ABSTRACT

Objective To know the plasma lipids level and influencing factors among adults in Tongxiang city,and to provide evidence for prevention and intervention of dyslipidemia.Methods Multistage stratified cluster sampling method was conducted to select participants in Tongxiang city.Questionnaire interview,physical examination and blood lipid detection were performed among the selected subjects.Models of the univariate and multivariate unconditional logistic regressions were used to analyze the influencing factors of dyslipidemia.Results A total of 600 people were involved in this study.The prevalence and standardized rates of dyslipidemia were 36.33% and 36.11 %,and abnormal rates of HDL -C,TG,TC and LDL -C were 27.33%,15.67%,4.83% and 4.83%,which standardized were 29.17%,15.1 3%,3.1 8% and 3.12%, respectively.The abnormal rate of LDL -C in females was higher than that in males (P <0.05).Borderline abnormal rate of TC and dyslipidemia rates were increased with age (P <0.05),and the abnormal rate of HDL -C were decreased with age (P <0.05).Multivariate logistic regression analysis showed that BMI (OR =3.398),diabetes mellitus(OR =1.694), hypertension (OR =1.717)and total static time(OR =1.712)were the risk factors for dyslipidemia.Conclusion The prevalence of dyslipidemia was high among the adults in Tongxiang city,especially the abnormity of HDL -C and TG. Community comprehensive prevention and control of dyslipidemia should be carried out according to its risk factors.

ABSTRACT

Objective To exam the influence of psychological factors,occupational factors and dietary habits on malignant tumors.Methods A total of 2 471 patients with malignant tumor were selected from 2010 to 2012 and 1 to 1 matching method was used controlling sex and age distribution in the case and control groups.Data collected by interviewing was analyzed by univariate and multivariate conditional logistic regression.Results 12 related factors including 6 psychological factors were found in relation to malignant tumor after univariate logistic regression analysis.The multivariate logistic regression analysis showed that harmful occupational history (OR =1.731,95%CI:1.337 ~2.242),often having fried food (OR =1.889,95%CI:1.398 ~2.551),heavy salty diet(OR =1.457,95%CI:1.169 ~1.818),psychological problems (OR =1.274,95%CI:1.003 ~1.617),poor marital life (OR =2.007,95%CI:1.111 ~3.623)and lack of self -regulation (OR =1.460,95%CI:1.189 ~1.793 )were the risk factors.Conclusion Malignant tumor was related to a variety of exposure factors and psychological factor has become an important risk factor for malignant tumor in Tongxiang city.

ABSTRACT

<p><b>OBJECTIVE</b>To study the histopathologic features, differential diagnosis and pathogenesis of diabetic cardiomyopathy.</p><p><b>METHODS</b>The clinicopathologic features of 40 autopsy cases of diabetes mellitus were studied. The hearts from another 40 cases of non-diabetic elderly deceased were used for comparison.</p><p><b>RESULTS</b>In the 40 cases of diabetes studied, 36 cases (90.0%) showed microscopic myocardial cell death. Focal interstitial fibrosis was observed in 37 cases (92.5%). On the other hand, similar myocardial cell death and patchy interstitial fibrosis was seen in 8 cases (20.0%) and 9 cases (22.5%) of non-diabetic hearts, respectively. The difference between the two groups was statistically significant (P < 0.01). The mural thickness of intramyocardial blood vessels was significantly increased in diabetic group (20.6 microm +/- 4.2 microm) than in non-diabetic group (7.2 microm +/- 5.2 microm), P < 0.01.The myocardial changes in diabetic group however were similar to those in non-diabetic group with systemic hypertension.</p><p><b>CONCLUSIONS</b>Pathologic diagnosis of diabetic cardiomyopathy relies on detailed histologic examination of heart tissue and clinical correlation of a long history of diabetes mellitus. Exclusion of other possible etiologies is also essential. The myocardial cell death observed may be due to the ischemic effect induced by diabetic microangiopathy in cardiac muscle.</p>

ABSTRACT

Objective To study effects of early non-invasive ventilation (NIV) application in treating elderly patients with acute left ventricular failure induced respiratory failure. Method Totally 32 elderly patients with acute left ventrieular failure induced respiratory failure, admitted from August 1997 to February 2007, received NIV treatment, and were retrospectively studied. There were 22 male and 10 female, aged (81.5?8.6) yearsdd. The changes of rahs, respiration rate, heart rate, arterial blood gas, cardiac function before and after NIV application were compared. According to the application time of NIV, 32 patients were divided into two groups: group A (early NIV application group, n=17) and group B (non-early NIV application group, n= 15). The time to improve the symptoms, the application time of NIV, cure rates, tracheal intubation rates and mortality were compared between the two groups. Results Thirty of the 32 patients survived, cardiac function was improved from New York classⅣtoⅠ~Ⅱ, respiratory rate, heart rate and blood pressure significantly decreased, PaO_2 and SaO_2 significantly increased and PaCO_2 significantly decreased. ABSTRACT

Objective To find out whether the levels of serum creatinine and urea,creatinine clearance rate and GFR estimated by using different equations correlate with the exact measurement of GFR with ~(99m)Tc-DTPA in elderly patients with chronic kidney disease.Methods Renal function was determined by using ~(99m)Tc-DTPA technique in 83 elderly patients aged 65—96 years.Blood and urine tests were done at the same time.Estimated clearance was determined by using the equations of Cockcroft-Gauh,MDRD(modification of diet in renal disease)predigestion and MDRD seventh,and the correlation coefficients between the above parameters and ~(99m)Tc-GFR were determined.Veracity for the diagnosis of renal dysfunction was calculated by using receiver operating characteristic(ROC) curves.Results Cockcroft-Gault formula(r=0.833),MDRD predigestion formula(r=0.811), MDRD seventh formula(r=0.803),creatinine(r=-0.672),Ccr(r=0.632),urea(r=-0.612), and albumin(r=0.444)were correlated significantly with ~(99m)Tc-GFR in this sequence.If GFR 30, 40,50,60ml/min were defined respectively as renal impairment thresholds and areas under the curve (AUC)were compared,the best parameter for estimation of GFR was the Cockcroft-Gault formula. Then MDRD predigestion formula and MDRD seventh formula,creatinine and Ccr were less accurate in all different renal dysfunction thresholds.Conclusions The Cockcroft-Gault equation seems to be the best formula available for GFR estimation,the two equations of MDRD are the second,creatinine is the third,Ccr and urea are the last.
