ABSTRACT

【Objective】 To retrospectively analyze the serological and nucleic acid testing(NAT) data of voluntary blood donors from six blood banks in Tibet, in order to explore the positive impact of NAT on reducing the risk of infective transfusion in a regional scope. 【Methods】 From 2018 to 2022, 38 718 voluntary blood donors from blood centers of Tibet, Shannan, Shigatse, Naqu, Nyingchi and Ngari were tested for hepatitis B virus surface antigen (HBsAg), hepatitis C virus antibody (anti-HCV), human immunodeficiency virus antigen (HIV) and antibody (Ag/Ab1+2) serological determination by enzyme-linked immunosorbent assay (ELISA). At the same time, Haoyuan and Daan nucleic acid detection systems were used for the combined detection of HBV-DNA, HCV-RNA and HIV-RNA. The results of NAT of reactive ELISA samples were statistically analyzed. 【Results】 A total of 178 ELISA-/NAT+ samples were detected in Tibet over the past five years, including 170 HBV-DNA positive cases, 8 HCV-RNA positive cases, and 0 HIV-RNA positive cases, with the positive rate at 0.460%.The detection rate of 624 ELISA+/NAT+ samples was 1.61%.The age of blood donors with hepatitis B in Shigatse area was slightly higher than that in other areas, and the difference was statistically significant(P<0.05) . 【Conclusion】 The centralized detection of viral nucleic acid in Blood Center of Tibet Autonomous Region can effectively reduce the missed detection of transfusion transmitted diseases and guarantee the blood safety in the region.

ABSTRACT

Objective To investigate the role of HER2 and Ki-67 in colorectal cancer pathogenesis and their influence on the prognosis in the patients with colorectal cancer.Methods The expressions of HER2 and Ki-67 proteins in colorectal cancer tissues from 71 cases of colorectal cancer and 30 cases of para-cancerous normal tissues were detected by usingthe immunohistochemistry.The expressions of HER2,Ki-67 and their clinicopafhological parameters and prognostic significance were also analyzed.Results The positive expression rate of HER2 in colorectal cancer tissue (32.4%) was higher than that in para-cancerous normal tissues (10.0%);the positive expression rate ofKi-67 in colorectal cancer tissue (85.9%) was significantly higher than that in para-cancerous normal tissues (6.7 %),the difference was statistically significant (P<0.05).The expression of HER2 in colorectal cancer tissues was correlated with tumor differentiation degree,lymph node metastasis,number of lymph node metastasis and TNM staging (P<0.05).The expression of Ki-67 in colorectal cancer tissues was correlated with infiltration depth,lymph node metastasis,number of lymph node metastasis and TNM staging (P<0.05).HER2 in colorectal cancer tissues was positively correlated with Ki-67 (r=0.515,P=0.001).The 5-year disease-free survival rates in the HER2 positive group and the HER2 negative group were 48.4% and 80.8%,the difference was statistically significant (P<0.05).The 5-year diseasefree survival rates in the Ki-67 high expression group and the Ki-67 low expression group were 56.5 % and 85.5%,the difference was statistically significant (P<0.05).The 5-year survival rate in the HER2 positive group and the HER2 negative group were 60.9 % and 85.5 %,the difference was statistically significant (P<0.05).The 5-year survival rate in the Ki-67 high expression group and the Ki-67 low expression group were 63.9% and 85.5 %,the difference was statistically significant (P<0.05).The single factor analysis showed that tumor differentiation degree,infiltration depth,lymph node metastasis,number of lymph node metastasis,TNM staging,the expressions of HER2 and Ki-67 were related with prognosis in the patients with colorectal cancer (P<0.05).The Cox multivariate analysis results showed that tumor infiltration depth and TMN staging were the independent factors affecting prognosis in the patients with colorectal cancer.Conclusion The expressions of HER2 and Ki-67 are associated with the development of colorectal cancer.The 5-year disease-free survival rate and the 5-year survival rate in the patients with HER2 and Ki-67 positive become significantly decreased.

ABSTRACT

Transcatbeter aortic valve replacement(TAVR) has been operated successfully since 2002 in France and lasted for 16 years now.In this period,the operation number is explosive and the indication of operation is expansived progressively.But this progress is slow in China,even in Asia,since the expensive fee,special character of aortic anatomy of Asian and the concern of its' complication.Now we summarize this situations of TAVR in recently year and want to give a new ideal to our reader.

ABSTRACT

OBJECTIVE:To promote the healthy development of TCM decoction piece. METHODS:Based on analyzing the literature,national policy and data,the advantages and urgent problems of bringing TCM decoction piece into the National Essen-tial Medicine List were summarized,and the suggestions for its health development were provided. RESULTS:Bringing TCM de-coction piece into the National Essential Medicine List relieved the economic burden of patients and provided opportunity for the de-velopment of TCM decoction piece industry. However,it existed problems,such as production,circulation,storage and monitor-ing;mutual conflict in the relevant supporting policies;inconsistent medicare reimbursement;and urgent improvement of quality standard. CONCLUSIONS:The relevant departments should establish and improve the quality standard system of TCM decoction piece,and clear the varieties of TCM decoction piece in the National Essential Medicine List,perfecting supporting policies such as medicare reimbursement and attaching importance to the training of professional talents can ensure scientific and rational services for the public health.

ABSTRACT

Objective To investigate the effects of leflunomide(A771726) on the apoptosis of the rat glomerular mesangial cells and its possible mechanism .Methods The cultured rat glomerular mesangial cells were divided into the normal control group (adding DMEM culture solution containing 5% fetal calf serum) ,A771726 group(adding 50 A771726 on the basis of control group , making it a concentration as 50 μg/mL) ,LY294002 group (adding LY294002 on the basis of normal control group ,making its final concentration as 2 μg/mL) and the LY294002+A771726 group(first adding 2μg/mL LY294002 to interfere the mesangial cells for 2 h ,then adding A771726 50μg/mL) .After 48 h intervention ,its influence on mesangial cell apoptosis in each group were measured by flow cytometry .The expression of mTOR was measured by immunofluorescence .The expression of Caspase‐3 was measured by Western‐blot .Results Compared with the normal control group ,the apoptosis rate of rat glomerular mesangial cells in the A771726 group ,LY294002 group and LY294002+A771726 group were significantly increased ,the expression of mTOR was decreased ,the expression of Caspase‐3 was increased;compared with the A771726 group ,the apoptosis rates of rat glomerular mesangial cells in the LY294002 group and LY294002+ A771726 group were significantly increased ,the expression of mTOR was significantly de‐creased ,the expression of Caspase‐3 was significantly increased;compared with the LY294002 group ,the apoptosis rate of rat glo‐merular mesangial cells in the LY294002+ A771726 group was significantly increased ,the expression of mTOR was significantly decreased ,while the expression of Caspase‐3 was significantly increased .Conclusion Leflunomide may induce the rat mesangial cells apoptosis through down - regulating the PI3K/Akt/mTOR signaling pathway ,moreover ,leflunomide may synergize with LY294002 to induce rat mesangial cells apoptosis .

ABSTRACT

<p><b>OBJECTIVE</b>To assess the safety and clinical efficacy of transjugular intrahepatic portosystemic shunt (TIPS) with various stents for treating patients with cirrhosis and esophageal gastric varices bleeding.</p><p><b>METHODS</b>One hundred and five patients were stratified according to stent type: bare stent group, covered stent-grafts group, combined stents group. Rates of success, shunt insufficiency, rebleeding, patient survival, and major complications were observed. The shunt insufficiency rate, rebleeding rate, and survival rate were calculated by the life tables method, the Kaplan-Meier analytical curve, and the log-rank test; a p-value less than 0.05 was considered statistically significant.</p><p><b>RESULTS</b>The overall success rate of all TIPS for treating the esophageal gastric varices bleeding was 100%. The overall shunt insufficiency rates at 6-, 12-and 24-months post-TIPS were 8%, 9% and 16%, rebleeding rates were 2%, 6% and 17%, and survival rates were 100%, 97% and 94%. The shunt insufficiency rate was 26% in the bare stent group, 14% in the covered stent-grafis group, and 5% in the combined stents group (x2=1.00, P=0.61). The rebleeding rate was 33% in the bare stent group, 7% in the covered stent-grafts group, and 3%in the combined stents group (x2=1.69, P=0.43). The survival rate was 92% in the bare stent group, 93% in the covered stent-grafts group, and 100% in the combined stents group (x2=1.91, P=0.39). The shunt insufficiency rates were higher in patients with splenectomy than in those without splenectomy (30% vs.14%; x2=4.15, P=0.04). The intraperitoneal hemorrhage rates in the covered stent-grafis group and the combined stents group were significantly lower than that in the bare stent group (0% vs 0% vs 13%; x2=8.88, P=0.01).</p><p><b>CONCLUSIONS</b>TIPS with an 8 mm stent effectively treated and prevented esophageal gastric varices bleeding in patients with cirrhosis. Intraperitoneal hemorrhaging caused by TIPS was significantly decreased in the covered stent-grafts group and combined stents group,which represented an improvement in safety of this treatment. However, the influence of covered stent-grafis and combined stents towards the clinical efficacy of TIPS needs further study.</p>

ABSTRACT

Objective To study the safety and efficacy of fast track program in anorectal surgery perioperative period. Methods One hundred and sixty-nine cases of rectal cancer were divided into the study group of 86 patients with fast track program, and the control group of 83 patients with traditional programs by random digits table. Both groups were compared from the time out of bed, the first intestinal discharge time,intravenous fluids stopping time,length of hospital stay,total cost of treatment and the incidence of postoperative complications. Results The study group compared with the control group: the first intestinal discharge time [(33.6 ± 12.9) h vs. (81.7 ± 20.1) h], intravenous fluids stopping time [(4.5 ±1.3) d vs.(7.4 ± 1.6) d],and length of hospital stay [(5.6 ± 1.2) d vs.(8.9 ±2.7) d],the total cost of treatment [(15 000 ± 3000) yuan vs. (16 000 ± 4000) yuan], the differences were statistically significant (P < 0.01 or < 0.05),and had less incidence of postoperative complications in study group than that in control group [5.8% (5/86) vs. 16.9% (14/83)], the difference was statistically significant (P < 0.05).Conclusions Fast track program in anorectal surgery perioperative period is safe and effective, beneficial,conducive to rehabilitation of patients.

ABSTRACT

Tepoxalin is a potent inhibitor of both the cyclooxygenase and lipoxygenase pathways of the arachidonic acid cascade, as well as a potent anti-inflammatory and control-pain (postoperation, arthritis et. al.) agent. The new method about the use of novel synthesis reagents and the first using ionic liquid as reactive solvent to synthesize tepoxalin were presented in this paper. The ionic liquid can be easily recycled and reused for several runs efficiently. The analgesic activity of tepoxalin was detected by acetic acid test on mice. The analysis of variance showed that oral administration of tepoxalin could significantly inhibit the number of writhing response within 1 hour and prolong the latent time in a dose dependent manner as compared with CMC control group (P < 0.05). At the same time, tepoxalin had the same analgesic activity as diclofenac sodium.

ABSTRACT

Objective To study the expression of Survivin in breast carcinoma combined with prognosis of ten-year follow-up.Methods The expression of Survivin was investigated by immunohistochemistry in 60 cases of breast cancer and 20 tumor surrounding tissue.All the cases were performed with prognosis of tenyear follow-up.Results The expression rate of Survivin in breast carcinoma(68.3%)was higher than that in fibroadenoma of bast tissue(25%)(χ2=20.03,P<0.01).Survivin didn't experss in tumor surrounding tissue.There Was no relationship among the expression of Survivin and the histological differentiation,clinical staging,and lymph node metastasis of breast carcinoma.The expression of Survivin was correlated with the survival years of patients.Conclusion The expression of Survivin was up-regulated in breast carcinoma,which indicated that survivin may play an important role of the genesis and development of breast cancer,but it may play negative correlated with breast carcinoma.
