ABSTRACT

@#<b>Objective</b> To investigate the current situation of quality control in medical linear accelerators in Hebei Province, China. <b>Methods</b> The main performance indices of some medical linear accelerators in Hebei Province, 2019—2021 were tested according to the current relevant effective standards GB 15213—2016 <i>Medical Electron Accelerators-Functional Performance Characteristics and Test Methods</i> and WS 674—2020 <i>Specification for Testing of Quality Control in Medical Linear Accelerator</i>. <b>Results</b> A total of 175 medical linear accelerators were tested from 2019 to 2021, and the annual pass rates were 72.4% in 2019, 75.9% in 2020, and 79.4% in 2021. The overall pass rate of initial inspection was 76.0%, and the pass rate of reinspection was 100.0%. The index with the lowest pass rate in the initial inspection was square X radiation field uniformity, followed by dose deviation and square X radiation field symmetry. <b>Conclusion</b> Medical institutions should strengthen quality control management, conduct regular testing of medical linear accelerators, and timely correct dose deviation, so as to ensure the treatment effect of patients.

ABSTRACT

Objective To investigate the application of radiological diagnosis and treatment equipment and distribution of medical radiation levels in medical institutions at various levels in Shijiazhuang, China in 2019, and to lay a sound foundation for further radiation protection and management. Methods A universally designed questionnaire was used to investigate and compile data on the level of the sampled hospitals, the number of radiation workers, equipment information, the number of outpatients and emergency patients, the number of inpatients, and the frequency of radiological diagnosis and treatment; the application frequency of each radiological diagnosis and treatment item was calculated based on the demographic data of Shijiazhuang. Results In Shijiazhuang, there were 390 medical institutions (excluding dental clinics) certified for radiological diagnosis and treatment, with a total of 4262 radiation workers and 1215 radiological diagnosis and treatment devices; 1.11 radiological diagnosis and treatment devices were available per 10 000 people, and 3.89 radiation workers were available per 10 000 people. The number of annual outpatients and emergency patients was 30 208 471, the number of inpatients was 1 981 295, and the total number of people or times receiving radiological diagnosis and treatment was 5 987 230. The application frequency of medical radiation was 546.70 persons/times per 1000 people, with the highest frequency for X-ray diagnosis (534.63 persons/times per 1000 people), followed by diagnosis and treatment with nuclear medicine (6.16 persons/times per 1000 people), and the lowest frequency for radiotherapy (1.17 persons/times per 1000 people). In terms of regional distribution, the highest frequency of medical radiation fell in Yuhua District (1602.97 persons/times per 1000 people), and the lowest frequency fell in Shenze County (203.21 persons/times per 1000 people). Conclusion The development of medical radiation is imbalanced in Shijiazhuang, with high-quality medical resources concentrated mainly in the main urban area, thus resulting in long-term overworking of hospital staff and equipment in some areas. The government and health administration departments should strengthen macro-control and the rational allocation of medical resources; medical institutions at various levels should rationally use radiological diagnosis and treatment equipment, strengthen judgments on the justness of radiation, and strengthen the training of radiation workers on protection knowledge and radiation protection optimization.

ABSTRACT

Objective To explore risk factors for coronary artery aneurysms (CAA) in children with Kawasaki disease (KD),and reduce the rate of serious sequelae of cardiovascular diseases.Methods All the patients were diagnosed as KD in Children’s Hospital Affiliated to Medical College of Zhejiang University from January 2009 to January 2014.A total of 679 cases was included,and 42 with concurrent CAA,181 with concurrent coronary artery expansion (CAD),and 456 without coronary artery damage cases (non-coronary artery lesion,NCAL).Coronary artery damage was related to factors such as gender,age,fever,white blood cell count (WBC),hemoglobin (Hb),platelet count (PLT),erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),alanine aminotransferase,serum potassium,plasma amino terminal brain natriuretic peptide precursor (NT-proBNP),and acrylic ball resistance.SPSS 18.0 software package was used for risk factor analysis.Qualitative data using chi-square test,was used to analyze the high risk factors of CAA group,and logistic multivariate regression analysis was also used.Results Chi-square test showed that male,with febrile days > 14 d,NT-proBNP ≥ 1 000 ng/L,immunoglobulin resistance was more likely to have KD concurrent CAA (P < 0.05).Multiple Logistic regression analysis showed that male [OR =4.092,95% CI (1.514,11.060),P =0.004],febrile days >14 d [OR =12.436,95% CI (4.599,33.631),P =0.000],NT-proBNP≥ 1 000 ng/L [OR =3.305,95% CI (1.401,7.794),P =0.005],and immunoglobulin resistance [OR =3.842,95 % CI (1.562,9.453),P =0.000] were independent risk factors for KD concurrent CAA.Conclusions Male children,febrile days > 14 d,NT-proBNP≥ 1 000 ng/L,and immunoglobulin resistance were independent risk factors of CAA.

ABSTRACT

Objective To analyze the influence of radial neck fracture on the upper limb function in older children,and to evaluate the curative effect of plate fixation in the treatment of Salter-Harris Ⅱ type of radial neck fracture.Methods The clinical data of 16 older children with radial neck fractures who treated by T plate internal fixation were retrospectively analyzed.16 cases were closed fractures,merging radial nerve injury in 1 case,3 cases of distal humerus fractures,1 case with rib fractures,fractures were Salter-Harris Ⅱ type,adopt steel plate internal fixa tion.Results 1 6 patients were followed up for 9-1 8 months.X-ray healing time was 6-9 weekson average 7.6 weeks.No malunion and no healing,no case of epiphyseal injuries.1 patient appeared steel block forearm supination.Postoperative patients with forearm pronation were greater than 90° supination were greater than 60 °.No radial nerve injury.Evaluation results were good.Conclusion Clinical treatment effect of open reduction and plate internal fixation in the treatment of older children with Salter-Harris Ⅱ type of radial neck fracture is satisfactory.Surgical treatment not only can enhance the fracture stability,but also can do the elbow joint function exercise to prevent dysfunction much earlier.

ABSTRACT

Objective To discuss surgical method and clinical curative effect of the headless compression cannulated screw percutaneous minimally invasive treatment of Evans -JensenⅠ,type Ⅱ of intertrochanteric fracture. Methods The clinical data of 21 cases of Jensen Evans and type II femoral intertrochanteric fractures who were treated by headless compression cannulated screw minimally invasive treatment were retrospectively annalyzed.In all patients,male in 6 cases,female in 13 cases;63 -86 years old,average 71.79 years.According to the Evans -Jensen norm and curative effect evaluation according to the improved Harris standard.Results Follow -up time was 6 -18 months (mean 12.15 months),two of them respectively in the six months after death from acute myocardial infarc-tion (ami)and 11 months of cor pulmonale,the rest of 17 cases of bony healing.Harris,the average score (67 -95points)at the time of the last follow -up,including 14 cases of optimal good in 2 cases,fair 1,poor in 1 case,was 88.89%.Conclusion Headless compression cannulated screw percutaneous minimally invasive treatment of Evans-Jensen Ⅰ,type Ⅱ intertrochanteric fractures has the advantangtages of minimally invasive,reliable fixation,high rate of fracture healing,allowing early hip not functional exercise load conditions and obtaining ideal therapeutic effect.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the therapeutic effect of double fill nine tastes soup in treating children recurrent respiratory infection (RRTI) and the change of immune function.</p><p><b>METHOD</b>77 RRTI patients were randomly selected into observation and control groups. The observation group was treated with Chinese medicine- double fill nine tastes soup,water frying points 2 times oral. The control was treated with transfer factor oral liquid,every 10 mL,2 times daily oral. Treatment periods were both two months. IgA, IgG, IgM and IL-12, TNF-alpha, INF-gamma were detected before and after treatment to assess the clinical effects and the changes of immune factors, meanwhile, a health group was established.</p><p><b>RESULT</b>Before treatment, compared with the health group, the serum IgA, IgG, IgM, IgE, IL-12, TNF-alpha, IFN-gamma in both groups were significantly different (P < 0.01). After treatment, the ratio of IgA, IgG, Ig M, IL-12, TNF-alpha, IFN-gamma in two groups were significantly different (P < 0.01). Compared with the recurrence rate and clinical effects, the observation group was better than control, and the differences were significant (P < 0.01).</p><p><b>CONCLUSION</b>Double fill nine tastes soup has significant effects in treating recurrent respiratory infection (RRI) and enhance the immune function in children.</p>
