ABSTRACT

It has attracted much attention worldwide that the application of artificial intelligence (AI) in primary screening and clinical diagnosis and treatment of eye diseases. In recent years, this technology has also been widely used in various grass-roots eye disease management, effectively improving the current situation of weak eye disease diagnosis ability and shortage of human resources in primary medical institutions. At present, there is no reference standard or guideline for the management mode, implementation content and management method of vision health management based on this technology, which are in urgent need of standardization. The article described the work mode exploration of AI-assisted grass-roots visual health management in Shanghai and shared practical experience. The aim is to provide reference for other provinces in China to carry out relevant work.

ABSTRACT

Objective: To study the inhibitory action of self-designed dithiothreitol(DTT) eye drops on selenite-induced rat cataract(simulating the senile cataract). Methods: Twelve-day-old SD rats (body weight about 25 g) were injected with Na2SeO3 subcutaneouly, quaque die alterna, for 3 times (0.2 ml/time, with a total amount of 210 μg). Six days later over 90% of rats developed nuclear cataracts. The right eyes of the rats were treated with 25 mmol/L DTT eye drops (4 times/day, 1 drop/ time) for 3 weeks. The left eyes were taken as control and were treated with normal saline at the same dosage. The lens were examined using slit lamp and the diameter of the cataract plaques were measured using vernier cursor. The eyes of cataract models were also treated with 25 mmol/L DTT (n = 89), pirenoxine eye drops (PDE, Japan, n = 22), pirenoxine sodium eye drops (PSED, China, n = 24, 2 died during the experiment), and normal saline (n = 45). Each eye was administered with 1 drop/time, 4 times/day for 3 weeks. Then slit lamp was used to examine the cortical lesions of both eye and the lens opacity. Results: The DTT eye drops significantly reduced the diameter of the cataract plaques and significantly reduced the rate of the cortical opacity after the nuclear cataract. The inhibitory action of DTT eye drops on the selenite-induced cataract was superior to pirenoxine eye drops(Japan) and pirenoxine sodium eye drops(China). Conclusion: Our study shows that the DTT eye drops has satisfactory inhibitory action on selenite-induced cataract in rats.

ABSTRACT

OBJECTIVETo evaluate the efficacy and safety of phacoemulsification with foldable posterior chamber intraocular lens(PC-IOL) implantation in the management of acute or chronic primary angle-closure glaucoma(APACG or CPACG) with cataract.

METHODSTwenty-six cases(28 eyes) with simple cataract and 60 cases(70 eyes)of PACG with cataract, including 14 eyes of preclinical APACG, 29 eyes of paroxysmal APACG, 27 eyes of CPACG. Visual acuity distributed from hand movement to 0.8. Phacoemulsification and intraocular lens implantation were performed and patients were followed up for(8.5+/-4.5) months.

RESULTIn simple cataract group, postoperative IOP did not decrease(t=1.9201, P>0.05); in preclinical APACG group, postoperative IOP decreased (t=3.9910, P<0.01); in paroxysmal APACG group, postoperative IOP decreased (t=4.7441, P<0.01); in CPACG groups, postoperative IOP decreased (t=4.4976, P<0.01). In APACG and CPACG groups postoperative antiglaucoma medication reduced. In APACG and CPACG groups, angle of anterior chamber was widen. Preoperative central and peripheral ACD of APACG and CPACG were much less than those of postoperative. In 56 eyes of PACG, visual acuity was corrected in 48 eyes(85.7%) including 28 eyes(50%) with corrected visual acuity>0.5.

CONCLUSIONThe phacoemulsification with foldable posterior chamber intraocular lens implantation is beneficial for PACG with cataract, and the curative efficacy of APACG is better than that of CPACG.
