ABSTRACT

To establish a quantitative analysis of multi-components by single marker (QAMS) for the determination of Aster souliei Franch., the relative correction factors (fx) of neochlorogenic acid, cryptochlorogenic acid, rutin, isoquercitrin, isochlorogenic acid B, isochlorogenic acid A, isochlorogenic acid C, quercetin, apigenin and kaempferol were established by ultra-high performance liquid chromatography with chlorogenic acid as internal reference. Meanwhile, the content of each component was determined by the external standard method (ESM) and QAMS, and a linear regression model was established to verify the feasibility and accuracy of the QAMS. Hierarchical clustering analysis (HCA) and orthogonal partial least square discriminate analysis (OPLS-DA) were used to evaluate the quality of 23 batches of A. souliei. The results showed that the repeatability of each fx was good. The average content of neochlorogenic acid, cryptochlorogenic acid, rutin, isoquercitrin, isochlorogenic acid B, isochlorogenic acid A, isochlorogenic acid C, quercetin, apigenin and kaempferol in 23 batches of A. souliei by QAMS was 0.165, 0.234, 6.115, 0.478, 0.484, 3.359, 1.382, 0.210, 0.172, and 0.057 mg·g-1, respectively. The mean content determined by the ESM method was 0.163, 0.235, 6.172, 0.479, 0.483, 3.343, 1.413, 0.207, 0.171, and 0.056 mg·g-1. The results of HCA and OPLS-DA analysis show that 23 batches of A. souliei can be divided into two groups based on caffeic acid content. The content of the first group was between 0.873 to 5.647 mg·g-1, while the second was between 8.524 to 16.705 mg·g-1. This QAMS method can be used to simply and quickly evaluate the quality A. souliei.

ABSTRACT

Objective:To investigate the effect of Jiaomu Gualou Decoction combined with bisoprolol on myocardial microcirculation and oxidative/antioxidant balance in patients with heart failure.Methods:A total of 128 patients with heart failure who met the inclusion criteria from March 2020 to February 2021 in Dagang Hospital of Traditional Chinese Medicine, Binhai New Area, Tianjin were divided into 2 groups by random number table method, with 64 in each group. On the basis of conventional treatment, the control group was given oral bisoprolol, and the study group was given Jiaomu Gualou Decoction and oral bisoprolol. Both groups were treated continuously for 4 weeks. TCM syndrome scores were performed before and after treatment. The left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), and left ventricular end-diastolic diameter (LVEDD) were detected by ultrasonic diagnostic equipment, and the frequency, duration and total myocardial ischemia load of the 24-hour ECG were recorded. Lipid peroxide (LPO) was detected by fluorescence method, SOD, MDA and GSH-Px were detected by colorimetric method. The adverse events were recorded and clinical response was evaluated.Results:The response rate was 93.75% (60/64) in the study group and 79.69% (51/64) in the control group, and the difference between the two groups was statistically significant ( χ2=5.49, P=0.019). After treatment, the scores of shortness of breath, phlegm, sternocostal fullness, and fatigue in the study group were significantly lower than those in the control group ( t values were 8.48, 8.15, 8.86, and 6.88, respectively, all Ps<0.001). After treatment, the LVEF of the study group [(53.26±5.18)% vs. (48.65±5.27)%, t=4.99] was significantly higher than that of the control group, and the LVESD [(42.59±3.26) mm vs. (46.98±3.55) mm, t=7.29], LVEDD [(52.79±4.15) mm vs. (57.48±4.60) mm, t=6.06] significantly lower than the control group ( P<0.01). After treatment, frequency of ST segment fall [(2.51±0.42) times/24 h vs. (3.79±0.55) times/24 h, t=14.80], duration [(15.26±3.45) min/24 h vs. (22.65±3.71) min/24 h, t=11.67] and total myocardial ischemia load [(25.79±5.13) mm/min vs. (38.02±5.44) mm/min, t=13.09] were significantly lower than those in the control group ( P<0.01). After treatment, the levels of serum GSH-Px and SOD in the study group were significantly higher than those in the control group ( t values were 10.97, 14.37, respectively, all Ps<0.001), while the levels of LPO and MDA were significantly lower than those in the control group ( t values were 7.50, 11.04, respectively, all Ps<0.001). During the treatment period, the incidence of adverse events was 4.69% (3/64) in the study group and 7.81% (5/64) in the control group, with no significant difference between the two groups ( χ2=0.13, P=0.715). Conclusion:The Jiaomu Gualou Decoction combined with bisoprolol can improve the cardiac function and myocardial microcirculation in patients with heart failure, promote the body's oxidation/antioxidant balance, relieve the clinical symptoms of patients, and improve the response effect safely.

ABSTRACT

Objective To determine the potential sources of food safety risk and food safety related awareness of employees in the farmer markets in Yangpu District of Shanghai, and to provide evidence for food safety management. Methods The investigation on food safety and food-borne disease knowledge were conducted in 13 farmer markets in Yangpu District, and to assess the impact of employees′ behavior on food safety. Results A total of 368 employees in the 13 farmer markets were included in the study.Employees with high school education and above had better knowledge of food safety (P < 0.001).There was significant difference in the physical examination and habit of smoking in public places among the employees with different education levels.In addition, the employees in various positions in the markets differed significantly in the knowledge of food safety and behavior, such as taking physical examination, washing hands before meal and continuing working when feeling sick. Conclusion Food safety related risk in the farmer markets in Yangpu District mainly originates from the inadequate knowledge of the employees with low education levels, which further affects their behavior.Market management should strengthen the training of food safety related knowledge in the employees and advocate healthy behavior, such as taking physical examination and stop smoking, which contributes to creating a healthy work and shopping environment.

ABSTRACT

According to the different organs involved in patients, the concept of localized Eosinophilic granulomatous polyvasculitis(EGPA) was first proposed by consensus in 2018, which provides a basis for the early diagnosis and treatment of EGPA. The prognosis of patients with EGPA is related to the initial treatment. The treatment of EGPA depends on the severity of the disease, the organs involved and whether the disease is active or not. The overall treatment plan is divided into two stages: induction remission and maintenance therapy. Induced remission therapy mainly includes hormones and/or immunosuppressive agents(e.g. cyclophosphamide). After remission, azathioprine or methotrexate is recommended for maintenance therapy. In 2015, the global consensus of EGPA diagnostic and therapeutic experts recommended that the duration of treatment should be at least 24 months after remission. In combination with the latest international progress, the first Expert Consensus on Diagnosis and Treatment of EGPA in China 2018 consensus introduced the application of Bio-Targeting drugs in EGPA.

ABSTRACT

Objective:To observe the effect of Changji'an prescription on intestinal permeability in diarrhea-predominant irritable bowel syndrome (IBS-D) rats and explore its mechanism for treatment of IBS-D. Method:Male SD neonatal rats were randomly divided into five groups:normal group, model group, pinaverium bromide group(0.018 g·kg-1), high-dose(33.48 g·kg-1) and low-dose (16.74 g·kg-1)Changji'an prescription groups. Except for the normal group, the IBS-D model was established by the combination of maternal and infant separation+acetic acid stimulation+restraint stress. After drug treatment, the ultrastructure of rat intestinal mucosa was observed by using transmission electron microscopy and the plasma D-lactate level was detected by enzyme-linked immunosorbent assay (ELISA). The expression levels of tight junction proteins Occludin and Claudin-1 were detected by Western blot. The mRNA expression levels of Occludin,Claudin-1 and zonula occluden(ZO)-1 were detected by real time polymerase chain reaction (Real-time PCR). Result:As compared with the normal group, the intestinal mucosal epithelial cells were damaged in IBS-D model group, and the microvilli arrangement was sparse and tight junction was widened, and some were not obvious,and the plasma D-lactate level in IBS-D rats was increased significantly (PPD-lactate level in pinaverium bromide group and high-dose Changji'an prescription group was significantly decreased (PD-lactate level in the low-dose group Changji'an prescription group had a tendency to decrease with no statistical difference. The mRNA and protein expression levels of Occludin and Claudin-1 and the mRNA expression of ZO-1 in the colon of rats in each administration group were higher than those in the model group (PConclusion:The therapeutic effect of Changji'an prescription on IBS-D may be achieved by improving the intestinal permeability.

ABSTRACT

Objective To observe the effect of myocardial transcription factor MRTF-A on myocardium inflammation and its mechanism.Methods Totally 30 rats were randomly divided into the sham,ischemia-reperfusion (myocardial ischemia 30 min and reperfusion 2 h),and MRTF-A groups(myocardial ischemia 30 min and reperfusion 2 h & Lentivirus infection MRTF-A) (n=10 each group).Serum myocardial enzyme activity was detected by biochemical analysis,myocardial infarct size detected by TTC,and degree of myocardial injury was measured by HE staining.The TLR4 and TRIF expression was analyzed by immunohistochemistry and qPCR.Results Compared with the sham group,the MRTF-A group significantly increased the activity of serum myocardial enzymes CK-MB and LDH (P<0.05).The infarct area of myocardial tissue was gray-white,and the infarct area was (54.31±3.07)% (P < 0.05).Myocardial fibrosis was disorder,myocardial cell was swollen and burst,and inflammatory cell infiltration was obvious.Protein and mRNA expressions of TRL4 and TRIF were significantly up-regulated (P<0.05).Compared with the ischemia-reperfusion group,the levels of CK-MB and LDH were significantly reduced after myocardial infection with MRTF-A (P<0.05).The myocardial infarction area was significantly reduced to (16.74±4.26)% (P< 0.05).The myocardial structure was nearly normal with mild edema.Protein and mRNA expression of TRL4 and TRIF decreased significantly (P<0.05).Conclusions The overexpression of transcription factor MRTF-A in myocardial cells alleviates the myocardial ischemia reperfusion injury by inhibiting the TLR4/TRIF signaling pathway and reducing the serum myocardial enzyme activity and myocardial damage.

Aim To investigate the effects of Periplane-ta americana extract Ento-A on the immune function in immunosuppressed mice . Methods Immunosup-pressed mouse model was induced by intraperitoneal injection of cyclophosphamide in KM mice .To evalu-ate the effects of Ento-A on the immune function in im-munosuppressed mice , neutral red method and MTT assay were used respectively to detect the effects of En-to-A on the phagocytosis of peritoneal macrophages and T cell proliferation rate in mice; with sheep red blood cell as immunogen , the effects of Ento-A on the pro-duction of serum hemolysin were evaluated;peripheral blood was tested and immune organ index calculated . Results Compared with model control group , the high, medium and low doses of Ento-A could improve the expression of serum hemolysin in immunosup-pressed mice ( P<0.01 ) , and increase the spleen in-dex(P<0.01) and thymus index (P>0.05), signifi-cantly increased the content of WBC ( P<0.01 ) , PLT ( P<0.01 ) , HGB ( P<0.01 ) , while the contents of RBC was on the rise , with no significant difference ( P>0.05 ) in peripheral blood , significantly enhanced phagocytic function and T lymphocyte proliferative abil-ity in a dose-dependent manner ( P<0.01 ) .Conclu-sion Ento-A can enhance the immune function of im-munosuppressed mice .

ABSTRACT

<p><b>OBJECTIVE</b>To study the association of vitamin D level with asthma control and pulmonary function in children with asthma.</p><p><b>METHODS</b>A total of 150 children with asthma were enrolled as observation group, and 55 healthy children were enrolled as control group. According to the level of asthma control, the children were divided into good control group, partial control group, and non-control group. Chemiluminescence microparticle immunoassay was used to measure the serum level of 25-hydroxyvitamin D [25(OH)D] for all groups. According to the level of 25(OH)D, the asthmatic children were divided into normal vitamin D group, vitamin D insufficiency group, and vitamin D deficiency group. Pulmonary function was measured for all asthmatic children.</p><p><b>RESULTS</b>The observation group had a significantly lower serum level of 25(OH)D than the control group (25± 7 ng/mL vs 29± 4 ng/mL; P<0.05). The normal vitamin D group had the highest asthma control rate, followed by the vitamin D insufficiency group and the vitamin D deficiency group (P<0.05). There was no significant difference in pulmonary function among the three groups (P>0.05).</p><p><b>CONCLUSIONS</b>Asthmatic children have a lower serum level of 25(OH)D than healthy children. The serum level of 25(OH)D is associated with the level of asthma control and has no association with pulmonary function.</p>

ABSTRACT

Objective To clarify the impairment mechanisms of acute hyperglycemia in the first-phase insulin se-cretion in mice. Methods The mouse model of acute glucose toxicity was established by glucose infusion through jugular vein catheterization. The glucose and insulin levels were assessed by IPGTT and OGTT in the mice of acute hyperglycemia and control groups. The histology of pancreatic islets was observed using HE staining and the insulin granules and other cy-toplasmic organelles were observed by electron microscopy. Results The mouse model of acute hyperglycemia was suc-cessfully established. The IPGTT showed that the blood glucose level was decreased by 87% ( 10. 3 ± 0. 33 mmol/L vs. 19. 3 ± 1. 66 mmol/L) at 15 min in the acute hyperglycemia group compared with the control group. The OGTT showed that the blood glucose level was decreased by 85% (9. 8 ± 0. 31 mmol/L vs. 18. 16 ± 1. 01 mmol/L) at 30 min in the acute hy-perglycemia group compared with the control group. However, the peak values of insulin secretion were delayed in both IPGTT and OGTT. Insulin levels at 2. 8 and 16. 7 mmol/L glucose stimulation in the acute hyperglycemia group was de-clined by 46% and 67% than the control group, respectively (P<0. 05). Residual insulin content in isletβcells was de-clined by 49% at 2. 8 mmol/L and 94% at 16. 7 mmol/L glucose infusion than the control group (P<0. 05). The histolo-gy showed irregular structure of pancreatic islets in the acute hyperglycemia group. The electron microscopy revealed that the amount of insulin granules was decreased, and more cytoplasmic vacuoles and swollen mitochondria were observed. Conclusions Acute intravenous glucose load decreases insulin content of isletβcells, leading to decrease and delay of the first-phase insulin secretion.

ABSTRACT

Objective To evaluate the effect of mindfulness intervention (MBI) on physical and mental symptoms in patients with diabetes mellitus. Methods Data of the relevant MBI in the physical and psychological symptoms were reviewed in patients with diabetes. The quasi-experimental studies or randomized controlled trials were used to retrieval from May 2016, and back into the study of references. According to the inclusion and exclusion criteria, the literature was screened and the quality was evaluated to extract the data. Review Manager 5.3 software was used for Meta-analysis. Results The total of 9 studies and 474 patients were included in this study. Patients were divided into MBI group (n=251) and control group (n=223). Meta-analysis showed that mindfulness therapy was effective in relieving depression, anxiety, stress and diabetes distress symptoms in diabetic patients compared with those of controls [MD=-5.35, 95%CI:-7.11--3.59;MD=-1.91, 95%CI:-3.04--0.79;MD=-4.65, 95%CI:-6.68--2.44;MD=-6.37, 95%CI:-10.84--1.90], the differences were statistical significance (P < 0.05). The improvement of the negative emotions was sustained long-term effect, and patients can benefit from it. The effects of MBI on the quality of life, whether short-term or long-term, were inconsistent and required further verification. The effects of glycemic control were generally better in MBI group than those of the control group. Conclusion Mindfulness intervention shows beneficial non-drug adjuvant therapy in reducing depression, anxiety, stress and diabetes related distress in patients with type 2 diabetes. The effects of MBI on improving the quality of life, blood glucose control need further verification.

ABSTRACT

<p><b>OBJECTIVE</b>To study the development of waist circumference (WC) and hip circumference (HC) in children and adolescents in Shandong province, and to provide scientific data for developing related reference values for screening central obesity in Chinese children and adolescents.</p><p><b>METHODS</b>Using data from 'Student physical fitness and health surveillance 2010 project' in Shandong province, a total of 42 275 students aged 7 - 18 years were selected to participate in this study. WC, HC, waist-to-hip ratio (WHR) and wrist-to-stature ratio (WSR) of subjects were measured. Comparison of data from the current study and other similar studies was made.</p><p><b>RESULTS</b>The mean values of WC increased with age, and boys all significantly higher than girls in all age categories (P < 0.01). There were two crosses on gender regarding the HC curves: before the age of 11 years, with mean HC higher in boys than in girls, but girls were higher than boys between the age of 12 to 14 while boys were also higher than girls after the age of 15 years. The 50(th) percentiles (P(50)) of WC of children and adolescents aged 7 to 18 years in Shandong appeared above the figures by 1.3 - 3.1 cm (boys) and 1.2 - 2.0 cm (girls) from 15 provinces in China as well as above the data from Hong Kong by 1.9 - 5.4 cm (boys) and 2.0 - 6.5 cm (girls), respectively. Overall, 20.20% of the boys and 16.57% of the girls had a WC of ≥ 90(th) percentile and 15.73% and 7.38% of the boys and girls had a WHtR of ≥ 0.5 which both showed significant differences between genders (P < 0.01).</p><p><b>CONCLUSION</b>Children and adolescents from Shandong province had a high level of WC.</p>

Objective To study the prevalence and characteristics of blood pressure (BP) among children and adolescents at normal weight but with abdominal obesity.Methods Using data from the ‘Student physical fitness and health surveillance 2010 project' in Shandong province,a total of 38 816 students aged 7-17 years were selected to participate in this study.Stature,body weight,waist circumference (WC),systolic blood pressure (SBP) and diastolic blood pressure (DBP) of these subjects were measured.Body weight status and abdominal obesity were defined by body mass index (BMI) and WC,respectively.Results In total,the proportions of thinness,normal weight,overweight and obesity defined by BMI were 5.37%,72.47%,12.92% and 9.24% respectively.5.86% of the children and adolescents with normal weight had abdominal obesity,with normal weighted girls (7.19%) having higher prevalence of abdominal obesity than boys (4.33%) (P<0.01).The Z-scores of SBP and DBP for both boys and girls were all significantly higher in the normal weight but with abdominal obesity groups than in both normal weight and WC groups (P<0.01).Conclusion Children and adolescents under normal weight but with abdominal obesity had higher BP level need to be identified and considered as high-risk individuals.Related intervention programs should also be targeted to this population.

ABSTRACT

Objective To explore the relationship between body composition index and blood pressure of children, and to provide bases for early prevention against adult diseases. Methods A total of 4326 children aged 7-12 participated in this study, with height, weight, skinfold thickness (SFT)and blood pressure(BP)of all subjects measured. Body fat percentage(BF%)were calculated by regression equation, fat mass index(FMI)and fat-free mass index(FFMI)were calculated according to following expressions: FMI(kg/m2)=BF% × weight/height2 and FFMI(kg/m2)=(weight - BF% × weight)/height2. Results Systolic blood pressure(SBP)and diastolic blood pressure(DBP)were positively correlated with FMI and FFMI in both boys and girls. Correlation coefficients between SBP, DBP and FMI were 0.432-0.531, 0.316-0.450 for boys, and 0.413-0.485, 0.345-0.421 for girls respectively and the correlation coefficients between SBP, DBP and FFMI were 0.214-0.366, 0.090-0.250 for boys, and 0.108-0.383, 0.063-0.214 for girls respectively. The coefficient between BP and FMI were larger than those between BP and FFMI. The mean values of FMI and FFMI of children with high BP were significantly higher than those normal children, especially showed in FMI. Conclusion In order to prevent hypertension among children,priority should be comcentrated on controlling body fat and preventing obesity.

ABSTRACT

<p><b>OBJECTIVE</b>To observe serum adiponectin, free fatty acid (FFA) profile and other glucose and lipid metabolic parameters in essential hypertensive patients with metabolic syndrome (HPMS) or without metabolic syndrome (HP).</p><p><b>METHODS</b>The serum adiponectin was measured with the radioimmunoassay, FFA profile measured with the gas chromatography and mass spectrometer in 72 HPMS, 56 HP and 43 normal control subjects.</p><p><b>RESULTS</b>Serum adiponectin were significantly lower in HPMS group than those in the HP and control group (P < 0.05 or P < 0.01). Serum total free fatty acid (TFA), unsaturated fatty acid (UFA, linoleic acid, oleic acid, arachidonic acid, docosahexaenoic acid, eicosatrienoic acid), polyunsaturated fatty acid (PUFA) and n6PUFA were significantly higher in HPMS group than those in the HP and control groups (P < 0.05 or P < 0.01). Adiponectin was negatively correlated with body mass index, waist circumferences, waist hip ratio, triglycerides (r = -0.222, -0.235, -0.179, -0.194, P < 0.01 or P < 0.05, respectively) and positively correlated with high-density lipoprotein cholesterol (r = 0.336, P < 0.01); TFA and PUFA were positively correlated with body mass index, waist circumferences, fasting blood glucose, mean arterial pressure (r = 0.241 and 0.280, 0.198 and 0.188, 0.226 and 0.298, 0.274 and 0.334, P < 0.01 or P < 0.05, respectively) in all subjects.</p><p><b>CONCLUSION</b>Our data suggest that changes in serum adiponectin, FFA and n6PUFA might promote the development of metabolic syndrome in essential hypertensive patients.</p>

ABSTRACT

Objective To explore the relationship between free fatty acid(FFA)composition and other glucose and lipid metabolic parameters in patients with metabolic syndrome(MS)in community.
