ABSTRACT

Objective To explore the effect of virtual reality video games on gross motor skills of children with cerebral palsy.Methods The 56 patients with cerebral palsy treated in the Fifth Affiliated Hospital of Zhengzhou University from April 2015 to April 2017 were selected as the subjects.The age of the patients was (5.4 ± 1.2) years old (5-7 years old).According to the results of Gross Motor Function Classification System (GMFCS) assessment,all patients belonged to level Ⅰ-Ⅱ.All patients were randomly divided-into 2 groups:video-game group and conventional exercise group for 6 weeks of functional exercise.The evaluation criteria included the average exercise time,the 6-minute walk test (6MWT) and general motion function challenge module (GMFMS-CM) measurement and evaluation.Results The average exercise time of video games in video game group was (44.2 ± 6.1) minutes.The encouragement from the therapists in the intervention process was very important.The technical problems also had a relatively high proportion,and the technical guidance and training should be emphasized at the beginning of the study.The average score of the Likert scale showed that the practice program was suitable for the patients;GMFMS-CM in video game group was significantly higher [mean difference was 4.8,the quartile (IQP) was 4.68,Z =-2.025,P =0.035],while conventional exercise group showed an increasing trend which was not significantly different;6MWTshowed a significant increase in the mean level of video-game group (mean difference was 69.1 m,IQR was 40.2 m,Z =-2.108,P =0.045),conventional exercise group was significantly lower than the average level of subjects in video-game group and there was no significant change in the study period.Conclusion For children with cerebral palsy,routine functional rehabilitation exercise plus family-style virtual reality video game practice,on the one hand to improve patient compliance with the treatment,on the other hand can significantly improve their physical movement function,which is conducive for the patients' quality of life and self-care ability.

ABSTRACT

Objective:To explore influence of walking training on pulmonary function and blood glucose in aged pa‐tients with type 2 diabetes mellitus (T2DM) .Methods :A total of 163 T2DM patients treated in our department were selected .According to number table ,they were randomly divided into routine group (n= 80 ,performed DM diet and routine insulin therapy strictly ,and lived as usual) and intensive group (n=83 ,received intensive walking train‐ing based on strict DM diet and routine insulin therapy ,namely walking ≥10000 steps/d for eight weeks) .Pulmona‐ry function and blood glucose indexes were compared between two groups before and after intervention .Results:Compared with before treatment ,after treatment ,there was significant rise in pulmonary function and significant reductions in blood glucose indexes in intensive group , P0.05 .Conclusion:As a low intensity aerobic exercise ,walking training can effectively improve pulmonary function , glucose metabolism in aged patients with type 2 diabetes mellitus ,it is help to improve quality of life .

ABSTRACT

Aim To study whether Ophiopogonin D has an effect inhibitory on myocardial hypertrophy induced by AngiotensinⅡand its possible mechanism. Methods Rat myocardial cell line H9 c2 were cultured in vitro. The effect of Ophiopogonin D on cell vitality was tested by;H9 c2 cells were treated with AngⅡ 1μmol ·L-1 after 24h to induce the cardiac hypertrophy,then it was co-treated with different concentrations of Ophio-pogonin D were added for 24h. After above,the total protein content was detected by BCA method;Quantita-tive real-time PCR ( qRT-PCR ) technique was used to examine the expression of marker genes BNP and β-MHC mRNA ,which representing the function of hear-ing; Western blot was used method to detect the ex-pression of autophagy protein LC3 B and high-through-put screening technology was emptoyed to verify it. In addi-tion, the changes of mitochondrial membrane po-tential in H9c2 myocardial cell were also examined. Results The cell viability results showed that H9 c-2 cells exposed to different concentrations of AngⅡ had no significant effect on vitality compared with the con-trol group after 24 h,but high concentrations of Ophio-pogonin D ( 50 ~100μmol · L-1 ) could obviously in-hibit the cell activity. Ot-her experimental results showed that myocardial cells treated with AngⅡ for 24h could cause myocardial hypertrophy,which appar-ently displayed the growth level of specific hypertrophic gene mRNA expression and the marked increase of the total protein expression. As hypertrophy was activated by AngⅡ, cells autophagy would be significantly en-hanced at the same time, more-over, the mitochondrial membrane potential would be reduced. But the effects of Ophiopogonin D could significantly reverse those pathological changes. Conclusion All above experi-mental results indicate that Ophiopogonin D can in-hibitmyocardial hypertrophy induced by AngⅡand pos-sibly plays a critical role in cardiovascular protection.

ABSTRACT

Objective To explore the perioperative nursing method and effect of doing minimal invasive occlusion to children with congenital heart disease via subaxillary incision.Methods The data of 86 children patients with congenital heart disease underwent minimally invasive occlusion via right subaxillary incision were retrospectively analyzed and summarized nursing points including preoperative and postoperative mental nursing,postoperative postural care and sedative care,observing whether the occluder moved or detached and mechanical hemolysis,monitoring ECG and blood pressure,and preventing hemorrhage and thrombogenesis.Results One case of infracristal ventricular septal defect with frequent premature ventricular beat after occlusion was converted to thoracotomy surgery;one case of mixed atrial septal defect with heart rate increasing and dyspnea after 12 hours of occlusion was found occluder detached and immediately converted to remove the occluder and fix the atrial septal defect;The other children were successfully recovered and been discharged.Conclusions Full understanding and strengthened perioperative nursing care of complications of children with congenital heart disease after surgical minimally-invasive surgical closure is the key point to avoid the occurrence of fatal complications and ensure the children recover safely and smoothly.

ABSTRACT

Objective To investigate the effect of continuous health education on the control of glucose and the changes of behavior. Methods One hundred and twenty-six newly diagnosed type 2 diabetic patients were randomly divided into control group and observation group,each group included 63 patients. The control group just received health education in hospitalization period,and the observation group received continuous health education both in and after hospitalization. The control of glucose and the changes of behavior were evaluated 3 months and 1 year later. Results After 3 months and 1 year treatment,the FPG,2hPG and HbA1c were significantly decreased compared to pre-treatment both in control group and observation group(P<0. 05). The FPG,2hPG and HbA1c were significantly decreased in the ob-servation group compared to the control groups(P<0. 05). The behaviors contained that mastery of diabetic knowledge,control of food in-take,persist in physical exercise, rational administration, self-monitoring of blood glucose and regular reexamination were significantly im-proved both in control group and observation group compared to pre-treatment (P<0. 05),and the behaviors were significantly improved in the observation group compared to the control group (P<0. 05). Conclusion Compared to control group,observation group had more ad-vantages in the control of glucose and the changes of behavior in newly diagnosed type 2 diabetic patients.

ABSTRACT

OBJECTIVE To study the effect of the ethanol extract of Fructus Psoraleae(EEFP)on endogenous metabolites in rat urine based on metabolomics. METHODS Male SD rats were orally administered with EEFP at the doses of 0.54,1.08 and 1.62 g · kg-1,respectively,once a day for two consecutive weeks. Urine samples were collected for 12 h after the last administration. Data were acquired with the MassLynx software based on ultra-performance liquid chromatography quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS). The principal component analysis(PCA)and partial least squares-discriminant analysis(PLS-DA)were used to analyze the difference of endogenous metabolites in different groups,then putative biomarkers were found through the orthogonal partial least-squares-discriminant analysis(OPLS-DA),variable importance in the projection(VIP)and t test and their relative intensity were determined. RESULTS The results of PCA showed that samples of each group were clustered,all the groups were separated,and that the distance between the EEFP groups and the blank control group was increased in a dose-dependent manner. The relative contents of p-cresol glucuronide and galactose-beta-1,4-xylose were 40.0 ± 11.2,2.7 ± 2.6,16.8 ± 6.3 and 45.9 ± 16.4,32.6±22.1,8.0±8.3 in the EEFP 0.54,1.08 and 1.62 g·kg-1 groups,respectively,significantly lower than those of the control group,which were 107.0 ± 26.9 and 82.3 ± 13.6(P<0.01),respectively. The relative contents of 5-L-glutamyl-taurine,and gluconolactone were 22.4 ± 10.0,47.6 ± 19.1 and 138.2 ± 18.8,337.3±64.0 in EEFP 1.08 and 1.62 g·kg-1groups,respectively,significantly higher than those of the blank control group,which were 2.6±1.6 and 20.5±6.8,respectively(P<0.01). The relative content of D-pantothenoyl-L-cysteine was 74.2 ± 31.5 in the EEFP 1.62 g · kg-1 group,significantly higher than that in the blank control group(0.6±0.5)(P<0.01). As the dose of EEFP increased,D-pantothenoyl-L-cysteine,5-L-glutamyl-taurine,and gluconolactone had an upward trend(P<0.01),while galactose-beta-1,4-xylose and p-cresol glucuronide had a downward trend(P<0.01). CONCLUSION The two-week administration of EEFP has effect on the endogenous metabolites in urine. The substances identified are mainly related to energy metabolism,taurine,tyrosine and glucose metabolism.

OBJECTIVE@#To search the features and the treatment of vascular compressive syndrome caused by the facial, acoustic nerves.@*METHOD@#Ten cases of vascular compressive syndrome caused by the facial, acoustic nerves were included in the group,which were treated by microvascular decompression(MVD). Besides, the microanatomic relationship between the nerve and their adjacent vessel at the root exit zone (REZ) were observed under microscope or nasoendoscopy in MVD.@*RESULT@#Tinnitus, vertigo and facial spasm disappeared after MVD in 7 cases (70%), improved in 2 cases (20%), and relapse in 1 case (10%). All cases were found out vessels compressing at the root zone of the facial nerve and the auditory nerve.@*CONCLUSION@#The Clinical features of vascular compressive syndrome caused by the facial, acoustic nerves are facial spasm, tinnitus, and vertigo, for which microvascular decompression has a positive therapeutic effect as long as the diagnosis is correct.

ABSTRACT

ObjectiveReport an operation of off-pump occlusion of trans-thoracic minimal invasive surgery (OPOTTMIS) adopted on the therapy of 92 patients suffered with simple congenital heart diseases (CHD) of atrial septal defect ( ASD),ventricular septal defect ( VSD ) and patent ductus arteriosus (PDA) and summarize the experiences.Methods The clinical data of 92 CHD patients administrated with OPOTTMIS in our hospital during the time of July 2008 and July 2011 were analyzed retrospectively.Moreover,the relevant literatures were reviewed and the different kinds of treatments and their outcomes of CHD were compared.These patients were 3-56 years old and weighed 8.0-54.5 kg,with male and female 38 and 54 cases,including ASD 52 cases,VSD 26 cases and PDA 14 cases,respectively.Within the initial 3 months of postoperation,all the patients were followed up and managed to take the re-examination with electrocardiogram and echocardiogram one time for each month,and 3 months later,at least one time for every 3-6 months.Results91 cases were performed the OPTTMIS procedure successfully,1 child with sieve-like ASD transferred to open-heart surgery and 1 adult( 1.1% ) PDA died on the second day after operation for the pulmonary hypertension crisis.4 cases appeared mild residual shunt including 2 of ASD,1 of VSD and 1 of PDA.2 cases showed Ⅱ stage atrial ventricular block ( AVB ) within ASD and VSD,respectively.Also,post-operation hemothorax happened to 1 case ASD and 1 case VSD for the active bleeding of cardiac puncture site and the exploration and hemostatic operation were performed.91 (98.9%) patients were performed the OPOTTMIS operation successfully,the total complications within 72 hours was 6.5% (6/92),respectively.There were no heavy complications happened during peri-operation such as cardiac rupture,infective endocarditis,stroke,hemolytic and thrombus formation.90 patients outcomes of 3 months to 4 years follow-up with the ECG and UCG examination showed that there were no Ⅲ stage atrioventricular block,no obvious occluder shift and broken and no moderate cardiac valve regurgitation,no evident blood flow restriction of left ventricular outflow tract and descending aorta,only found mild residual shunt within 2 cases ( VSD and PDA each) and heart expansion compared to pre-operation within 2 PDA patients.However,all the patients'heart functions were in grade Ⅰ to grade Ⅱ according to NYHA standard.ConclusionOPOTTMIS is a safe,feasible and effective option on simple congenital heart diseases.

BACKGROUND: With wide application of biotechnological substitute materials, pelvic repair and reconstruction develop to a certain degree. Biomaterial patch is a major substitute for repairing injured pelvic fascia tissue, so it is widely used for pelvic reconstruction.OBJECTIVE: To investigate the feasibility, efficacy, and clinical value of biomaterial patch to pelvic reconstruction in the females.METHODS: Articles related to pelvic functional disorder, pelvic reconstruction, and application of patch implant were retrieved from PubMed (http://www.ncbi.nlm.nih.gov/PubMed) and (http://www.wanfangdata.com.cn) with the key words of "reconstruction of whole pelvic floor, mesh, synthetic mesh implants" in both Chinese and English between 1990 and 2008. Duplication studies were excluded. A total of 54 articles were initially retrieved, and 17 ones were included in the final analysis.RESULTS AND CONCLUSION:Pelvic organ prohpse, which was a major symptom of pelvic disorder in the females, caused by defect of pelvic supporting structure, injury, and functional disorder. Traditional operation could not solve fundamental question.At present, substitute materials for pelvic repair and reconstruction mainly include biomaterial patch (self-substitute materials, homogeneity substitute materials, and heterogeneity substitute materials) and artificial patch. All of them could substitute the injured pelvic fascia tissue; therefore, they were major substitute materials of pelvic tissue and widely used for pelvic reconstruction. Patch which was used for pelvic reconstruction realized the recovery of anatomic structure and caused functional recovery, with simple and easy processing. Additionally, patch application did not prolong operative time and cause complication, but induced well tolerance, security and reliability, and remarkable short-term effect on patients. However, the long-term efficacy should be further studied. The modified pelvic reconstruction is clinically valuable for patients with varying prolapsed sites.

ABSTRACT

Objective The purpose of this trial was to appraise the effects of both recognition-insight therapy and family education on rehabilitation of patients with schizophrenia. Methods 140 outpatients with schizophrenia were randomly divided into two groups (70 patients in each group). The family group was trained with recognion-insight therapy and family education while the control group only received services at outpatient department. During a three-year follow-up period, both groups were conformed with the schizophrenic diagnostic criteria of BPRS, CGI, SDSS and the self-made gauge. Results Both at the end of the 1st and the 3rd year, the family group was lower than the control group on scores of every gauge (P <0.01 ). On the other hand, the therapy compliance of the family group was significantly higher than the control group (P<0.01 ). Both relapse rates of the end of the Ist year and 3rd year (19.4%, 32.2%) were excessively lower than the control group (46.9%, 74.5%) (P<0.Ol ), while the family group's social function fine rate (73.1% , 62.7% ) was significantly higher than that of the control group (39.1% , 20.0%) (P<0.01). Conclusion The recognion-insight therapy and family education performed pivotal role in the rehabilitation of patients with schizophrenia.
