ABSTRACT

In order to solve the difficulties and challenges in the implementation of the original blood distribution and collection regulations caused by the expansion of hospital area, the extension of blood transfer time, the changeability of blood transfer environment, and the strain of personnel due to the increase of workload, as well as to ensure the accuracy of the information throughout blood remote verification and distribution and the safety of clinical blood transfusion, , Shanghai experts related to clinical transfusion and blood management had made a systematic study on the applicable scope and management rules of remote verification of blood distribution and collection, and formulated this Expert Consensus combined with the development status of digital, intelligent and remote communication technologies, so as to provide corresponding guidance for clinical medical institutions in line with the changes in reality.

ABSTRACT

Objective To investigate whether the amount of blood transfusion affects the lengths of stay (LOS),costs,and outcomes of hospital patients or not,and to prepare for the execution of patient blood management.Methods The data of hospital patients,who had been administrated with blood in our hospital during 2016,were collected.And the influence of blood transfusion volume on LOS,costs and outcomes of patients was analyzed retrospectively.Results LOS,costs and outcomes of patients vary significantly with the amount of blood transfusion (P<0.01).There were positive correlations between the total amount of blood transfusion and LOS,costs,and outcomes of patients.The Spearman correlation coefficient was 0.317,0.497,0.290,respectively (P<0.01).Plasma preparation transfusion volume has a great influence on LOS,costs,and outcomes than red blood cell (P<0.05).The transfusion volume of death patients was significantly higher than that of the survival (P<0.01).In particular,the amount of transfused plasma and precipitation was distinctly higher than that in death patients(P<0.01).Conclusion Blood transfusion volume affects LOS,costs and outcomes of hospital patients.The administration of plasma preparations should deserve more attention.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the protective mechanism of stearoyl-CoA desaturase 1 (SCD1) over-expression against the pro-apoptotic affects of palmitic acid on hepatocytes using the rat BRL cell line.</p><p><b>METHODS</b>Concentration effect curves were generated using the trypan blue exclusion test to assess the death rate of BRL cells upon exposure to a dilution series of palmitic acid. The multiplicity of infection (MOI) of a lentiviral expression vector, pGC-FU-GFP, was determined for the BRL cells. Unmanipulated BRL cells were divided into two groups: the non-palmitate groups were composed of ordinary cultured cells (CON) alone, infected with lentivirus empty expression vector (negative control, NC), and infected with lentivirus overexpressing SCD1 (SCD1-LV); the palmitate groups were composed of ordinary cultured cells plus palmitate (CON+) alone, infected with lentivirus empty expression vector plus palmitate (NC+), and infected with lentivirus overexpressing SCD1 plus palmitate (SCD1-LV+). SCD1 mRNA expression was detected by real-time PCR. Propidium iodide (PI) single-staining was used to detect apoptosis and assess the cell cycle. Inter-group differences were analyzed statistically.</p><p><b>RESULTS</b>The death rate of BRL cells increased significantly after 72 h of exposure to 400 mumol/L palmitate (P less than 0.01). The MOI of pGC-FU-GFP in BRL cells was 20. The expression of SCD1 was significantly higher in the SCD1-LV and SCD1-LV+ groups than in the respective controls (vs. CON: F = 289, P less than 0.01; vs. CON+: F = 1522, P less than 0.01). Palmitate exposure led to decreased expression of SCD1 (CON+ vs. CON, F = 22, P less than 0.05 and NC+ vs. NC: F = 34, P less than 0.05). The ratio of S stage cells was similar in all non-palmitate groups (CON, NC and SCD1-LV, P = 0.137). However, there was a significant apoptotic peak and lower ratio of S stage cells in the control palmitate groups (CON+ and NC+) and the activity of cell proliferation was decreased as well. The ratio of apoptotic cells was decreased significantly in the SCD1-LV+ group compared to the CON+ group (P less than 0.01).</p><p><b>CONCLUSION</b>The expression of SCD1 and its desaturation activity increased in BRL cells upon infection with the pGC-FU-SCD1-GFP lentiviral vector, suggesting that SCD1 over-expression can decrease palmitic acid-induced toxicity and apoptosis in hepatocytes.</p>

ABSTRACT

Objective To investigate the effect of apolipoprotein E (ApoE) gene polymorphism on lipid metabolism of renal transplant recipients before and after transplantation. Methods ApoE gene polymorphism was detected by Polymerase chain reaction restriction fragment length polymorphism and serum lipid levels were measured by biochemical method. Results Serum lipid levels in the recipients were increased significantly at 3rd month after renal transplantation, and were ulteriorly elevated at 6 th month and the first year. The patients with higher total serum cholesterol (TC) and triglyceride (TG) levels were only accounted for 2.9 % and 7.6 % respectively before renal transplantation, but for 28.6 % and 46.7 % respectively at 3rd month after renal transplantation ( P

ABSTRACT

Objective To investigate the effect of the gene polymorphism of cholesterol ester transfer protein (CETP) on the serum lipid levels in renal transplant patients.Methods Serum total cholesterol (TC),triglyceride (TG),low density lipoprotein (LDL),high density lipoprotein cholesterol (HDLC),apolipoproteins (Apo A1,B,E) and lipoprotein(a) [Lp(a)] were measured. Polymerase chain reaction-restriction fragment length polymophism (PCR-RFLP) was used to detect CETP gene polymorphism in renal transplant recipients.Results Serum levels of TC,TG,HDLC,LDLC,ApoB,ApoE in the renal transplant recipients were increased significantly after transplantation. The allele frequency and the distribution of the TaqⅠ(intron 1) and MSPⅠ(intron 8) genotypes showed no significant difference between the controls and the renal transplant recipients. The serum TG level was significantly higher in renal transplant recipients with the genotype TaqⅠB1/B1 than those in the patients with genotype TaqⅠ B1/B2 and B2/B2. But there was no statistical difference among the serum lipid levels in renal transplant recipients with different MSPⅠgenotypes.Conclusion The serum lipid levels were increased significantly in transplant after transplantation,and the patients with CETP genotype TaqⅠB1/B1 liable to develop hypertrglyceridemia.
