ABSTRACT

At present, Chinese residents have three iodine nutritional states: iodine deficiency, iodine appropriate and iodine excess. Appropriate iodine nutrition is crucial to the growth, development, and metabolic regulation of the body. Accurate assessment of individual iodine nutrition level is helpful to accurately understand the iodine status of the body and prevent iodine deficiency or excess. Previous indexes for assessing iodine nutrition include urinary iodine, thyroid stimulating hormone, goiter rate and so on, but these indexes have different degrees of shortcomings in individual iodine nutrition evaluation. This paper reviews the commonly used individual iodine nutrition evaluation indexes, and discusses the possibility of combined application of multiple indexes.

ABSTRACT

Objective:To evaluate the effect of clinical pathway implementation on medical efficiency and medical expenses of patients with two common rheumatic immune diseases " rheumatoid arthritis" and " ankylosing spondylitis" diseases by using diagnosis related group (DRG) related indicators.Methods:The data of patients with two common rheumatic immune diseases " rheumatoid arthritis" and " ankylosing spondylitis" included in the clinical pathway management from January 2017 to December 2019 in the Department of Rheumatology and Immunology of Jinhua Hospital, Zhejiang University School of Medicine were carried out. The impact of clinical pathway implementation on the average hospital stay, average cost and average drug cost of patients with the two diseases were analyzed and compared , so as to evaluate the effect of the implementation of the clinical pathway.Results:From the implementation of clinical pathway in 2017 to 2019, the number of patients admitted and total medical specialty services in the two groups of " rheumatoid arthritis" and " ankylosing spondylitis" increased year by year ( P<0.01). The average length of stay, average cost and average drug cost of patients in the " rheumatoid arthritis" disease group decreased year by year, with statistically significant differences between groups (all P<0.01). The average length of stay in the ankylosing spondylitis group was shortened year by year, and the difference was statistically significant ( P<0.01). Compared with 2017, the average cost in 2018 decreased significantly, and the difference was statistically significant ( P<0.01). There was no significant difference in average cost between 2018 and 2019 ( P>0.05). The average cost in 2018 was significantly higher than that in 2017 ( P<0.05). After analyzing the causes and optimizing the clinical pathway, the average cost in 2019 was significantly lower than that in 2018 ( P<0.01). Conclusions:Through the implementation of clinical pathways and continuous optimization of pathway connotation during use, the diagnosis and treatment efficiency of patients with " rheumatoid arthritis" and " ankylosing spondylitis" can be significantly improved, and medical costs can be reduced, which is in line with the current medical reform needs.

ABSTRACT

Objective@#To investigate the impact of transforming growth factor-β1 (TGF-β1) silencing by small interfering RNA (siRNA) on the expression of platelet-derived growth factor-β (PDGF-BB) and its receptor (PDGF-βR) in rats with hepatic fibrosis.@*Methods@#A total of 40 male Sprague-Dawley rats were randomly divided into normal control group, model group, TGF-β1 siRNA treatment group, and negative control group. All rats except those in the normal control group were given subcutaneous injection of 40% carbon tetrachloride to establish a rat model of hepatic fibrosis. The rats in the negative control group and the TGF-β1 siRNA treatment group were given tail vein injection of negative control plasmid or TGF-β1 siRNA plasmid twice a week at a dose of 0.25 mg/kg, and those in the normal control group and the model group were given the injection of sterile isotonic saline twice a week. The rats were sacrificed after 12 weeks and liver tissue samples were collected. Real-time PCR, immunohistochemistry, and Western blot were used to measure the expression of PDGF-BB, PDGF-βR, and phosphorylated extracellular signal-regulated kinase 1/2 (p-ERK1/2) in liver tissue. A one-way analysis of variance, the q test, and the Kruskal-Wallis test were used for statistical analysis based on data type.@*Results@#Compared with the model group and the negative control group, the TGF-β1 siRNA treatment group had significantly inhibited mRNA and protein expression of PDGF-BB and PDGF-βR (F = 24.785 and 22.92, P < 0.01), as well as significantly inhibited expression of p-ERK1/2 (P < 0.05).@*Conclusion@#Targeted TGF-β1 siRNA can effectively downregulate the expression of PDGF-BB, PDGF-βR, and p-ERK1/2 in liver tissue and thus help to improve hepatic fibrosis.

ABSTRACT

<p><b>OBJECTIVE</b>To establish Raji adriamycin (ADR)-resistant cell lines and analysis the expression of mitochondria Prohibitin (PHB) and microRNA-27a (miR-27a), as well as discuss its clinical significance.</p><p><b>METHODS</b>Built ADR-resistant Raji cells, detected their resistant index and drug-resistant spectrum and stability, observed their morphology and growth characteristics in general; evaluated the expression of phb mRNA and miR-27a in ADR-resistant cells (Raji/A) and sensitive cells (Raji/S) via real-time quantitative polymerase chain reaction (RT-PCR).</p><p><b>RESULTS</b>The ADR-resistant Raji cell lines were built; expression of PHB1 mRNA in Raji/A was evidently higher than in Raji/S (P<0.05), the expressing difference of PHB2 mRNA in Raji/A and Raji/S was statistically meaningless (P>0.05), the expression of miR-27a in Raji/A was much higher than in Raji/S (P<0.05).</p><p><b>CONCLUSION</b>By building the experimental model of Raji ADR-resistant cell lines, high expression level of PHB1 and miR-27a were detected in the cell lines, indicating that PHB1 and miR-27a may be associated with ADR-resistance of Raji cells.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the validity of three currently used chronic myeloid leukemia (CML) scoring systems (Sokal CML prognostic scoring system, Hasford prognostic scoring system, and EUTOS prognostic scoring system) in chronic phase CML (CP-CML) patients.</p><p><b>METHODS</b>One hundred and thirteen CP-CML patients taking hydroxyurea, interferon or imatinib were enrolled in this study. All patients were stratified into different groups according to each scoring system and different therapies respectively. And overall survival (OS) rates were observed and compared among different groups.</p><p><b>RESULTS</b>Three scoring systems were effective predictors of OS in CP-CML patients, and EUTOS scoring system may predict more validly. Hasford scoring system was well correlated with Sokal and EUTOS scores (r=0.792, P<0.01 and r=0.624, Plt;0.01). The correlation between Sokal and EUTOS scores was relatively weak (r=0.508, Plt;0.01). Despite imatinib's marked effect in prolonging survival of CML patients, it was incapable of prolonging survival in Sokal, Hasford and EUTOS high- risk patients.</p><p><b>CONCLUSION</b>EUTOS prognostic scoring system may predict better than Sokal and Hasford systems in CML patients. Classification of patients based on the three scoring systems may assist in the choice of appropriate therapy for CML.</p>

ABSTRACT

The 8p11 myeloproliferative syndrome is a rare hematologic malignancy that involves the fibroblast growth factor receptor 1 (FGFR1) gene at chromosome 8p11.The clonal change of the FGFR1 rearrangement is present in BM cells,and in the T-lymphoma cells in the LN.The hematological stem cells transplantation is the only method that can cure this rare hematologic neoplasma.

ABSTRACT

@#ObjectiveTo investigate the effect of Jiawei Glutinous decoction on gouty nephropathy. Methods54 patients with gouty nephropathy were randomized into two groups, 31 patients in the treated group were treated with Jiawei Glutinous decoction, and 23 patients in the control group were treated with allopurinol. Blood uricacid (BUA), renal function, β2-microglobulin (β2-MG) and 24 h urine protein of patiens in two groups were measured before and after treatment.ResultsImprovements of symptoms and renal function as well as decreasing of BUA, β2-MG and 24h PRO in the treated group was superior to those in the control(P<0.05).ConclusionJiawei Glutinous decoction present good effect in treating gouty nephropathy.
