ABSTRACT

Objectives To evaluate the effectiveness of bedside noninvasive scoring system in diagnosis of coronary heart disease (CAD).MethodsSix hundred and twelve patients with suspected CAD in our hospital were enrolled (343 males and 269 females) from September 2008 to October 2010,with an average age of 55 ± 7 y.The detailed history was taken; physical examination,resting electrocardiogram,blood biochemistry,treadmill exercise test and/or 12 lead Holter monitoring,64 or 256 rows CT coronary artery imaging and coronary artery angiography were performed in all patients.The risk factors for CAD were screened by multiple questionnaire surveys with Delphi method.The risk factors were stratified according to the results of expert survey: heavy smoking,diabetes mellitus,typical angina,positive treadmill exercise test and positive Holter monitoring electrocardiogram were included in the highest risk factors with an integrated scores of 8 ; dyslipidemia of 3 items,hypertension complicated with left ventricular hypertrophy were high risk factors with an integrated scores of 6; males≥40 y,medium smoking,dyslipidemia of 2 items,pathoglycemia,heavy drinking,positive ECG and post-menopause females were moderate risk factors with integrated scores of 4; Low risk factors contain moderate drinking and dyslipidemia of item,were classified as low risk factors with an integrated scores of 2. The bedside noninvasive scoring system was evaluated in all patients and the results were compared with those from multi-slice spiral CT or coronary angiography.ResultsWhen integrated score ≥ 24 was set as the cut-off level for diagnosis of CAD,thesensitivity,specificity,positive predictive value and accuracy were 89.95%,85.63%,94.03%and 88.73% respectively.When integrated score≤ 14 was set as the exclusion criteria of CAD,the sensitivity,specificity,positive predictive value and accuracy were of 93.10%,82.86%,98.09% and 84.80% respectively.The accuracy was lower than that of multi-slice spiral CT or coronary angiography( P <0.05 ).ConclusionsThe bedside noninvasive scoring system is effective for preliminary diagnose of CAD,but need to be further improved.

ABSTRACT

Objective To investigate the effect of arsenic trioxide on human coronary smooth muscle cells(HCSMCs). Method After the subculture of HCASMCs, cells were divided into 4 groups: control group and three arsenic trioxide (4.0 μmol/L) groups as the arsenic agent co-cultured with cells for 12,24 and 48 h. Flow cytometry and TUNEL were used for counting the ntmnber of cell apoptosis. RT-PCR was used to detect the expression of E2F-1 mRNA and Western blot was used to measure the levels of Bcl-2 ond Bax. Results Arsenic trioxide inhibited the proliferation of HCASMCs. When arsenic trioxide was 4.0 μmol/L, the living cells were ( 8.44 ±0.10) × 105/mL,signifiantly than that of control group (16.44 ± 1.34) × 105/mL, P < 0.05. This inhibition of proliferation cycle was produced by affecting S and G2-M phase, which did not appear in the control group. Apoptotic cells increased from 16.0% ± 3.1% to 38.7% ± 2.7% (P < 0.05) detected by TUNEL. The arsenic agnet decreased the Bcl-2 level and increased Bax. The expression of E2F-1 mRNA was decreased in 4.0 μmol/L arsenic trioxide group. Conclusions Arsenic trioxide exerts the apoptotic effect on human coronary smooth muscle cells.

ABSTRACT

Objective To investigate the apoptotic effects of arsenic trioxide on E2F-1 and EMAP-Ⅱ of hu-man coronary artery smooth muscle cells (HCASMCs). Method HCASMCs proliferated after culturing cells.Cells were divided into 4 groups: control group and arsenic trioxide (4.0μmol/L) 12 h group, 24 h group and 48 h group. Flow cytometry was used for counting number of apoptotic cells, RT-PCR was used for detecting the ex-pression of E2F-1 mRNA and Western blot was employed to get the level of EMAP-Ⅱ. Results Arsenic trioxide inhibited the proliferation of HCASMCs (living cells in 4.0 μmol/L arsenic trioxide were (8.44±0.10)×10~5/mL vs. control (16.44±1.34)×10~5/m (P <0.05), and the 4.0μmol/L arsenic trioxide inhibited proliferation cy-cle via affecting S and G2M phases, while those were not found in control group. E2F-1 mRNA expression was de-creased in 4.0 μmol/L arsenic trioxide group. Western blot test showed EMAP- Ⅱ level was also decreased in 4.0 μmol/L arsenic trioxide group. Conclusions Arsenic trioxide has apoptotic effect on smooth muscle cells of hu-man coronary artery and decrease the expression of E2F-1 mRNA and the level of EMAP- Ⅱ.

ABSTRACT

Objective To investigate the role of interferon ?on smooth muscle cells proliferation and migration after balloon injury. Methods Animal model of rabbit iliac artery balloon injury was set up, smooth muscle cells derived from injured artery were cultured. Smooth muscle cells were divided into four groups ( control, TGF ? 1, IFN ?, TGF ? 1 and IFN ?).Cells from each group were treated with medium or TGF ? 1 (10 ng/ml) and/or IFN ?(500 u/ml) for 72 h separately. Smooth muscle cell proliferation was determined by cell count and MTT, migration of smooth muscle cells was also detected. Matrix metalloproteinase 2 was also detected by zymography. ABSTRACT

Objective The function of regional myocardium of patients with myocardial infarction(MI) treated by intra-coronary stenting was determined with quantitative tissue velocity imaging(QTVI).Methods Twenty healthy controls and 24 patients with acute anterior myocardial infarction underwent QTVI with high-frame rates. The myocardial tissue velocity imaging in the apical 4-chamber view and apical 2-chamber view and apical longitude view were digitized and stored. Off-line ventricular walls regional velocity profiles along long axis were obtained, and systolic peak velocities (Vs) and early and late diastolic peak velocities (V E and V A) of 12 regional segments were measured.Results Under normal and ischemic conditions, 6 ventricular walls showed significant basal-apical myocardial velocity in the long-axis reduction in V S and V E and V A were significantly reduced in the abnormal segments in patients with MI. Conclusion QTVI can be used to analyze regional myocardial motion in the long-axis quantitatively and synchronously;intra-coronary stenting can obviously improve regional myocardial function of left ventricle,especially the diastolic function.

ABSTRACT

Objective To explore the feasibility of right ventricular outflow tract index in the evaluation on right ventricular systolic function and its relationship to other right ventricular functional indices generally accepted in clinical practice. Methods Sixty-one patients with heart failure were randomly selected and divided into elevated right side pressure group and normal right side pressure group by pressure gradient ≥35 mm Hg aquired from tricuspid regurgitation velocity. Twenty-four healthy adults were selected as control group. All the people experienced transthoracic echocardiography. Right ventricular longitudinal excursion(RVLX), right ventricular ejection fraction(RVEF), right ventricular outflow-tract fractional shortening(RVOTFS) and right ventricular ejection time(RVET) were measured, and their differences were compared between healthy adults and heart failure patients as well as between elevated right side pressure group and normal right side pressure group. Relationships of RVOTFS with other right ventricular functional indices were also analyzed. The present study were to identify a dose-response relationship and study the histologic correlates. Group A : The rabbit's iliac segments of normal control,were performed in radiofrequency balloon angioplasty.
