ABSTRACT

Cancer cell membrane (CCM) derived nanotechnology functionalizes nanoparticles (NPs) to recognize homologous cells, exhibiting translational potential in accurate tumor therapy. However, these nanoplatforms are majorly generated from fixed cell lines and are typically evaluated in cell line-derived subcutaneous-xenografts (CDX), ignoring the tumor heterogeneity and differentiation from inter- and intra- individuals and microenvironments between heterotopic- and orthotopic-tumors, limiting the therapeutic efficiency of such nanoplatforms. Herein, various biomimetic nanoplatforms (CCM-modified gold@Carbon, i.e., Au@C-CCM) were fabricated by coating CCMs of head and neck squamous cell carcinoma (HNSCC) cell lines and patient-derived cells on the surface of Au@C NP. The generated Au@C-CCMs were evaluated on corresponding CDX, tongue orthotopic xenograft (TOX), immune-competent primary and distant tumor models, and patient-derived xenograft (PDX) models. The Au@C-CCM generates a photothermal conversion efficiency up to 44.2% for primary HNSCC therapy and induced immunotherapy to inhibit metastasis via photothermal therapy-induced immunogenic cell death. The homologous CCM endowed the nanoplatforms with optimal targeting properties for the highest therapeutic efficiency, far above those with mismatched CCMs, resulting in distinct tumor ablation and tumor growth inhibition in all four models. This work reinforces the feasibility of biomimetic NPs combining modular designed CMs and functional cores for customized treatment of HNSCC, can be further extended to other malignant tumors therapy.

ABSTRACT

Objective:To explore the detection and segmentation of ischemic core infarct volume of the acute stroke in diffusion weighted imaging (DWI) images using cascaded VB-Net.Methods:MRI data of 1 500 patients (2 456 lesions) with acute ischemic stroke in Henan Provincial People′s Hospital from December 2016 to December 2018 were retrospectively analyzed. Firstly, manual segmentation of ischemic core was performed on DWI images (b=1 000 s/mm 2), and then all data were divided into training set, validation set and independent test set by 8∶1∶1. Then, the cascaded VB-Net was constructed, and the core infarct was automatically detected and segmented in the test set. Interclass correlation coefficient (ICC) was used to evaluate the consistency of volume size measured by manual segmentation and cascaded VB-Net. The patients were divided into large ischemic core lesion group (ischemic core volume ≥10 ml) and small ischemic core lesion group (ischemic core volume<10 ml), and the Dice coefficient difference between the two groups was compared using Mann-Whitney U test. Results:In independent test set, cascaded model had the detection rate of 94.6% (243/257) with Dice coefficient of 0.76 (0.68, 0.84). The agreement of cacade VB-Net segmented [4.19(1.21,14.13)ml] and manual segmented ischemic core infarct volume [4.08(1.19,17.92)ml] was high (ICC=0.97, P<0.001). There was no significant difference in Dice coefficient between large and small lesion groups [0.76 (0.69, 0.85), 0.76 (0.67, 0.84), Z=-0.44, P=0.657]. Conclusions:The cascaded VB-Net model provided a tool to realize automatic detection, segmentation, and calculation of ischemic core infarct volume. It has good segmentation accuracy and high consistency with manual segmentation, which can provide an auxiliary decision-making tool for the selection of treatment plans.

ABSTRACT

In order to clean the endoscopic tube more effectively, a new double-headed endoscopic cleaning brush was used in this study. A total of 130 colonoscopies were selected from the Digestive Endoscopy Center of Ruijin Hospital, Shanghai Jiao Tong University School of Medicine from June 2019 to August 2020. The colonoscopy cleaning sequence was marked with odd and even number. Colonoscopies marked with the odd number were assigned to the conventional group ( n=65) which received back and forth cleaning with the single-headed endoscope cleaning brush. Colonoscopies marked with the even number were assigned to the experimental group ( n=65) which received one-way cleaning with a nylon brush at the head and a dense non-woven brush at the tail. The cleaning methods for endoscopes were in accordance with Flexible Endoscopic Cleaning and Disinfection Technical Specification WS507—2016. ATP bioluminescence tests and bacterial quantitative cultures were applied to evaluate the cleaning effects of the two methods. The results showed that ATP relative light unit (RLU) decreased in both the experimental group and the conventional group [530.63 RLU (26-3 559 RLU) VS 270.87 RLU (20-1 415 RLU)] before and after cleaning the endoscope tube, showing significant difference between the two groups ( Z=-2.894, P<0.05). After scrubbing, the positive rate of bacterial culture on the brush head was 86.2% (56/65) for the double-headed non-woven brush head, 46.2% (30/65) for the double-headed nylon brush head and 32.3% (21/65) for the single-headed nylon brush head with significant differences among the three groups ( χ 2=41.046, P<0.05). The cleaning effect of the new double-headed endoscopic cleaning brush is better than that of the conventional single-headed endoscopic cleaning brush for soft endoscope cleaning. Non-woven brush is better than nylon brush in eliminating bacteria.

ABSTRACT

Objective:To analyze the advantages of the impact of physical activity management under medical supervision on body components and healthy physical fitness.Methods:A total of 106 healthy adults who participated in physical activity management activities in the Health Management Center of Sichuan Academy of Medical Sciences & Sichuan Provincial People′s Hospital from April 1 st 2020 to May 31 th 2021 were enrolled. The subjects were equally divided into a medical supervision group and a self-supervision group with random number table. Both groups performed a 8-week physical activity intervention, followed by the model of “a combination of online home physical activity with offline team exercise, and team exercise interaction with individual physical activity guidance”, while health education was provided and wearable devices were worn to collect exercise data. The medical supervision group completed the set number of exercises under the supervision of the team, with on-site exercise guidance. They were required to participate in weekly one-to-one on-site instruction. The self-management group carried out exercises and participated in weekly one-to-one instruction in accordance according to their own will with online feedback. During the study, 6 cases withdrew, and 52 cases in the supervision group and 48 in the self-management group were obtained. The t and Wilcoxon tests were used to compare the body components and physical fitness of the two groups before and after intervention. Results:The amount of exercise in the supervision group was significantly higher than that in self-management group (1 359 vs 615), and there was no significant differences in exercise intensity (mean heart rate, maximum heart rate and exercise duration) between the two groups (all P>0.05). After the intervention, the body weight, body mass index (BMI), percent body fat, subcutaneous fat area and strength fitness of the supervision group were significantly lower than those before the intervention [(55.36±6.37) vs (56.11±6.33) kg, (21.48±2.85) vs (21.73±2.47) kg/m 2, (27.38±5.34) % vs (28.53±4.99) %, (119.56±48.45) vs (128.70±50.10) cm 2, (24.48±6.37) vs (26.07±5.29) kg], while the cardiorespiratory endurance, flexibility fitness and agility fitness were significantly higher [(33.57±5.06) vs (30.32±4.59) ml·kg -1·min -1, (14.71±7.51) vs (10.54±7.54) cm, (0.44±0.08) vs (0.48±0.05) s] (all P<0.05). After the intervention, the body weight, muscle mass and lean body mass in self-management group were all significantly higher than those before the intervention [(56.83±8.09) vs (56.45±8.22) kg, (38.28±3.19) vs (38.24±5.17) kg; (40.80±4.16) vs (40.32±4.95) kg], and agility fitness was significantly lower [(0.64±0.39) vs (0.49±0.05) s] (all P<0.05). It was also found that improvements regarding the following three indicators were more significant in the supervision group: visceral fat, balance and agility fitness (all P<0.05). Conclusions:Physical activity intervention under medical supervision can significantly improve the body components and physical fitness in healthy adults, as well as exercise compliance and validity.

ABSTRACT

Objective:To investigate the MRI and pathological features of intramuscular fibro-adipose vascular anomaly (FAVA).Methods:The clinical and imaging data of 44 patients with intramuscular FAVA confirmed by pathology from December 2012 to March 2021 in Henan Province People′s Hospital were retrospectively analyzed. Twenty-five females and 19 males were included, with the age of (15±6), from 5 to 29 years old. The clinical and MRI features including the type, location, boundary, signal intensity, enhancement mode and degree, and the vascular flow voids in the lesion were summarized and compared with pathological results.Results:The thigh and calf muscles were involved in 1 patient simultaneously, and 1 site was involved in 43 patients, including 20 calf muscles, 15 thigh muscles, 5 forearm muscles, 1 upper arm muscle, 1 gluteal muscle, and 1 shoulder muscle. The gastrocnemius muscle of lower leg was most commonly involved (13/44), followed by soleus muscle (10/44) and quadriceps femoris muscle (9/44). All the lesions were solid on MRI, including 24 cases of focal mass type, 15 cases of diffuse infiltration type and 5 cases of local infiltration type. The long axis of all the lesions were consistent with the long axis of the muscles. All lesions showed inhomogeneously moderate hyperintensity on T 1WI and T 2WI, and significantly hyperintensity on fat suppression T 2WI. All lesions showed tortuous and dilated abnormal vessels, of which 18 cases showed vascular flow voids. Thrombosis was found in 10 cases. On contrast-enhanced imaging, the lesions showed moderate to obvious inhomogeneous enhancement. Pathologically, the diseased skeletal muscle was infiltrated by fibrous tissue, fat components, irregular abnormal veins and vessels, which led to inhomogeneous MRI signals. Among the 7 patients who underwent human PIK3CA gene mutation detection, and 6 were mutant. Conclusions:Intramuscular FAVA has certain characteristics in clinic, MRI imaging and histopathology, and its MRI signal characteristics can reflect its complex pathological components.

ABSTRACT

Objective@#To obtain the dietary exposure of lead in primary, middle and high school students, and to provide basic foundation for food safety risk assessment and management for children and adolecsents.@*Methods@#Weighing method, 3-days hour dietary survey, combining with the food frequency questionnaire, was applied to obtain basic dietary data in 616 primary, middle and high school students selected through multi-stage randdom sampling method from Pudong new area of shanghai. Data on lead concentrations were derived from the food safety risk monitoring system, which included 1 145 samples.@*Results@#Lead was detected in 568 of 1 145 samples (49.61%) from 10 categories of foods. The exceeding standard rate was 3.58% (41 samples exceeded). The average content of lead was (0.12±0.25) mg/kg, fungi and algae, beans and the aquatic products were found to be the primary food sources of lead exposure. The average dietary lead exposure in primary, middle and high school students was 9.94 μg/kg per week, accounted for 39.76% of PTWI. The lead exposure level at the 97.5th percentile which accounted for 118.24% of PTWI was 29.56 μg/kg. Vegetables, beans and cereal foods were the mainly source of dietary lead exposure, with average dietary lead exposure 2.57, 2.44 and 1.43 μg/kg peer week, accounted for 10.26%, 9.76% and 5.74% of PTWI, respectively.@*Conclusion@#Lead is present in some foods available in local markets in Pudong New Area, Shanghai. Low level of dietary exposure to lead is found in primary, middle and high school students, however, it depends on dietary pattern. Continued efforts are needed to reduce the dietary exposure of lead in school students.

ABSTRACT

Objective:To explore the value of radiomics features extracted from multi-parameter MRI (mp-MRI) in preoperative prediction of triple negative breast cancer (TNBC) and non triple negative breast cancer (NTNBC).Methods:The clinical and preoperative-MRI data of 371 patients with breast cancer confirmed by surgical pathology from January 2017 to July 2019 in Henan Provincial People′s Hospital were retrospectively analyzed. Based on the results of immunohistochemistry (IHC) and fluorescence in situ hybridization (FISH) from postoperative pathological specimens, the cancer was classified as TNBC and NTNBC. Patients were randomly assigned to a training set ( n=250) and a validation set ( n=121). Quantitative radiomics features were extracted from three-dimensional lesions based on dynamic contrast enhanced-T 1WI (DCE-T 1WI) and fat-suppressed T 2WI sequences, and 32 quantitative radiomics features were selected by Mann-Whitney U test, elastic network, and support vector machine recursive feature elimination. Three radiomics signatures were constructed by using the algorithm of support vector machine based on the quantitative radiomics features extracted from fat-suppressed T 2WI, DCE-T 1WI and the mp-MRI of their combination. The prediction performances were evaluated by receiver operating characteristic (ROC) curve and the area under the ROC curve, accuracy, sensitivity, and specificity were calculated. Results:There were 61 patients with TNBC and 310 patients with NTNBC. The clinicopathological characteristics between NTNBC and TNBC were statistically different in the pathological grade (χ2=24.544, P<0.001). Other clinicopathological characteristics (age, maximum diameter of mass, vascular tumor thrombus, axillary lymph nodes) were not statistically differences between NTNBC and TNBC ( P>0.05). The radiomics signature presenting the best performance for predictive TNBC and NTNBC were based on mp-MRI radiomics features. The area under the ROC curve, accuracy, sensitivity, and specificity were 0.91[95% confidence interval (CI) 0.881-0.932], 86.0%, 84.4% and 86.3% in training set, and 0.84 (95%CI 0.807-0.868), 75.2%, 68.7% and 76.1%, in validation set, respectively. Conclusion:Radiomics based on mp-MRI features can be a effectively potential tool for predictive TNBC and NTNBC breast cancer and provide scientific basis for clinicians to make treatment decisions.

The consecutive chest CT scans were followed up for all patients with an average of 4 scans performed per patient during the hospitalization. The shortest interval between each scan was 2 days and the longest was 7 days. The shape, number and distribution of lung shadows, as well as the characteristics of the lesions on the CT images were reviewed.@*RESULTS@#The obvious shadows infiltrating the lungs were shown on CT images in 50 cases, for other 2 cases there was no abnormal changes in the lungs during the first CT examination. Ground-glass opacities (GGO) were found in 48 cases (92.3%), and 19 cases (36.5%) had patchy consolidation and sub-consolidation, which were accompanied with air bronchi sign in 17 cases (32.7%). Forty one cases (78.8%) showed a thickened leaflet interval, 4 cases (7.6%) had a small number of fibrous stripes. During hospitalization, GGO lesions in COVID-19 patients gradually became rare,the fibrous strip shadows increased and it became the most common imaging manifestation. The lesions rapidly progressed in 39 cases (75.0%) within 6-9 days after admission. On days 10-14 of admission, the lesions distinctly resolved in 40 cases (76.9%).@*CONCLUSIONS@#The chest CT images of patients with COVID-19 have certain characteristics with dynamic changes, which are of value for monitoring disease progress and clinical treatment.

ABSTRACT

The aim of this study was to provide a summary estimate of depression prevalence among people with ankylosing spondylitis (AS) in comparison to those without AS. A systematic literature search was conducted using PubMed, Embase, PsycINFO, Web of Science, the Cochrane database library, China National Knowledge Infrastructure, and Wanfang Database from their inception to December 2016. The results showed that thirty-one eligible studies involving 8,106 patients were analyzed. Fifteen methods of defining depression were reported. The overall pooled prevalence of depression was 35% (95% CI, 28–43%), with high between-study heterogeneity (I²=98.8%, p<0.001). The relative risk of depression among people with AS was 1.76 (95% CI: 1.21–2.55, eight studies, n=3,006) compared with people without AS. The depression score [standardized mean difference (SMD)=0.43, 95% CI: 0.19–0.67, seven studies, n=549] was higher in AS patients than in controls. The main influence on depression prevalence was the sample size and country of origin. In conclusion, one-third of people with AS experience symptoms of depression. Depression was more prevalent in AS patients than in controls. Further research is needed to identify effective strategies for preventing and treating depression among AS patients.

ABSTRACT

Objective To explore the classification of gliomas according to the theory and method of radiomics. Methods In this study, 161 pathologically confirmed glioma patients were retrospectively selected from 2012 to 2016 including 52 low-grade gliomas and 109 high-grade gliomas.Three hundred and forty-six quantization features were extracted from the MRI images, including shape, density, texture and wavelet imaging features. Mutual information and logistic regression model were used to select feature reduction and prediction model. The predictive ability of the model was validated using 10-fold cross-validation. Results Nineteen radiomics features were chosen from 346 quantization features. The sensitivity of the model was 96.3% (105/109), the specificity was 78.8% (41/52), the area under the curve (AUC) was 0.952 7, and the accuracy was 90.7%(146/161). Conclusion The solution proposed in this paper showed that radiomics can non-invasively and quickly provide an adjunct to the clinical grade of glioma with high accuracy.

Objective To construct a questionnaire for patients to participate in hand hygiene promotion in medical institutions,and provide a measuring tool for patients to participate in hand hygiene promotion.Methods A ques-tionnaire was constructed by methods of literature research,expert panel discussion,pre-investigation,and Delphi. Results This questionnaire was designed respectively for patients and health care workers,each contained 20 ques-tions,involving hand hygiene perception,willingness to participate in promoting hand hygiene,and participation method,et al.Conclusion A questionnaire for patients to participate hand hygiene promotion in medical institutions is preliminary constructed,which can be used for evaluating patients’and HCWs’willingness and relevant influen-cing factors of patients’participation in hand hygiene promotion in medical institutions.

ABSTRACT

Objective To investigate the effects ofLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets on diabetic complications in patients with type 2 diabetes mellitus (T2DM). Methods Seventy-eight patients with T2DM were randomized into a TCM treatment group and a conventional treatment group by random number table method, 39 in each group. The patients in the conventional treatment group were regularly using antihyperglycemic drugs or insulin. On the basis of conventional treatment group, the patients in the TCM treatment group were additionally treated withLiuwei-Dihuang Pills and Ginkgo Biloba Leaves Extract Tablets. Intima-media thickness (IMT) of internal carotid artery, vibration perception threshold (VPT) and diabetic retinopathy in both groups were detected before the treatment, 12 and 36 months after the treatment.Results The VPT in the left foot 12 months after the treatment in the TCM treatment group were significantly lower than that in the conventional treatment group (13.98 ±4.38 Vvs. 18.70 ±2.43 V;t=2.764, P=0.008). IMT of internal carotid artery (left: 0.81 ± 0.16 mmvs.0.70 ± 0.10 mm,t=3.120,P=0.003; right: 0.81 ± 0.17 mmvs. 0.73 ± 0.12 mm,t=2.286,P=0.026), VPT (left foot: 14.82 ± 6.45 Vvs. 20.63 ± 9.75 V,t=2.714, P=0.009; right foot: 16.73 ± 7.10 Vvs. 20.73 ± 10.35 V,t=2.001,P=0.048) and incidence of diabetic retinopathy (35.9%vs. 53.8%;χ2=5.804,P=0.016) 36 months after the treatment in the TCM treatment group were significantly lower than those in the conventional treatment group.ConclusionsLiuwei-Dihuang Pills combined with Ginkgo Biloba Leaves Extract Tablets can effectively reduce the progression of diabetic complications in patients with T2DM.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the characteristics of brachial-ankle pulse wave velocity (baPWV)in populations having health check-up and to explore its relationship with risk factors of cardiovascular diseases.</p><p><b>METHODS</b>13 841 people who had health check-up were included and their baPWV were tested by automatic arteriosclerosis measurement system. All people were inquired about their diseases history, while height, weight, body mass index(BMI), blood pressure, fasting blood-glucose as well as serum lipid were also measured.</p><p><b>RESULTS</b>With increasing of age, baPWV were increased both in males and females. The baPWV of male aged less than 60 years was significantly higher than that of female less than 60. Multi-variable regressive analysis showed the age, systolic pressure,BMI, fasting blood-glucose, uric acid and triglyceride were the risk factors of baPWV.</p><p><b>CONCLUSION</b>BaPWV values of health check-up population in Chengdu area demonstrated different characteristics in different age and gender group. Attention should be paid on baPWV measurement among elder people, especially the elder women, in order to prevent cardiovascular events happening in high risk population.</p>

@#Stroke is the disease which caused by many risk factors.The relationship between insulin resistance(IR) and young adult stroke is one of the focuses now in the neurological field.IR refered to the series of pathological and physiological changes,it happens when the response of the target tissue organ to insulin is depressing or losing.There are many ways of detection of IR,the insulin sensitivity index(ISI) is one of the most satisfied indexes to reflect the sensitivity to insulin.IR could cause a series of pathological and physiological changes,which caused hyperinsulinemia,hyperglycemia,hyperpiesia,blood-fat derangement,fibrinolytic system anomaly,blood vessel endothelium change,artherosclerosis,et al,maybe in ways of many metabolic risk factors.Atherosclerosis is the main etiopathogenisis factor in young adult stroke,as a conclusion,IR is also existed in young adult stroke.But,does IR exist in all types of young adult stroke? What is the mechanism? Is the level of IR related with the pathogenetic condition and prognosis? These questions are still not clear at present,and the study on the relationship between IR and young adult stroke is important to prevent young adult stroke.

ABSTRACT

Objective To establish a sandwich enzyme linked immunosorbent assay (ELISA) method for determination of von Willebrand factor propeptide (vWF-pp) in plasma and to determine the levels of plasma vWF-pp in 152 healthy volunteers, 36 patients with coronary heart disease and 38 cases of the normal control group with the same ages.Methods The levels of vWF-pp in 152 healthy volunteers with different ages and different genders and in 36 patients with coronary heart disease were determined with the sandwich enzyme linked immuno-sorbent assay (ELISA).Results The average level of vWF-pp in 152 healthy volunteers was 496.4?167.2 ?g/L.The level of vWF-pp in the 50~60 years old group was significantly higher than that in the less than 40 years old group (P0.05). There was significant difference between the 51~60 years old male group and the other three male groups respectively, the 16~20 years old group, the 21~30 years old group and the 31~40 years old group (P

ABSTRACT

AIM:To investigate effects of propolis-ethanol extract on platelet activity in flowing blood.METHODS: Intima-injury model was made by in vitro perfusion.Coverslips was coated by using fibrinogen and human Ⅲ type collagen to detect platelet adhesive rate when blood went through fibrinogen and collagen surface in 1 000/s shear rate.Three experimental groups were set up: negative control group(24% ethanol),experimental group(0.1 g/L,24% propolis-ethanol extract) and positive control group(0.1 g/L,ferulic acid).RESULTS: Intima-injury model was made.On fibrinogen and collagen,platelet adhesion on area of experimental group was lower than that in negative control group(P0.05).Propolis-ethanol extract decreased more obviously the platelet adhesion area on fibrinogen surface than that in collagen surface(P
