ABSTRACT

Objective:To investigate the expression of Mucosal-associated invariant T (MAIT) cells in the peripheral blood of ankylosing spondylitis (AS) patients, and to analyze its clinical significance.Methods:Sixty-four AS patients from September 2020 to September 2022 in Fujian Provincial Hospital and 60 healthy controls were enrolled. The expression of MAIT cells and CD69 +MAIT cells were detected by immunofluorescence. Spondylarthritis research consortium of Canada (SPARCC) scores of sacroiliac joint magnetic resonance imaging (MRI) were calculated and were analyzed based on clinical data. The changes of MAIT cells were observed in 7 patients with AS after treatment. t test, variance analysis and Pearson correlation analysis were used for statistical analysis. Results:The expression of MAIT cells in the peripheral of AS patients was significantly lower than that in the controls [(2.81±0.27)% and (4.46±0.86)% respectively, t=2.33, P=0.022], while CD69 +MAIT cells were significantly higher [(12.0±1.0)% and (7.8±1.2)% respectively, t=2.31, P=0.024]. The level of CD69 +MAIT cells were significantly positively correlated with erythrocyte sedimentation rate (ESR) ( r=0.39, P=0.010), C-reactive protein (CRP) ( r=0.36, P=0.012), ASDAS ( r=0.35, P=0.013) and SPARCC scores ( r=0.38, P=0.006) of AS patients. ASDAS scores and CD69 +MAIT cells obviously decreased in 7 treated AS patients ( F=7.62, P=0.007 and F=4.97, P=0.027 respectively). Conclusion:The expression of peripheral MAIT cells in AS patients is lower than that in healthy controls, but the number of the activated MAIT cells is increased. Levels of activated MAIT cells may in some extent reflect the inflammatory state and disease activity of AS, as well as inflammatory destruction of sacroiliac joint, and therapeutic effect. MAIT cells may partially participate in the inflammatory response and play a role in the pathogenesis of AS.

ABSTRACT

Objective:To investigate the expression of Mucosal-associated invariant T (MAIT) cells in the salivary gland of primary Sj?gren′s syndrome (pSS) patients, and to analyze its clinical significance.Methods:Thirty-two pSS patients and 27 non-pSS controls who had salivary gland biopsies were enrolled. The expression of MAIT cells was detected by immunofluorescence and then analyzed based on clinical data. T test, variance analysis and Spearman correlation analysis were used for statistical analysis. Results:The expression of MAIT cells in the salivary gland of pSS patients was significantly higher than in controls [(2.40±0.33)/gland and (0.79±0.13)/gland respectively, t=4.24, P<0.01]. In pSS patients with mouth dryness ( n=26), MAIT cells in the sali-vary gland were significantly increased compared with those without [(2.73±0.38)/gland vs (0.95±0.15)/gland, t=2.24, P=0.03]. In patients with dental caries ( n=17), MAIT cells in the salivary gland were also significantly increased [(3.13±0.54)/gland vs (1.57±0.20)/gland, t=2.57, P=0.02]. The expression of MAIT cells in the salivary gland of pSS patients was positively correlated with erythrocyte sedimentation rate (ESR) and European League Against Rheumatism Sj?gren′s Syndrome Disease Activity Index (ESSDAI) scores ( r=0.37, P=0.04 and r=0.65, P<0.01 respectively). In pSS patients with positive SSA antibody ( n=23), the amount of MAIT cells was more than controls [(2.89±0.40)/gland vs (1.13±0.32)/gland, t=2.61, P=0.01]. The level of MAIT cells in the salivary gland was higher in pSS patients with more lymphocytic foci [one foci: (1.50±0.49)/gland, two foci: (2.29±0.52)/gland, three foci(3.66±0.59)/gland; F=4.22, P=0.02]. Conclusion:The expression of MAIT cells in the salivary gland of pSS patients is significantly higher than non-pSS controls, and is correlated with oral symptoms, disease activity and the production of autoantibodies. This may suggest that MAIT cells may participate in the local inflammation response and play a role in the pathogenesis of pSS.

ABSTRACT

Objective: To study the dimensional accuracy of the Co-Cr crowns prepared by selective laser melting(SLM) technology and to evaluate their internal and marginal fit by "Virtual seating". Methods: The Co-Cr metal crowns were fabricated with 2 different methods(n = 10): milling wax with lost-wax method in the control group and SLM in the test group. After collecting the data of the die and crowns, "virtual seating" was completed by the software of reverse engineering. The gaps between the die and crowns on the 2D cross-sectional were measured by the same software. At last, crowns were cemented on the dies. And the thickness of the crowns and the cement films were measured under a scanning electron microscope. Data were statistically analyzed. Results: The most appropriate pre-set die spacer thickness was 50 μm. The relative errors of the crown thickness of SLM group and the control group were 1. 80% and 2. 20% respectively(P> 0. 05). No statistically significant difference was found in both internal and marginal fit between the 2 groups (P> 0. 05). And there was no statistical difference between the 2 measuring methods (P> 0. 05). Conclusion: SLM technique achieves clinically values for internal and marginal fit. "Virtual seating" can be used for evaluate crown dimensional accuracy.

ABSTRACT

Objective:To evaluate the clinical efficacy,toxicity,and prognostic factors of nab-paclitaxel as first-line treatment for elderly patients with advanced lung squamous carcinoma.Methods:This was a prospective study.Forty patients enrolled in the Second Affili-ated Hospital of Fujian Medical University were treated with nab-paclitaxel(260 mg/m2,ivggt d1),and a period of three weeks was considered as one session.The effects were evaluated after two cycles.Results:All 40 patients were followed up and appraised.Two patients achieved complete remission,13 achieved partial remission,13 achieved stable disease,and 12 achieved progressive disease. The objective response rate was 37.5% and the disease control rate was 70.0%.The progression-free survival(PFS),median overall sur-vival,and 1-year survival rate was 6.3 months,12.6 months,and 62.5%,respectively.The main hematologic toxicities were neutrope-nia and anemia,and the main non-hematologic adverse events were fatigue,constipation,nausea,vomiting,muscle aches,and hear-ing loss.Most patients could tolerate these toxic reactions.Moreover,Cox multivariate regression analysis showed that the neoplasm stage,Eastern Cooperative Oncology Group performance status,response rate,and PFS were independent factors for the survival rate (P<0.05),while age was not related to patient prognosis(P>0.05).Conclusions:Nab-paclitaxel as single drug and first-line therapy for elderly patients with advanced lung squamous carcinoma is effective and safe.

ABSTRACT

Objective:To compare the clinical efficacy of oxycodone hydrochloride controlled-release tablets (OHCT) and pa-tient-controlled intravenous analgesia (PCIA) in the treatment of intractable cancer pain. Methods:Retrospective analysis was conduct-ed to evaluate the intractable cancer pain of 89 elderly patients who were admitted to the medical oncology departments of The Second Affiliated Hospital of Fujian Medical University and the Fuzhou General Hospital of Nanjing Military Command between September 2012 and March 2014. Among the 89 patients, 47 were treated with OHCT, and 42 received PCIA. The total dosage ranged from 60 mg/d to 400 mg/d PO q12h for patients in the OHCT group, whereas abackground dose+patient-controlled dosemode was adopted for patients in the PCIA group. The therapeutic efficacy, presence of adverse reactions, cost of treatment, and degree of patient satisfaction were compared between the two groups. Results:The average dosages of analgesics in the two groups were almost the same (P>0.05). Visual analogue scale (VAS) values and daily average VAS values were both lower in the PCIA group than in the OHCT group at 24 h after analgesia (P0.05). Conclusion:Pa-tients who received PCIA attained better analgesia and exhibited less adverse reactions than those who received OHCT whereas the treatment cost and patient satisfaction did not differ in both groups.

ABSTRACT

Objective To investigate and analyze the health care seeking behavior,financial burden and relative factors of patients with rheumatic diseases in Fujian Province.Methods Patients diagnosed with rheumatic diseases were investigated in Fujian Provincial Hospital from December 2013 to February 2014,including demographic data,health care seeking behavior and financial burden.Relevant factors were analyzed by Logistic regression.Results ① In this study,474 patients were enrolled.The ratio of male to female was 1∶2.38.② At the onset of symptoms,51.9%(246 cases) of patients visited a doctor as soon as possible,and 36.1%(171 cases) of them went to hospital within a month after disease onset.③ 74.1%(351 cases) of patients could adhere to medication regimen prescribed by doctors,and the most common reason for stopping medication was symptoms relieved (33.3%,41 cases).④ 63.1%(299 cases) of patients learned about the knowledge of rheumatic diseases from hospitals and doctors.⑤ 34.2%(162 cases) of patients spent 1 000-5 000 yuan every month on treatment.⑥ Insurance type could affect the time of first visit to hospital (P=0.004),while income and cost of therapy might impact patients' compliance (P=0.013,P=0.004).Conclusion Most patients will go to hospital as soon as possible and adhere to treatment.Economic situation is the main factor influencing health care seeking behavior of patients with rheumatic diseases.

ABSTRACT

Objective To assess the rationality of using CT for clinical staging in esophageal carcinoma.Methods 103 esophageal carcinoma patients with radical resection were selected,patients were examined by CT before surgery.To compare the clinical staging on the basis of CT findings with pathology.Results Lymph node enlargement were mentioned in 29 cases (28.2 %) with CT findings before surgery,but 70 cases (68.0 %) by pathology.The CT findings and pathology were inconsistent (x2 =11.719,P =0.001).Although the pathology staging in the cases which were positive with CT findings could be later (Z =-3.04,P =0.002),there was no correlation between the two methods (x2 =10.885,r =0.055,P =0.055).Conclusion The accuracy rate in esophageal carcinoma clinical staging by CT before surgery is low.Combining with PET-CT or endoscopic ultrasonography may improve the accuracy rate.

ABSTRACT

Objective To analyze the clinical features,therapy and outcome of systemic lupus erythematosus (SLE) combined with lupus myelopathy (LM).Methods Ten SLE patients combined with LM treated in Department of Rheumatology and Immunology,People' s Hospital from 1990 to 2011 were retrospectively analyzed and 43 cases of SLE combined with LM reported home and abroad were reviewed.Results All the ten patients were women with age of 23-53 (36.9 ± 3.4) years old and duration of 1-18 years.MRI of spinal cord revealed long T2 signal in one case,and normal in two cases.Seven patients received methylprednisolone pulse plus cyclophosphamide (CTX),two were given glucocorticoid pulse only,and one was given moderate dosage of glucocorticoid,CTX and plasma exchange (PE).The results revealed that four patients received complete recovery,four received partial recovery,and two received no improvement.Conclusions LM is a rare but severe complication of SLE with poor prognosis,which usually occurs in early phase of young SLE patients.Pulse methylprednisolone and CTX may be effective.Early and active treatment may improve the outcome.
