ABSTRACT

Objective@#To investigate the trends in incidence of malignant tumors in Yuyao City, Zhejiang Province from 2011 to 2018, so as to provide insights into formulation of the malignant tumor control strategy.@*Methods@#Data pertaining to the incidence of malignant tumor in Yuyao City from 2011 to 2018 were captured from the Chronic Disease Surveillance Module of Ningbo Municipal Digital Disease Control and Prevention Platform. The crude incidence, Chinese population-standardized incidence, global population-standardized incidence, cumulative incidence (0 to 74 years of age), and truncated age-standardized incidence (35 to 64 years of age) of malignant tumors were estimated, and the trends in incidence of malignant tumors were analyzed in Yuyao City from 2011 to 2018 using average annual percentage change (AAPC).@*Results@#The crude incidence, Chinese population-standardized incidence, global population-standardized incidence, cumulative incidence (0 to 74 years of age), and truncated age-standardized incidence (35 to 64 years of age) of malignant tumors were 433.78/105, 289.37/105, 224.22/105, 25.83% and 83.34/105 in Yuyao City from 2011 to 2018, with AAPC of 5.90%, 3.31%, 3.31%, 2.84% and 4.24%, respectively (all P<0.05), and the incidence of malignant tumors appeared an overall tendency towards a rise. The incidence of malignant tumors increased with age in Yuyao City from 2011 to 2018, and the incidence of malignant tumors was significantly higher in women than in men at ages of 20 to 54 years, while higher incidence was seen in men than in women at ages of 55 years and older. The ten most common malignant tumors included lung cancer, gastric cancer, colorectal cancer, liver cancer, breast cancer, esophageal cancer, thyroid cancer, pancreatic cancer, prostate cancer and cervical cancer, accounting for 78.84% of all malignancies, and the the incidence of lung cancer, colorectal cancer, breast cancer, thyroid cancer, pancreatic cancer and prostate cancer appeared a tendency towards a rise year by year, with AAPC of 8.37%, 5.36%, 6.42%, 24.81%, 5.49% and 18.78%, respectively (all P<0.05). @*Conclusions@#The of malignant tumors appeared a tendency towards a rise in Yuyao City from 2011 to 2018, and lung cancer, thyroid cancer, specific female cancers, male gastrointestinal and prostate cancers should be given a high priority for cancer control in Yuyao City in the future. Early screening and early diagnosis of cancers should be facilitated among high-risk populations.

ABSTRACT

With the increasing incidence of diabetes in China, the related complications also show a trend of " spreading" , and diabetes foot (DF) is one of the most serious complications of diabetes. The arterial disease of lower limbs in patients with DF are relatively complex, and the recovery of arterial blood supply of lower limbs is the key to improve the quality of life and limb preservation rate of patients with DF. At present, endovascular therapy is the first choice for the treatment of diabetic foot ischemic lesions in China because of its minimally invasive and quick recovery. In order to treat DF in a better and more standardized way and make patients benefit more, this paper summarized the strategies of endovascular treatment for DF ischemia by combining the objectives of DF treatment and the principles of endovascular treatment.

ABSTRACT

Objective:To explore the feasibility and efficacy of subpatellar artery balloon molding in the treatment of diabetic foot ulcer caused by arterial ischemia.Methods:The clinical data of patients with diabetic foot ulcer caused by subpatellar artery disease treated in Xuanwu Hospital of Capital Medical University from December 2020 to April 2022 were retrospectively analyzed. Among them, 29 patients received medical balloon dilatation (drug balloon group) and 30 patients received balloon dilatation alone (simple balloon group). The improvement of lower limb ischemia at 3 and 6 months after surgery was analyzed in the two groups. The observation indicators included case-fatality rate, limb preservation rate, ulcer healing, Rutherford grading and pain score.Results:There was no significant difference in preoperative Rutherford grading between the two groups ( P>0.05). Three and six months after operation, the Rutherford grading in both groups was significantly improved compared with that before surgery (all P<0.05), and there was no statistical significance between the two groups ( P>0.05). There was no significant difference in preoperative pain scores between the two groups ( P>0.05). The pain scores of both groups were significantly decreased 3 and 6 months after surgery ( P<0.05), and there was no statistical significance between the two groups ( P>0.05). Three and six months after surgery, the wound ulcer healing rate in the drug balloon group was higher than that in the simple balloon group [51.7%(15/29) vs 43.3%(13/30), P=0.519; 86.2%(25/29) vs 50.0%(15/30), P=0.002]. There was no death or amputation in the two groups 3 and 6 months after surgery. Conclusions:Balloon dilatation can improve severe limb ischemia of diabetic foot. Compared with balloon dilatation alone, drug balloon dilatation is more beneficial to the healing of ulcer wounds in diabetic limb ischemia patients.

ABSTRACT

Objective:To investigate the clinical characteristics and surgical treatment experience of carotid body tumor (CBT).Methods:The clinical data of 12 patients with CBT admitted to the Department of Vascular Surgery, Xuanwu Hospital, Capital Medical University from March 2013 to August 2020 were analyzed retrospectively. Among them, there were 6 males and 6 females, aged 31-83 years, with a median age of 57 years. Among the 12 patients, 2 patients were not treated surgically. The body mass index (BMI), tumor side, maximum diameter of tumor, and tumor classification, operation time, intraoperative bleeding volume, postoperative drainage volume and time, postoperative hospital stay and postoperative complications of patients undergoing surgery were recorded.Results:BMI of the 12 patients was 17.19-29.07 kg/m 2, with an average of (24.05±3.95) kg/m 2. Among the 12 patients, there were 4 tumors on the left side, 6 tumors on the right side and 2 patients had bilateral tumors. The maximum diameter of the tumor was 1.7-8.7 cm, with an average of (4.05 ± 1.89) cm. Among the 2 patients with bilateral tumors, 1 patient underwent staged resection within 9 months and 1 patient only removed the larger tumor. A total of 10 patients underwent surgical resection. All excised tumors were confirmed histopathologically to be paraganglioma. The average operation time and the amount of bleeding was(164.73 ± 74.39)min and 341.82 mL respectively. The drainage time was 1-3 d, with an average of (1.73 ± 0.65) d. The cumulative drainage volume was 22-237 mL, with an average of (77.18 ± 57.47) mL. Classification of 11 surgically resected tumors: 3 patients (3/11, 27.3%) were Shamblin Ⅰ, 7 patients (7/11, 63.6%) were Shamblin Ⅱ and 1 patient (1/11, 9.1%) were Shamblin Ⅲ. There were 1 patient of hematoma and 1 patient of acute cerebral infarction after operation. One patient with decrease in muscle strength of right limb, other surgical patients complained no complications such as stroke and cranial nerve injury when discharged. Patients undergoing surgery were hospitalized for 8-20 days, with an average of (13.36 ± 3.61) d. Conclusions:CBT is a rare paraganglioma in clinic. Surgical resection is an effective method to treat CBT. Careful operation should be carried out to avoid serious complications such as wound hematoma, cranial nerve injury and ischemic stroke.

ABSTRACT

Objective:To investigate the clinical characteristics as well as short-term and long-term prognostic factors of patients with Stanford type B aortic dissection (TBAD) with hypertension.Methods:Patients with TBAD who received thoracic endovascular aortic repair (TEVAR) admitted to Xiangyang Central Hospital from January 2014 to December 2018 were enrolled. The baseline data of patients admitted to the hospital were collected through the case management system, including gender, age, underlying diseases (hypertension, diabetes, coronary heart disease), smoking history, drinking history, duration of pain, vital signs at admission [heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP)], laboratory results [white blood cell count (WBC), platelet count (PLT), neutrophil/lymphocyte ratio (NLR), serum creatinine (SCr), C-reactive protein (CRP), D-dimer, ascending aorta diameter], etc. The clinical characteristics of TBAD patients with hypertension were analyzed. Logistic regression model and Cox proportional risk model were used to analyze the impact of hypertension on the short-term and long-term all-cause deaths after TEVAR in TBAD patients.Results:Among 227 TBAD patients, 160 cases (70.5%) were complicated with hypertension, while 67 cases (29.5%) were not. The average age, the proportion of diabetes and coronary heart disease, and the level of SBP, DBP and SCr at admission of TBAD patients with hypertension were higher than those of TBAD without hypertension [age (years old): 53.1±11.9 vs. 42.8±14.1, combined with diabetes: 8.8% vs. 1.5%, combined with coronary heart disease: 6.3% vs. 0%, SBP (mmHg, 1 mmHg = 0.133 kPa): 147.9±18.1 vs. 136.9±15.2, DBP (mmHg): 93.9±11.9 vs. 89.1±13.8, SCr (μmol/L): 97.8±25.4 vs. 89.8±23.6, all P < 0.05]. The short-term mortality of TBAD with hypertension group was significantly higher than that of TBAD without hypertension group [6.3% (10/160) vs. 0% (0/67), χ2 = 4.386, P = 0.036]. 227 patients with TBAD were followed up for 3-66 months, with a median follow-up time of 25 months. There was no significant difference in long-term mortality between TBAD patients with and without hypertensive during discharge follow-up [13.1% (21/160) vs. 9.0% (6/67), χ2 = 0.784, P = 0.376]. Further multivariate Logistic regression analysis and Cox regression analysis did not indicate that hypertension was an independent risk factor for short-term and long-term death in TBAD patients [odds ratio ( OR) and 95% confidence interval (95% CI) were 13.477 (0.541-330.215), 1.012 (0.990-1.035), both P > 0.05]. Age and HR were independent risk factors for the short-term mortality of TBAD patients [ OR and 95% CI were 15.287 (1.051-226.415), 0.026 (0.002-0.840), both P < 0.05]. Age, PLT and D-dimer were independent risk factors for the long-term mortality of TBAD patients [ OR and 95% CI were 1.808 (1.205-2.711), 0.555 (0.333-0.924), 1.482 (1.035-2.122), respectively, all P < 0.05]. Conclusions:The TBAD patients with hypertension have older age, high rates of diabetes or coronary heart disease. However, hypertension is not an independent risk factor for short-term and long-term mortality in TBAD patients.

ABSTRACT

The clinical data of 4 patients with peripheral vascular rupture admitted in the Hetian Regional People′s Hospital from March 2012 to October 2017 were retrospectively analyzed. The 4 patients were all males, aged 17-59 years. Among them, 2 cases caused by firearm injury, 1 case was caused by a car accident and 1 case was caused by a sharp injury; 2 cases were incompletely ruptured in femoral artery, 1 case was completely ruptured in popliteal artery, and 1 case was completely ruptured in axillary artery. All patients underwent successful reconstructive surgical procedures, and the skin temperature, skin color, and arterial pulse of the affected limb returned to normal after operation. The followed-up time was 28-95 months, with an average of 47.8 months. Blood flow in the reconstructed vessel was unobstructed. There were no complications. It is indicates that for peripheral vascular rupture if the diagnose and treatment are timely, the satisfactory results can be achieved.

ABSTRACT

Objective:To explore the method and effect of endovascular treatment to innominate artery stenosis or occlusion.Methods:The data of 11 patients with stenosis or occlusion of innominate artery from January 2014 to November 2019 at Xuanwu Hospital of Capital Medical University were collected. All patients received endovascular treatment. We summarized the changes of clinical symptoms, surgical methods, perioperative complications, stent patency, and analyzed the changes in systolic blood pressure and peak blood flow velocity on the involving side.Results:All 11 patients underwent endovascular treatment. The surgical technique success rate was 100%. All patients were followed up. The follow-up time was 4-69 months, with an average of (30.1±23.4)months. 2 patients used cerebral umbrella during the operation. 1 patient was performed ipsilateral carotid endarterectomy, 1 patient underwent contralateral carotid stent implantation, 1 patient was diagnosed as severe stenosis of the innominate artery and left common carotid artery, and an innominate artery stent implantation was performed at one stage, left common carotid artery stent implantation was performed after half a year. We done operation from the femoral artery puncture approach (6 patients), brachial artery puncture approach (2 patients), axillary artery and femoral artery puncture approach (1 patients), and right common carotid artery and the femoral artery puncture approach (2 patients). 3 patients had in-stent restenosis at 6, 7and 12 months after stenting, respectively. 1 patient underwent balloon dilatation, and 2 patients underwent re-stent implantation. We have not do further intervention to 1 case of in-stent occlusion occurred 14 months after the stenting, for the clinical symptoms did not improve significantly. The clinical re-intervention rate in this group was 3/11, and the primary patency rate was 7/11. The secondary patency rate was 10/11. The symptoms of 10 patients were relieved and the weakness of right upper extremity was not significantly changed in 1 patient. No puncture point complications occurred in all patients, and no cerebral infarction occurred during the perioperative period. There were statistically significant differences in systolic blood pressure, blood pressure difference and peak blood flow velocity before and after the operation ( P<0.05). Conclusions:Endovascular treatment of innominate arterial stenosis or occlusion was safe and effective, and the appropriate surgical approach and plan should be selected according to the lesion characteristics and the whole body conditions.

Objective To investigate the effect of endovascular treatment of cerebral infarction with right aortic arch and Kommerell's diverticulum.Methods Retrospective analysis was done to assess the treatment effect of a case from vascular surgery,Xuanwu Hospital of Capital Medical University.This case was diagnosed as cerebral infarction with right aortic arch and Kommerell's diverticulum.Results One month after the treatment of cerebral infarction,we successfully used the thoracic aortic stent to isolate the Kommerell's diverticulum.There were no operative complications occurred.The stent had good shape and smooth blood flow was seen in the stent.Conclusions Endovascular treatment to cerebral infarction with right aortic arch and Kommerell's diverticulum is safe and feasible,and the clinical outcome requires further long-term follow-up.

ABSTRACT

Re-use of contrast agent during endovascular treatment in patients with contrast agent allergy may lead to severe allergic reaction.For patients with localized stenosis of external iliac artery,endovascular therapy (stenting) is the first choice.Whether patients with severe iliac artery stenosis with contrast allergy can be treated with endovascular therapy is unknowed.A case of iliac artery stenting without contrast agent is reported in this paper.The successful implementation of this operation requires adequate preoperative and intraoperative preparation,and the accurate determination of the stenosis site and the presence or absence of residual stenosis during the operation.

Objective To study the effect of warfarin individualized drug regimen based on gene detection on anticoagulation in patients with acute deep vein thrombosis (DVT).Methods From January 2016 to June 2017,62 patients with DVT diagnosed for the first time in vascular surgery of the Affiliated Hospital of Jining Medical College were analyzed retrospectively,including 33 in the individualized group and 29 in the experience group.All patients were diagnosed by Color Doppler Ultrasound (CDUS).Gender,age,the results of the initial international standardized ratio (INR) measurement,the time from the first dose to the INR standard,and whether the INR exceeds the standard in the process of increasing the dose were collected.Results There was no significant difference in sex,age and initial INR between the two groups (P > 0.05).The time of reaching the standard in the individualized group was significantly lower than that in the experience group [(13.8 ± 6.8) d vs (17.6 ± 7.5) d,P < 0.05];There was no significant difference in INR between the two groups (P ± 0.377).Conclusions For the patients with DVT for the first time,the individualized drug regimen of warfarin based on the guidance of gene detection can shorten the time of reaching the standard without increasing the risk of bleeding.

Objective To investigate the clinical significance of antegrade lower limb venography in varicose veins of lower limbs.Methods The data of 230 cases of varicose veins of lower limb in Department of Vascular Surgery,Xiangyang Central Hospital from June 2018 to August 2019 were examined by antegrade lower limb venography.All patients were divided into simple varicose veins group (group A,67 cases) and secondary varicose veins group (group B,63 cases) according to the patency of deep veins.The data of sex,limb side,swelling and ulcer were analyzed respectively.Results There was no significant difference in the results of sex between the two groups (57.5% vs 63.5%,P > 0.05).The incidence of left side in group A was lower than that in group B (59.3% vs 76.2%,P < 0.05).And the incidence of swelling of lower extremities and ulcer in boot area in group A was lower than that in group B (31.1% vs 49.2%,P<0.05;18.6% vs31.7%,P<0.05 respectively).Conclusions Combined with swelling of lower extremities,ulcer in boot area or the varicose veins in left side had a higher incidence of secondary varicose veins.Anterogrede venography of lower extremities can accurately distinguish the classification of varicose veins of lower extremities and improve the diagnostic coincidence rate,which has significant clinical significance.

About 25%-40% of ischemic stroke is in the posterior circulation,in which 20% of posterior circulation stroke is caused by vertebral artery stenosis.Patients with symptomatic vertebral artery stenosis have a high risk of recurrent stroke.At present,the treatments of vertebral artery stenosis include medication,open surgery and interventional therapy.In this paper,the endovascular treatment of vertebral artery stenosis is reviewed,especially the progress of endovascular treatment is expounded,in order to promote the development of its treatment.

However, lower patency rate, in-stent restenosis, stent fracture become critical problems in clinical practice. With the development of endovascular techniques, debulking atherectomy can effectively solve these problems via removing plaque and enlarging lumen capacity. Among debulking atherectomy techniques, laser ablation and directional atherectomy are the main methods in treating arteriosclerosis obliterans. This article reviews the applications and the progresses of the two methods.

In this paper, the endovascular treatment of vertebral artery stenosis is reviewed, especially the progress of endovascular treatment is expounded, in order to promote the development of its treatment.
