ABSTRACT

Objective:To investigate the effect of Kuntai capsule combined with fenmotong in the treatment of perimenopausal syndrome and its effects on sex hormone, anti-Mullerian hormone (AMH) and bone mineral density (BMD).Methods:One hundred and eighteen patients with perimenopausal syndrome admitted to Ningbo Development Zone Central Hospital from January 2018 to January 2019 were randomly divided into observation group (59 cases) and control group (59 cases). The patients in the control group were treated with Fenmotong, while the patients in the observation group were treated with Kuntai capsule on the basis of the control group. Both groups took 28 d as a cycle for 6 consecutive cycles. The changes of Kupperman menopausal index (KMI), sex hormone, AMH and bone mineral density before and after treatment were compared between the two groups.Results:The KMI score of the observation group was lower than that of the control group [(6.27 ± 1.42) scores vs. (9.89 ± 1.74) scores]( t=12.381, P<0.05). The level of serum estradiol (E 2) in the observation group was higher than that in the control group [(195.48 ± 12.43) pmol/L vs. (165.65 ± 10.76) pmol/L], while the levels of serum luteinizing hormone (LH) and follicle-stimulating hormone (FSH) in the observation group were lower than those in the control group [(35.46 ± 1.76) U/L vs. (39.92 ± 2.13) U/L, (36.57 ± 2.34) U/L vs. (40.38 ± 1.89)U/L]( P<0.05). The serum AMH level of the observation group was higher than that of the control group [(3.24 ± 0.68) μg/L vs. (1.97 ± 0.57) μg/L] ( t=10.994, P<0.05). The BMD of the observation group was higher than that of the control group [(0.87 ± 0.13) g/cm 2 vs. (0.71 ± 0.07) g/cm 2]( t=8.324, P<0.05). Conclusions:Kuntai capsule combined with Fenmotong has a good effect in the treatment of perimenopausal syndrome. It can improve the level of sex hormones, AMH and bone mineral density.

ABSTRACT

Objective To observe the clinical efficacy of LiQiDingChuan pill combined with Dexamethasone on the treatment of patients with acute asthma.Methods 93 patients with acute asthma from July 2015 to September 2016 were randomly divided into the observation group(47 cases)and the control group(46 cases).The control group were received dexamethasone, and the observation group were given LiQiDingChuan pill combined with Dexamethasone.The main symptoms disappeared time, forced expiratory volume in 1 second, peak expiratory flow rate value, hs-CRP, TNF-α and clinical efficacy were recorded and analyzed.Results ①After treatment, wheezing cough, chest tightness and wheezing disappeared time were(4.71±0.59)d,(2.85±0.49)d,(4.93±0.70)d in the observation group, which were shorter than(7.76±1.02)d,(5.16±0.74)d,(8.67±1.12)d in the control group, and the differences were statistically significant(P<0.05).② After treatment, FEV1 and PEF levels were(65.69±3.74)V/L,(218.92±16.09)L/s, which were better than(57.45±3.62)V/L,(176.34±15.14)L/s in the control group, and the differences were statistically significant(P<0.05).③ After treatment, hs-CRP and TNF-α levels were(6.76±0.78)mg/L,(7.99±1.87)pg/mL, which were better than(6.76±0.78)mg/L,(7.99±1.87)pg/Ml in the control group, and the differences were statistically significant(P<0.05).④The total effective rate in the observation group was 91.49%, which was significantly higher than 71.74% in the control group, and the differences were statistically significant(P<0.05).Conclusion Liqidingchuan pills combined with dexamethasone treatment has a good therapeutic effect on acute phase of asthma, which can reduce asthma related symptoms, reduce the inflammation level in the patients, optimize the forced expiratory volume in 1 second and peak expiratory flow rate.The combination is better than the single drug.

In vivo Mammalian Bone Marrow Micronucleus Test is included in the standard battery genotoxicity testing,with great application prospects in medicine,public health,food and drug safety evaluation fields.Establishing standardized experimental methods and conditions in GLP condition and accumulating a certain range of background data could effectively ensure the reliability of the test system,and also provide strong basis to support the experimental data.We herein summarized the background data of mouse and rat bone marrow micronucleus tests performed from 2007 to 2015,to expound the standardized data collection method for rodent animal bone marrow micronucleus test.

ABSTRACT

Objective To study the expression and significance of fatty acid synthase ligand ( sFas ) and soluble fatty acid synthase receptor (sFasL) on the neoadjuvant chemotherapy of breast cancer.Methods 84 cases of patients with breast cancer neoadjuvant chemotherapy were recorded from January 2015 to January 2016 in our hospital as breast cancer group,another 84 cases of healthy subjects as control group,the two groups were measured sFas,the positive rate of sFasL.At the same time,respectively before and after neoadjuvant chemotherapy, detection of patients serum sFas, sFasL levels and breast cancer.According to the clinical curative effect were divided into effective group and ineffective group , compared two groups of serum sFas, sFasL levels.Results Breast cancer group called sFas and sFasL positive rate was 58.33% (49/84) and 100.00%(84/84),were significantly higher than that of control group 38.10%(32/84) and 0.00% (0/84);The level of sFas in patients with neoadjuvant chemotherapy was significantly lower than that of neoadjuvant chemotherapy, and the level of sFas in neoadjuvant chemotherapy group was significantly higher than that in ineffective group, the difference was statistically significant (P<0.05).Conclusion In the course of neoadjuvant chemotherapy for breast cancer,the efficacy of neoadjuvant chemotherapy can be effectively detected by detecting the level of serum sFas in patients with breast cancer , while sFasL has no significant change in the neoadjuvant chemotherapy of breast cancer.

ABSTRACT

Objective To study nuclear factor E-2 related factor 2(NrF-2) and heme oxygenase -1(Ho-1) expression and to explore its clinical significance.Methods From February 2016 to June 2016, 176 cases human gastric tissue paraffin blocks selected in department of pathology in Hangzhou Cancer Hospital, including 78 cases of gastric cancer, 78 cases of normal tissue were analyzed.And the expressions of NrF-2 and Ho-1 were detected by immunohistochemistry in two groups of human gastric tissue.Results The levels of NrF-2 and Ho-1 in gastric cancer group were(0.752 ± 0.098),(0.862 ±0.081) were significantly higher than those in normal group(0.381 ±0.068),(0.412 ±0.083);NrF-2, the positive expression rate of Ho-1 in gastric wall invasion degree of T3 +T4 with lymph node metastasis, NTM stage III and IV gastric cancer tissues were significantly increased.The differences were statistically significant ( P <0.05 ) .Conclusion NrF-2 and Ho-1 play the important role in the development of gastric cancer and NrF-2, the positive expression rate of Ho-1 in the gastric carcinoma and gastric wall invasion, lymph node metastasis and NTM staging are closely related.The clinical diagnosis if the combined detection of NrF-2 and Ho-1, is conducive to the early diagnosis of gastric cancer and the prognosis of the disease.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the expression of microRNA-10a-5p (miR-10a) in laryngeal epithelial premalignant lesions (LEPL) and to analyze the correlations of its dysregulation with clinicopathologic parameters of LEPL.</p><p><b>METHODS</b>According to the WHO classification (2005), 94 cases of LEPL were grouped into mild dysplasia (MID), moderate dysplasia (MOD), severe dysplasia (SD) and carcinoma in situ ( CIS). The expression of miR-10a in 94 cases of LEPL was detected by quantitative real-time polymerase chain reaction (qRT-PCR), and correlated with the clinical and follow-up data of all LEPL patients.</p><p><b>RESULTS</b>miR-10a was down-regulated in LEPL with increasing grade of dysplasia. There was significantly statistical difference between low-risk ( MID + MOD) and high-risk ( SD + CIS) lesion groups (P = 0.03). The linear regression analysis showed that miR-10a was correlated with grade and gender of LEPL (P < 0.05).</p><p><b>CONCLUSIONS</b>Down regulation of miR-10a may be a useful molecular marker for grading of LEPL and diagnosis of early laryngeal cancer.</p>

ABSTRACT

Objective:To investigate the rationality of prophylactic antibiotics application during perioperative period in the pa-tients with type Ⅱ incision operation in gynecology and obstetrics in our hospital after the intervention. Methods:Totally 274 medical records of obstetrics and gynecology patients with typeⅡincision operation in our hospital from April 2012 to March 2013 were collect-ed as the control group, another 321 medical records of obstetrics and gynecology patients with typeⅡincision operation in our hospital from April 2013 to March 2014 were collected as the intervention group. The rationality of prophylactic antibiotics application before and after the intervention was compared. Results:Compared with those in the control group, the average administration time, the hos-pitalization time and the cost of antimicrobial agents were significantly decreased in the intervention group (P0. 05). Compared with those in the control group, the incidence of irrational choice of antibacterial drugs, the combined use without indication, irrational medication time,over-time use, inappropriate use and improper choice of solvents in the intervention group was significantly reduced ( P<0. 05). Conclusion:After the intervention, the rationality of prophylactic antibiotics application during perioperative period in the patients with type Ⅱ incision operation in gynecology and obstetrics in our hospital is improved significantly.

ABSTRACT

Objective To investigate the effect of plant estrogen on serum hormone levels, NOS and oxidative stress products in patients with uterine fibroids with iron deficiency anemia.Methods 80 cases of hysteromyoma patientswith uterine fibroids with iron deficiency anemia were selected, according to the different medication were randomly divided into experiment group and control group,40 cases in each group.Two groups were treated with anemia correction , mifepristone oral ,and the experiment group were treated on the base of the control group with plant estrogen, 12 weeks for a course.serum sex hormone, NOS, SOD and MDA level were detected.ResuIts Compared with the control group, the indexes of patients in the experimental group were lower(P<0.05), level of serum NOS were higher(P<0.05), level of serum SOD were higher(P<0.05), level of serum MDA were lower ( P<0.05 ) .ConcIusion Plant estrogen can improve the serum levels of serum hormone, NOS and oxidative stress levels of the treatment of uterine fibroids, worthy of clinical promotion.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the histopathological subjective difference of laryngeal epitheliual premalignant lesions and to analyze the prognosis of every grade of laryngeal epitheliual premalignant lesion.</p><p><b>METHODS</b>According to 2005 WHO classification system, 237 cases of laryngeal epitheliual premalignant lesions were reviewd by 3 pathologists, meanwhile, the histopathological consistency among 3 pathologists was estimated with κ-statistics method. The Kaplan-Meier survival analysis were performed to assess the prognosis of every grade of laryngeal epitheliual premalignant lesion with follow-up data.</p><p><b>RESULTS</b>There was totally a moderate consistency among 3 pathologists (κ values: 0.598 9), the histopathological difference was mainly between mild dysplasia and moderate dysplasia and between severe dysplasia and carcinoma in situ. The follow-up duration was between 18-120 months, and the median follow-up duration was 38 months. The carcinoma trsnformation rate was 8.44% (20/237) with an average malignant transformation duration of (19.45 ± 7.32) months, which was increased with the degree of dysplasia(mild dysplasia:1.67%, moderate dysplasia:3.57%, severe dysplasia:10.60%, carcinoma in situ:18.18%), no difference could be found between mild and moderate dysplasia, between moderate and severe dysplasia; Moreover, there was no difference between severe dysplasia and carcinoma in situ (P > 0.05).</p><p><b>CONCLUSIONS</b>The histopathological variability brought the uncertainty for classification of laryngeal epitheliual premalignant lesions, which might have an impact on clinical management of laryngeal epitheliual premalignant lesions; there was obvious difference in carcinoma transformation between low-risk and high-risk laryngeal epitheliual premalignant lesions, and more attention on treatment and prognosis assessment should be paid for severe dysplasia as carcinoma in situ.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore academic significance and guiding function played by subspecialty groups of the Society of Pediatrics, Chinese Medical Association on Chinese pediatric clinical practice through a statistical analysis of the articles published by the subspecialty groups.</p><p><b>METHOD</b>Bibliometric methods were used to analyze the number of articles, article types, total citations, highly cited articles and the distribution of citing journals.</p><p><b>RESULT</b>Totally 7 156 articles were published in Chinese Journal of Pediatrics from 1993 (31) to 2012 (51), of which 187 by subspecialty groups of pediatrics (2.6%), with a total citations of 11 985. Among them, 137 articles were cited with a citation rate of 73.3% and average citations for each article was 64.1. Articles classified as clinical guidelines had been totally cited for 10 900 times with average citations of 123.86 per article. The article on Neonatal hypoxic ischemic encephalopathy diagnosis and clinical index was cited 1 791 times ranked in highly cited literatures. All the top three cited literature periodicals were core journals of pediatrics, and 10 periodicals among the top 20 were in pediatrics and the rest in other medical fields.</p><p><b>CONCLUSION</b>The number of the articles published by the subspecialty groups of pediatrics was increasing year by year though the portion it in the total number of the articles in the journal was not large. However, the citation frequency of the articles by the subspecialty groups of pediatrics was high, making an obvious contribution to the total citations of Chinese Journal of Pediatrics. The total citation rate of clinical guideline articles and their average rate was higher than those of other articles published in this journal, which meant that this type of articles provided academic references with guiding significance for clinical practice of pediatrics and for other medical fields as well.</p>

ABSTRACT

Objective To investigate the effect of astragalus polysaccharide (AP) on proliferation and apoptosis of human acute myelocytic leukemia cell line KG-1a and the underlying mechanisms.Methods Human acute myelocytic leukemia cell line KG-la was cultured under 37 ℃ and 5 % C02,when appropriate cell passage and cryopreservation were performed.After treatment for 48 h with different concentrations of AP,the proliferative activity and apoptosis rate of KG-1a cells were examined by CCK-8 assay and flow cytometry,respectively.The expressions of p-Akt and bcl-2 protein in AP-treated KG-1a cells were evaluated by Western blot.The expression of PTEN mRNA by quantified real-time PCR.Results The proliferative activity of KG-la cell was obviously suppressed by AP treatment,and the inhibition rate increased in a dosedependent manner.The flow cytometry showed that,compared with the control group,the apoptosis rates of KG-1a cells were significantly increased after treatment with AP for 48 h.The early apoptosis rates were (1.98±0.16) %,(12.60±0.48) %,(16.31±0.73) %,the late apoptotic rates were (3.11±0.19) %,(17.17±1.40) %,(21.17 ± 0.81)%,the differences were statistically significant (P < 0.05).Western blot showed that the expressions of p-Akt and bcl-2 protein in KG-1a cells decreased significantly after treatment with AP (P < 0.05).In contrast,the mRNA level of PTEN increased (P <0.05),which was shown by real-time PCR.Conclusion AP could repress cellular proliferation activity of KG-1a cells,which could be attributed to inhibition of PTEN-PI3K-Akt signaling pathway.

ABSTRACT

Objective To investigate the clinical and cytogenetic features of myelodysplastic syndrome (MDS) ,and to investigate the application of the International Prognostic Scoring System Revised (IPSS-R) in myelodysplastic syndrome .Methods A retro-spective analysis was conducted on 112 patients diagnosed with MDS on basis of French-American-British(FAB) and World Health Organization(WHO) criterion .Conventional cytogenetics are performed to investigated the cytogenetics changes .Results Karyo-type abnormalities are found in 46(41 .1% ) patients .Cytogenetics subgroups are devided into 5 groups according to IPSS-R criteri-on :very good ,good ,intermediate ,poor ,very poor ,which account for 2 cases(1 .8% ) ,48 cases (42 .9% ) ,41 cases (36 .6% ) ,10 ca-ses (8 .9% ) ,11 cases (9 .8% ) ,with a median survival time(MS) 59 ,36 ,15 ,10 month respectively ,except for very good .Conclusion It is of great guiding significance of IPSS-R criteria for prognostic stratification .

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the genotoxicity of bone cement.</p><p><b>MATERIAL AND METHODS</b>Test article was mixed of liquid and the powder extract of bone cement. Using bacterial reverse mutation test (Ames test) and mouse lymphoma Assay (MLA) with and without metabolic activation S9. Ames test was performed by the plate incorporation method for its ability to induce reverse mutations in three treatment dosage groups: 25 microl/plate, 50 microl/plate, 100 microl/plate using S. typhimurium TA98, TA100, TA1535, TA1537 and E. coli WP2 uvrA; In MLA, the mutation frequency (MF) and the percentage of small colony mutants (SC%) of L5178 tk(+/-) cells induced by bone cement at final concentration of 0.125%, 0.25% and 0.5% (V/V) were calculated and assayed with +S9/3 h, - S9/3 h, -S9/24 h.</p><p><b>RESULTS</b>Slight precipitations of two dosage group 50 microl/plate and 100 microl/plate were observed after phosphoric acid buffer solution or S9mix added. Growth inhibition effect exists in TA100, TA1535 and WP2 uvrA to varying degrees. Other groups did not cause obvious increases in the mean number of revertant per plate compared with negative control (DMSO). The result of MLA indicated no significant MF or SC% increases (P>0.05) observed comparing with negative control under all test conditions.</p><p><b>CONCLUSIONS</b>The bone cement did not induce reverse mutation at his(-)/trp(-) and there was no obvious damage effect to gene tk(+/-) or chromosome under this study condition.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the potential developmental toxicity of Radix Ophiopogonis decoction in SD rats.</p><p><b>METHOD</b>Timed-pregnant SD rats were given Radix Ophiopogonis decoction (26.9 g x kg(-1)) or vehicle (distilled water) by gavage on gestation days 6-17. Maternal clinical sign, abortions, premature deliveries, and body weight were monitored throughout gestation. At termination (gestation days 20) pregnant females were evaluated for clinical status and gestational outcome; live fetuses were examined for gender, external, visceral and skeletal malformation and variations.</p><p><b>RESULT</b>No deaths, premature deliveries or dose-related clinical signs were attributed to Radix Ophiopogonis decoction. Maternal body weight and body weight gain were not affected. There were no effects on fetus weight and viability, incidences of fetal malformation and variation.</p><p><b>CONCLUSION</b>These results demonstrated that Radix Ophiopogonis decoction had no detectable adverse effects in either the treated F0 female rats or the fetuses.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the genotoxicity of Cortex Fraxini decoction.</p><p><b>METHOD</b>Mouse lymphoma assay (MLA) and mouse bone marrow micronucleus test (MNT) were used. In MLA, four dose levels of 1.71, 3.42, 6.83 and 13.65 g (crude drug) x L(-1) were exposed with I5178Y cells for 3 hours with and without metabolic activation, then expressed for 2 days. The mutation frequency plates were prepared and incubated for 12-13 days. Colony size in each plate was scored, and the total mutation frequency and the percentage of small colony mutants were calculated. In MNT, contained three dose levels of 7.14, 14.28 and 28.55 mg (crude drug) x kg(-1) and 10 ICR mice (5 males/5 females) were in each group. The mice were given in every 24 hours by oral gavage twice and sacrificed after 24 hours of the last dosing. Both femur bones were collected to prepare the smear. For each mouse, the number of micronucleated polychromatic erythrocytes (MNPCE) in 2 000 polychromatic erythrocytes was counted, and the mean of rate of MNPCE of each group was calculated.</p><p><b>RESULT</b>In MLA, the results indicated that the total mutation frequency of four dose levels showed a dose-dependent increase, there was statistically significant difference at high concentrations compared with negative control (P < 0.01), and the percentage of small colony mutants was similar with positive control in the absence of metabolic activation. The total mutation frequency of each dose level was similar with negative control in the presence of metabolic activation. In MNT, the results indicated that the decoction did not show inhibitory action for bone marrow, and the induced rate of MNPCE of each group was not significantly increased comparing with negative control.</p><p><b>CONCLUSION</b>Cortex Fraxini decoction induces the tk(+/-) gene mutation and chromosome damage in L5178Y cells in vitro without metabolic activation, it hints that the direct mutagens may be within the test article. Cortex Fraxini decoction does not show chromosome damage of bone marrow in ICR mice, it has not genotoxicity in vitro/in vivo with metabolic activation under this study condition.</p>

ABSTRACT

<p><b>BACKGROUND</b>To evaluate the response, adverse effects and survival of MVP regimen and TVP regimen.</p><p><b>METHODS</b>Sixty six patients with advanced non-small cell lung cancer were randomized into two groups:MVP arm (32 patients, mitomycin C 6-8 mg/m² d1, vindesine 2-3 mg/m² d1 and d8, cisplatin 70-80 mg/m² d1) and TVP arm (34 patients, pirarubicin 40-50 mg/m² d1, vindesine and cisplatin were the same as arm MVP). Characteristics of the patients were similar in two arms. All patients received two to four cycles of chemotherapy.</p><p><b>RESULTS</b>The overall responses were 34% (11/32) in the MVP arm and 56% (19/34) in the TVP arm. There were 1 complete response, 10 partial responses in the MVP arm and 1 complete response, 18 partial responses in the TVP arm. TVP regimen appeared to have a higher objective response, but no statistically significant difference in the response was observed between two regimens (Chi-square=2.269, P=0.132). Main side effects were hematological toxicities. Grade III+IV hematological toxicities were significantly higher in the patients of arm TVP than arm MVP, especially neutropenia (79% vs 44%, Chi-square=7.458, P=0.006). Median survival time was 12 months vs 8 months, and 1-, 2-, 3-year survival rates were 53% vs 24% (Chi-square=4.943, P=0.026), 17% vs 6%, 6% vs 0, for arm TVP and arm MVP, respectively..</p><p><b>CONCLUSIONS</b>MVP regimen has a lower response rate and longer survival time but less hematological toxicities than TVP regimen. The results suggest MVP regimen is a safe and active regimen for advanced non-small cell lung cancer.</p>
