ABSTRACT

Objective To study the clinical value of whole -body magnetic resonance diffusion weighted imaging(WB-DWI) in evaluating the chemotherapy response for lung cancer,thus to provide evidence for optimizing clinical imaging examination. Methods From October 2017 to May 2018,60 patients with lung cancer confirmed by histopathology in Linfen Central Hospital were selected. The patients underwent DWI examinations before chemotherapy and after two cycles of chemotherapy. The change of tumor size,distant metastasis and apparent diffusion coefficient (ADC) value were compared before and after chemotherapy. The correlation between the change rate of ADC value and the shrinkage rate of tumor size in the effective group was analyzed. Results Of 60 cases,1 case had new cerebral metastases after chemotherapy. There were statistically significant differences in ADC value [(1. 12 ± 0.33) ×10 -3mm2/svs.(1.56±0.40) ×10 -3mm2/s]andtumorsize[(4.63±2.75)cmvs.(2.28±1.45)cm] between before and after chemotherapy in the effective group(t= -3. 954,4. 711,all P<0. 01). There was correlation between the change of ADC value and tumor size(r=0. 34,P<0. 05). Conclusion WB-DWI can not only detect the change of tumor size and distant metastasis quickly and effectively,but also can observe the microscopic changes of tumor cells by measuring ADC value. So it can predict the early therapeutic response of the tumor and make effective evaluation for the staging and chemotherapy response of lung cancer.

ABSTRACT

With the rapid development of China′s aging population, palliative care has been greatly developed in China. And advance directives (ADs), as an important part to promote the development of palliative care, because of the lack of legal protection and a wide range of social identity, its development is relatively slow. In China, most domestic studies are cognitive investigation. This paper describes the related concepts, research background and significance of ADs, analyzing the related factors affecting patients make ADs, puts forward the specific content of ADs intervention, including the intervention object, the leader of ADs, intervention sites, methods of ADs intervention etc., so as to provide suggestions for the development of ADs intervention in China.

ABSTRACT

Objective To establish a simple,rapid and sensitive nucleotide polymorphisms genotyping method in order to conduct the routine clinical detections under the simple laboratory condition by this method.Methods Based on the ligase-agarose gel electrophoresis,the oligonucleotide detection probes of mutational sites was designed.The detection underwent the detection probe connecting,purification and universal amplification,finally the mutation genotypes of detection sites were judged by the ap-peared bands in the agarose gel electrophoresis(AGE).With the 3 SNP sites EGFR,c.2573T>G(L858R),EGFR,c.2582T> A (L861Q)and EGFR,c.2155 G>T(G719C)in epidermal growth factor receptor(EGFR)gene as the detection objects,the plasmid template and plasma circulating DNA sample in lung cancer were performed the detection.Results The established method was easy to operate with higher specificity and sensitivity.After 20-30 cycles of PCR amplification,the genotype of detection sites was clearly estimated according to the amplification band.When detecting the mixed alleles in the heterogeneous sample,minimal 2.5%mutation alleles could be detected out.This method and the direct sequencing method could respectively detect 6 cases and 2 cases of heterozygotes mutation in the SNP site of L858R among 62 samples of lung cancer.Conclusion The established detection method for SNP genotyping is suitable to the routine mutation detection on the heterogeneous samples under the simple laboratory condi-tion.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the changes in the afferent discharge activities of the sensory nerve endings in muscle spindles of rats with hindlimb immobilization.</p><p><b>METHODS</b>Plaster cast was used immobilize the hindllimbs of rats. Using air-gap technique, the spontaneous discharge of the muscle spindles and its responses to perfusion with succinylcholine (0.05 mg/ml) and suspension in an extended position were observed in isolated muscle spindles from rats with hindlimb immobilization for 3, 7, and 14 days.</p><p><b>RESULTS</b>The muscle spindles of rat soleus showed a sharp decrease in spontaneous discharge frequency (P<0.01) and response to succinylcholine perfusion after 3 days of hindlimb immobilization (P<0.05). Significant changes of the firing rate in an extended position was observed in rats after a 14-day immobilization (P<0.01). The duration of individual spikes was significantly prolonged following hindlimb immobilization (P<0.01).</p><p><b>CONCLUSION</b>Muscle spindle discharges decrease significantly in rats following hindlimb immobilization, which might be related to reduced contractile properties of the muscle spindle.</p>

ABSTRACT

Breathing gas carries important physiological information. Technology for detection of breathing gas has become a research focus because of the advantages of nondestructive sampling and convenient operation. Proton transfer reaction mass spectrometry (PTR-MS) plays an irreplaceable role because of the advantages of high sensitivity, fast response and good specificity. In this paper, the principle of PTR-MS is introduced first, followed by research progress of PTR-MS in the field of breathing gas detection. Factors influencing the test results are analyzed. Finally, future prospects of development for PTR-MS in the field of breathing gas detection are discussed.

ABSTRACT

@# Objective To observe the alterations in soleus nerve conduction velocities induced by hindlimb immobilization. Methods 24 healthy Sprague-Dawley rats were randomly divided into control group, immobilization 7 days group and immobilization 14 days group with 8 cases in each group. The plaster cast was fixed as the method to induce a hindllimb immobilization model in rats. The afferent thin fibers conduction velocity and the latency of M wave were observed with electrophysiological technique. Results 7 days after immobilization,the conduction velocity of muscle spindle primary afferent fiber decreased (P<0.05), the latency of M wave significantly prolonged 14 days after immobilization (P<0.05). Conclusion The soleus nerve conduction velocities decrease significantly following hindlimb immobilization and the alterations in afferent fibers are more serious than that in efferent fibers.

ABSTRACT

Objective: This paper is aimed at to evaluate B7-H1 expression as induced by human cytomegalovirus [HCMV] in extravillous cytotrophoblast cell line HPT-8 and possible underlying mechanism


Method: Real time PCR and flow cytometry were used to determine B7-H1 mRNA and protein before and after HCMV infection in HPT-8 cells. Western blot analysis was used to determine the level of MAPK phosphorylation in HPT-8 cell lines infected with HCMV


Results: 100TCID50 was found to be the most effective dose, capable of stimulating B7-H1 mRNA and protein expression in HPT-8 cells. When empty control group was considered to have a B7-H1 mRNA value of 1, B7-H1 mRNA was 4.32 in 100TCID50 group. In flow cytometry study, mean fluorescence intensity [MFI] of 100TCID50 group was 16.14, while empty control group was 1.34. Both mRNA and protein expression were found to be significantly increased [P < 0.05] in 100TCID50 group compared to empty control group


The result of Western blot analysis showed increase in B7-H1 expression caused by the extracellular signaling that was related to ERK activation and the ERK inhibitor U0126 was found to reverse this increase


Conclusion: HCMV upregulates B7-H1 expression in human extravillous cytotrophoblast cell line HPT-8, which is related to MAPK activation. Our result would be helpful in finding better therapies against intrauterine HCMV infection

ABSTRACT

Objective To investigate the methods of nursing safety management for indwelling catheters. Methods The nursing safety management measures included the establishment of three-level monitoring network of safety management for indwelling catheters,standardization of 25 kinds of catheter identification cards,formulation of prevention procedure and reporting system of catheter extrusion,design of catheter management handbook,organization of training and examination on the relative skills for nurses. Results Nurses' safety awareness of indwelling catheters was strengthened. And nurses could distinguish different catheters promptly and provide efficacious and safe nursing services. There was no nursing error,nursing dispute and patient complaints towards nursing care of indwelling catheters. Conclusion The implementation of nursing safety management for indwelling catheters can ensure medical and nursing safety,and promote the continuous improvement of quality of nursing.

ABSTRACT

Objective To explore the curative effect and patient's tolerance combined transcatheter arterial chemoembolization(TACE) and three dimensional conformal radiotherapy(3DCRT) for primary hepatocellular carcinoma.Methods 38 patients were treated by combined TACE and 3DCRT.The treatment regimen consisted of TACE first,it took TACE of Doxombicin 50 mg/m2,Fluorouracil 600 mg/m2,Cisplatin 60 mg and iodine Oil 10~15 ml.repeated every four weeks.thlee times totally.After resting one or two months,followed by 3DCRT(44~56 Gy/7~11f/3~4w),the planning target volume(PTV)was covered by the 80%~90% isodose curve and the target dose was prescribed to PTV.Resuits All patients completed the planned treatment without interruption.Hematological and radiation-induced toxicities as shown by the WHO staging system were 52.6% of grade 1~2 acute gastrointestinal tract reaction and 47.4% of grade 1~2 bone marrow inhibition but they were tolerable.The complete response(CR)and partial response(PR)rates were 23.7% and 63.2%.respectively,with a CR+PR rate of 86.8%.The median survival was 18.5 months.The 1-,2-and 3-year survival rates were 73.7%,57.9% and 34.2% respectively. Conclusion Combined TACE and 3DCRT iS better to short-term curative effect,it has lesser toxicities and it is a satisfactory therapy for unreseetable primary hepatocellular carcinoma.Long-tem survival and late toxicities need further observation.

ABSTRACT

AIM: To explore the effects of high glucose a t fluctuant concentrations on glucose transport activity and expression of phosp hoinositide 3-kinase (p85) in primary cultured skeletal muscle cells. ME T HODS: Rat skeletal muscle cells were cultured at fluctuant glucose conce ntrations (5, 25mmol/L) for 48h. Then the glucose uptake and the expression of p hosphoinositude 3-kinase were measured. RESULTS: The skeletal mu s cle cells treated with fluctuant high concentration glucose showed the impairmen t of the basal and insulin-induced increase in glucose uptake and significant de crease in p85 protein expression as well as p85 mRNA. Particularly, there was a significant decrease in p85 protein and mRNA expression in the high fluctuate l evel of glucose concentration. CONCLUSION: The exposure to fluct uant high concentration of glucose inhibits glucose uptake and induces insulin r esistance in skeletal muscle cells.
