ABSTRACT

Objective To explore the effects of family empowerment program on comprehensive care ability and care preparedness for main caregivers of patients with initial stroke. Methods A total of 64 main caregivers of patients with initial stroke were enrolled from a tertiary hospital in Hangzhou from January 2017 to July 2017,and assigned into the intervention group and the control group according to different wards, with 33 cases in the intervention group and 31 cases in the control group. Both groups received routine care, and the intervention group also received family empowerment program. The stroke caregivers' comprehensive care ability questionnaire and the Caregiver Preparedness Scale were used to evaluate the effects of the intervention. Results Before the intervention, there were no statistically significant differences in disease-related knowledge, daily and disease-related care skills,self-stress and health management, coping strategies, and total scores of comprehensive care ability, and caregiver preparedness scores between two groups(P>0.05).After the intervention, the total scores and each dimension score of comprehensive care ability, and caregiver preparedness scores of two groups were significantly higher than those before the intervention, and the differences were statistically significant(P<0.05).Conclusion Family empowerment program can effectively improve comprehensive care ability and care preparedness of main caregivers of patients with initial stroke, making the caregiver playing a positive role in the rehabilitation of stroke patients.

ABSTRACT

Colorectal cancer is currently one of the carcinomas with highmortality in the world, and its incidence is increasing year by year. In the course of treatment, the patients with colorectal cancer prone to metastasis and recurrence because of the presence of v-Kiras2 Kirsten rat sarcoma viral oncogene homolog (K -Ras) gene mutations, which seriously limits the treatment of colorectal cancer. Drug resistance is a major problem in oncotherapy, which is more prominent in the treatment of K -Ras -mutated colorectal cancer, and the effective treatment strategies are still lacked in present. With the increase of reports about the formation mechanisms of drug resistance, recurrence and metastasis, as well as the discovery of molecular biomarkers in colorectal cancer with K -Ras gene mutations, the new drugs and the corresponding combination regimens appear constantly. Furthermore, the therapy of K -Ras -mutated colorectal cancer is more rational, but there are still many problems. Therefore, this paper focuses on the lasted studies on colorectal cancer with K -Ras -mutations, and reviews the progress in K -Ras mutation characteristics, treatment difficulties and research breakthroughs as well as the future prediction, hoping to provide some useful references for the subsequent clinical treatment and research.

ABSTRACT

High-grade gliomas (HGG) are a group of malignant tumours with high recurrence rate, rapid progression, poor prognosis and short median survival. Patients suffer with severe symptoms after operation, which seriously affect the quality of life of patients. Palliative care is an important nursing measure to improve the quality of life of patients with malignant tumors. This paper reviews the current situation and research progress of palliative care in patients with advanced glioma at home and abroad, and provides the evidence and guidance for medical staff to carry out palliative care of patients with high grade glioma.

ABSTRACT

Objective The coronary slow flow phenomenon (CSFP) is a coronary artery disease with a benign course, but its pathological mechanisms are not yet fully understood. The purpose of this controlled study was to investigate the cellular content of blood in patients diagnosed with CSFP and the relationship of this with coronary flow rates. Methods Coronary angiographies of 3368 patients were selected to assess thrombolysis in myocardial infarction (TIMI) frame count (TFC) values.Seventy eight of them had CSFP,and their demographic and laboratory findings were compared with 61 patients with normal coronary flow. Results Patients'demographic characteristics were similar in both two groups.Mean corrected TFC (cTFC) values were significantly elevated in CSFP patients (P < 0.001).Furthermore,hematocrit and hemoglobin values, and eosinophil and basophil counts of the CSFP patients were significantly elevated compared with the values obtained in the control group (P =0.005,P =0.047,P =0.001 and P =0.002).The increase observed in hematocrit and eosinophil levels showed significant correlations with increased TFC values (r =0.288 and r =0.217).Conclusion Significant changes have been observed in the cellular composition of blood in patients diagnosed with CSFP as compared to the patients with normal coronary blood flow. The increases inhematocrit levels and in the eosinophil and basophil counts may have direct or indirect effects on the rate of coronary blood flow.

ABSTRACT

ObjectiveTo investigate the role of ischemic postconditioning on lung ischemic/reperfusion injury induced by cardiopulmonary bypass (CPB) in combined mitral and aortic valve replacement, and evaluate the effects of different protocol of ischemic postconditioning, and analyze its possible mechanisms.Methods24 patients diagnosed as rheumatic mitral and aortic valve disease with or without tricuspid valve disease were randomly divided into 3 groups (8 in each):Control group( group A),patients undergoing routine cardiac surgery; Postconditioning group Ⅰ (group B ),patients undergoing routine cardiac surgery and ischemic postconditioning by occlusion of the pulmonary artery of five cycles of 15 s ischemia and 15 s repeffusion before the pulmonary artery totally restoring peffusion; Postconditioning group Ⅱ (group C),patients undergoing routine cardiac surgery and ischemic postconditioning by occlusion of the pulmonary artery of five cycles of 30 s ischemia and 30 s repeffusion before the pulmonary artery totally restoring perfusion. The Oxygenation Index (OI) was measured at pre -operation and 1 h,2 h,3 h,6 h and 12 h after terminating CPB.The plasma contents of MDA were detected by Enzyme-linked Immunosorbent Assay at pre -operation,1 h,3 h,6 h and 12 h after terminating CPB.ResultsCompared with group A and C,OI in group B significantly increased at 1 h,2 h,3 h,6 h after terminating CPB (283.25 ±56.47 vs 384.76 ±29.17 vs 310.50 ±65.71,265.75 ±58.78 vs 381.75 ±29.67 vs 310.50 ±48.17,283.75 ±73.15 vs389.74±39.34 vs 317.87 ±78.41,310.37 ±52.00 vs 401.62 ±4 2.89 vs 337.25 ± 64.06,all P <0.05),and the plasma contents of MDA in group B reduced at 1 h,3 h,6 h,12 h after terminating CPB (4.64±0.63 vs 3.88 ±0.20 vs 4.38 ±0.41,5.75±0.49 vs 4.44 ±0.34 vs 5.28 ±0.76,4.42±0.31vs 3.77 ± 0.40 vs 4.35 ± 0.54,3.74 ± 0.31 vs 3.19 ± 0.17 vs 3.64 ± 0.24,all P < 0.05 ), However, there were no significant differences in OI and MDA between group C and A (all P >0.05 ).Conclusions Ischemic postconditioning by occlusion of the pulmonary artery of 5 cycles of 15 s ischemia and 15 s reperfusion attenuates lung oxygenation function injury induced by CPB in double valves replacement, and it is possibly mediated by reducing MDA produced by lipid peroxidation. However, ischemic postconditioning by occlusion of the pulmonary artery of 5 cycles of 30 s ischemia and 30 s repeffusion may not have significant protective effect for lung oxygenation function injury induced by CPB in double valves replacement.
