ABSTRACT

To develop the environment scale of unintentional injury in the home for children aged 0-6 years living in urban area of China, and test its validity and reliability. The content of the environment scale was established through the literature review, expert consultation and pilot study. A total of 1 104 children aged 0-6 years in urban area of Changsha were enrolled in this study by using a multi-stage stratified cluster random sampling method. The questionnaire was used to collect the basic information of children, the incidence of unintentional injury and the status of home environment. The reliability of the scale was tested by using Cronbach's α coefficient and split-half reliability coefficient. The content validity and construct validity were tested by using Pearson correlation analysis and factor analysis. All children were divided into two groups according to the incidence of unintentional injury in the home and the discrimination validity of the scale was tested by using -test. The scale had 54 items in 6 dimensions. The number of eligible questionnaires was 1 074, including 554 (51.6%) from boys and 519 (48.3%) from children under 3 years old. The incidence rate of unintentional injury and in-home injury was 18.34% (197 children) and 10.71% (115 children). The internal consistency reliability (Cronbach's α) coefficient for the scale was 0.87 and the split-efficacy reliability coefficient was 0.82, both meeting the standard of reliability above 0.70. The Pearson correlation coefficient between each dimension and the whole scale ranged from 0.53 to 0.84 (all values <0.001). The common factor cumulative variance contribution rate of the scale was 58.34%. There were 54 items with factor loadings greater than 0.30. The root mean square error of approximation, comparative fit index and goodness-of-fit index were 0.07, 0.61 and 0.71, respectively. The score of scale in children with injury was significantly higher than that in children without injury (0.022). The validity and reliability of the environment scale for unintentional injury in the home for children aged 0-6 years old in the urban area of China are good.

ABSTRACT

Objective To evaluate the changes of macrophages and expression of Rac1 in the inflammatory site of Crohn's disease,and to investigate the effects of 6-thioguanine (6-TG) and peptidoglycan on apoptosis of human peripheral blood monocyte-macrophage by regulating Rac1 signaling pathway.Methods Ten patients with Crohn's disease and eight healthy controls diagnosed were enrolled at Department of Gastroenterology and Hepatology,Tianjin Medical University General Hospital from January 2013 to January 2014.The number of macrophages,apoptosis and expression of Rac1 in the inflammation sites and non-inflammation sites of intestinal mucosa were detected in both patients and controls.Peripheral blood mononuclear cells (PBMCs) were sorted by CD14 immunomagnetic beads.The apoptosis of monocytes,expression of Rac1 and related apoptosis signaling molecules were detected in patients treated with peptidoglycan,6-TG and Rac1 inhibitor NSC23766 and another 15 healthy donors.Results The number of macrophages and apoptotic cells significantly increased in the inflammatory group of Crohn's disease patients compared with the non-inflammatory group.The expression of PAK1,downstream molecular of Rac1 signaling pathway of macrophages was also significantly higher in the inflammatory group of Crohn's disease patients than that in healthy controls and non-inflammatory group.Compared with control group,anti-apoptotic signals (NF-κB,Bcl-xL and STAT-3) in PBMCs increased in the peptidoglycan group,while slightly decreased in 6-TG group.6-TG and NSC23766 significantly promoted peptidoglycan-related anti-apoptosis [peptidoglycan group (8.6±3.7)%,peptidoglycan + 6-TG group (42.0±2.7)%,peptidoglycan + NSC23766 group (58.5±6.9)%,P<0.05].Conclusions Peptidoglycan plays a role in the pathogenesis of Crohn's disease by recruiting macrophages.However,6-TG inhibits peptidoglycan-induced activation of Rac 1 signaling pathway leading to macrophage apoptosis.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the efficacy and safety of Wuji Powder (WP) and a small dose aripiprazole in treatment of antipsychotic drug-induced phlegm dampness type amenorrhea.</p><p><b>METHODS</b>Seventy female schizophrenic patients with antipsychotic drug-induced galactorrhea-amenorrhea syndrome (GAS) were recruited and randomly assigned to the treatment group and the control group, 35 in each group. All patients received antipsychotic drug therapy. Patients in the treatment group additionally took WP, while those in the control group took aripiprazole (at the daily dose of 5 mg, once daily). The therapeutic course for all was 4 weeks. Prolactin levels and obesity indices[body weight, waist aircumstance, body mass index (BMI) and waist-hit ratio (WHR)] were determined before and after treatment. The efficacy was evaluated.</p><p><b>RESULTS</b>The treatment course was completed in 95.71% of patients. The total effective rate of the 33 patients of the treatment group was 93.94% (31/33), while it was 91.18% (31/34) in the 34 patients of the control group. There was no difference in the total effective rate between the two groups (P > 0.05). Prolactin levels in both group after treatment were significantly lower than those of the baseline (P < 0.01). There was no significant difference in prolactin levels between the two groups after treatment (P > 0.05). Compared with before treatment, body weight, BMI, waist circumstance, and waist-hip ratio obviously decreased after treatment, showing significant difference when compared with the control group (P < 0.05). There was no significant difference in body weight, BMI, waist circumstance, and waist-hip ratio in the control group between before and after treatment (P > 0.05).</p><p><b>CONCLUSIONS</b>Both WP and aripiprazole could lower high prolactin levels of schizophrenics with phlegm dampness type amenorrhea. They showed equivalent efficacy. But WP showed more obvious effect in reducing obesity indices.</p>

ABSTRACT

Objective To explore the effects of lower esophageal leiomyoma on esophageal dynamics.Methods A total of 25 patients with lower esophageal leiomyoma,31 patients with gastroesophageal reflux disease (GERD) and 16 healthy controls were selected.The differences of high resolution esophageal manometry results were compared among the groups.The t-test or one way analysis of variance was performed for normally distributed measurement data comparison.Rank sum test was used for non-normally distributed measurement data comparison.Results Among 25 patients with lower esophageal leiomyoma,14 patients accompanied with acid reflux,heartburn,chest pain or other GERD symptoms.In the group of patients with lower esophageal leiomyoma,the lower esophageal sphincter pressure (LESP),the lower esophageal sphincter relaxation rate (LESRR),low esophageal body pressure and peristaltic contraction percentage were (9.00 ± 6.30) mmHg (1 mmHg =0.133 kPa),53.0 % (0,334.0%),(34.66±18.33) mmHg and 55.6% (0,100.0%),respectively,which were lower than those of the healthy control group ((16.25 ± 3.71) mmHg,86.5% (49.0%,103.0%),(57.75 ± 22.49) mmHg,100.0% (80.0%,100.0%)),and the differences were statistically significant (t=-4.150,Z=-2.353,t=-3.601,Z=-3.798; all P<0.05).However there were no significant differences in esophageal dynamics indexes between patients with lower esophageal leiomyoma accompanied GERD symptoms and without GERD symptoms (all P>0.05).There were no significant differences in esophageal dynamics indexes between the GERD patients and patients with lower esophageal leiomyoma accompanied GERD symptoms (all P>0.05).LESRR level of patients with lower esophageal leiomyoma (71.4%(45.5%,150.0%)) increased after endoscopic submcosal dissection (Z=-2.194,P=0.028).Conclusions Lower esophageal leiomyoma may affect esophageal motor function,which contributed to anti-reflux esophageal function decline.Lower esophageal leiomyoma combined with gastroesophageal reflux symptoms are not uncommon.

ABSTRACT

<p><b>OBJECTIVE</b>To study the histogenesis of giant cell tumor (GCT) and factors related to tumor recurrence, invasiveness and malignant transformation.</p><p><b>METHODS</b>The clinical features, radiologic classification, surgical approach, pathologic findings, immunophenotypes and follow-up data of 123 cases of GCT were analyzed.</p><p><b>RESULTS</b>There was a significant correlation between tumor recurrence and radiographic classification (P = 0.032), over-expression of CD147 (P = 0.034) and p53 (P = 0.005), and surgical approach (P = 0.0048) in GCT. The biologic behavior showed no correlation with intramedullary infiltration, cortical bone involvement, parosteal soft tissue extension, tumor thrombi, fusiform changes of mononuclear tumor cells, mitotic count, Ki-67 index, coagulative tumor necrosis, secondary aneurysmal bone cyst formation, and adjoining bony reaction. The positive rate of p63 in stromal cells of GCT (79.7%, 94/118) was significantly higher than that in chondroblastoma (44.7%, 21/47), osteosarcoma (22.2%, 10/45) and other giant cell-rich tumors.</p><p><b>CONCLUSIONS</b>GCT is a bone tumor of low malignant potential. It is sometimes characterized by locally invasive growth, active proliferation, coagulative necrosis, secondary aneurysmal bone cyst and surrounding bony reaction. It is difficult to predict the biologic behavior of GCT. Over-expression of p53 in the tumor cells and CD147 in all components of GCT correlate with tumor invasiveness, recurrence and malignant transformation. Selection of suitable surgical approach with reference to radiologic classification is considered as an important factor in reducing the recurrence rate.</p>

ABSTRACT

The gastrointestinal tract harbors a complex microbiota,which contains almost 30 genera,400 to 500 species.The factors influencing human intestinal microbiota include host and environmental ones,with the most important environmental factor being diet.Dietary fiber,fat,and protein have different impact on gastrointestinal microbiota and the microbiota in turn plays an important role in maintaining health.Its variation can lead to many diseases.Knowledge of the interaction between diet and gastrointestinal microbiota may be conducive to reaching a better understanding of some diseases,and to discovering better preventive and therapeutic strategies.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the depression and anxious status among transferred injured survivors in Wenchuan earthquake in Sichuan province.</p><p><b>METHODS</b>A total of 43 transferred injured survivors were investigated by questionnaire exploring their trauma symptoms and mental health status.</p><p><b>RESULTS</b>High rates of trauma symptoms were remarkably observed in these survivors. Of all the respondents, 60% had some emotional symptoms and sleeping difficulties. About one third of respondents experienced recurrent and intrusive distressing recollection of event, 16 (37.21%) experienced nightmare, 15 (34.88%) had flashback and 7 (16.28%) of them tried to avoid relative stress.</p><p><b>CONCLUSION</b>Many mental symptoms were observed in transferred injured survivors. The two major factors of mental stress were emotional symptoms and re-experience of the disaster.</p>
