ABSTRACT

OBJECTIVE@#To investigate the toxicity management and efficacy evaluation of BCMA-chimeric antigen receptor T cells(CART) in the treatment of relapsed and refractory multiple myeloma (MM).@*METHODS@#The efficacy and adverse reactions of 21 patients with MM who received BCMA-CART treatment at the First Affiliated Hospital of Wenzhou Medical University from December 2017 to September 2020 were evaluated, and the efficacy assessment and survival analysis for high-risk patients and non-high-risk patients were evaluated.@*RESULTS@#After infusion of BCMA-CART cells in 21 MM patients, the number of effective cases was 17, of which the complete remission (sCR/CR) was 10, and the partial remission (VGPR/PR) was 7. The median OS time for all patients was 19.4 months, and the median PFS time was 7.9 months. The number of patients with extramedullary disease(EMD), high-risk genetics, and ISS stage Ⅲ were 5, 15 and 8, and the effective number was 3, 11 and 6, respectively. The treatment of 3 patients without high-risk factors was effective. The median OS and median PFS of patients with EMD were 14.2 and 2.5 months, respectively, which were shorter than those of patients without EMD (19.4 months and 8.9 months, respectively). The median OS and median PFS of patients with high-risk cytogenetic factors and ISS Ⅲ were not significantly different from those of non-high-risk patients. Cytokine release syndrane (CRS) occurred in 20 patients, of which 14 cases were Grade 1 CRS, while 6 were Grade 2, no CRS of Grade 3 or above occurred. IL-6 receptor inhibitors were used in 9 patients. All CRS were controlled effectively, and no patients had neurological toxicity.@*CONCLUSION@#BCMA-CART is a certain curative effect in the treatment of relapsed and refractory multiple myeloma, and the adverse reactions can be well controlled through close monitoring and timely treatment.

ABSTRACT

This experiment was to study the constituents of the roots of Viburnum setigerum through various column chromatographic techniques. Thirteen compounds were obtained and their structures were identified using chemical and spectroscopic methods as (7αH, 8'αH)-4, 4', 8α-trihydroxy-3, 3', 9-trimethoxy-7, 9'-epoxylignan (1), (7αH, 8'αH)-4, 4', 8α, 9-tetrahydroxy-3, 3'-dimethoxy-7, 9'-epoxylignan (2), alashinol G (3), alashinol F (4), (-)-secoisolariciresinol (5), (7R, 7'R, 8R, 8'S)-3, 3'-dimethoxy-7, 7'-epoxylignane -4, 4', 9, 9'-tetraol (6), (7αH, 8αH, 8'βH)-4, 4', 7'α, 9-tetrahydroxy-3, 3'-dimethoxy-7, 9'-epoxylignan (7), loganin (8), dihydroquercetin (9), protocatechuic acid (10), 4-hydroxy-3-methoxy-benzoic acid (11), adoxoside (12), and catechin (13). Compound 1 was a new compound. Compounds 3-7 and 11 were reported from the genus Viburnum for the first time. All compounds were separated from this plant for the first time.

ABSTRACT

Objective·To investigate the effects of insulin-like peptide 6 (Insl6) on renal fibrosis and calcification in unilateral ureteral obstruction (UUO) mice. Methods·Twenty-four SPF male mice with genotypic background of C57BL/6 were divided into Sham (n=8), UUO+saline (n=8) and UUO+Insl6 (n=8) groups randomly. Mice were sacrificed 10 days after operation and renal tissues of surgical side were obtained. Sirus red staining, Masson staining and alizarin red S staining were used to verify the level of collagen and calcium deposition. TGF-β1 expression was determined by Western blotting. Realtime-PCR was used for determining TGF-β1, BMP2, Col1a1, and Col2a1 mRNA expression. Results·Compared with sham group, fibrotic area especially collagen Ⅰ , calcium deposition, TGF-β1 protein, and TGF-β1, BMP2, Col1a1, and Col2a1 mRNA expression in UUO+saline group significantly increased (all P<0.05). As compared with UUO+saline group, fibrotic area especially collagen Ⅰ, calcium deposition, TGF-β1 protein, and TGF-β1, BMP2, Col1a1, and Col2a1 mRNA expression in UUO+Insl6 group significantly decreased (all P<0.05). Conclusion·Insl6 inhibits UUO-induced renal fibrosis and calcification, which may be related to regulation of TGF-β1, collagen Ⅰ , BMP2 and collagen Ⅱ expression levels.

ABSTRACT

Objective· To prepare nanocarriers capable of improving the immunogenicity of cholesteryl ester transfer protein (CETP) hapten. Methods · Prepare CETP polypeptide modified by thiol in Fmoc, then Sulfo-SMCC were used to prepare CETP peptide-ovalbumin (OVA) nanoparticle and CETP peptide - OVA molecule conjugates in a two-step reaction scheme. The particle size and zeta potential of nanovaccine were determined and the morphology was observed. New Zealand White rabbits were vaccinated by subcutaneous injection of vaccine and serum collected from rabbits was detected by ELISA assay for the analysis of antibodies, high density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterol (LDL-C). The rabbits were randomly allocated to the PBS group (n=3), traditional vaccine group (n=3), and nanovaccine group (n=3). Results · Nanovaccine targeting CETP were successfully synthesized, with about 70 nm in size and about -8.81 mV in zeta potential, and possessed homogeneous spherical shape under transmission electron microscopy. Compared with traditional vaccine group, rabbits in nanovaccine group got higher antibodies. Conclusion · Nanovaccine improve immunogenicity of CETP haptens, and stimulate experiment rabbits to produce higher antibodies.

ABSTRACT

The morbidity and mortality rates of lung cancer in Xuanwei city, Yunnan province, China are the highest in the global scope. Since 1980s, the studies conducted in this region supported by international health organizations have been focused on the epidemiology, pathogenesis and molecular biology of Xuanwei lung cancer. With the development of molecular detection techniques and the application of next-generation sequencing technique, great progress has been achieved in understanding the environmental risk factors which cause this disease and the improvement of diagnostic methods and therapeutic measures in Xuanwei city. This review surveys the recent development in research of lung cancer in this area from the perspectives of environmental risks, mutation and expression of cancer-related genes, and the diagnosis and drug treatment. Moreover, the positive role of next-generation sequencing technique in diagnosis and treatment of Xuanwei lung cancer will be discussed.

ABSTRACT

Objective To establish an HPLC method for simultaneous analysis of danshensu (DSS), protocatechuic acid (PA) and hydroxysafflor yellow A (HSYA) in plasma samples, and to study the pharmacokinetics of DSS, PA and HSYA in Danhong Injection in normal and cold-coagulation and blood-stasis model rats. Methods The cold-coagulation and blood-stasis rat models (n=6) were made by continuous stimulation with ice water for 20 days; another 6 normal rats served as controls. The concentrations of DSS, PA and HSYA in the plasma were determined by RP-HPLC (0. 2% formic acid water[A]-methanol [B], gradient elution, wavelength detection: 280 nm [0-40 min] and 402 nm [40-60 min]) at 2, 5, 10, 15, 20, 25, 30, 40, 50, 60, and 90 min after administration of Danhong Injection via the tail vein. The pharmacokinetic parameters were calculated with DAS 3. 0 software. Results DSS and PA had an open two compartment model and HSYA had an open three compartment model. Compared with the normal groups, the model group had significantly increased maximum plasma concentration (Cmax) of DSS, PA and HSYA, the distribution half-life time (t1/2a) of DSS, and elimination half-life time (t1/2a, t1/2γ), area under curve (AUC), and apparent volume (V) of PA and HSYA (P1/2a) of PA and HSYA and area under curve (AUC) of DSS(P<0. 05). Conclusion DSS has a lower distribution and bioavailability under the condition of cold-coagulation and blood-stasis, while PA and HSYA show a faster distribution, slower elimination, increased apparent volume and bioavailability, indicating a better clinical effect.

ABSTRACT

Compounds with serotonin reuptake inhibition/5-HT(1A) dual activity were used to build 3D pharmacophore model as a training molecules by Discover Studio. Based on the model, 8 novel aryl piperazine benzo[b][1,4] oxazine derivatives were designed and synthesized, and their structures were confirmed by 1H NMR and HR-MS. Biological evaluation illustrated that compounds VI(1) and VI(7) showed potent functional activities at both 5-HT transporter and 5-HT(1A) receptor, which can be used as lead compounds to guide future research of design and synthesis of potent novel compounds.

ABSTRACT

To compare the sampling errors from cluster or unequal probability sampling designs and to adopt the unequal probability sampling method to be used for death surveillance.Taking 107 areas from the county level in Shaanxi province as the sampling frame,a set of samples are drawn by equal probability cluster sampling and unequal probability designs methodologies.Sampling error and effect of each design are estimated according to their complex sample plans.Both the sampling errors depend on the sampling plan and the errors of equal probability in stratified cluster sampling appeares to be less than simple cluster sampling.The design effects of unequal probability stratified cluster sampling,such as πPS design,are slightly lower than those of equal probability stratified cluster sampling,but the unequal probability stratified cluster sampling can cover a wider scope of monitoring population.Conclusions:Results from the analysis of sampling data can not be conducted without consideration of the sampling plan when the sampling frame is finite and a given sampling plan and parameters,such as sampling proportion and population weights,are assigned in advance.Unequal probability cluster sampling designs seems to be more appropriate in selecting the national death surveillance sites since more available monitoring data can be obtained and having more weight in estimating the mortality for the whole province or the municipality to be selected.

ABSTRACT

To explore novel monoamine reuptake inhibitor with antidepressant activity, a series of substituted aryl alkanol piperidine derivatives were designed and synthesized. All of them were new compounds, and their structures were confirmed with 1H NMR and HR-MS. The results showed that compounds 4, 5 and 8 displayed strong 5-HT, NA and DA reuptake inhibiting activities in vitro. Among the tested compounds, 4, 5 and 13 exhibited potent antidepressant activities in the mice forced swimming test. Compounds 4 and 5 have potent antidepressant activities and are worth further development.

ABSTRACT

Objective To observe whether sodium arsenite(NaAsO2)can activate the expressions of hemeoxygenase-1(HO-1)of normal human liver cell line named Chang liver.Methods Chang liver cells were exposed to NaAsO2 at 10 μmoL/L,0(contml),2,6,12,24 h and at 0(control),5,10,25,50 μmol/L in 12 h, followed by the measuring of the expressions of HO-1 protein in ceUs with western blot.Results In 10 μmol/L groups Chang liver cells exposed for 6,12,24 h cultured in vitro,the expressions of HO-l protein(3.97±0.72, 12.92±2.98,23.29±3.82)was significantly higher than that of control(1.00±0.00),and compared with the control, the difference being statistically significant(F=85.83,P<0.01;t=-9.42,-8.95,-13.83,respectively,P< 0.05 or<0.01).In 12 h,5,10,25 and 50 μmol/L groups cultured in vitro,the expressions of HO-1(16.34±0.25, 7.75±0.39,7.93±0.14,12.48±0.35)was significantly higher than that of control(1.00±0.00).and compared with the control,the difference being statistically significant(F=85.83,P<0.01;t=-36.25,-30.19,-86.40, -56.40,respectively,all P<0.01).Conclusion Inorganic arsenic call induce the activation of HO-1,promote the expression of protein in a time-and dose-dependent manner.

<p><b>OBJECTIVE</b>To analyze the chemical constituents of Hedyotis diffusa.</p><p><b>METHODS</b>Column chromatographies were used to isolate and purify the chemical constituents of this plant, and their structures were identified by spectral analysis and physicochemical properties.</p><p><b>RESULTS AND CONCLUSION</b>Seven compounds were isolated and identified as p-coumaric acid (I), methyl-p-coumarate (II), 2-formyl-5- hydroxymethylfuran (III), quercetin (IV), kaempferol (V), beta-sitosterol(VI) and daucosterol(VII), respectively, among which the compounds II and III were isolated from Hedyotis diffusa for the first time.</p>

ABSTRACT

The study was conducted to assess the nutritional status and levels of body mass index (BMI, kg/m2) and to evaluate the geographical distribution of male youths of the Han nationality in China. In total, 60,773 male youths, aged 18-20 years, of the Han nationality, were categorized into underweight, normal-weight, overweight, and obesity according to the international adult BMI cut-offs. Different levels of nutritional status and BMI of male youths of the Han nationality were compared among different areas. The mean BMI for the whole country was 20.6 in urban areas and 20.0 in rural areas. BMI increased from 20.1 among 18-year old youths to 20.5 among 20-year old youths. The prevalence of underweight among the male youths was 21.6%, while the prevalence of overweight and obesity were 4.6% and 0.6% respectively. For urban youths, the prevalence of underweight, overweight, and obesity were 21.0%, 6.8%, and 1.1% respectively, while these were, respectively, 21.9%, 3.3%, and 0.3% for rural youths. The nutritional status of the male youths in North-China was at the highest level (21.1) among the six areas, and the prevalence of underweight, overweight, and obesity were 14.3%, 9.1%, and 1.4% respectively. The highest prevalence of underweight was 29.8% in the North-West region, and the lowest prevalence of overweight was 2.2% in the South-Middle region, while the lowest prevalence of obesity was 0.2% in the South-West region. The nutritional status of the male youths was significantly different among different areas. Underweight was still prevalent in all male youth groups. Nonetheless, overweight was more prevalent among urban youths than among rural youths and was more prevalent in the North region than in the South region.

@#ObjectiveTo explore the application of matching in medical scientific data classification and coding and establish the multi-classific groups and manage the datas effectively.MethodsMedical science data subsection coding rules were designed using matching method.ResultsMain classification had five codes, which standed for three grades. Phylum (the first grade) was letter code, subphylum (the second and third grades) was number code, great class (the fourth and fifth grades) was number code. In the process of designing matching classification, given classification and coding system were used.ConclusionThe merging between traditional classification and matching method is very importment in the process of specializing classification to multi-factor matching, so as to merge and organize the data information.

OBJECTIVE@#To establish a model based on artificial neural network in the prediction of nosocomial infection risk.@*METHODS@#Clinical data of 27,352 inpatients extracted from hospital information system were cleaned and coded, and the model of prediction in nosocomial infection risk was developed based on artificial neural network.@*RESULTS@#The structure of artificial neural network is {16-6-1}-BP, and the fit rate of prediction was 0.9891. The area under ROC curve was 0.986.@*CONCLUSION@#Artificial neural network model can be used as a tool for nosocomial infection forecasting, which can provide supplementary information for the diagnosis and control of nosocomial infection.

ABSTRACT

<p><b>OBJECTIVE</b>To understand and analyze the infection situation of Helicobacter pylori (H. pylori).</p><p><b>METHODS</b>Extensively reviewing Chinese literature collecting the related with electronic documents in combination with manual retrieve and using Meta-analysis to do a quantitative analysis.</p><p><b>RESULTS</b>Slight difference in the infection rate of H. pylori between men and women (95% CI: 0.0579-0.0963) was noticed. The infection rate of H. pylori in children whose parent was positive with infection of this bacteria was higher than that of children whose parent was negative (95% CI: 0.3378-0.5042).</p><p><b>CONCLUSION</b>The infection rate of H. pylori showed gender difference with nature of family aggregation. Epidemiological studies of H. pylori was comprehensive and involved many aspects. Further investigation needs to be focused on infection rate and other risk factors.</p>

ABSTRACT

The aim of the present study was to investigate the effect of ERK on 17beta-estradiol (E(2)) inhibition of vascular smooth muscle cell (VSMC) proliferation in rats after vascular injury. Common carotid artery balloon-injury (Inj) model was established in ovariectomized rats (OVX). Female SD rats were randomly divided into 4 groups: OVX, E(2)+OVX, OVX+Inj, and E(2)+OVX+Inj groups. The thickness of the vessels, the plasma content of NO, and the expression of ERK, phosphorylated ERK as well as eNOS protein were measured. The results showed that compared with OVX, the vessel wall was significantly thickened and the plasma content of NO was significantly decreased in OVX+Inj group. E(2) significantly decreased the vessel thickness but increased the plasma NO content after balloon injury. E(2) inhibited the expression of ERK, phosphorylated ERK and induced the eNOS expression. There is a positive correlation between plasma NO content and eNOS protein expression, while there is a negative correlation between plasma NO content and the thickness of vessel. The plasma NO content and the expression of ERK protein were negatively correlated. These results suggest that E(2) increases the vascular eNOS protein expression and NO release, leading to the inhibition of VSMC proliferation after balloon injury by inhibiting the ERK and phosphorylated ERK protein expression.

ABSTRACT

Clinical epidemiologic data and animal experimental studies regard estrogen as being protective against the development of cardiovascular diseases. The mechanisms by which estrogen affects the development of vascular diseases are not clear. Recent studies demonstrated that the cardiovascular protective effects of estrogen are closely related to nitric oxide (NO) pathway. Our previous study proved that estrogen inhibited the proliferation and oncogene expression of vascular smooth muscle cells (VSMCs) induced by endothlin 1 (ET-1) and serum,this effect was mediated by NO release. In the present study, we investigated the role of inducible nitric oxide synthase (iNOS) in the VSMCs cycle arrest induced by 17 beta-estradiol (E(2)). The effects of E(2) on iNOS activity and protein expression in cultured rat VSMCs and the influence of NOS inhibitor N(G)-nitro-L-arginine methylester (L-NAME) on the inhibitory effect of E(2) on cell cycle were investigated. NOS assay kit was used to measure the activity of iNOS and protein expression of iNOS was determined by Western-blot. Cell cycle analysis was accessed by flow cytometry. The results obtained showed that E(2) increased iNOS activity of VSMCs but not in a dose-dependent manner. E(2) 10 nmol/L increased the iNOS activity of VSMCs distinctly at two time points: 30 min and 12 h. These effects were significantly inhibited by estrogen receptor (ER) antagonist Tamoxifen (0.1 micromol/L) and NOS inhibitor L-NAME (1 micromol/L). E(2) increased iNOS protein expression of VSMCs in a dose-dependent manner. The effect of E(2) on iNOS protein expression of VSMCs started at 3 h, distinctly increased at 12 h and then decreased. Tamoxifen significantly inhibited the E(2)-induced iNOS protein expression of VSMCs. ET-1 increased cell percentage of S phase and G(2)+S/G(1). This effect was inhibited by E(2). L-NAME significantly attenuated the inhibitory effect of E(2) on cell cycle of VSMCs. The results suggest that E(2) induced G(1) arrest of VSMCs, which was associated with an increase in iNOS activity and protein expression of VSMCs. These effects were at least mediated by estrogen receptor partly.

BACKGROUND: To know whether the laryngeal mask airway (LMA) triggers a pharyngo-esophago- gastric reflex during general anesthesia, we compared the esophageal motility of patients with an LMA or endotracheal tube (ETT) in place. METHODS: Fifty patients (ASA I or II) scheduled for elective orthopedic surgery with general anesthesia were randomly allocated into LMA (n = 30) or ETT (n = 20) groups. The esophageal manometric inputs were recorded continuously using an ambulatory esophageal manometric recorder and divided into five perioperative phases (preanaesthesia, induction, operation, LMA or ETT rejection, and arousal phase). RESULTS: The peristaltic percent and number of contractions per minute were significantly decreased during induction, operation, LMA or ETT rejection and arousal phases compared with preanesthetic phases in both the LMA and ETT groups. However, there were no significant group differences in any corresponding perioperative phases. CONCLUSIONS: We suggest that during general anesthesia the use of a LMA does not provoke significantly different esophageal peristalsis compared with an ETT. Thus, the LMA is unlikely to potentiate gastric regurgitation and reflux during general anesthesia by stimulating the pharyngo-esophago-gastric reflex.
