ABSTRACT

ObjectiveWearing protective masks for a long time causes a large number of frontline health care workers to suffer different degrees of pressure injury or facial skin rupture in response to COVID-19. This paper aims to analyze the occurrence characteristics and related factors of pressure injury related to devices, and provide the basis for taking countermeasures. Methods There was online investigation of skin injuries caused by wearing protective equipment in medical staff. Descriptive analysis was carried out on the occurrence characteristics of pressure-induced injury, and influencing factors were analyzed through logistic regression model. ResultsThere were a total of 2901 valid questionnaires. The results showed that the incidence of pressure injury caused by protective equipment was 26.34%, mainly in the bridge of the nose (20.41%), cheek (20.23%), auricle (17.82%) and forehead (8.86%). Multivariate Logistic regression model analysis showed that the major associated factors, which presented increasing risk, were sweating and dampness (OR=12.72, 95%CI 8.36-17.30), wearing level-3 protective equipment (OR=3.55, 95%CI 2.47-5.08), wearing level-2 protective equipment (OR=3.37, 95%CI 2.47-4.60), wearing time (OR=1.29, 95%CI 1.05~1.58) and occupation (OR=1.57, 95%CI 1.00~2.49). Conclusion There is the high incidence of pressure injuries to health care workers caused by protective equipment against COVID-19. The main risk factors for facial stress injury of medical staff are sweating and dampness, wearing level-3 and level-2 protective equipment and wearing time.

ABSTRACT

With the promotion and application in medicine and sanitary fields, the reporting quality of Bland-Altman agreement evaluation is worrying. This study aimed at developing a set of reporting standards for Bland-Altman agreement evaluation as the guidance for proper application to improve the reporting quality.A research group was launched to work on reporting standards for Bland-Altman agreement evaluation, and all the requirements for personnel on various levels were made clear.Early in the study,we carried out articles reviews, regular meetings, expert consultations, literary evaluation, item integration and extraction. Then,we invited a multi-disciplinary panel of experts to improve overall design, evaluate reporting items and form the first draft by brainstorming method.Multiple rounds of expert consultations were also conducted with reference to Delphi method to integrate expert advice and form the basic framework of reporting items. Finally, through the thorough analysis and demonstration,we proposed the reporting items for the agreement evaluation of Bland-Altman method (RiBAM) as the recommended report.A list of recommended items called RiBAM was formed, which consisted of 17 first-level items and 23 secondary-level items.RiBAM is more comprehensive and systematic as an important reference for improving the quality of reports by avoiding the omission of reporting contents and achieving the clarity, integrity and transparency of the report.RiBAM recommended items can be a guide for authors in reporting Bland-Altman agreement evaluation as well as a basic reference for journal editors, peer reviewers and readers.

ABSTRACT

Source data and its source documents are the foundation of clinical research. Proper source data management plays an essential role for compliance with regulatory and GCP requirements. Both paper and electronic source data co-exist in China. Due to the increasing use of electronic technology in pharmaceutical and health care industry, electronic data source becomes an upcoming trend with clear advantages. To face new opportunities and to ensure data integrity, quality and traceability from source data to regulatory submission, this document demonstrates important concepts, principles and best practices during managing source data. It includes but not limited to: (1) important concepts of source data (e.g., source data originator, source data elements, source data identifier for audit trail, etc.); (2) various modalities of source data collection in paper and electronic methods (e.g., paper CRF, EDC, Patient Report Outcomes/eCOA, etc.); (3) seven main principles recommended in the aspect of data collection, traceability, quality standards, access control, quality control, certified copy and security during source data management; (4) a life cycle from source data creation to obsolete is used as an example to illustrate consideration and implementation of source data management.

ABSTRACT

Safety assessment in clinical trials is dependent on an in-depth analysis of the adverse events to a great extent. However, there are difficulties in summary classification, data management and statistical analysis of the adverse events because of the different expressions on the same adverse events caused by regional, linguistic, ethnic, cultural and other differences. In order to ensure the normative expressions, it's necessary to standardize the terms in recording the adverse events. MedDRA (medical dictionary for regulatory activities) has been widely recommended and applied in the world as a powerful support for the adverse events reporting in clinical trials. In this paper, the development history, applicable scope, hierarchy structure, encoding term selection and standardized query strategies of the MedDRA is introduced. Furthermore, the practical process of adverse events encoding with MedDRA is proposed. Finally, the framework of statistical analysis about adverse events is discussed.
