ABSTRACT

Objective:To explore the reliability of Xpert MTB/RIF (Xpert) in the diagnosis of pulmonary tuberculosis and to understand the characteristics of retreated patients, so as to provide clues for the investigation of retreated patients.Methods:905 patients sputum with suspected tuberculosis from July 2019 to December 2019 were collected from the Tuberculosis Department of Changsha Central Hospital. Xpert was used for rapid screening of tuberculosis. MGIT960 and Roche culture method were used for rapid sputum culture and drug sensitivity, respectively. Rifampin (RIF) resistance was compared between Xpert and Roche culture. The gender, age, RIF resistance, occupation and diabetes mellitus of patients with positive Xpert were compared, and the risk factors of RIF resistance were analyzed by binary logistic regression.Results:The total detection rate of Xpert was 28.7%(260/905) in 905 suspected pulmonary tuberculosis patients; RIF resistance rate accounted for 20.8%(54/260) in 260 positive patients, and the main mutation types were probe-E and probe-D. There was no significant difference between traditional drug sensitivity and Xpert method in RIF resistance detection ( P>0.05); There was no significant difference between Xpert method and traditional drug sensitive cases in predicting isoniazid (INH) resistance ( P=0.293); there were significant differences in age ( t=-3.835, P<0.05), gender, RIF resistance rate and proportion of farmers between newly treated and retreated patients (χ 2=5.091, 16.862, 4.808, P<0.05); Binary logistic regression showed that retreatment was a risk factor for RIF resistance ( OR=3.739, P=0.004). Conclusions:Xpert has high clinical value in the diagnosis of pulmonary tuberculosis and RIF resistance; The retreated patients were older, and the proportion of male farmers was larger, with higher resistance rate to RIF.

ABSTRACT

To evaluate the efficacy and safety of Songling Xuemaikang Capsules combined with conventional Western medicine in the treatment of essential hypertension. PubMed, VIP, CNKI, Wanfang and other databases were retrieved from the establishment of the database to February 2020 for clinical randomized controlled trial(RCT) about Songling Xuemaikang Capsules combined with conventional Western medicine in the treatment of essential hypertension. The literatures were screened out according to the inclusion criteria, and RevMan 5.3 software was used for Meta-analysis. A total of 3 100 patients in 27 RCTs were enrolled. According to Meta-analysis, Songling Xuemaikang Capsules combined with conventional Western medicine could effectively reduce systolic blood pressure(MD=-7.88,95%CI[-9.68,-6.08],P<0.000 01) and diastolic blood pressure(MD=-7.85, 95%CI[-9.07,-6.62], P<0.000 01), triglyceride(MD=-0.46, 95%CI[-0.66,-0.26], P<0.000 01) and total cholesterol(MD=-0.92, 95%CI[-1.49,-0.35], P=0.001), but increase HDL cholesterol(MD=0.51, 95%CI[0.28, 0.73], P<0.000 01), with a better effect than the Western medicine group alone. The results of LDL-C analysis showed that there was no significant difference between the two groups(MD=-0.91, 95%CI[-1.82, 0.01], P=0.05). The subgroup analysis suggested that reduced systolic blood pressure may be related to the use of ARB. There was a close correlation between CCB drugs and the decrease of diastolic blood pressure. In addition, there was no significant difference in the compliance and the incidence of adverse reactions. Clinical application of Songling Xuemaikang Capsules combined with Western medicine in the treatment of patients with essential hypertension has clear efficacy and certain safety. More clinical randomized controlled trials are needed for verification in the future.

December 2019 witnessed the outbreak of COVID-19 in Wuhan city. Tongji Hospital, as a designated hospital for treatment of critical patients, shoulders the diagnosis and treatment tasks of numerous critical patients of COVID-19.For the purpose of handling their medical records and effectively preventing the nosocomial infection of the disease, the hospital put in place both the electronic signature system of patients based on a Wechat applet and a paperless medical record total solution based on the data center. These measures overcome the challenges incurred by patients′ signature on paper-based records and medical records archiving during the epidemic. On the other hand, they can not only downsize the paper-based medical records, minimize the risk of infection caused by paper-based medical records via physical contacts, but also effectively save the hospital of its operating costs and improve its efficiency of clinical work.

ABSTRACT

Objective@#To compare the clinical efficacy and drug safety between oral apatinib combined with conventional chemotherapy and conventional chemotherapy alone for the treatment of osteosarcoma and soft tissue sarcoma patients with pulmonary metastasis.@*Methods@#Thirty-three osteosarcoma and soft tissue sarcoma patients with pulmonary metastasis who were treated in the Department of Bone and Soft Tissue Tumor Surgery, Cancer Hospital of China Medical University from January 2015 to December 2017 were enrolled in this study. Patients with osteosarcoma received methotrexate, adriamycin (ADM), cisplatin (CDDP), ifosfamide (IFO) sequential regimen; patients with soft tissue sarcoma were treated with IFO and ADM regimen. Eighteen of these patients received an additional oral dose of apatinib. The patients were followed up regularly for changes in primary tumors and metastases, adverse reactions and prognosis.@*Results@#Before treatment, the maximum diameter of pulmonary metastases in patients of apatinib group and routine treatment group were (4.46±1.70) cm and (4.53±2.00) cm, respectively, without significant difference (P=0.909). After treatment, the maximum diameter of pulmonary metastases in patients of apatinib group was (1.46±1.39) cm, significantly smaller than (3.02±1.20) cm of routine treatment group (P=0.002). After treatment, the maximum diameter of the primary lesions in the apatinib group and the conventional treatment group median decreased 0.31 cm and 0.12 cm, respectively, without significant difference (P=0.542). After treatment, the maximum diameter of the lung metastases in the apatinib group median decreased 0.59 cm, significantly more than 0.18 cm of the conventional treatment group (P=0.027). The median progression-free survival (PFS) was 9.4 months in the 33 patients. The median PFS was 9.6 months and 8.3 months in the apatinib group and the conventional treatment group, respectively, without significant difference (P=0.593). Specific adverse reactions both occurred in apatinib group and routine treatment group, mainly including oral mucosal reactions and digestive tract reactions (including nausea, vomiting and diarrhea).@*Conclusions@#Apatinib can effectively reduce the volume of primary and metastatic lesions in patients with bone and soft tissue sarcoma accompanied by lung metastasis without reducing the survival rate or causing uncontrollable adverse reactions. The safety and clinical efficacy of apatinib are significant.

ABSTRACT

Osteosarcoma is a malignant tumor of mesenchymal origin and occurs mostly in young people, which is characterized with high malignancy, poor prognosis, easy recurrence and metastasis, as well as a great difficulty in treatment. Osteosarcoma is rich in blood supply, and its growth, invasion and metastasis highly depend on the tumor new angiogenesis. The process of angiogenesis is mainly initiated by various pro-angiogenic factors secreted by tumor cells, and the anti-angiogenic targeted therapy has been taken based on the targets spot and signal pathways of various angiogenic factors, which can effectively suppress the various biological behaviors of the osteosarcoma and improve the survival rate. However, a series of problems, such as drug resistance, side effects, different evaluation criteria, and difficulties in selecting effective treatment strategies also exist in the process of anti-angiogenesis therapy, and thus, further studies are needed. This paper mainly reviews the clinical trials of anti-angiogenic drugs, main barriers in usage, as well as the feasible treatment strategies in clinical practice.

ABSTRACT

Objective At present, modern medicine cannot yet clarifythe mechanism of radiation-induceddamage (RID) to salivary glands and its treatment and protective measures remain in the exploration stage .This study was to explore the mechanism of RIDto the submandibular gland and observe the effect of Dark Plum Spray ( DPS) on the submandibular gland after RID in order to provide some evidence for its further application . Methods Using the random number table , 84 healthy Wistar male rats were divid-ed into a normal, an RID model, and an experimental group ,all fed normally.The rats of the RID model groupwere left untreated , while those of the experimental group were intervened with DPS , tid, after RID.On the 1st, 7th, 14th, and 28th day after irradiation, 7 rats were taken from each group for measurement of the body weight, collection of the saliva , and calculation of the salivary flow rate.The submandibular glands were harvested for determination of the mRNA and protein expressions of autophagy-related Atg5 by RT-PCR and Western blot . Results In the 7th day after irradiation , the average body weight in NG[(239.87±16.50)g] was significantly higher (P<0.05) than UG[(213.84±14.42)g] and EG[(222.71±11.14)g].In the 14th ,28th day after irradiation, the average body weight in UG and EG were significantly lower ( P<0.05) than NG;the average body weight in EG was significantly higher ( P<0.05) than UG.In the 1st , 14th day after irradiation , the salivary flow rate in UG and EG were lower than NG but there were no significantly difference(P>0.05).In the 7th day after irradiation, the salivary flow rate in NG[(49.29±16.90)μL/min] and EG[(50.99±6.79)μL/min] were significantly higher (P<0.05) than UG[(30.13±13.19)μL/min].In the 28th day after irradiation, the salivary flow rate in NG[(69.29±11.32)μL/min] were significantly higher (P<0.05) than UG[(49.26±14.13)μL/min] and EG[(46.56±13.60)μL/min] .In the 1st , 7th and 14th day after irradiation , RT-PCR showed that the expressions of Atg 5 in UG and EG were significantly higher(P<0.05) than NG.In the 1st , 7th and 14th day after irradiation, Western blot showed that the expressions of Atg 5 in UG and EG is on the upper trend than NG;In the 1st , 7th day after irradiation , there were no significantly difference between UG and EG .In the 28th day after irradiation , Western blot showed that the expressions of Atg 5 in UG is on the declining trend than NG . Conclusion The autophagic activity of submandibular gland cells may associated with early radiation -induced injury , and Dark Plum Spray may en-hance the action of theanti-apoptosis cytokine in repairingradiation-induceddamageto the submandibular gland .

BACKGROUND: There is little information available in the mechanism of radiation-induced salivary gland injury, and its treatment and prevention are still at the exploratory stage.OBJECTIVE: To establish a rat model of radiation-induced xerostomia with 18 Gy electron beam and to observe the pathological changes of the submandibular gland and changes in saliva ingredients.METHODS: Totally 115 Wistar rats were randomly divided into exposure and control groups: the rats in the exposure group were subjected to anesthesia, and the submandibular gland received 18 Gy electron beam radiation to establish the model of radiation-induced xerostomia. The rats in the control group were only anesthetized but not exposed to radiation. The water intake was recorded at 21 dys after modeling. The saliva was collected and the submandibular gland was removed at 1, 3, 7, 14, 21, 28, 35 and 42 days to detect the saliva volume and submandibular gland index,and the morphological changes of the submandibular gland were observed by hematoxylin-eosin staining.RESULTS AND CONCLUSION: At 1-21 days after modeling, the average daily water intake was (6.42±1.91) mL in the exposure group and (4.82±1.20) mL in the control group, respectively (P < 0.05). During 42 days after modeling, the saliva secretion volume in the exposure group was lower than that in the control group, which was the lowest on day 7,and the difference was significant at 7, 21, 28 and 42 days after modeling between two groups (P < 0.05). The submandibular gland index in the exposure group was significantly lower than that in the control group at 1 and 21-42 days after modeling (P < 0.05). Hematoxylin-eosin staining results showed that in the exposure group, the rat submandibular gland appeared with inflammatory infiltration, glandular atrophy and karyopyknosis that were aggravated with time until day 42. To conclude, the rat model of radiation-induced xerostomia is established successfully with 18 Gy beam, characterized as increased water intake, decreased saliva volume and progressive aggravation of pathological injury of the submandibular gland.

Objective The mechanism of inhibition of the proliferation and migration of osteosarcoma cells by miR-646 was investigated.Methods The effects of miR-646 on the proliferation of U2OS cells were detected by MTT assay.The effects of miR-646 on U2OS cell migration were analyzed by Tmnswell assay.MIRDB was used to predict the binding sites of miR-646 and epidermal growth factor receptor (EGFR),which were confirmed in a luciferase reporter assay.Western blotting was conducted to detect the effect of miR-646 on the EGFR pathway.Results The MTT test showed that transfection of miR-646 significantly inhibited U2OS cell proliferation;the 24 h inhibition rate was 21.76%± 1.05%.In contrast,the 24 h promotion was 18.89%± 0.81%.Transwell experiments showed that miR-646 inhibited the migration of osteosarcoma U2OS cells.MIRDB predicted that miR-646 bound in the 3'-untranslated region of EGFR,and luciferase reporter gene experiments showed that miR-646 directly acted on EGFR.Western blotting experiments demonstrated that miR-646 inhibited the proliferation and migration of U2OS cells to some extent by inhibiting the EGFR pathway.Conclusion miR-646 can inhibit the proliferation and migration of osteosarcoma cells by inhibiting the EGFR pathway.

ABSTRACT

To analyze the medication features and the regularity of prescriptions of traditional Chinese medicine in treating patients with Qi-deficiency and blood-stasis syndrome of chronic heart failure based on modern literature. In this article, CNKI Chinese academic journal database, Wanfang Chinese academic journal database and VIP Chinese periodical database were all searched from January 2000 to December 2015 for the relevant literature on traditional Chinese medicine treatment for Qi-deficiency and blood-stasis syndrome of chronic heart failure. Then a normalized database was established for further data mining and analysis. Subsequently, the medication features and the regularity of prescriptions were mined by using traditional Chinese medicine inheritance support system(V2.5), association rules, improved mutual information algorithm, complex system entropy clustering and other mining methods. Finally, a total of 171 articles were included, involving 171 prescriptions, 140 kinds of herbs, with a total frequency of 1 772 for the herbs. As a result, 19 core prescriptions and 7 new prescriptions were mined. The most frequently used herbs included Huangqi(Astragali Radix), Danshen(Salviae Miltiorrhizae Radix et Rhizoma), Fuling(Poria), Renshen(Ginseng Radix et Rhizoma), Tinglizi(Semen Lepidii), Baizhu(Atractylodis Macrocephalae Rhizoma), and Guizhi(Cinnamomum Ramulus). The core prescriptions were composed of Huangqi(Astragali Radix), Danshen(Salviae Miltiorrhizae Radix et Rhizoma) and Fuling(Poria), etc. The high frequent herbs and core prescriptions not only highlight the medication features of Qi-invigorating and blood-circulating therapy, but also reflect the regularity of prescriptions of blood-circulating, Yang-warming, and urination-promoting therapy based on syndrome differentiation. Moreover, the mining of the new prescriptions provide new reference and inspiration for clinical treatment of various accompanying symptoms of chronic heart failure. In conclusion, this article provides new reference for traditional Chinese medicine in the treatment of chronic heart failure.

Objective To investigate the changes of serum protein fingerprint in patients with lung cancer with deep venous thrombosis.Methods Eighteen case patients with lung cancer were selected,including 8 case of lung cancers with thrombosis and 10 cases of lung cancers with no thrombosis.Surface enhanced laser desorption ionization protein-time of flight mass spectrometry (SELDI-TOF-MS) was used to analyze serum protein content of two groups in the same mass to charge ratios(M/Z),then drew the protein peaks that content difference was statistically significant.Results The M/Z of lung cancer with thrombus group and control group were 5911,1216,4187,1019,4293,the protein peaks had significant differences between the two group (43.81±7.74,6.37±5.02,2.97±0.35,35.96± 12.10,9.65±4.37;15.35± 12.69,2.06±0.37,4.67± 1.35,15.94±6.47,14.65±8.80;t =5.334,4.800,2.981,4.639,4.596;P< 0.05).Compared with the control group,the decrease of protein peak M/Z were 5911,1019,1216,and the increase of protein peak M/Z were 4293,4187 in the deep venous thrombosis group.Conclusion In the serum of patients with tumors SELDI profiles M/Z are 5911,4293,4187,1019,1216 of SELDI protein fingerprinting can be considered in patients with thrombosis of tumor specific markers.

ABSTRACT

BACKGROUND: Spontaneous recovery of severe alopecia areata is rare and the condition is difficult to treat. OBJECTIVE: The aim of this study is to investigate and compare the effects and safety of steroid pulse therapy between oral and intravenous administrations between 1999 and 2010 at the Department of Dermatology, National Cheng Kung University Hospital. METHODS: Data were retrospectively retrieved. A satisfactory response was defined as more than 75% hair regrowth in the balding area. RESULTS: A total of 85 patients with more than 50% hair loss were identified and treated, with an overall satisfactory response rate of 51.8%. The mean follow-up time was 37.6 months, with a relapse rate of 22.7%. Patients with alopecia areata (hereafter, AA) of recent onset within one year showed higher response rates (p<0.001) and lower relapse rates compared to patients with AA persisting for more than 1 year. Further, even in patients with alopecia totalis, alopecia universalis or ophiasis type, early treatment resulted in a satisfactory response rate of 47% among the treated patients. In general, oral therapy was as effective and well-tolerated as intravenous therapy. CONCLUSION: The response rate is determined by disease severity and time of intervention, not by the administration form of steroid pulse therapy. Oral steroid pulse therapy can be considered as the first-line treatment for patients with severe AA of recent onset within one year.

Objective To demonstrate the result that progression of disease and prognosis of gastrointestinal cancer patients are correlated with the expression of the cluster and the abundance of lost goodwill target (LGT) fingerprint. Methods To research the period of survival and the time of death, 61 patients treated by SELDI (Surface enhanced laser desorption/ionization-time of flight-mass spectrometer)were involved in. According to the fingerprint spectrum obtained by SELDI and the appearance of fingerprints of LGT, these patients were divided into four groups namely non-peak, single-peak, double-peak, tri-peak and multi-peak groups. By means of Multi-factor analysis of variance and Cox regression analysis of risk, we compare four groups each other so as to find out one group in which patients have the greatest risk of death.Results Among the fingerprint spectrums of patients suffered from gastrointestinal cancer, which were obtained by SELDI, the patients with negative expression of LGT had low risk of death, which were lower than those of single-peak and double-peak groups, especially lower than those of tri-peak and multi-peak groups;the risk of death among the tri-peak and multi-peak groups were much higher than single-peak, the difference had statistical significance (P <0.05), and the death rate is rather high. Conclusion Among the serums from cancer patients tested by SELDI, the expression of LGT with tri-peak and multi-peak suggested that patient' s state of illness is about to deteriorate. This kind of fingerprint spectrum can be regarded as positive indicators,and the negative expression of LGT suggested that patient will have a good prognosis with a very low risk of death in the near future.

Objective To analyze the serum related proteomic fingerprints when Lost Goodwill Target (LGT) proteomic fingerprints drifting from negative to positive in the advanced lung adenocarcinoma patients. Methods The serum proteomic fingerprints of 31 advanced lung adenocarcinoma patients whose LGT fingerprints drifted from negative to positive were detected by SELDI and CM10 protein chip. More than 10 % cluster and M/Z values from 11,000+H to 12,000+H was regarded as LGT positive, otherwise as negative. Different fingerprints were screened by Biomarker Wizard 3.1 and Biomarker Wizard software and the decision tree model was established. Results There were 16 statistically different protein peaks when LGT fingerprints drifting from negative to positive, including 10 up-regulation proteomic fingerprints (M/Z:11531, 11483, 11686, 11394, 11822, 11323, 11911, 12450, 5811 and 5709) and 6 down regulation proteomic fingerprints( M/Z: 4126, 13927, 13784, 7001, 1959 and 2741). Conclusion By SELDI and CM10 protein chip detection, up-regulating fingerprints of M/Z 11531, 11483, 11686, 11394, 11822 and 11323 were regarded as the subtype of LGT when it drifting from negative to positive, while up-regulation of M/Z 11911,12450, 5811 and 5709 and down-regulating of M/Z 4126, 13927, 13784, 7001 and 1959 were regarded as the related fingerprints when LGT drifting from negative to positive. The above different fingerprints are constituted of the fingerprints library when LGT fingerprints drifting from negative to positive and it will provide a platform for studying the LGT proteins.

ABSTRACT

Objective To analyze the disposition characteristic of LGT fingerprints and the related fingerprints for advanced gastric carcinoma patients when the LGT fingerprints changing. Methods SELDI and CM10 protein chip was used to detect the serum protein fingerprints of 81 cased of advanced gastric cancer patients. After 1 year follow-up, all the patients were detected by SELDI again. According to the expression condition of LGT fingerprints, the patients were divided into 4 groups: A group(LGT changing from negative to positive) 25 cases, B group (LGT changing from positive to negative) 15 cases, C group (LGT fingerprints keeping negative) 29 cases and D group (LGT fingerprints keeping positive) 12 cases. The influencing factor should be rejected. And the remaining fingerprints were LGT fingerprints' subtypes and therelated fingerprints. The different proteomic fingerprints were analyzed by Biomarker Wizard 3.1 Software.Results After rejecting the influencing fingerprints, the up-regulation fingerprints in A group were 11473, 11821, 11664, 11409, 11552 and 11947 (M/Z), and no down-regulation fingerprints. In B group, 3264 was upregulation, and 6523, 11509, 11669, 11413, 11351 and 6483 were down-regulation. In C and D group, there was not statistically differential protein fingerprint. Conclusion M/Z up-regulating fingerprints including 11473, 11821, 11664, 11409, 11552 and 11947 can be regarded as the subtype of LGT when it changing from negative to positive; down-regulating including 11509, 11669, 11413 and 11351 can be regarded as the subtype of LGT when it changing from positive to negative; 3264 up-regulating and 6523 and 6483 downregulating can be regarded as the related fingerprints when LGT changing from positive to negative; and when LGT keeping negative or positive in patients with advanced gastric cancer, there was no differential protein fingerprints.

ABSTRACT

Objective Exploration of TSH, FT3, FT4 changes in critical condition prior to death.Methods 84 patients that were anti-tumor therapy acceptable and not associated with the treatment of thyroid disease in patients with terminal tumors. Age from 24 to 85-year-old, male 56, female 28. The use of SELDI technology (surface-enhanced laser description / ionization time-of-flight mass spectrometry protein chip technology) checks to M/Z for the 11 100 + H~11 900 + H peak of the cluster, divided into the positive group for critically ill patients with early warning protein (LGT) and the negative group for it. All patients were given TSH, FT3, FT4 inspection. The results were statistically analyzed. Results TSH, FT3, FT4 in the LGT positive group was significantly lower than in the negative LGT group. There was no correlation between TSH,FT3 and FT4 in the LGT positive group;In the negative LGT group FT3 and FT4 had a positive correlation,and FT3 and TSH no relevance. Conclusion Thyroid function is normal in the stage of cancer patients stable condition. Possible pituitary-hypothyroidism may occur as the disease advances to the end or before death.This may be an important factor causing the acceleration to the death of the patients. However, the cause and effect between this phenomenon and the early warning of protein fingerprint group(LGT) is still not clear. In critical condition checking thyroid function and render supplementary amount of thyroid hormone in accordance with the checking results may be beneficial.

ABSTRACT

Objective By surface-enhanced laser desorption / ionization time-of-flight mass spectrometry(SELDI-TOF-MS), the serum pmteomic fingerprints related with the changing of malignant tumor patients' serum glucose after chemotherapy was selected and constructed as an predictive model. Methods By SELDI-TOF-MS, the serum of 182 malignant tumor patients who had received chemotherapy were tested, and the pmteomic fingerprints were received. After 2 years follow-up, all the patients were divided into 3 groups: the euglycemia group(136 people), the carbohydrate tolerance abnormality group(27 people), and the diabetes mellitus group (19 people). The proteomic fingerprints were analyzed by Biomarker Wizard Software and the idio-proteomic fingerprint of protective models were constructed by BPS (biomarker pattern software). Results The diagnosis model composed with 2 proteins (M/Z values were 5298 and 9608) could classify the carbohydrate tolerance abnormality group, and the diabetes mellitus group correctly. In the test model, the sensitivity and specificity were 81.48 %(22/27) and 100.00 %(17/17) respectively, the accuracy was 88.64 % (39/44). The diagnosis model composed with 3 proteins (M/Z values were 10324, 2761 and 4084) could classify the diabetes mellitus group and the euglycemia group correctly. In the test model, the sensitivity and specificity were 62.35 %(53/85) and 88.24 %(15/17) respectively, the accuracy was 66.67 %(68/102). The diagnosis model composed with 6 proteins (M/Z values were 5895,6010,6099,3930,5430 and 2495) could classify the diabetes mellitus group and the the carbohydrate tolerance abnormality group correctly. In the test model, the sensitivity and specificity were 77.65 %(66/85) and 96.30 %(26/27) respectively, the accuracy was 82.14 %(92/112). Conclusion SELDI-TOF-MS could be utilized to analyze protein profiling in screening serum glucose changing-related biomarkers and developing diagnostic and predictive patterns, and the developed patterns may be used to predict the changing of serum glucose after chemotherapy in malignant tumor patients.

ABSTRACT

Objective To investigate the expression of CEA, NSE and CYFRA21-1 in lung cancer, and the significance of combined determination of three tumor markers in the detection of lung cancer. Methods CEA, NSE and CYFRA21-1 levels in serum of 65 patients with lung cancer, 50 patients with benign lung diseases and 38 normal adults were measured by enzyme linked immunosorbent assay (ELISA). The association of CYFRA21-1, NSE, and CEA level with the type of lung cancer in pathology were also studied. Results In comparison, the serum levels of CEA, CYFRA21-1 were increased more obviously in patients with lung cancer than that of patients with benign lung diseases and the normal adults (P<0.01). The levels of serum and the sensitivity of CEA, NSE and CYFRA21-1 were related to pathology type. The sensitivity and specificity increased by combined measurement of CEA, NSE, and CYFRA21-1. Conclusion These findings suggest that the serum CEA, NSE and CYFRA21-1 levels is increased in patients with lung cancer, and the increasing extents is not same in lung cancer with different pathology types. CEA, NSE and CYFRA21-1 are significant in adjuvant diagnosis of lung cancer.

ABSTRACT

Objective Verification of the serum proteomic fingerprints of NSCLC (non-small cell lung cancer) patients related to their advantage in Gefitinib therapy. Methods 27 NSCLC patients who have been treated with Gefitinib for more than 1 month and attained certain efficacy were taken for examination. They were divided into 3 groups, CR+PR, SD and PD, according to the evaluation criteria of solid tumor curative efficacy. All patients received SELDI (SELDI-TOF-MS surface-enhanced laser desorption/ionization time-of-flight mass spectrometry technique) inspection before treatment with Gefitinib. The shifting procedure was based on the abundance of the fingerprint :"M/Z 8693 50H+". According to the abundance of this fingerprint founded, the patients were divided into 4 populations: 1. The advantage population (10 patients, abundance 10 %);2. The disadvantage population (7 patients, abundance≥30 %);3. Suspected population A (5 patients, abundance 11%-15 %);4. Suspected population B (5 patients, abundance 16~ 29%). The ratio of patients in each of the 3 groups (CR + PR, SD and PD) into each of the above 4 populations were analyzed. Results It showed that: 1. In the advantage population, the ratio dispersed from CR + PR group is 9/10, those from SD group is 1/10 and from PD is 0.2. In the disadvantage population, the ratio dispersed from CR + PR group is 1/7, those from SD group is 1/7 and from PD is 5/7. In the suspected A group, the ratio from CR + PR group is 3/5, those from SD group is 1/5 and from PD is 1/5. 4. In the suspected B group, the ratio from CR + PR group is 3/5, those from SD group is 1/5 and from PD is 1/5. Conclusion This fingerprint "M/Z 8693 50H+" with abundance 10% after SELDI inspection is the index choosing advantage patients suitable to receive Gefitinib therapy.

ABSTRACT

Objective To explore the correlation between the increasing of blood glucose and cytokines in malignant tumor patients after chemotherapy.Methods According to the blood glucose after chemotherapy,155 cases of malignant tumor patient were divided into 3 groups:euglycemia group,IGT(impaired glucose tolerance)group and DM(diabetes mellitus)group.The variance of cytokines in the 3 groups were analyzed.Results Compared with pre-chemotherapy,IL-6,TNF-αincreased greatly in IGT group and DM group,while IL-10 decreased greatly.The statistical difference were obvious compared with euglycemia group.Conclusion There are close correlation between the increasing of blood glucose and cytokines in malignant tumor patients after chemotherapy.
