ABSTRACT

Objective To determine the median effective dose(ED50)of dezocine inhibiting re-sponses to insertion of laryngeal mask airway(LMA)when combined with propofol in the elderly pa-tients.Methods American Society of Anesthesiologists physical statusⅠorⅡ patients, aged 66-75 yr, with body mass index of 20-25 kg∕m2, were included in this study.Anesthesia was induced with dezocine at the initial dose of 0.2 mg∕kg and propofol which was simultaneously administered by target-controlled infu-sion.The initial target plasma concentration of propofol was 1 μg∕ml, and the concentration was increased in increments of 0.5 μg∕ml every 3 min until the target concentration 3 μg∕ml was achieved.LMA was inserted when bispectral index value reached 50-60.The dose of dezocine was determined using the up-and-down method.The response to insertion of LMA was defined as positive when patients developed coughing, laryn-gospasm and∕or body movement during insertion or within 3 min after insertion.The dose of dezocine was in-creased∕decreased in the next patient if the insertion response was positive or negative.The ratio between the two successive doses was 0.8.The ED50and 95% confidence interval of dezocine inhibiting responses to in-sertion of LMA were calculated.Results When combined with propofol, the ED50of dezocine inhibiting re-sponses to insertion of LMA was 0.126 mg∕kg, and the 95% confidence interval was 0.110-0.143 mg∕kg.Conclusion The ED50of dezocine inhibiting responses to insertion of LMA is 0.126 mg∕kg when combined with propofol in the elderly patients.

ABSTRACT

Objective To evaluate the effect of electric vagal stimulation on postoperative cognitive dysfunction in aged rats.Methods Thirty healthy Sprague-Dawley rats of both sexes,aged 18-20 months,weighing 390-550 g,were randomly divided into 3 groups (n=10 each) using a random number table:control group (group C),surgery group (group S),and electric vagal stimulation group (group V).Incision of the right carotid sheath was performed,and the vagal nerve was exposed after anesthesia in S and V groups.The right vagus nerve was stimulated for 30 min with continuous electric rectangular pulses (1 ms,10 Hz,1-2 V) after surgery in group V.The Morris water maze place navigation test was performed at 4,3,2 and 1 days before surgery and 2 days after surgery.The cognitive function was assessed using spatial probe and open field tests conducted at 2 days after surgery.The escape latency,the number of crossing the platform,the number of crossing the grid,the number of standing on the back legs,and the time animals spent in the central square were recorded.After the end of behavioral testing,venous blood samples were collected from the jugular vein for determination of concentrations of serum tumor necrosis factor-alpha (TNF-α),interleukin (IL)-1β and IL-6 using enzyme-linked immunosorbent assay.Results Compared with group C,the number of crossing the platform,the number of crossing the grid,and the number of standing on the back legs were significantly decreased,the escape latency and the time animals spent in the central square were significantly prolonged,and the concentrations of serum TNF-α,IL-1β and IL-6 were significantly increased at 2 days after surgery in group S (P<0.05).Compared with group S,the number of crossing the platform,the number of crossing the grid,and the number of standing on the back legs were significantly increased,the escape latency and the time animals spent in the central square were significantly shortened,and the concentrations of serum TNF-α,IL-1β and IL-6 were significantly decreased at 2 days after surgery in group V (P<0.05).Conclusion Electric vagal stimulation can improve the postoperative cognitive dysfunction in aged rats.

ABSTRACT

Objective To explore therapeutic effect of integrated traditional Chinese and western medicine therapy on reynolds syndrome,and to provide effective and reliable operation method for treatment of the disease. Methods 104 cases with reynolds syndrome were randomly divided into observation group (59 cases)and control group (45 cases)by random number table method.The observation group was treated with aspirin,nifedipine combined with traditional Chinese medicine dialectical therapy,the control group was treated with aspirin and nifedipine. The cold water test ,fist test results and hemorheology indexes of the two groups were measured before and after a period of treatment.The therapeutic effects were evaluated after one course.Results After treatment,the fist test, cold water test positive rates of observation group were 32.2%,23.7%,which were lower than 75.6%,57.8% of the control group (χ2 =19.199,12.504,all P 0.05 ). Conclusion In the treatment of reynolds syndrome,the integrated traditional Chinese and western medicine therapy has better curative effect and low drug adverse reaction rate,with good clinical application value.

ABSTRACT

Objective To learn the effect of cinepazide maleate combined with Shuxuetong in the treatment of ischemic vertigo.Methods 1 81 cases with ischemic vertigo were randomly divided into the observation group (98 cases)and the control group(83 cases)by random number table method.The observation group was treated with cinepazide maleate combined with Shuxuetong,and the control group was treated with cinepazide maleate.The blood flow velocity of vertebral artery and basilar artery of the two groups were measured before and after a period of treat-ment.The therapeutic effects were evaluated after one course.Results The peak systolic velocity and end diastolic velocity of right vertebral artery,left vertebral artery and basilar artery of the two groups were higher than before treat-ment,the differences were statistically significant(t =1 1 .21 5,1 2.51 0,6.030,6.81 4,7.052,7.730,1 9.884,1 7.774, 1 0.954,1 9.320,1 9.483,1 9.1 98,all P 0.05).Conclusion In the treatment of ischemic vertigo,cinepazide maleate combined with Shuxuetong has better curative effect and low incidence rate of drug adverse reaction.

ABSTRACT

OBJECTIVE To evaluate theanalgesic efficiency and safety of patient-controlled intravenous analgesia (PCIA) with sufentanil, lornoxicam and sufentanil combined with lornoxicam after Han-uvulopalatopharyngoplasty (H-UPPP) surgery. METHODS Sixty patients after H-UPPP surgery for PCIA were randomly divided into three groups: group S (sufentanil 2.0 ?g/kg), group L (lornoxicam 0.5 mg/kg) and group SL (sufentanil 1.0 ?g/kg + lornoxicam 0.4 mg/kg). The efficiency of analgesia was assessed by VAS (visual analogue scale) and Ramsay score at 2, 6, 12, 24, 48 hours after PCIA. The MAP, HR, SpO2 and side effects were also recorded. RESULTS Group S and SL showed good pain relief and sedation. In Group L VAS score was higher but Ramsay score was lower than that of group S and SL at 2, 6, 12 hours after PCIA (P
