ABSTRACT

Objective:To analyze the clinical characteristics, endoscopic manifestations, complications and related risk factors of button battery ingestion in 58 children, thus providing the basis for clinical diagnosis and treatment.Methods:The medical data of 58 children with button battery ingestion were collected and researched at Children′s Hospital Affiliated to Zhengzhou University from September 2015 to September 2020.The demographic information, battery impaction location, duration, symptoms, mucosal injury level, battery size, treatment, complications and follow-up results were analyzed.Results:The average age of the patients with button battery ingestion was (25.7±15.4)months, including 40 boys(68.9%). The average retention time of the battery in digestive tract was 13.8(2, 96) h. Vomiting, salivation, dysphagia, cough and fever were the common chief complaints.There were 29(50.0%) cases of grade I mucosal injury, as well as 14(24.1%) cases, 10(17.2%) cases and 10(17.2%) cases for grade Ⅱ, grade Ⅲ and grade Ⅳ, respectively.Additionally, common complications included esophageal stenosis, esophageal perforation and esophageal-tracheal fistula.Logistic regression analysis showed that location(esophagus) and diameter(≥15 mm) of battery incarceration were important predictors of complications.Conclusion:The degree of mucosal damage is associated with the diameter and impaction location of battery.The button battery embedded in the esophagus is prone to complications, while the ones retained in the stomach were not vulnerable to serious complications.Endoscopy and other related examinations should be performed again in 1 to 3 weeks after the button removal to determine the outcome of complications and to intervene in time.

ABSTRACT

Objective:To investigate the clinical characteristics, endoscopic features and management of acquired tracheoesophageal fistula (TEF) by esophageal foreign bodies in children.Methods:The clinical data and follow-up data of 21 children with acquired TEF who were treated in Children′s Hospital Affiliated to Zhengzhou University from January 2008 to January 2019 were retrospectively analyzed.Results:A total of 21 cases with esophageal foreign bodies were button batteries, irregular iron sheets, game coins, jujube seed, animal bone sheets, and fish thorn.The statistical results suggested that the cases of button batteries were 7 cases (33.33%), 4 cases of jujube pit (19.05%), 3 cases of irregular iron (14.29%) and 3 cases of animal bone (14.29%), 2 cases of game coin (9.52%), 2 cases of fish thorn (9.52%). All foreign bodies were removed by endoscopy.Esophageal perforation with TEF was discovered in 17 cases (80.95%) during the operation.Esophageal perforation with TEF was found in 4 cases (19.05%) within 2 week after the operation, and no death occurred.A total of 13 case fistula size ≤5 mm (61.90%), 4 case fistula size>5 mm and ≤10 mm (19.05%) and 4 case fistula size >10 mm (19.05%). The treatment methods included gastrointestinal decompression and nasojejunal nutrition tube support in 10 cases (47.61%), gastrostomy and jejunostomy in 4 cases (19.05%), surgical repair in 4 cases (19.05%) and endoscopic titanium clip suture in 3 cases (14.29%). Five cases (23.81%) were healed in 3 months, 6 cases (28.57%) in 5 months, 4 cases (19.05%) in 8 months, and 2 cases (9.52%) in 12 months.Conclusions:Acquired TEF in children are mostly caused by special foreign bodies embedded in the esophagus, and endoscopic management is effective for fistulas with less trauma.Appropriate operation and intervention should be taken as early as possible.

ABSTRACT

Objective:To explore the clinical characteristics and treatment strategies of upper gastrointestinal perforation caused by ingested foreign bodies in children.Methods:A retrospective analysis of 32 patients with upper gastrointestinal perforation secondary to foreign bodies in Henan Children′s Hospital from January 2011 to February 2019 was performed.Some basic information was collected, including age, gender, type of foreign bodies, location of perforation, time of swallowing, clinical symptoms, treatment methods and outcomes.Results:Among the 32 children, there were 20 cases (62.5%) were male and 12 cases (37.5%) were female.The predilection age was 0 to <3 years old [21 cases (65.6%)]. The most common type of upper gastrointestinal foreign bodies was jujube pits [13 cases (40.6%)], followed by button batteries [10 cases (31.2%)], magnets [3 cases (9.4%)], fish bones [2 cases (6.3%)], lollipop stick [2 cases (6.3%)], button battery combined with magnet [1 case (3.1%)] and metal pin [1 case (3.1%)]. Majority of perforation was located in the esophagus [25 cases (78.2%)], especially in the upper esophagus [15/25 cases, (60.0%)]. The common symptoms of upper gastrointestinal perforation include fever [22 cases (68.8%)], dysphagia and/or salivation [21 cases (65.6%)], vomiting [24 cases (75%)], abdominal pain [5 cases (15.6%)] and expiratory dyspnea [1 case (3.1%)]. All the foreign bodies were successfully extracted by endoscopy [28 cases (87.5%)] and surgery [4 cases (12.5%)]. Moreover, all the patients recovered with both treatments of indwelling gastrointestinal decompression tube and enteral nutrition, including nasojejunal tube [24 cases (75.0%)], endoscopic hemoclip and nasojejunal nutrition [2 cases (6.3%)], and surgical operation [6 cases (18.7%)].Conclusions:The upper gastrointestinal foreign body is a common emergency in children, and might cause perforation, especially jujube pits, button batteries and magnets.The endoscopic procedure is safe and effective for the removal of foreign bodies.However, the surgical intervention is required as soon as possible due to the failure of endoscopic treatment.

ABSTRACT

As a common disease in pediatric outpatient and emergency departments,foreign bodies in digestive tract of children can occur in all ages. Foreign bodies in digestive tract is swallowed accidently or deliberately,which can neither be digested nor be absorbed in gastrointestinal tract. Early and timely diagnosis of foreign bodies in digestive tract which failed to pass spontaneously through the gastrointestinal tract or caused gastrointestinal tissue damage should be fully assessed and perfect auxiliary examination. Emergency endoscopic treatment with high success rate,less complications and quick recovery of organ function has become the best treatment for foreign bodies in digestive tract and is worth for extensive application in the clinic.

ABSTRACT

Objective To explore the clinical features, endoscopic changes, and histopathological features of abdominal type Henoch-Scho..nlein purpura (HSP) in children. Methods The clinical data of 151 children with HSP who had abdominal pain as the primary symptom from June 2011 to June 2016 were analyzed retrospectively. Results A total of 151 children with HSP, (87 males and 64 females) aged 4.8 - 11.5 years, were enrolled. All of them had abdominal pain. There were 100 cases of cutaneous purpura (66.22%), 72 cases of vomit (47.68%), 59 cases of haematemesis (39.07%), 44 cases of hematochezia (29.13%) and 8 cases of joint swelling and pain (5.29%). Peripheral blood leucocytes increased in 114 cases (75.49%), C-reactive protein increased in 48 cases (31.78%), and albumin decreased in 21 cases (13.90%). It indicated the localized thickening of the intestinal wall in 49 cases (32.45%) by B-ultrasound. Gastroscopy was performed in 96 cases and main findings were patchy hemorrhage and erythema (89 cases, 92.70%), erosion or ulcer accompanied by bleeding (68 cases, 70.83%), ecchymosis (37 cases, 38.54%) and hematoma like process (13 cases, 8.60%). The descendent duodenum was the most common and had the most serious lesions, followed by gastric antrum and duodenal bulb. Histopathology showed capillary inflammation in 37 cases, 10 of which were accompanied by Helicobacter pylori infection.Enteroscopy was performed in 55 cases and main findings were punctate hemorrhage and erythema (49 cases, 89.09%), and erosion or multiple ulcer with hemorrhage (27 cases, 49.09%). The terminal ileum was the most common and had the most serious lesions, followed by rectum. Conclusions The clinical features of abdominal type HSP in children are varied, and it often involves descendent duodenum and terminal ileum. Endoscopic examination is valuable for the diagnosis of abdominal type HSP in children.

ABSTRACT

Objective To observe what changes the brown adipose tissue (BAT) of T2DM rat models would have,including morphology,function and specially expressed uncoupling protein (UCP1) after the gastric bypass (Roux-en-Y,RYGB) and to explore the effects of RYGB on BAT of T2DM rat models and its related mechanism in order to provide a theoretical and experimental basis for treatment of T2DM patients with RYGB.Methods SD rats were given a high-fat and high-sugar diet for two weeks,by injecting streptozotocin (STZ) 30 mg/kg intraperitoneally to build models.Blood glucose was measured after 72 h and 1 week by the fast blood glucose meter.The models were built successfully if blood glucose at both times were ≥ 16.7 mmol/L.Feeding environment:individually caged,standard rat feed,natural circadian cycle,indoor temperature (18±2)℃,indoor humidity (50±2)%.50 rats were randomly selected and dividing into four groups according to intervention methods:diabetes operation group (group A,n=10),undergoing RYGB surgery with the whole stomach kept;diabetes sham operation group (group B,n=10),the same anesthesia and incision as the previous RYGB group.The operation mode was anterior gastric wall incision and suture,jejunum transection in corresponding position and in situ anastomosis with the same suture method as group A;diabetes control group (group C,n=10),normally feeding after building models;and the last one was the healthy control group (group D,n=10):no special treatment,adequate water feeding ensured.The rest of rats remained to be used.The body mass (BM),fasting blood glucose (FBG),fasting serum insulin(Fins)before and at the 1st,2nd,4th and 8th week after surgery were measured.The number of transversal ceils was calculated by IPP6.0 image software and the average radius of fat cells was calculated.UCP1 expression was tested with western blot.Results ① The fasting blood glucose,fasting serum insulin level and the body weight of dia betic rats were higher than those of the control group,but the insulin sensitivity index was significantly lower.② HE Staining showed:diabetes operation group (group A) rats,compared with diabetes control group and diabetes sham operation group(group B),had obviously higher brown fat cell counts transversally and average radius,and the difference was statistically significant (P<0.01).Diabetes operation group (group A) rats had no significant difference from the healthy control group(group D) rats,and the diabetes control group (group C) rats had no significant difference from sham operation group (group B) rats as well.③ Western blot showed that after the gastric bypass surgery,compared with the diabetes sham operation group (group B) and the diabetes control group (group C),UCP1 expression of brown adipose tissue of the diabetes operation group (group A) increased significantly (P<0.05).The diabetes sham operation group (group B) had no significant difference from the diabetes control group (group C),and the diabetes operation group(Group A) had no significant difference from the healthy control group (Group D) as well (P>0.05).Conclusion RYGB can reduce the body mass and insulin resistance (IR) of diabetic rats and,at the same time,promote the expression of UCP1 of brown adipose tissue.RYGB might increase the activity of brown adipose tissue by regulating the UCP1 signaling pathway to improve body's insulin resistance.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of laparoscopic Roux-en-Y gastric bypass on the short-term immune function of type 2 diabetic patients and to explore the correlation between the immune regulatory effect and blood glucose control.</p><p><b>METHODS</b>Clinical data of 28 patients with type 2 diabetes mellitus who underwent laparoscopic Roux-en-Y gastric bypass surgery in our hospital during January 2014 to January 2015 were retrospectively collected. Fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), immunoglobulin (IgG, IgA, IgM), T lymphocytes CD3(+), killer T cells CD3(+)CD8(+), helper T cells CD3(+)CD4(+), natural killer(NK) cell subsets CD16(+),CD56(+) and B cell subsets CD19(+),CD45(+) levels were detected and compared between before and after surgery. Correlation of immunoglobulin and immune cell subsets with the level of FBG and HbA1c were examined.</p><p><b>RESULTS</b>After operation, FBG and HbA1c decreased significantly from (5.78±1.15) mmol/L to (14.21±1.89) mmol/L, and (9.96±0.97)% to (6.87±0.69)%, respectively (all P<0.05), and immunoglobulin (IgG, IgA, IgM) increased significantly [IgG: from(9.41±1.23) g/L to (12.74±1.61) g/L, IgM: from (1.71±0.22) g/L to (2.43±0.39) g/L, IgA: from (1.25±0.26) g/L to (1.97±0.23) g/L, all P<0.05]. Besides, T lymphocytes CD3(+), killer T cells CD3(+)CD8(+) and B cell subsets CD19(+)CD45(+) elevated significantly as well [T lymphocytes CD3(+): from (55.3±3.8)% to (67.6±4.6)%, killer T cells CD3(+)CD8(+): from (15.6±5.3)% to (28.7±4.2)%, B cell subsets CD19(+)CD45(+): from (8.4±3.7)% to (18.1±4.1)%, respectively, all P <0.05]. There was no significant difference in the expression of helper T cells CD3(+)CD4(+) and natural killer cell subsets CD16(+)CD56(+) between pre-operation and post-operation(all P>0.05). Levels of immunoglobulin (IgG, IgA, IgM), CD4(+)/CD8(+) ratio and B cell subsets were negatively correlated with HbA1c and FBG levels (IgG with FBG: r=-0.865, IgA with FBG: r=-0.887, IgM with FBG: r=-0.902, CD4(+)/CD8(+) with FBG: r=-0.956, CD19(+)CD45(+) with FBG: r=-0.834; IgG with HbA1C: r=-0.859, IgA with HbA1C: r=-0.957, IgM with HbA1C: r=-0.843, CD4(+)/CD8(+) with HbA1C: r=-0.912, CD19(+)CD45(+) and HbA1C: r=-0.885, all P<0.05), but the proportion of NK cells was not significantly correlated with HbA1c and FBG (P>0.05).</p><p><b>CONCLUSIONS</b>Laparoscopic Roux-en-Y gastric bypass has good effect on the immune function of patients with type 2 diabetes mellitus. Improvement of immune function is correlated with the control of FBG and HbA1c.</p>

ABSTRACT

Objective To optimize the prescription of GA and GB hydrophilic gel matrix tablets; To study the in vitro release mechanism. Methods On the basis of the results of the mono-factor investigation, mixture uniform design was used to optimize the handicraft molding prescription of GA and GB hydrophilic gel matrix tablets. The release mechanism was investigated by the vitro of the GA and GB hydrophilic gelmatrix tablets to accumulate releasing rate to conduct linear fitting. Results The optimized prescription of GA and GB hydrophilic gel matrix tablets was: powder: HPMC: lactose=23:24:53. Conclusion Mixture uniform design can be used to optimize the prescriptions of GA and GB hydrophilic gel matrix tablets, and the results are accurate. The hydrophilic gelmatrix tablets release medicine by non-Fick mechanism, and the medicine release is in accordance with zero-order.

ABSTRACT

<p><b>OBJECTIVE</b>To establish a new optimized extraction process based on dynamic changes in extract concentration during the extraction process.</p><p><b>METHOD</b>Sanqi was taken as an example to compare the concentration of marker components, total solids yield and similarity evaluation of the fingerprint images of extracts by orthogonal design optimization process and dynamic optimization process to assess the feasibility of dynamic process optimization method.</p><p><b>RESULT</b>The concentration of marker components and total solids yield vary less than 5%, with a 100% of similarity in fingerprints, indicating basic coincidence between extracts by the two optimized processes. The dynamic process optimization process could save 79% of the extraction time.</p><p><b>CONCLUSION</b>In the extraction time, the dynamic optimization process is superior to the orthogonal design optimization process in that it can significantly reduce the extraction time and lower production costs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish the reference extraction method for determining five components contained in Pueraria labta decoction and detecting the applicability and feasibility of the application of reference extracts according to the quality standard of traditional Chinese medicine.</p><p><b>METHOD</b>Zorbax C18 chromatographic column (4.6 mm x 250 mm, 5 microm) was used, with methanol-0.02% phosphoric acid (25:75) as the mobile phase and the detection wavelength was set at 250 nm. Self-prepared reference extracts of P. labta was used for determining the content of 3'-hydroxy puerarin, puerarin, 3'-methoxy puerarin, isoflavoues aglycone-8-C-apiose-(1--> 6) glucoside and daidzin and compare with the experimental results obtained with the single reference substance determination method.</p><p><b>RESULT</b>The reference extraction determination method showed high precision, stability and reproducibility, with coincident determination results with the results obtained with the single reference substance determination method.</p><p><b>CONCLUSION</b>The reference extraction determination method of P. labta can be used for quality control of P. labta herbs. The study preliminarily proves the feasibility of the application of P. labta in traditional Chinese medicine.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe effect of oral scorpio and scolopendra powder on T-cell subsets in peripheral blood and intestine from rats with collagen induced arthritis (CIA).</p><p><b>METHOD</b>60 rats were randomly divided into 6 groups: normal control group, model control group, low-dose scorpio and scolopendra group, middle-dose scorpio and scolopendra group, high-dose scorpio and scolopendra group, and type II collagen group. Rat's rheumatoid arthritis was induced by collagen II (C II). Level of T-cell subsets from peripheral blood and intestine was measured by flow cytometry.</p><p><b>RESULT</b>CD4+ T cellular level was obviously increased (P < 0.05 or P < 0.01) or kept increased tendency in peripheral blood and intestine from the model group compared with that of the normal group, while the ratio of CD4+/CD8+ in intestine was obviously descent but the contrary in peripheral blood (P < 0.05 or P < 0.01). CD4+, CD8+ T cellular level in intestine were obviously descent and the ratio of CD4+ /CD8+ increased in all treated groups when compared with in the model group (P < 0.05 or P < 0.01). However, CD4+ T cellular level and the ratio of CD4+/CD8+ in peripheral blood were remarkablely decreased.</p><p><b>CONCLUSION</b>The mechanism that scorpio and scolopendra could treat rat's rheumatoid arthritis may be regulating balance of T-lymphocyte subsets in peripheral blood and intestine.</p>

ABSTRACT

To establish kinetic assay method for the analysis of hemolysis and to investigate dynamic hemolytic process of polysorbate 80. The UV-VIS spectrum of heme changes when hemoglobin is released continuously during the hemolytic process. Therefore, dynamic hemolytic curve was determined as a new way to characterize the kinetic process of interaction between polysorbate 80 and red blood cells. The effect of polysorbate 80 on blood cells could be perfectly investigated by the hemolytic dynamics. Dynamic hemolytic parameters of polysorbate 80 were calculated according to the hemolytic curves. The constants of hemolytic rate and maximum hemolytic rate of polysorbate 80 had fine linear relationships at the range of 1-20 mg x mL(-1) and 5-20 mg x mL(-1), respectively. In comparison with the present official method such as macroscopic observation and static spectrophotometric methods, kinetic spectrophotometry has the advantages of real time, on-line determination, sensitive, objective, good reproducibility and 2-dimensional information acquired. Therefore, as a biological technique, kinetic spectrophotometry could be applied to evaluate the quality of polysorbate 80 and to screen other solubilizing excipients.

AIM:To establish a method for simultaneously determining luteolin and negletein in Mosla chinensis Maxium collected in picking times.METHODS: The Angilent Zorbax,Extend C-18 column(4.6 mm×250 mm,5μm) was used with methanol-water (containing 0.4% phosphonic acid)(50: 50) as mobile phase.The flow rate was 1.0 mL/min.The detection wavelength was set at 350 nm.RESULTS : The method had the good linear relationship within the range of 0.04-0.61 μg for luteolin and within the range of 0.20-3.01 μg for negletein,respectively,with the correlation coefficient of 0.999 9.The average recoveries of the two flavones were 97.32%,98.60%,respectively.The contents of luteolin and negletein in Mosla chinensis Maxium.varied in different picking times,which reached the its maximum at florescence (264.8 μg/g,2 746.3 μg/g) in August,respectively.CONCLUSION: The method is fast,rapid,convenient,accurate and can be used for quality control of Mosla chinensis Maxium.

ABSTRACT

Objective To explore the value of low resection of rectal carcinoma in pull-through anus operation and its influence factor with rectal carcinoma distant metastases along intestine wall.Method Clinical pathological and visited data of 43 patients with rectal carcinoma were analyzed prospectively.Resuits Forty--one cases were successfully accepted resection in pull-through anus (95.3%).No patient suffered from stoma leak,3 cases appeared recurrence after operation.Compared between high differentiation patients and mid differentiation patients,there was no significant difference, while the difference of comparing between high or mid differentiation patients and low differentiation patients respectively,there was significant difference.The difference of comparison among Dukes stage A,B or C was significant.Conclusions The right length of incising distant intestine tube is more than 2.0,3.0,4.0 cm respectively for rectal careinoma of high,mid and low differentiation or that of Dukes stage A,B or C.low resection of rectal carcinoma in puB-through anus should be considered as a regular operation.
