ABSTRACT

Background The community is the main place for people's daily activities. A livable environment will improve the subjective well-being of residents. Objective To understand the current status of subjective well-being of residents in Pudong, Shanghai, and explore the impact of community environmental factors on residents' subjective well-being. Methods Using quota sampling, 6000 permanent residents from 12 sub-districts or towns in Pudong, Shanghai were selected to participate in an questionnaire survey. The questionnaire included three parts: participants' basic information, community environmental factors (neighborhood aesthetics, fitness environment, public service and security, natural environment), and subjective well-being. Using a multiple regression model, the influence of community environmental factors on the subjective well-being of residents was analyzed. Results A total of 5887 questionnaires were recovered, and the valid recovery rate was 98.1%. The subjective well-being score of the survey respondents was (7.03±1.61) points. There was no statistical difference in the subjective well-being score of study subjects of different gender and marital status groups; while those with different ages, education levels, occupations, and self-evaluated economic status showed statistical differences in their subjective well-being score (P<0.05). The multiple logistic regression analysis results showed that after controlling general demographic characteristics, with the low level as the control group (according predetermined cut-off values of 33.3% and 66.7%, the community environmental factors were divided into high-, medium-, and low-level groups), the OR values of subjective well-being of the high- and medium-level neighborhood aesthetics groups were 1.393 (95%CI: 1.173-1.654) and 1.235 (95%CI: 1.080-1.412); the OR values of the high- and medium-level fitness environment groups were 2.297 (95%CI: 1.929-2.734) and 1.349 (95%CI: 1.166-1.560); the OR values of the high- and medium-level public service and security groups were 1.101 (95%CI: 0.943-1.285) and 1.039 (95%CI: 0.905-1.193); the OR values of the high- and medium-level natural environment groups were 4.248 (95%CI: 3.321-5.434) and 1.652 (95%CI: 1.374-1.986), respectively. Conclusion Community environment factors could affect residents' subjective well-being, and good neighborhood aesthetics, fitness environment, natural environment have positive effects.

ABSTRACT

ObjectiveTo explore the effective components and mechanism of Epimedii Folium in the treatment of oligoasthenotspermia by using network pharmacology and molecular docking technique. MethodThe main active components and corresponding target genes of Epimedii Folium were screened out from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). Target genes of oligospermia were obtained by GeneCards and Online Mendelian Inheritance in Man (OMIM) database. Uniprot was used to correct all genes. The drug-active component-key target regulatory network was constructed by Cytoscape3.9.0, and the key active components were screened out according to the degree value. The active components and common targets of the disease were uploaded to STRING 11.5 database to construct the Epimedii Folium and oligoasthenotspermia target protein-protein interaction (PPI) network, and the key protein targets were screened out according to the degree value. The key targets of gene ontology (GO) functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were performed using DAVID database. Protein Data Bank (PDB) and TCMSP were used to obtain the molecular structure of target proteins and active components. AutoDock Vina 1.1.2 was used to perform molecular docking of the active components and the core protein targets. Finally, icariin, the active component of Epimedii Folium, was used to intervene in the rat model of oligoasthenotspermia to verify the effect of icariin on the expression level of protein targets. ResultTwenty-three active components from Epimedii Folium were screened out, and 50 common targets and 6 core targets of oligoasthenotspermia and Epimedii Folium were obtained, including tumor protein p53 (TP53), epidermal growth factor receptor (EGFR), prostaglandin-endoperoxide synthase 2 (PTGS2), cysteine aspartate-specific protease (Caspase)-3, erb-b2 receptor tyrosine kinase 2 (ERBB2), and caspase-9. Through GO enrichment and KEGG pathway enrichment analysis, the active components of Epimedii Folium were mainly involved in the P53 signaling pathway, the pathways in cancer, cell proliferation, and apoptosis, etc. Molecular docking results indicated that icariin, quercetin, and 8-isopentenol had strong binding ability to target protein. The results of icariin intervention experiment showed that as compared with the control group, the expression of target proteins in testis of rats with oligoasthenotspermia was significantly down-regulated. As compared with the model group, icariin significantly up-regulated the expression of target protein in testis of rats with oligoasthenotspermia (P<0.05). ConclusionEpimedii Folium treats oligoasthenotspermia through regulating the P53 signaling pathway, the pathways in cancer, cell proliferation, and apoptosis by icariin, quercetin, and 8-isopentenol.

ABSTRACT

ObjectiveIn the context of COVID-19 prevention and control, to summarize the current situation of the public health personnel team development against the pandemic at a county-level center for disease control and prevention (CDC) in Shanghai’s extra-large urban area, to analyze and summarize the current situation and existing problems of the public health personnel team of the district and county-level CDCs, and to propose measures to strengthen and improve the development. MethodsTaking the Shanghai Pudong New Area Center for Disease Control and Prevention, which serves a population of 5.68 millions, as an example, through information access, individual interviews, questionnaires, holding symposiums, etc., combined with the deployment of personnel in the handling of the COVID-19, we described the Pudong New Area CDC’s talent team characteristics, personnel recruitment and loss, and investigated the factors affecting the construction of talent team. ResultsFrom 2014 to 2020, Pudong CDC recruited 115 people, and 65.71% of them chose to work there. 68 people resigned, with an average of 6.8 people leaving every year, with a fluctuating upward trend in resignation rate (χ2=0.158,P<0.05). Since we entered the phase of regular COVID-19 prevention and control in 2020, the staff have been overloaded with work, based on the number of hours spent dealing with the pandemic on the spot and the number of people on the job. In 2017 to 2019, the salary of staff was basically maintained at the level of 2016 (average annual growth of about 2.5%); it lacked targeted talent recruitment (one-time resettlement fee, etc.) and talent retention incentives (monthly housing subsidy for new employees, etc.). According to the questionnaire’s results, 79.23% of the respondents believed that the talent incentive mechanism needs to be improved, 73.43% believed that a good environment for talent growth needs to be created, and 50.24% thought that the employment mechanism needs to be reformed. It is difficult to meet the needs of pandemic prevention and control under the new situation with the current number of public health personnel, the recruitment of talents is insufficient, the turnover is too high, and there is a gap between the ability of some public health personnel and what is required of them under the new situation and new tasks. ConclusionThe quantity and quality of public health personnel for disease control in Shanghai can hardly meet the needs of pandemic prevention and control under the new situation. The pertinence and systematization of public health talent training urgently need to be strengthened, and the incentive mechanism needs to be improved.

ABSTRACT

ObjectiveTo systematically review the intervention effect of Chinese medicine on the structure and function of testicular Sertoli cells in animal models of impaired spermatogenesis. MethodThe databases, such as China National Knowledge Infrastructure (CNKI),VIP,Wanfang Data,EMbase,and Pubmed,were searched for experimental studies on the effect of Chinese medicine on the structure and function of testicular Sertoli cells in animal models with impaired spermatogenesis. The included studies were evaluated for risks of bias,and the outcome indicators were analyzed with RevMan and Stata software. ResultThirty studies were included,involving 37 randomized controlled trials (RCTs). As indicated by the Meta-analysis results, compared with the model group,Chinese medicine increased sperm density(SMD=2.42,95% confidence interval(CI)[1.47,3.37],P<0.000 01), promoted sperm motility(SMD=2.35,95%CI [1.70, 2.99],P<0.000 01), up-regulated the protein and mRNA levels of Vimentin (related to Sertoli cell cytoskeleton), elevated the levels of Occludin and Claudin-11 (related to tight junction of blood-testis barrier), boosted the levels of β-catenin and N-cadherin (related to adherens junction of blood-testis barrier), raised the level of connexin 43 (Cx43, related to gap junction of blood-testis barrier), improved the function of Sertoli cells, increased the serum content of Inhibin B (INHB), and up-regulated the levels of testicular follicle-stimulating hormone receptor (FSHR), INHB mRNA, androgen-binding protein (ABP) mRNA, transferrin(TF),stem cell factor(SCF),SCF mRNA,glial cell line-derived neurotrophic factor (GDNF),GDNF mRNA,bone morphogenetic protein 4(BMP4),and BMP4 mRNA (P<0.05). ConclusionChinese medicine can effectively increase sperm density and motility of animal models of impaired spermatogenesis,and improve the structure and function of testicular Sertoli cells. However,affected by the quality of the included studies,the above conclusion needs to be further verified by relevant high-quality studies.

ABSTRACT

Background The health and safety climate of workplace has an important impact on the physical and mental health of workers. There is no available scale for the evaluation of workplace health and safety climate in China at present. Objective This study aims to sinicize and evaluate the reliability and validity of the Health and Safety Climate Survey. Methods The English version of Health and Safety Climate Survey was translated to Chinese and back-translated to English, and followed by expert evaluation to develop a Chinese version. Quota sampling method was used to select 2600 employees from 16 enterprises and public institutions in Pudong New Area of Shanghai and to evaluate the reliability and validity of the Chinese version of the scale. Structural validity, convergent validity, and discriminant validity were included in validity evaluation. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to test structural validity. In EFA, an acceptable factor loading of items on their respective dimensions was > 0.60. In CFA, an acceptable root mean square error of approximation (RMSEA), a standard absolute fitness index, was < 0.05. Comparative fit index (CFI) and parsimony-adjusted non-normed fit index (PNFI), measures of incremental improvement and fit, > 0.90 indicated a good fit. Convergent validity was evaluated by average variance extracted (AVE), and an AVE > 0.50 indicated good convergent validity. Discriminant validity was determined as good by the square root of AVE greater than the correlation coefficient between latent variables. Cronbach’s α coefficient was used to evaluate the internal consistency of the scale; the internal consistency considered was very good with an α > 0.90. Cronbach’s α coefficient may cause underestimation of reliability in the case of error correlation. Therefore, composite reliability (CR) calculated based on structural equation model >0.70 indicated that the CR was good. Results The response rate was 95.69%. The Chinese version of Health and Safety Climate Survey included 22 items belonging to six dimensions which was extracted by the EFA, and the cumulative variance contribution rate was 74.79%. The corrected RMSEA was 0.045, the PNFI was 0.970, and the CFI was 0.975 in the CFA, indicating good fit. The dimensional AVE values were all greater than 0.50, indicating good CV. The square root of AVE was greater than the correlation coefficient between latent variables, indicating good DV. The Cronbach’s α was 0.90 for the total scale and were above 0.70 for all dimensions. The CR was 0.93 for the total scale and were above 0.70 for all dimensions, indicating that the overall reliability of the scale was good. Conclusion The Chinese version of Health and Safety Climate Survey has good reliability and validity, and can be further improved in field application.

Objective To explore the relationships between neuronal nicotinic acetylcholine receptor subunit gene polymorphisms and type 2 diabetes in Chinese han schizophrenics.Methods Five single nucleotide polymorphisms(SNPs )of CHRNA3 (rs1317286),CHRNA4 (rs1044396),CHRNA 7 (rs6494212) and CHRNA5 (rs16969968,rs684513) gene were analyzed in a sample of 346 schizophrenics with type 2 diabetes and 360 schizophrenics without type 2 diabetes.The five markers were genotyped by the TaqMan fluorogenic detection method with the ABI7900.Results There were no significant differences in alleles and genotypes distribution of the five genes between two groups (P>0.05).For the CHRNA 7(rs6494212),there were significant difference in genotypes (P=0.039) and alleles distribution (P<0.021) between two groups in male patients.The haplotypes constructed by markers of CHRNA5 were not associated with the 2 diabetes in Chinese Han male schizophrenics.The interaction analysis revealed a significant association between models made up by rs1317286,rs1044396,rs6494212,rs684513and 2 diabetes in Chinese Han male schizophrenics (P=0.002).Conclusion The CHRNA7(rs6494212) gene may be one of common susceptible genes for 2 diabetes in Chinese Han male schizophrenics.There is a significant association between models made up by rs131726rs1044396,rs6494212,rs684513 and schizophrenics with type 2 diabetes.

ABSTRACT

Objective · To investigate the effect of metformin on insulin resistance (IR)-related metabolic parameters in olanzapine-bearing rats.Methods · Thirty female SD rats were randomly divided into metformin intervention group, olanzapine group and control group. During the first 6 weeks,5 mg /(kg·d) olanzapine was given to the two test groups . The control group was given the same amount of saline. From the 7th week, the intervention group began to combine with metformin 500 mg / (kg·d), while the olanzapine group combined with the same amount of saline, continuing for 4 weeks.At the end of 6th week and 10th week, intraperitoneal glucose tolerance and homeostasis model assessment IR index were assessed. Results · The area under the glucose tolerance curve (P=0.040) and the IR index (P=0.000) were significantly higher for the intervention group and the olanzapine group than the control group at the end of 6th week. At the 10th weekend, the glucose tolerance (P=0.015) and IR index in the intervention group were significantly lower than those in the olanzapine group (P=0.001). Conclusion · Metformin may rectify the impaired glucose tolerance and improve IR induced by olanzapine partly.

ABSTRACT

OBJECTIVE: Duloxetine hydrochloride is a selective serotonin (5-hydroxytryptamine) and norepinephrine reuptake inhibitor. It is approved for effective treatment for major depressive disorder. The pharmacokinetics (PK) of duloxetine has been studied, but few pharmacokinetics properties in Chinese subjects are available. This study explored the dose proportionality and determined duloxetine levels in human plasma by comparing the PK properties after administration of single or multiple doses in healthy volunteers. METHODS: Thirty-six subjects were divided randomly into three groups and received a single dose of 15, 30, or 60 mg duloxetine. Those who received 30 mg continued on to the multiple-dose phase and received 30 mg daily for 7 days. Liquid chromatography/mass spectroscopy was applied to determine concentrations. The PK properties were calculated and included maximum plasma concentration (Cmax), time when maximum plasma concentration was reached (Tmax), time when half-maximum plasma concentration was reached (t1/2), area under the plasma concentration-time curve from time zero to the last measurable concentration (AUC0-t), mean concentration levels (AUC0-infinity), and apparent total clearance of the drug from plasma after oral administration (CL/F). RESULTS: The standard calibration curve was linear in the concentration range 0.11-112 ng/ml (r>0.992). Linear PK properties were found at doses of 15-60 mg. The Cmax and AUC were proportional to dose, but the Tmax and t1/2 did not increase with increasing dose. CONCLUSION: No significant differences in the PK parameters were found among the three groups during the single-dose phase. The AUC and Cmax were greater in the multiple-dose phase, indicating duloxetine accumulation following multiple-dose administration.
