ABSTRACT

[Abstact] Purpose To investigate the value of apparent diffusion coefficient (ADC) of diffusion weighted imaging (DW1) in the evaluation of the efficacy of treatment of nonsmall cell lung cancer (NSCLC) with tyrosine kinase inhibitor (TKI).Materials and Methods Nineteen patients with advanced NSCLC who treated with TKIs from May 2014 to December 2015 were recruited prospectively. All patients underwent CT and MRI scans three times before targeted therapy, after 1 week and 4 weeks of treatment. The tumor maximal diameter and mean ADC value at different time points were compared. The correlations between mean ADC value and tumor maximal diameter and maximum diameter change rate at different time points were analyzed.Bland-Altman analysis was performed to confirm the reproducibility of measurements. Results The ADC value after 1 week of treatment was significantly increased compared with that before treatment (t=-6.329,P<0.05),but the change of tumor maximal diameter was not significant (P>0.05).The ADC value was significantly increased after 4 weeks of treatment compared with that before treatment (t=-4.878,P<0.05),and the change of tumor maximal diameter was also significant (t=7.054,P<0.05).The negative correlation was detected between the ADC before treatment and the maximal diameter change ratio after 4 weeks of treatment (r2=-0.474,P<0.05).In the group of responders, there was significant difference between the ADC value before treatment and 1 week after treatment (P<0.05).There was significant difference in ADC value between the groups of responders and non-responders after 4 weeks of treatment (P<0.05).The Bland-Altman analysis showed good reproducibility among the operators. Conclusion The ADC value after 1 week of targeted therapy is more sensitive to the change of tumor than the maximal diameter, and the ADC value before treatment has a certain value in predicting the maximum diameters of the target after 4 weeks of targeted therapy with repeatability.

ABSTRACT

OBJECTIVETo evaluate the efficacy and safety of wheat cellulose particles (testa triticum tricum purify, Fiberform) in the treatment of internal hemorrhoid.

METHODSA multicenter randomized controlled clinical trial was adopted. From October 2015 to July 2016, 60 patients with internal hemorrhoid were enrolled from three medical centers, including Department of Anorectum, Shuguang Hospital of Shanghai University of Traditional Chinese Medicine, Department of Anorectum, The Third Affiliated Hospital of Nanjing University of Chinese Medicine, Department of Anorectum, The Second Affiliated Hospital of Guangzhou University of Chinese Medicine. Enrollment criteria: Patients aging from 18 to 65 years old; being diagnosed with the second or third grade internal hemorrhoid; having good communication skills and being able to complete the records and follow-ups according to the research program requirements.

EXCLUSION CRITERIAPatients combined with other anal diseases, or whose anus tube or rectum suffering occupying lesions; patients currently using other methods and defecation drugs in the treatment of their hemorrhoids; patients in pregnancy or with diseases of heart, liver, kidney or metabolic disorders; patients suffering from constipation due to other diseases and drugs, and long-term laxatives abusers. According to the random number table method, 60 patients were randomly divided into the combined treatment group [30 cases, wheat cellulose particles 1 bag each time, 2 times per day; Diosmin tablet 2 pills, 2 times per day] and the single treatment group [30 cases, Diosmin tablet 2 pills, 2 times per day]. The treatment courses for both groups were 7 days. According to the four-grade scoring method, the efficacy evaluation would be made on six indicators, which were the degree of hematochezia or bleeding, the degree of pain, hemorrhoid prolapse, the shapes and properties of stool, the defecation frequency and the defecation duration. The higher the score a participant got, the more severe the symptom was. The effectiveness was evaluated by the scoring reduction rate, and marked effectiveness and effectiveness were both found to be effective. Incidence of adverse events was compared between two groups before the treatment, and on postoperative 3-day and 7-day respectively.

RESULTSThere were no significant differences between two groups in gender, age, internal hemorrhoids gradings, disease course, and onset time as well as the baseline data, such as the degree of hematochezia or bleeding before the treatment, the degree of pain, hemorrhoid prolapse, the shapes and properties of stool, the defecation frequency and the defecation duration (all P>0.05). After the seven-day treatment, there was significant difference in effective rate between combination group and single group [96.7%(29/30) vs. 66.7%(20/30), Z=-4.376, P=0.000]. Meanwhile, the scores of combined group and single group in hematochezia or bleeding were 0(0, 1) and 0(0, 2) (Z=9.241, P=0.002); in shapes and properties of stool were 0(0, 1) and 0(0, 1) (Z=5.364, P=0.021); in defecation frequency were 0(0, 1) and 0(0, 2) (Z=7.552, P=0.006); and in defecation duration were 0(0, 1) and 0(0, 2) (Z=4.425, P=0.035), whose differences were all significant. The scores of pain degree and hemorrhoid prolapse of two groups also decreased, but the difference was not statistically significant (P>0.05). During the treatment, abdominal pain, diarrhea and other adverse reactions were not observed in participants of two groups.

CONCLUSIONCombination therapy of wheat cellulose particles (testa triticum tricum purify, Fiberform) can significantly improve the efficacy of internal hemorrhoid with safety and tolerance.

ABSTRACT

Aim To establish a cell model targeting on GLP-1 R,and evaluate its function by the cAMP assay, for screening the new class of GLP-1 analogues as anti-diabetic candidates. Methods An eukaryotic expression vector pEGFP-GLP-1 R was constructed and transfected into HEK293A cells. After selecting with G41 8,a cell line stably expressing GLP-1 R-GFP was estab-lished.The expression and the cellular distribution of GLP-1 R-GFP fusion protein were investigated by Western blot and fluo-rescence microscopy. Then, the activity of GLP-1 analogue Lira-glutide was evaluated by monitoring the content of cAMP via HTRF using this cell model. Results GLP-1 R-GFP-293A cell line was successfully established.GLP-1 R-GFP fusion proteins were mainly distributed in the cell membrane. The dose-respon-sive relationship experiments revealed that cAMP could be effec-tively stimulated by Liraglutide using this cell model.Conclu-sion This cell model could be used to detect the bioactivity of GLP-1 analogues in vitro, which lays a foundation for the screen-ing of GLP-1 analogues and small GLP-1 R agonists.

ABSTRACT

PurposeTo study the MRI and pathological features of ovarian endometrioid adenocarcinoma (OEC) in order to evaluate the value of MRI in diagnosing OEC. Materials and Methods The MRI imaging features of 8 patients confirmed by surgery and pathology were analyzed retrospectively and were compared with the results of surgery and pathology. The MRI outcome and the related histological findings were further analyzed. Results Six out of the eight patients had unilateral tumor, 3 tumors in the left and the other 3 in the right; 2 patients had masses in the both ovaries. The total number of masses was ten.The diameters of the tumors ranged from 3.5 to 16.5 cm, with the average size of (10.5±4.1) cm. The border of 2 tumors was partially fuzzy and that of the other 6 was clear. The MRI scans showed that 8 tumors were cystic-solid and the other 2 were solid with heterogeneous signals. The solid components mainly presented slightly short T1 signals and long T2 signals; the cystic ones revealed long T1 and T2 signals. The enhanced scanning showed that the solid components of 8 tumors were patchy and obviously enhanced and the other 2 had mild or moderate enhancement. The cystic components were not hyper-intense. The enhanced MRI scans of two cases of primary endometrial carcinoma with metastases to the ovaries showed thickened endometrium and mild hyper-intense.Conclusion MRI can reveal the pathological features of OEC and clearly presents the forms, components and the relationship with its surroundings of tumors. Therefore, MRI is of great importance to the clinical diagnosis of OEC.

ABSTRACT

Objective To explore the associations between vitamin D receptor ( VDR) Fok- Ⅰ and Apa- Ⅰ polymorphisms and myasthenia gravis (MG) in Chinese Han population. Methods Polymorphisms of VDR Fok- Ⅰ and Apa-Ⅰ were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. The frequencies of genotypes and hyplotypes were compared among 286 normal controls and 267 MG patients in different subgroups classified by gender, age of onset, presence of thymoma, and Osserman classification sat maximal severity in the follow-up.The association between the genotypes and maximal severity of MG and short-term glucocorticoid treatment were also investigated. Results There were no significant differences in frequencies of genotypes and hyplotypes of both Fok-Ⅰ and Apa-Ⅰ between MG group and control group, and among subgroups of MG.The Fok- Ⅰ showed no statistical difference between the patients with better and less improvement after short-term treatment of glucocorticoid. The frequency of Apa-Ⅰ alleles in the patients with better improvement (55/186,29.6% ) significantly differed from the less improved group ( 7/48,14.6%,OR = 2.46,95% CI 1.04-10.43,x2 = 4.400,P = 0.036).The patients with the genotype A A/Aa were more likely to improve better after the treatment(48/93,51.6%) than in the worse group(7/24,29.2%,OR =2.59,95% CI 0.98-14.60,x2 =3.858,P= 0.049).Conclusions Alleles and genotypes of VDR-Fok- Ⅰ and Apa-Ⅰ were not found to be related with MG onset and severity.MG patients with VDR-Apa-A allele may have better improvement short-term treatment of glucocorticoid.
