ABSTRACT

Objective:To establish and implement a management information system for patients with stomas based on the hospital information system.Methods:Relying on information system resources of Beijing Cancer Hospital, Beijing Institute for Cancer Research, the management information system for patients with stomas was developed by the Nursing Department and the Information Center from August 2018 according to the requirement of whole process management for patients with stomas.Results:A whole process management information system was constructed for patients with stomas including stoma construction, stoma care, management of complications and stoma closure. It was applied to 12 departments covering clinical departments and stoma clinics of the hospital, including 326 cases with stomas and 6 358 cumulative visits. 28 cases with skin complications around stoma were detected and the rate of complications was 8.59% (28/326) from May 2019 to October 2020.Conclusions:The management information system for patients with stomas can realize the whole-process management from stoma construction, stoma care, management of complications to stoma closure, and provide basis and guidance for nursing staff to implement professional nursing timely and accurately, which can also provide basis for continuously improving the quality of stoma care.

ABSTRACT

Objective@#To introduce Revised American Pain Society Patient Outcome Questionnaire for patients with cancer pain in China, and to test its reliability and validity.@*Methods@#Cross-cultural adjustment was conducted on the basis of the Revised American Pain Society Patient Outcome Questionnaire of the American Pain Association published on the official website through expert review and pre-experiment. A convenience sample of 153 hospital patients with cancer pain was recruited. And the data were analyzed for reliability and validity.@*Results@#The adjusted Revised American Pain Society Patient Outcome Questionnaire contains 18 core items, which are easy to understand and can be completed within 10 minutes. Reliability test: the Cronbach α coefficient of internal consistency for the total scale was 0.735. The Cronbach α coefficient of pain intensity dimension was 0.233, the Cronbach α coefficient of sleep interference dimension was 0.891, the Cronbach α coefficient of activity interference dimension was 0.830, the Cronbach α coefficient of emotion dimension was 0.846, the Cronbach α coefficient of pain management related side effects dimension was 0.591, and the Cronbach α coefficient of perception dimension of pain care was 0.633. Validity test: The total content validity of the scale was 0.98, and the content validity of each item ranged from 0.82 to 1.00.@*Conclusion@#The adjusted Revised American Pain Society Patient Outcome Questionnaire has good reliability and validity, providing an effective assessment tool for medical institutions to evaluate the quality of cancer pain management.

Objective:To establish and implement an information system for case management based on the Jiahe Electronic Patient Record System, Hospital Information System (HIS), Laboratory Information System (LIS) and Picture Archiving and Communication Systems (PACS).Methods:Relying on the hospital information system resources, an information system for case management was developed by nursing department and information department according to the clinical characteristics of cancer patients and the needs of the completely informative case management. Case managers were put in charge of the system in clinical practice.Results:The information system for case management was established which provides the whole-process including taking the case, diagnosis, treatment, follow-up and closing the case. The study consisted of 3 901 patients in case management, in which 2 396 cases closed, 2 744 cases received multi-disciplinary team discuss from January to December 2018. The planning treatments implementation rate was 96.85% (3 756/3 878) and the timely follow-up rate was 95.60% (4 501/4 708) through the implementation of case management.Conclusion:The information system for case management can realize the whole-process management from taking case stage until the end of follow-up, provide a convenient and practical information management for post duty of the case manager, and also a quantitative basis for the supervision of the quality of the case management.

ABSTRACT

Objective To establish and implement a central venous linemanagement information system based on the hospital information system (HIS). Methods The central venous line information management system was developed by the infusion therapygroup and the information technology department according to the requirement of completed central venous line management and the Chinese intravenous nursing practice standard. Results ① A complete information management system was constructed for central venous line from placement, maintenance and management of complications to drawing tube, and it operated smoothly. ②Totally, 4304 cases of PICC, 14,330 cases of CVC and 123 cases of PORT were collected from January 2016 to December 2017. The information of the maintenance number, catheter retention time, complications can be counted. Conclusions The central venous vein management information management system can realize the whole-process management, and ensure the continuous evaluation, maintenance and tracking of the patient's central venous line, making detailed statistical analysis of catheter- related complications, whichcan provide reliable basis for continuous nursing quality improvement.

ABSTRACT

Objective To investigate the current status of knowledge and attitudes of oncology nursing staffs on pain management in cancer patients. Methods A total of 232 oncology nursing staffs from 7 Grade III-A Hospitals in 4 provinces and municipalities were surveyed using General Information Questionnaire and knowledge and attitudes survey regarding pain (KASRP) (2014) questionnaire (Chinese version). Results The results of this survey showed that the average score of knowledge and attitude about pain management of 232 oncology nursing staffs was (21.56±4.01) points, and the average correct rate was 52.59% . There were no significant differences in the scores of different education background, years of nursing work, years of cancer clinical nursing care, and the nature of the hospital they worked in (P>0.05). However, there was a difference in the score of nursing staffs with different professional titles (F=5.885, P=0.003). Besides that, difference also showed in scores of nursing staffs with different experiences in caring cancer pain patients (F=3.216, P=0.024). Conclusion The knowledge and attitude of pain management in oncology nursing staffs are in low level, the command of knowledge is not enough, meanwhile, the pain management ability is insufficient. Therefore, it is necessary to strengthen the training of pain related knowledge of nursing staffs and improve their pain management ability.

ABSTRACT

Objective@#To evaluate the reliability and validity of Chinese-version Good Death Inventory among oncology nurse specialists.@*Methods@#A further revision has been made in the Chinese-version Good Death Inventory from Taiwan experts through expert review and preliminary experiment. The reliability and validity of the Chinese-version GDI was evaluated among 180 oncology nurse specialists.@*Results@#The Chinese-version of GDI was easy to understand. Most respondents finished the whole scale in 25 minutes and the valid response rate of the scale was100%. The I-CVI value of each item was 0.67-1.00, S-CVI/Ave was 0.938. KMO was 0.832, and the revised Chinese-version GDI consisted of 54 items in 18 dimensions (Core 10 Domains and Optional 8 Domains). The total scale′ s Cronbach′ s alpha coefficient was 0.950, split-half reliability coefficient was 0.832. Cronbach′ s alpha of Core 10 Domains was 0.938 and that of Optional 8 Domains was 0.876.@*Conclusions@#The Chinese-version GDI is a valid scale to measure end-of-life care comprehensive outcomes from Chinese oncology nurse specialists′ perspective.

ABSTRACT

Objective To investigate the effect of case management in the perioperative period of patients with gastrointestinal tumor. Methods The quasi-experiment study were took to design the research. A total of 100 patients were enrolled in this study. Patients were divided into control group (n=50) and intervention group (n=50) by the admission time. The control group received routine nursing care, while the intervention group received routine perioperative care and case management during the perioperative period. The uncertainty of illness was compared between the two groups at the time point admission, on discharge and 5 weeks after surgery. The satisfaction during the process of treatment services, postoperative complications and return visit rate were compared too. After 5 weeks follow up, 10 cases in the control group and 4 cases in the intervention group were lost to follow-up, 86 cases were included in data analysis. Results Two groups patients all had medium uncertainty of illness at the three-different time points(preoptrative, 1 day before discharge and 5 weeks after surgery). The scores of the control group were 103.38 ± 14.94, 109.58 ± 16.06 and 113.90 ± 15.95 respectively.Those of the intervention patients were 108.02 ± 15.29,109.89 ± 16.52 and 113.41 ± 14.70.The uncertainty of illness elevated on the 5th week after surgery. Significant statistical differences were identified in the uncertainty of illness within each group (F=14.117, P<0.01). Significant statistical difference was identified in the visit rate, 100% (46/46)vs.87.5% (35/40) (χ2=6.105,P<0.05). The score of preoperative satisfaction of intervention patients (14.41 ± 1.13) was obviously higher than that of control patients (13.58 ± 1.55) (t=-1.89, P<0.05). Whereas, incidence of postoperative complications was not statistically significant between two groups. Conclusions The specificity of gastrointestinal tumor and the complexity of treatment bring about great pressure to patients. As the type of entire journey care model, the case management could integrate the fragmented management of conditional management model. Meantime, it also could effectively improve the medical experience and outpatient visit rate of patients with gastrointestinal tumor.

ABSTRACT

Objective To investigate burden on caregivers of patients with neuroendocrine tumors and to explore possibly related factors in a relative big sample. Methods A face-to-face interview was conducted with major caregivers of neuroendocrine tumors patients admitted to January 2016 to October 2016,applying the Zarit Caregiver Burden Interview(ZBI)scale to investigate the burdens and to explore possibly related factors. Results A total of 185 major caregivers participated. ZBI score was (35.41 ± 11.67)points,and there were only 13.5%(25/185)caregivers had no burden.Most(52.4%,97/185)were under moderate burden, and over 34.1%(63/185) were bearing severe burden. The multi-element linear regression results showed that related factors included the age,gender,education background,duration of care giving, the lasting period of the patients' disease, and the family's monthly income (P<0.05). Conclusions Nearly 90% caregivers of patients with neuroendocrine tumors were under burdens,mostly on a moderate level.In addition,the burdens were even higher when the caregivers were elder,female and highly educated, longer care duration, loner disease duration and lower family income, to which special attention should be paid,and more education and support should be provided clinically.

ABSTRACT

Objective To study the reference values and related factors of tympanometry for children between 2 and 15 year-old .Methods From December 2014 to March 2015 ,a total of 120 children (240 ears total) among 2 to 15 year-old (6 .95-3 .12) with Type A tympanograms received the singe -frequency component tympanometry test ,including acoustic equivalent volume (Vea ) ,peak-compensated static admittance (Ytm ) ,tympanometric peak pressure (TPP) and tympanometric gradient (TG) .With the results of each test ,the relationships were conducted according to the differences of age and gender .The outcomes also were compared with the recent studies abroad .Re-sults The difference of TPP was statistically significant among the gender group ,and the differences of Vea and TPP were statistically significant between different age groups ,2-6 year pre-school children and 7~15 yr school children .Conclusion The normative values of each tests were significantly different between Chinese and western children of 2-15 years .We would speculate that the differences of TPP in the age and gender group have changed with the physiological characteristics of Eustachian tubes and eardrums and the differences among male and female .

ABSTRACT

This article reported hospice care for a patient with pancreatic cancer in the end-of-life stage. We relieved the patient's pain and other painful symptoms,provided psy-chological counseling and support,respected the patient's wishes and autonomy,and guided family members to ef-fectively participate in end-of-life care. Through the above interventions,we attained the goal that the patient passed away peacefully and their caregivers lived without regrets.

ABSTRACT

Objective To determine the cut-off points of Psychological Distress Scale of Cancer Patients by using the ROC curve, in order to provide reference for clinical application of it. Methods A questionnaire containing both the Psychological Distress Scale of Cancer Patients and Symptom Checklist-90 (SCL-90) were filled by 364 cancer patients in Beijing Cancer Hospital. Later the cut-off points of the four latitudes of the Scale were evaluated by ROC curve through comparing the two scales. Results By using the latitudes of the SCL-90 scale as a reference, the areas of all the four latitudes of the Scale were:anxiety (0.815, P<0.01); depression (0.790, P<0.01); interpersonal disability (0.829, P<0.01);suspiciousness (0.670, P<0.01) respectively. The following cut-off points could result in the most ideal sensitivity and specificity: 11 for anxiety; 14 for depression; 7 for interpersonal disability and 4 for suspiciousness. Conclusions The Psychological Distress Scale of Cancer Patients can provide great values to the diagnosis of psychological problems related with anxiety, depression, and interpersonal disability, while low valuable reference for diagnosis of suspiciousness. It can provide good diagnosis value which contributes to evaluation of psychological problems of cancer patients.

ABSTRACT

Objective To explore the moves that creating magnetic work environment in hospital, in order to improve the nurses job satisfaction and reduce the turnover rate then stable the nursing team. Methods Through a series of measures such as the establishment of scientific rational allocation of human resources, implementing fair performance evaluation and allocation mechanism, carrying out the nurse grading management system to create magnetic nursing work environment, the nurse job satisfaction and turnover rate were compared before and after the measures. Results In January 2014 a total of 520 questionnaireswere issued, 519 valid questionnaires, and the effective recovery rate was 99.81%(519/520);And 559 were valid of 577 issued questionnaires, which was 96.88% (559/577). The overall score of nurses′job satisfaction increased from (3.20±0.46) points in 2013 to (3.41±0.52) points in 2014 (t=9.40, P= 0.000); The nurse turnover rate fell to 1.97%(13/661) from 5.13% (30/585) (χ2= 10.27, P= 0.003). Conclusions That is very important for remain stable and high quality nursing team to carry out scientific and fine management according to the demand of nurses, so as to create a safe, harmonious, positive, cooperative magnetic nursing work environment which have development direction and space.
