ABSTRACT

Background and Objectives@#Treatment choice for fungal ball is endoscopic endonasal removal. However, it is not easy to remove fungal elements from the maxilla using only an endonasal approach. To overcome this difficulty, we introduced a cotton pledget technique and evaluated its efficacy through a cadaveric study and clinical research.Materials and Method: A cadaveric study was performed using 10 half heads of seven cadavers. The ease and safety of the cotton pledget technique were compared to those of a previously reported technique. In clinical research, we enrolled 52 patients who underwent surgery with the cotton pledget technique and 36 patients who underwent surgery using the conventional endoscopic approach. Demographic factors, preoperative Lund-Mackay (LM) score, sinonasal outcome test (SNOT) score, surgical morbidity, and incomplete removal rate were analyzed. @*Results@#The cadaveric study showed that the cotton pledget technique was easier (p=0.011) than the conventional technique. In addition, clinical evaluation showed that the cotton pledget group had significantly lower incomplete removal rate than that of the control group (p=0.010). @*Conclusion@#The cotton pledget technique is an easy and safe method that enables fungal ball removal more effectively than the conventional technique without need for inferior meatal antrostomy (IMA) or the Caldwell-Luc (CL) approach.

ABSTRACT

PURPOSE: Leptin is a highly hydrophilic 16-kDa protein which is produced in the adipose tissue and participates in the regulation of food intake and energy expenditure. The aim of the present study was to examine the relation between umbilical cord blood leptin concentration and intrauterine growth. METHODS: Ninety-seven full-term newborn infants who were born in Yeungnam University Hospital from July to August 1998 were included in the study. They were divided into 3 groups related to birth weight : appropriate for gestational age(AGA) group(n=73), large for gestational age(LGA) group(n=17), small for gestational age(SGA) group(n=7). Birth weight, head circumference, mid-arm circumference, mid-arm circumference to head circumference ratio, Ponderal index, and BMI were measured at birth. Maternal body weight and placental weight were measured. Leptin concentrations of cord blood and maternal serum were measured by a RIA method, and testosterone, estradiol, insulin, c-peptide, glucose, white blood cell, hemoglobin, platelet count of cord blood were also measured. RESULTS: Leptin concentration in cord blood was positively correlated to birth weight and body length. Leptin concentrations(microgram/L) in cord blood were significantly different among groups(10.1+/-1.1 in LGA group, 8.7+/-0.9 in AGA group, 1.7+/-0.1 in SGA group). There was a statistically significant difference in leptin concentration of cord blood between female and male infants(11.6+/-1.9, versus 6.7+/-0.9). There was no significant correlations between leptin concentration of cord blood and placental weights or maternal leptin concentration. Therefore leptin concentration of cord blood can not inflect maternal leptin concentration but intrauterine fetal growth. CONCLUSION: Leptin in cord blood might originate mainly from fetal adipose tissue rather than the placenta, and may be related to fetal growth.

ABSTRACT

From January 1976 until December 1980 we treated 12 male and 6 female children between the ages of 1 month and 15 years for ureteropelvic obstruction. Diagnosis was based on excretory urography and retrograde pyelography. The male and female ratio was 2:1. The patient in this series were distributed from 1 month old to 15 years old. The obstruction was on the left side in 10 cases, on the right side in 5 and bilateral in 3. Of the patients 7 presented because of an abdominal mass and 6 because of flank pain. The pathologic lesion encountered at ureteropelvic obstruction, are intrinsic ureteropelvic obstruction in 16 aberrant blood vessel in 1 and fibrous band in 2. Dismembered pyeloplasty was done in 10 cases, and nephrectomy in 9 cases. Preliminary nephrostomy was done in 3 cases. And secondary nephrectomy in 1 case. Of the 10 repairs 9 were protected by nephrostomy drainage plus ureteral stent.

ABSTRACT

It is clear that creatine plays a reservoir of high energy phosphate bond as creatine phosphate and maintains ATP levels in skeletal muscle and nervous tissues. Creatine and creatine kinase activity are required to utilize creatine phosphate as high energy phosphate. The contents of creatine in testis of rats were determined by the method of Van Pilsum and compared with other organs for the study of the physiological role of creatine in testis. Creatine content of tests was 39.82+/-3.36 ug/g wet tissue compared with skeletal muscle, 52.92+/-10.25 ug/g wet tissue. It was relatively high compared with brain (15.45+/-6.49 ug/g wet tissue), heart (20.0+/-2.91 ug/g wet tissue), kidney (29.55+/-2.52 ug/g wet tissue) and liver (12.68+/-1.94 ug/g wet tissue). Creatinine content of testes (45.84+/-4.08 ug/g wet tissue) was very high, compared with skeletal muscle (24.14+/-7.73 ug/g wet tissue), heart .(23.71+/-4.73 ug/g wet tissue), brain (17.24+/-1.19 ug/g wet tissue), kidney (14.92+/-3.45 ug/g set tissue), and liver (9.59 +/-1.26 ug/g wet tissue). I suppose that creatine in testis of rats may be a part of potent system for generation of ATP from ADP hydrolyzing creatine phosphate.

ABSTRACT

During the last several years, benign tumors of the ureter have been reported with increasing frequency. Fibroepithelial polyps, accounting for about 25 per cent of these tumors. Herein we present two cases of multiple fibroepithelial polyps of the ureter in female. A review of the literature and discussion of treatment are included.

ABSTRACT

Involvement of both testes in disseminated malignant lymphoma is unusual case. Lymphoma of the testis is the most common testicular tumor in patients more than 50 years old. Including all age groups lymphoma of the testis is the most common simultaneous bilateral testis. Although the natural history of lymphoma of the testis is variable, 90 percent of the patients die of generalized lymphoma within 2 years. This report concerns a case of the bilateral involvement of both testes by malignant lymphoma.

ABSTRACT

Leiomyoma of the urethrovaginal septum is rare condition. There are only 14 documented cases of leiomyoma of the female urethra and less than 250 cases of leiomyoma of the vagina in the literature. The physical and cystoscopic features are helpful in establishing the benign nature and the extent of the lesions. This report describes two cases of leiomyoma of the urethrovaginal septum that were confirmed pathologically at our Severance Hospital.
