ABSTRACT

Objective To summary the application of computer aided three-dimensional visualization technique in different kinds of mandibular detects reconstruction with vascularized free fibular flap.Methods Five patients diagnosed of recurrent mandibular ameloblastoma underwent the virtually operation by software with a thin-slice CT scan,which determined shape and position of the free flap,and preoperative resin model and modules were designed and prefabricated.Then the titanium plates were bent precisely,and the surgical reconstruction proceeded exactly as expected from the modeling process.Results All operations were achieved successfully.The extent of the lesion was coincident with the preoperative CT results,and the vascularized free fibular flaps were harvested and implanted into the defect regions accurately.The mean distance was (0.23 ±2.10) mm compared with the virtual fibula.The mean time of operation was 4.4 hours.Following up the mean 12.8 months,all patients obtained the excellent functional and cosmetic outcomes.Conclusion The procedure of operation was simplified and the time was shortened with reconstruction exactly using computer aided three-dimensional visualization technique,and the clinical therapeutic efficacy of reconstruction of mandibular defect with vascularized free fibular flaps was improved notably.

ABSTRACT

To investigate the therapeutic effects and adverse reaction of acupuncture plus medication and single medication for post-stroke depression(PSD). Methods: In an open control study, 93 cases of the patients with depression were randomly allocated to three groups:30 cases in the acupuncture-medication group (AM), 30 cases in Prozac Group A (A), and 33 cases in Prozac Group B (B). The acupuncture-medication group was treated by oral administration of Prozac 20 mg/d plus acupuncture. Prozac Group A was treated by oral administration of Prozac 20 mg/d. Prozac Group B was treated by oral administration of Prozac 20-40 mg/d. All three groups were treated for 6 weeks. The therapeutic effects and adverse reaction were evaluated by Hamilton Depression Scale (HAMD) and Treatment Emergent Symptom Scale (TESS) respectively in the three groups. Results: The effective rate was 86.7% in the acupuncture-medication group, 63.3% in Prozac Group A and 87.9% in Prozac Group B.The therapeutic effect was better in the acupuncture-medication group than in Prozac Group A,and there was no significant difference in comparisonwith Prozac Group B (P>0.05). But the occurrence rate of drug adverse reaction was higher in Prozac Group B than in the acupuncture-medication group. Conclusion: Simultaneous application of acupuncture and medication is a therapeutic method with affirmative effect and less side effect for post-stroke depression.

ABSTRACT

Objective: To investigate if acupuncture can improve the curative effect of Western medicine on chronic prostatitis(CP). Methods: One hundred and ten patients with chronic prostatitis were randomly allocated to a treatment group of 80 cases and a control group of 30 cases. The clinical effects were evaluated according to cultures of bacteria in early-stream urine, midstream urine, prostate secretion(EPS) and after-massage urine(VB3). Results: Both the cure rate and the total efficacy rate were higher in the treatment group than in the control group(43/80 vs 10/30,P< 0.01; 75/80 vs 25/30, P< 0.05). In the treatment group there was no significant difference in curative effect between bacterial CP and nonbacterial CP(P > 0.05). In the control group the curative effect on bacterial CP was significantly superior to that on nonbacterial CP. Conclusion: Treatment with combined acupuncture and medicine is a good therapy for nonbacterial CP.

ABSTRACT

<p><b>OBJECTIVE</b>To study the adaptive alterations of elastic fibers in the bilaminar zone (BZ) of rabbit temporomandibular joint (TMJ) following disc displacement.</p><p><b>METHODS</b>Twenty-eight Japanese white rabbits were used in this study. The right temporomandibular joints of 20 of 28 rabbits were subjected to the surgical procedure of anterior disc displacement (ADD). Four rabbits in the surgical group were sacrificed at 2, 4, 6, 8 and 12 weeks after operation. Their temporomandibular joints were studied histochemically.</p><p><b>RESULTS</b>Elastic fibers were reduced in number and ran irregularly in the superior lamina of BZ from ADD rabbits. The jungly elastic fibers (EFs) could still be seen at 2 weeks after operation. At 4 weeks, the number of EFs decreased significantly; EFs lost their jungly arrangement and were shaped like rough dots, of which the arrangement and the lengths were different. Six weeks after operation, many EFs were replaced by distorted, uneven, non-oriented fine EFs, distributed unevenly and some thick or fine EFs that ran irregularly. The number of EFs decreased further and their arrangement was more deranged at 8 weeks. At 10 and 12 weeks, EFs in the superior lamina of BZ were similar to those at 8 weeks.</p><p><b>CONCLUSION</b>Our results show that EFs lost their function as well as their distribution and arrangement after disc displacement.</p>

ABSTRACT

Objective:To establishing model of anterior disc displacement(ADD) of temporomandibular joint(TMJ) by persistent inducement of traction through orbit. Methods:The inferior and posterior walls of orbit of right side in 8 rabbits were exposed. The anterior part of disc was vertically passed through using a suture needle with an elastic rubber. The double strands of the elastic rubber was stretched forward from 7 mm to 16 mm and fixed in the hole drilled in the zygomatic arch bellow inferior orbit. The rabbits were killed 1,2,4 and 8 weeks after operation respectively, then both TMJs of each were resected. The paraffin sections were made and stained with hematoxylin & eosin. Results:The mandible of the rabbits deviated to the left side with inclined attribution of incisors. The disc was displaced forward and the degree of ADD in the rabbits was similar. Conclusions:The disc anterior displacement of temporomandibular joint can be induced by persitent anterior traction of the disc.
