ABSTRACT

Objective:To explore the methods and effect of applying specialized training and multimedia education in key infection-related departments of a hospital to intervene standard precautions for clinical practice interns. Methods:Based on the cases of nosocomial infection at home and abroad, the Science and Education Section and Infection Management Section designed and produced videos about standard precautions and occupational protection. The content of these videos covered two fields including knowledge training and skill training, with emphasis on enhancing awareness of profession risks and skills of hospital infection control and occupational protection of 196 medical students through specialized training in key infection-related departments. The training effect of standard precautions education for the two groups of students was evaluated from the theoretical test scores, questionnaire survey on their attitude and compliance to standard precautions, and the incidence of their occupational exposure to needle stab wound. SPSS 18.0 was performed for t test and chi-square test. Results:Apart from the knowledge of the unfavorable factors of implementing standard precautions, there were significant differences between the experimental group and the control group in six aspects: theoretical test scores, hospital infection risk cognition, significance cognition of standard precautions, favorable factors of implementing standard precautions, self-efficacy evaluation and self-evaluation of compliance to standard precautions ( P<0.05). And there was significant difference in the incidence of occupational exposure to needle stab wound between the experiment group and control group (1.02% vs. 7.14%; χ2=4.69, P<0.05). Conclusion:Various teaching interventions of standard precautions during the entire internship period have expanded and deepened the contents of school education, enabling the students to form good occupational risk awareness and coping skills to protect the patients and themselves.

ABSTRACT

Objective To investigate current situation communication ability and training needs of standardized training nurses, in order to provide reference for improving the communication courses. Methods A cross-sectional study was conducted and randomized sampling method was used to choose 100 nurses who participated in standardized training in Chengdu Fifth People′s Hospital. Nurses' communication ability was evaluated by Nurse-Patient Communication Ability Scale, and whose training needs of communication were investigated. Results The average score of the Nurse- Patient Communication Ability Scale was 140.43 ± 13.94. The educational background (t=-3.717,P﹤0.01), standardized training time (t=-2.812, P﹤0.01), and whether they had received communication training in the last year (t=2.231, P﹤0.05) had significant influence on nurse-patient communication ability. 88.00% (88/100) thought it necessary to carry out nurse-patient communication ability training during standardized training. Among them, 88.64% (78/88) hoped to carry out humanistic knowledge training, 65.91% (58/88) hoped that at least four training methods would be combined, and 44.32% (39/88) hoped that teachers from multiple disciplines would conduct training. Conclusions The nurse-patient communication ability of standardized training nurses needs to be improved. It is necessary to strengthen the theoretical research and curriculum design of training, pay attention to practical teaching, so that the training effect can be improved.

ABSTRACT

Objective To explore the cognitive level of patients with breast cancer who were undergoing adjuvant endocrine therapy and their needs for related knowledge, so as to provide powerful references for making effectively compliance interventions. Methods An in-depth interview was conducted on 12 breast cancer patients who were undergoing adjuvant endocrine therapy. Data were analyzed by Colaizzi analysis procedure. Results The views on adjuvant endocrine therapy were very disagree and the main needs were: the related knowledge of adjuvant endocrine therapy, side effects of adjuvant endocrine therapy and its cope measures, the needs about healthy lifestyle during taking medicine, the needs of communicating with doctors and nurses, the regulation methods on alleviating psychological stress and releasing worries of the drug costs. Conclusion More efforts should made to strengthen the health education on adjuvant endocrine therapy and create effectively follow-up system, strengthen multi-discipline cooperation and doctor-patient communication, also we need deeply recognize the cognitive level about endocrine therapy of the patients and meet their psychological needs, aim directly interventions to relieve or eliminate their concerns, thus reinforce their believes of drug administration and promote the adherence of adjuvant endocrine therapy.

ABSTRACT

Hyoscyamine 6 beta-hydroxylase (H6H) is the last rate-limiting enzyme directly catalyzing the formation of scopolamine in tropane alkaloids (TAs) biosynthesis pathway. It is the primary target gene in the genetic modification of TAs metabolic pathway. Full-length cDNA and gDNA sequences of a novel H6H gene were cloned from Datura arborea (DaH6H, GenBank accession numbers for cDNA and gDNA are KR006981 and KR006983, respectively). Nucleotide sequence analysis reveals an open reading frame of 1375 bp encoding 347 amino acids in the cDNA of DaH6H, while the gDNA of DaH6H contains four exons and three introns, with the highest similarity to the gDNA of H6H from D. stramonium. DaH6H also exhibited the most identity of 90.5% with DsH6H in amino acids and harbored conserved 2-oxoglutarate binding motif and two iron binding motifs. The expression level of DaH6H was highest in the mature leaf, followed by the secondary root, and with no expression in the primary root based on qPCR analysis. Its expression was inhibited by MeJA. DaH6H was expressed in E. coli and a 39 kD recombinant protein was detected in SDS-PAGE. Comparison of the contents of scopolamine and hyoscyamine in various TAs-producing plants revealed that D. arborea was one of the rare scopolamine predominant plants. Cloning of DaH6H gene will allow more research in the molecular regulatory mechanism of TAs biosynthesis in distinct plants and provide a new candidate gene for scopolamine metabolic engineering.

ABSTRACT

Objective To observe the clinical effects of Shenqi Fuzheng Injection in treatment of sepsis in children and its effect on the serum levels of high mobility group protein-1 and tumor necrosis factor-α, lactic acid, C reactive protein, procalcitonin levels. Methods 140 patients with sepsis were chosen, and randomly divided into two groups, the observation group and the control group. Two groups were given treatment by the national standard of sepsis, while the control group only received basic treatment, the observation group on the basis of conventional treatment combined with Shenqi Fuzheng injection. Two groups of clinical curative effect and the levels of serum HMG-1, TNF-α, LA, CRP, PCT level were observed. Results After treatment, the observation group total effective rate (95.71%) was significantly higher than control group (70.00%),χ2 =16.29, P<0.01.Two groups before treatment, TNF-αand HMG-1 had no statistically significant difference. After treatment, 2 groups of HMG -1, TNF-αsignificantly lower than before treatment (P<0.01),which HMG-1,and TNF-αreduced significantly in observation group (P<0.01).Serum HMG-1 were positively correlated with TNF-αbefore and after treatment (P<0.05).After treatment, LA, CRP and PCT in 2 groups were significantly different compared with before treatment (P<0.01), and the observation group decreased significantly than control group (P<0.01).Conclusion The effect of Shenqi Fuzheng Injection in the treatment of children with sepsis is significant, which is worthy of clinical application.

ABSTRACT

Objective To evaluate the application effect of bedside evaluation of deglutition function and predictive nursing intervention on patients with dysphagia in neurology department. Methods 320 Patients with dysphagia were divided into the observation group and control group with 160 cases in each group. The observation group was given admission nursing evaluation and bedside evaluation of deglutition function while the control group only received admission nursing evaluation. The incidence of complications in each group were assessed. Results The incidence of complications such as aspiration, pulmonary infection, asphyxia and metabolic disturbance in the observation group was lower than that of the control group(P<0.01).Conclusion The bedside evaluation of deglutition function could accurately evaluate the degree of swallowing disorder and supply theoretical evidence for predictive nursing intervention by in structing clinical application of selective measures for patients with dysphagia. This could guarantee safe food intake and reductive incidence of complications of patients. So we advocated that bedside evaluation of deglutition function should be a routine admission nursing evaluation in neurology department.
