ABSTRACT

The distribution of pemphigus subclass autoantibodies in a patient with pemphigus vulgaris (PV) has been investigated by semiquantitative indirect immunofluorescence (IIF), using the HP series monoclonal antibodies specific for four human IgG subclasses on human foreskins. IgG1 and IgG4 intercellular substance-specific autoantibodies were detected in the serum of the patient, whereas IgG2 and IgG3 autoantibodies were absent. In addition to foreskins, human tonsillar epithelia were used as substrates of IIF for detecting the PV autoantibodies and it was one of satisfactory substitutes for monkey esophagus.

ABSTRACT

A 77-year-old woman with chronic renal failure and malnutrition had thick crusted plaques, erythematous papules and scaly patches on her entire body. The skin scraping and exfoliated skin scales showed the presence of numerous itch mites, Sarcopbes scabiei var horriums 8667 mites were counted from Ig of the collected samples. Twenty six cases a pruritic rash were reported among hospital personnel: mites were recovered from only one. Among other hospitalized patients, 2 cases of scabies were reported. One patient had pruritic papules and burrows on the left upper arm and the other had lesions in the axilla, mites were recovered from both. Subsequent evaluation suggested that they acquired scabies from a contaminated blood pressure monitoring cuff and a clinical thermometer. The contaminated medical instruments are considered to be fomites, a mode of dissemination of mites.

ABSTRACT

This study was designed to investigate the effect of local hyperthermia on contact hypersensitivity (CHS) and elucidate it's mechanism through assessment of number of epidermal LCs and transfer of spleen cells. Depilated dorsal skin of mouse was immersed into controlled water bath at 52 ℃ for 30 seconds in vivo. The number of epidermal LCs was counted by adenosine triphosphate staining, and CHS to 2,4-dinitro-l-fluorobenzene was assessed by ear swelling and transfer of spleen cells. The number of LCs was significantly reduced 1 to 3 days after the hyperthermia treatment and recovered to normal 5 days after the treatment. CHS was significantly suppressed in mice sensitized 5,7, or 10 days after hyperthermia treatment, but the suppression was meager in mice sensitized 1 or 3 days after the treatment. There is a discord between the number of LCs and degree of CHS. When mice received spleen cells from hyporesponsive donors, CHS was remarkably sup-pressed in the recipient mice compared with positive control. These findings suggest that tfeatment of local hyperthermia suppress CHS in mice, which may be associated with the induction of suppressor cells. The nature of the discord between the the number of LCs and degree of CHS in this investigation remains to be cleared by further studies.

ABSTRACT

Cryoglobulinernia occurs in about 5% of the cases of multiple myeloma. The most common finding in patient with cryoglobulinemia is ulceraticn that oceurs about ankle, hands, and occasionally the ears, upon prolonged exposue to cold. A 59-year old male had had 5 years of pain in his ears. There were black or dark brown colored ischemic ulcerations on his both helix. He also had mottled purpuric patches on his both ankles. A test for cryoglobulinema was positive. X-ray examination of the skull showed multiple punched but lesions. The bone marrow study revealed myeloma cell infiltration.

ABSTRACT

One hundred and nineteen cases of congenital melanocytic nevi diagnosed clinically and/or histopathologically st Departments of Dermatology, Plastic surgery and Pedia trics of 7 branch hospitals of Catholic University Medical College from October, 1986 to October, 1988 were reviewed by their incidence, distribution, age, size and elinical findings. The results were summarized as follows . l. Among 2g73 newborn babies, 33(1.5%) melanocytic nevi were found clinically. 2. Amomg 119 congenital melanocytie nevi, 31 located on the head and neck area, 47 on the trunk, and 41 on the extremities. 3. Thirty (252%) were less than 0.6cm in greatest diameter, 42(353%) nevi 0.6 to 15cm, 15(12.7%) nevi 1.6 to 3.0cm, 26(218%) nevi 3.1 to 10.0cm and 6(5.0%) nevi greater than 10.0cm. 4. a) Seventy three nevi(613%) were uniform in color and 46(38.7%) were mixed brown r black with variable proportion. b) Fifty three(44.5%) nevi were flat, 36(303%) were slightly elevated, 17(14.3%) had pebble stone appearance and 13(10.9%) were dorne shaped. c)Thirteen(11%) nevi had terminal hair on the surface.

ABSTRACT

A 53-year-old woman developed a psinful erythematous papular skin eruption around the biting site by a tick on the left lower abdomen. A excision biopsy specimen from the skin lesion showed perivsscular infiltration of inflammatory cells composed mainly of neutrophiles, lymphocytes, and a few eosinophiles and histiocytes throughout the dermis. Some of the vessels were dilated with protruding and proliferated endothelial cells. The tick was identified as an adult female of Ixodes nipponensis by the scanning electron microscopic examination.

ABSTRACT

Juvenile xanthogranuloma is characterized by one, or oftener many, yellow, re3dish yellow, or, brown 4-to-20 mm papules or nodules on the face, scalp, trunk, and mostly, extensor surfaces. It may involve internal organ besides the skin such as eye, gastrointestinal and genitourinary tracts. anthogranuloma in adult is usually solitary and dose not involve organs other than skin, and generally dose ncit resolve without treatment. Histopathologically, characteristic Touton giant cells usually appear numerously. We report a case of juvenile xanthogranulorna which occured on scalp of 49 year-old male showing typical clinical and histopathological findings.

ABSTRACT

The author was to study the types and distrbution of malocclusion in the 1217 patients with diagnostic charts, cephalograms and study models, who have been treated from March, 1973 to August, 1982 in the Department of orthodontics, Infirmary of Dental College, Yon Sei University. The results were as follows; 1. In the Angle's classification of malocclusion, it was presented that class I malocclusion was 671 persons (55.1%), class II division 1 malocclusion was 241 persons (19.8%), class II division 2 malocclusion was 30 persons (2.5%) and class III malocclusion was 275 persons (22.6%). 2. In the Angle's class I malocclusion, the crowding was most remarkable (39%). 3. In the Angle's class II malocclusion, 88 persions(32.5%) were due to the undergrowth of the mandible, and deep bith was most frequent. 4. In the Angle's class II malocclusion, 94 persons (34.2%) were in the pseudo type, 84 persons (29.8%) were due the the overgrowth of the mandible, and openbite and edge to edge bite were most frequent. 5. In the sexual distribution of the patients, female patients were 791 persons (65%), male patients were 426 persons (35%), and the number of females was 1.8 time then that of male. 6. In the age distrbution of the patientsm 572 persons(47%) were in the 9-13 years old. 7. In the regional distribution of the patients, most of them lived near the hospital.

18 years 5 months old male patient who came to the Dept. of Orthodontics, college of Dentistry, Yonsei University, width missing of the maxillary right canine was treated by means of full banding and removable expansion plate. The following results were obtained; 1. Anterior crowding and crossbite was improved. 2. molar relationship was improved. 3. facial profile was somewhat improved.

ABSTRACT

Unilaterl frontal hyperhidrosis, a rare disorder of the sweat gland with localized hyperhidrosis, ia discribed. A 13-year-old female patient had increased sweating on the left half of the forehead, recurring in summer season for 2 years. Quantitative sweat collection demonstrated that the sweat glands responsive to those stimuli, which were thermal (direct and indirect heat), gustatory and cholinergic stimulation. A marked increase in sweat gland activity was noted on the affected areas as compared to the contaralateral normal sites, in thermal and gustatory effect. But, carbachol stimulation was not responsive. Biosy specimen which taken from the positive site of starch iodine technique showed increase number of sweat glands and ducts with somewhat dilated sweat ducts.

ABSTRACT

We present two cases, a 15 and a 24 year-old females, showing extensive involvement of dermal melanocytic nevi, so called bilateral Ota nevus and bilateral Ito nevus, and nevus anemicus. Both females had bilaterally brownish or slate-bluish pigmented patches on the eyelids, the scleras, the zygomatics, the frontal and temporal areas, the alae nasi, the nasal bridges (bilateral Ota nevus), and, the suprascapular regions(bilateral Ito nevus). Nevus amenicus was noticed on the left anterior neck in both females. In addition, a 15 year old female has had a diffush bluish pigmented patches on the nearly whole body (Mongolian spot), reticulate patches of nevus anemicus on the dorsal back and both arms and nevus flammeus on the right cheek. These cases are showing extensive melanocytic nevi associated with vascular anomalies, which are very rare in the literatures.

ABSTRACT

Hand, foot and mouth disease, a distinctive clinical syndrome caused by a coxsackie virus, is clinically characterized by vesicles appearing on the hands, feet and in the mouth. The infection begins with a fever and mouth lesions consisting of small vesicles surrounded by red areolae on the buccal mucosa, tongue, soft palate and gingiva. The disease usually lasts spontaneously between 7 to 10 days after onset. We observed 20 cases of hand, foot, and mouth disease from July, 12th to September, 4th, 1979 and examined histopathologically and virologically. We have isolated only one viral strain showing cytopathic effect on HeLa cell among the five cases of acute stage and also observed that viral particle in the electronmicroscope.

ABSTRACT

A 3 year-old female patient was examined because of thumb-tip sized erythematous, verrucose, granulomatous mass on the nose, and coin to hen egg sized well demarked annular plaques with central atrophy and peripheral scaling, appeared on the left deltoid and clavicular area. The skin lesions showed so-called apple jelly color on diascopic examinatian. Biopsy from the lesions of nose and the left deltoid area revealed typical pictures of lupus vulgaris. Chest PA and flat abdomen revealed irregular bony destruction with reactive sclerosis involving left clavicle, 6th, 7th and 9th ribs and right 7th rib, and both iliac bones along the sacroiliac joint, and also noted unusual prominence of right paratracheal shadow. The patient was treated with isoniazid, ethambutol, rifampicin, and now is under our our observation
