ABSTRACT

Hereditary leiomyomatosis and renal cell cancer (HLRCC) syndrome is a rare hereditary disease and characterized by cutaneous leiomyoma, uterine leiomyoma and/or renal cell carcinoma, but rarely associated with vena cava embolism. We treated 1 case of HLRCC syndrome patients with inferior vena cava tumor emboli (Mayo grade Ⅳ), confirmed after genetic testing, the patient and her family refused further treatment. The patient died after two months of follow-up after discharge.

ABSTRACT

Metastatic seminal vesicle carcinoma is rare in clinic.The case of renal clear cell carcinoma metastasis to the seminal vesicle gland is more rare,which is difficult to be identified with bladder,prostate and colorectal tumors.In November 2017,1 case of renal clear cell carcinoma metastasis to the left side of the seminal vesicle gland was admitted in our hospital.Laparoscopy seminal vesicle metastasis tumor resection was performed.Regular bladder irrigation and sunitib oral intake were conducted.No evidence of local recurrence was found within 19 months follow-up.

ABSTRACT

Objective:To explore the effect of constructive therapeutic communication on preoperative anxiety of rectal cancer patients undergoing da Vinci robot surgery.Methods:Patients with rectal cancer who were hospitalized in gastrointestinal surgery ward from January 2016 to December 2018 in the hospital and were identified as anxious patients after the anxiety self-assessment were selected for study. The patients were numbered according to the order of entry, the odd number was control group, the even number was experimental group. Finally, control group was 48 cases, and the experimental group was 46 cases. The control group received routine nursing measures and the experimental group received constructive therapeutic communication intervention based on it. The changes of anxiety levels before and after intervention were evaluated by Self-rating Anxiety Scale(SAS) and compared between the two groups.Results:Before the intervention, the SAS score of the experimental group was 54.15±4.92, and the control group was 54.17±4.81. There was no statistically significant difference between the two groups ( t value was 0.014, P>0.05). After the intervention, the anxiety score in the experimental group was 45.39±5.82, and the control group was 51.81±4.96. The difference between the two groups was statistically significant ( t value was -5.761, P<0.05). Conclusions:The constructive therapeutic communication can effectively reduce anxiety of on preoperative anxiety of rectal cancer patients undergoing da Vinci robot surgery, and is superior to conventional nursing measures. Constructive therapeutic communication can effectively manage the adverse emotions of patients, promote the establishment and use of positive coping methods, and improve the compliance of treatment. It is worth clinical application.

ABSTRACT

Objective To evaluate the diagnostic value of magnetic resonance spectroscopy analysis in patients with prostate specific antigen (PSA) abnormal prostate disease. Methods The patients who had abnormal PSA from January 2017 to December 2018 in Weifang People′s Hospital were selected. Patients with prostate puncture indications were as research target. A total of 137 patients including 44 patients with prostate cancer and 93 patients with benign prostatic hyperplasia were diagnosed by puncture. All patients underwent magnetic resonance scan and enhancement and spectroscopy before surgery. The comparative value of magnetic resonance scan + enhancement, magnetic resonance spectroscopy and joint examination was obtained. Results Sensitivity of magnetic resonance imaging and enhancement was 77.3% (34/44), specificity was 86.0% (80/93), and accuracy was 83.2% (114/137). Sensitivity of magnetic resonance spectroscopy was 52.3% (23/44), specificity was 77.4% (72/93), and accuracy was 69.3% (95/137). The sensitivity of the combined application was 90.9% (40/44), specificity was 91.4% (85/93), and accuracy was 91.2% (125/137). Conclusions The application of magnetic resonance spectroscopy can increase the sensitivity, specificity and accuracy of magnetic resonance plain scan and intensive examination for diagnosis of prostate cancer.

ABSTRACT

Objective To determine the utility of musculoskeletal ultrasound imaging to support physical findings in different stages of recovery and in recommending treatment regimens.Methods Forty-three hemiplegic stroke survivors with shoulder pain were enrolled.Physical examinations were performed 3 and 6 months after the stroke using a visual analogue scale (VAS),a modified Ashworth scale,measurements of passive range of motion and Brunnstrom staging.This was coupled with sonography of the long head of the biceps,the subscapularis tendon,the supraspinatus tendon,the infraspinatus tendon,the acromioclavicular joint and the subacromial-subdeltoid bursa.Results Three months after the stroke the average VAS score correlated negatively with the average Brunnstrom stage of the shoulders (r=-0.39,P≤0.01),with the range of passive abduction (r=-0.56,P≤ 0.01),the range of passive external rotation (r=-0.36,P≤ 0.02),and the range of passive flexion (r=-0.37,P≤ 0.02).It was positively correlated with abnormal sonographic findings of the long head tendon of the biceps (r=0.32,P≤ 0.04),the subscapularis tendons (r=0.31,P≤0.04) and the supraspinatus tendons (r=0.53,P≤0.01).However,3 months later the VAS results were negatively correlated with Brunnstrom stage (r=-0.45,P ≤ 0.01),range of passive shoulder abduction (r=-0.60,P≤0.01),range of passive external rotation (r=-0.41,P≤0.01),and passive range of flexion (r=-0.52,P≤ 0.01),but positively correlated with spasticity (r=0.34,P≤ 0.03),biceps tendinopathy (r=0.45,P≤0.01),subscapularis tendinopathy (r=0.33,P≤ 0.03) and supraspinatus tendinopathy (r=0.56,P≤ 0.01).Rotator cuff injury was significantly more severe 6 months after stroke than at 3 months.The incidence of pain in the hemiplegic shoulder and abnormal sonographic findings of the biceps tendon and subscapularis tendon were significantly higher at 6 months than at 3 months after the stroke.Supraspinatus tendon problems were notable both at 3 and 6 months.Conclusions Pain in a hemiplegic shoulder is positively correlated with abnormal sonographic findings but negatively correlated with Brunnstrom stages and passive range of motion 3 and 6 months after stroke.Six months after stroke,pain is positively correlated with muscle tone.The tendon at the long head of the biceps and the rotator cuff are vulnerable to injury,especially at 6 months after stroke.

ABSTRACT

Objective To explore the clinical effect and feasibility of laparoscopic "making-dumplings"resection for gastrointestinal stromal tumor (GIST). Methods Fifty patients with GIST underwent laparoscopic "making-dumplings"resection from July 2010 to February 2015.We used suture to overhang the tumor , then sealed the lesion with linear cutting stapler like "making-dumplings", finally resected it with a more than 2 cm edge. Results The operation time was (95.8 ±34.2) min and the intraoperative blood loss was (40.2 ±17.9) ml.The tumor body of resected specimen was intact , and the resection margin was more than 2 cm.GIST was diagnosed by pathological examination and immunohistological staining .There were 5 cases of tumor diameter >5 cm, 23 cases between 2 cm and 5 cm, and 22 cases less than 2 cm.No residual tumor or lymph node metastasis was found . According to newly revised NIH risk grading standards , there were 20 cases of very low risk , 17 cases of low risk , 7 cases of moderate risk, and 6 cases of high risk.The mean follow-up time after operation was 24 months (range, 8-36 months).There was one case of omentum metastasis after 3 months operation which was reoperated , and the remaining 49 cases had no recurrence or metastasis under gastroscopy , ultrasonography and CT scanning . Conclusion Laparoscopic "making-dumplings"resection for GIST is feasible and effective, being worthy of further clinical application .

<p><b>OBJECTIVE</b>To explore the expression of KIF18A gene protein in gastric cancer tissues and its association with the prognosis of patients.</p><p><b>METHODS</b>Twenty fresh paired gastric cancer specimens and adjacent normal mucosa(at least 5 cm from the edge of tumor) from 20 gastric cancer patients undergoing operation in Department of General Surgery at the First Affiliated Hosptial of Anhui Medical University between March 2015 and July 2015 were collected. Real-time PCR was used to examine KIF18A mRNA expression in above specimens. Meanwhile, paraffin embedded cancer tissue samples from 129 gastric cancer patients undergoing operation and 23 samples of randomly selected normal gastric tissue(adjacent non-cancer tissue) were collected to establish the microarray. Immunohistochemistry method was applied to detect the KIF18A protein expression in the microarray after confirmation by pathologists. Association of KIF18A expression with clinicopathological features in gastric cancer patients was evaluated. Cox proportional hazard model was used to identify prognostic risk factors.</p><p><b>RESULTS</b>Among 20 fresh paired gastric cancer specimens, mRNA expression of KIF18A in 16 specimens was obviously lower than that in adjacent normal tissues. The positive rate of KIF18A protein expression in gastric cancer tissues was significantly lower than that in normal gastric tissues in microarray[45.0%(58/129) vs. 69.6%(16/23), P=0.041]. KIF18A protein expression was significantly associated with invasion depth (P=0.008) and TNM staging (P=0.032). The median overall survival of all the 129 patients was 44.0(95% CI: 39.78-49.24) months. The three-year survival rates of patients with high and low KIF18A expression were 67.2% and 36.6% respectively(P=0.020). Cox regression analysis showed that KIF18A expression was an independent protective factor of the prognosis of gastric cancer patients (HR=0.570, 95% CI:0.335 to 0.970).</p><p><b>CONCLUSIONS</b>KIF18A expression is down-regulated in gastric cancer tissue, which may play a critical role in gastric cancer carcinogenesis. Lower expression of KIF18A is associated with poor prognosis of gastric cancer patients. KIF18A may be a potential prognostic marker of gastric cancer.</p>

ABSTRACT

Objective To study the technique and clinical effect of endoscopic surgery using high frequency electrotome with suction device for nasopharyngeal angiofibroma. Methods In this study, we retrospective analyzed the clinical data of 10 patients treated between January 2009 and December 2015. All these patients were treated by endoscopic surgery using high frequency electrotome with suction device. The average blood loss during surgery, tumor residual, recurrent tumor and duration of recovery were evaluated. Results The tumor was completely removed in all 10 patients. The average blood loss was 465 ml, there was no postoperative complication, all patients had no residual tumor, and there was no recurrent tumor over a follow-up of 6~18 months. Conclusion Though it was not preoperative transcatheter arterial embolization, the endoscopic surgery using high frequency electrotome with suction device can be used to treated nasopharyngeal angiofibroma which either limited to nasopharyngeal cavities and paranasal sinus with intraoperative blood pressure control.

ABSTRACT

Objective To observe the effect of Chaihushugansan combined with fluoxetine on interleukin -6 ( IL-6 ) , interleukin -1β( IL-1β) , tumor necrosis factor ( TNF-α) in the treatment of patients with postpartum depression.Methods 83 patients from November 2013 to July 2015 in Jiaxing Maternity and Child Health Care Hospital were randomly divided into 41 patients of observation group and 42 patients of control group.The control group were treated with fluoxetine treatment, the obsercation group received Chaihushugansan on the basis of control group.The Hamilton depression scale (HAMD) and Hamilton anxiety scale (HAMA),IL-6, IL-1β, TNF-α, high-sensitivity C-reactive protein (hs-CRP) and adverse reactions were followed up and recorded.Results The HAMD and HAMA score in observation group post-treatment were (8.31 ±2.05,9.03 ±2.08)points, which were lower than (13.96 ±2.16, 13.61 ±2.14) points in control group (P<0.05).The serum IL-6 and IL-1βlevels in observation group post-treatment were (6.59 ±3.20,5.01 ±2.83)pg/mL, which were lower than (10.64 ±3.86,9.31 ±3.42)pg/mL in control group (P<0.05).The serum TNF-αand hs-CRP levels in observation group post-treatment were ( 9.16 ±2.01 ) pg/mL, ( 3.62 ±1.06 ) mg/L, which were lower than ( 12.30 ±2.37 ) pg/mL, (5.29 ±1.14)mg/L in control group (P<0.05).The total adverse reaction rate in observation group was 7.32%, significantly lower than 23.81% in control group ( P <0.05 ) .Conclusion Chaihushugansan combined with fluoxetine has a good therapeutic effect in the treatment of postpartum depression, could significantly reduce the IL-6, IL-1β, TNF-α levels, with fewer side effects, it is better than fluoxetine alone.

ABSTRACT

Objective To explore the serum anti-Mullerian hormone (AMH) level in women of childbearing age with normal menstrual cycles. Methods A total of 1 423 women with regular menstrual cycles were selected and divided into 5 groups according to their ages, i.e.≤25, 26-30, 31-35, 36-40,≥41 years. Their serum levels of AMH were measured, and the relationship between AMH and age was analyzed. Results The serum AMH levels of 5 groups according to ages (≤25, 26-30, 31-35, 36-40, ≥41 years) were 3.62, 3.10, 2.27, 1.07, 0.45μg/L, respectively. The comparison of serum AMH levels in different age groups had significant difference (P<0.01). Serum AMH level declined with increasing age,and dropped significantly after 36. The serum AMH level and age showed a negative correlation with significant difference (r=-0.374, P<0.01). Quadratic regression of logAMH proximally reflected the relationship between AMH and age. Conclusion AMH determination for women of childbearing age could provide reference for the evaluation of ovarian function.

ABSTRACT

Objective To investigate the effects of laparoscopic and open resection with pelvic autonomic nerve preservation (PANP) on sexual function of male patients with lower rectal cancer. Methods Total 177 male patients with lower rectal cancer received surgery from September 2008 to December 2013 were enrolled into two groups: the laparoscopic PANP group (n = 105) and the open PANP group (n = 72). The classifications of erectile and ejaculatory functions were used to evaluate the sexual functions of patients at 6 months and 12 months post-operation, respectively. The effect of different operation on the sexual function of the male patients was compared between the two groups. Results The incidence rates of erectile dysfunction at 6 months and 12 months post-operation in the laparoscopic group were lower than those in the laparotomy group (P < 0.05). The incidence rates of ejaculatory dysfunction at 6 months and 12 months post-operation in the laparoscopic group were also lower than those in the laparotomy group (P < 0.05). Conclusion The laparoscopic resection with PANP in patients with lower rectal cancer can not only clearly reveal pelvic autonomic nerve and effectively protect them, but also reduce the incidence of postoperative sexual dysfunction.

Objective To explore significance and impact of hypoxia inducible factor 1α,metastasis associated with colon cancer 1 and transcription factor FOXO4 in gastric carcinoma and clinical pathological characteristics and prognosis .Methods 110 samples diagnosed as gastric carcinoma and 46 samples diagnosed as pericarcinomatous tissue which was 5 cm far away from cancer were collected in first affiliated hospital of Anhui medical university from December 2006 to May 2008 .Immunohistochemical stai-ning was performed in these samples to detect the expressions of HIF-1α,MACC1 and FOXO4 .And the correlation between devel-opment of gastric carcinoma and expressions of HIF-1α,MACC1 and FOXO4 was analyzed with the help of patients′messages .Ka-plan-Meier method and Cox regression were used to analyze patients′5 years survival rate and the factors .Results The expression levels of HIF-1α(P<0 .001) and MACC1(P<0 .001) in gastric carcinoma tissues were significantly higher than pericarcinomatous tissues ,however the expression levels of FOXO4(P<0 .001) was significantly decreased in gastric carcinoma tissues .Statistical a-nalysis revealed that the expressions of HIF-1αand MACC1 were apparently increased with the lower degree of cell differentiation , the greater degree of tumor infiltration ,accompanied with lymph node metastasis and higher TNM scores ,but lowered the expres-sion of FOXO4 factor .There were significant correlations between expressions of the three factors in gastric carcinoma .The expres-sions of HIF-1α,MACC1 and FOXO4 had an apparent influence on the patients′5 years survival rate .And they were independent factors on the prognosis of gastric carcinoma .Conclusion HIF-1α,M ACC1and FOXO4 are abnormal expression in gastric cancer tissue ,and the clinical pathological features of gastric cancer and the prognosis of patients with gastric cancer are closely related .

ABSTRACT

Objective To explore the expression of THBS 2 and its clinical significance in gastric cancer . Methods The mRNA and protein expression levels of THBS2 were assessed in 14 paired of gastric cancer specimen and corre-sponding normal mucosa using quantitative real-time PCR and Western blot analysis. Immunohistochemistry of TH-BS2 on a population-based tissue microarray consisting of 129 gastric cancer cases was used for evaluating the TH-BS2 prognostic significance. Kaplan-Meier method and Cox′s proportional hazards model were used in survival anal-ysis. Results Both mRNA and protein expression of THBS2 in 12 gastric cancer tissues were remarkably lower than the corresponding normal tissues among total 14 pair tissues. Consistent with the results of our Western blot, THBS2 expression was significantly inhibited in gastric cancer tissues as that in the normal controls in TMA ( P=0. 031) . Overexpression of THBS2 had a significant correlation with favorable prognosis of gastric cancer patients ( P=0. 002 ) and decreasing THBS2 expression was associated with the poor histological grade of gastric cancer his-tological grade ( P=0. 005 ) . Conclusion This study suggests THBS2 is down-regulation in gastric cancer tissue versus normal gastric tissue, and it may play a critical role in gastric cancer carcinogenesis and may be a potential prognosis predictor of gastric cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To identify ticks and determine the Borrelia (B.) burgdorferi genotype from four counties of northern Xinjiang.</p><p><b>METHODS</b>Sheep ticks were collected from 6 surveillance sites in four counties including Shihezi, Shawan,Yining and Chabuchaer. All ticks were initially screened out based on morphological methods and 16S rRNA sequence analysis. B. burgdorferi was detected and cultivated with BSK-H medium. Combined with nested PCR, silver nitrate staining was employed to detect B. burgdorferi. Genotype of isolated B. burgdorferi was determined by Sequencing and phylogenic analysis based on 11 conference sequences.</p><p><b>RESULTS</b>Hyalomma asiaticum asiaticum, Haemaphysalis punctata, Dermacentor marginatus and Rhipicephalus turanicus were identified from more than 900 ticks. Out of 24 tubes from 102 representative tick specimens, 16 tube were positive for B. burgdorfer. Sequencing of 5S-23S rRNA intergenic spacer showed 98.6%-99.5% identities to B. burgdorferi Sensu Stricto(B31). Results from the analysis of OspC genotype showed consistent with that of 5S-23S rRNA intergenic spacer.</p><p><b>CONCLUSIONS</b>16 strains of B. burgdorferi Sensu Stricto were isolated in four counties, from northern Xinjiang. Additionally, B. burgdorferi Sensu Stricto was isolated from Rhipicephalus turanicus first time in China.</p>

Objective To explore the significance of the Self -assessments of Voice Handicap Index (VHI) and The objective detection of Dysphonia Severity Index (DSI) in polyp of vocal cord perioperatively .Methods The Self -assessments of VHI had been completed in 28 patients with polyp of vocal cord before the operation and one week after the operation ,including function(F) ,physiological(P) ,emotion(E) ,and the sum denoted by T .In the meantime ,The DSI had been completed in these patients by acoustic analysis of DIVAS 2 .5 .After one month ,19 pa-tients had been re -examined .Results The DSI value was increasing ,but there was no significant difference be-tween pre-operation and one week after the operation(P>0 .05) ,indicating that the function of vocal cord was not recovered .In VHI scale ,the parameters were significantly different between the time preoperatively and one week postoperatively in F ,E and T(P0 .05) ,indicating that the function of vocal cord was not completely recovered as same as the result of DSI .After one month ,there were significant differences in DSI and VHI ,which showed a full recovery of vocal cord fanction .Conclusion The Self -assessments of VHI and The objective detection of DSI in polyp of vocal cord can evaluate simultaneouly the function of laryngeal .Additionally ,there was a good correlation between the DSI and the VHI score .

Objective To investigate the feasibility and safety of radical resection of colorectal carcinoma with laparoscopy.Methods Clinical data of 48 patients who underwent laparoscopic colorectal cancer resection and 131 patients who underwent traditional radical resectin were retrospectively analyzed.Results The incision length,first flatus time,intestinal intervention times,the first meal time,urinary catheterization time,gastric tube retaining time,drainage tube lien time,analgesia time of the laparoscopic group significantly decreased compared with those of conventional surgery ( P < 0.05 ).There was no significant difference in complication occurrence,postoperative hospitalization time and overall hospitalization time between the two groups( P > 0.05 ).The average operation time of laparoscopic surgery is longer than that of the traditional group (P < 0.05 ).There was no significant difference between 2 groups in regarding of perioperative bleeding rate,volume of intraoperative and postoperative blood transfusions,mass maximal diameter,the distance between the distal and proximal margin and the mass in colon specimens,as well as the two variables in rectum specimens,the resected lymph node number and positive lymph node number (P >0.05).Conclusion Laparoscopic resection for colorectal cancer,a safe and feasible technology,is of the same short-term curative effect with traditional open surgery,but the follow-up radical curative effect is yet to be confirmed.

ABSTRACT

Objective To investigate the application of the multiple locus variable numbers of tandem repeats(MLVA) in genotype Mycobacterium tuberculosis(MTB) strains isolated from Uygur in south of Xinjiang, and to understand the characteristics of genotype and distribution. Methods One hundred and fifty-one Mycobacterium tuberculosis strains were collected from Uygur in south of Xinjiang which contains three regions, Kashgar, Hotan and Kizilsu kirghiz. Twenty-four tandem repeats loci in the total genome of MTB were analyzed by PCR and agarose gel electrophoresis method. The characteristics on polymorphism of DNA fingerprinting of 151 MTB strains were analyzed with BioNumerics 5.0 software. Results Twenty-four MLVA loci of 151 MTB strains were analyzed respectively. The results showed that there were obvious polymorphisms of VNTRs. The clustering of genotype showed that these strains could be categorized into 8 gene groups( Ⅰ , Ⅱ , Ⅲ ,Ⅳ, Ⅴ ,Ⅵ,Ⅶ ,Ⅷ) and 151 genotypes. Sixty-seven isolates(44.4% ) belonged to group Ⅵ. 23.2% were group Ⅷ including 35 genotypes, 20. 5% were group Ⅳ including 31 genotypes. The group Ⅵ prevailed mostly in the Kashgar. The group Ⅲ prevailed mostly in the HOTAN. Conclusion The results showed there were obvious polymorphisms of VNTRs of Mycobacterium tuberculosis clinical strains preliminarily. Group Ⅵ was the predominant prevalent strain in south of Xinjiang.

ABSTRACT

Objective To develop a method for the determination of acrylamide in flocculant by gas chromatography-mass spectrometry.Methods Acrylamide was derived through bromination and extracted by organic solvent.GC-MS method was used to identify the target substance and quantify acrylamide.Results The recoveries obtained from samples spiked with standards were in the range of 70.2%-109.2% and relative standard deviations (n=5) was less than 5.0%.Linearity of peak area was obtained over a wide range of concentration (4.3-2 000 ?g/ml),and the correlation coefficients was 0.999 4.Conclusion The method is reliable,selective and reproducible and it is applicable to the determination of the acrylamide in flocculant.
