ABSTRACT

Objective To investigate the fertility history and demand for reproductive health services of new residents in Shanghai. Methods A questionnaire survey was conducted among 1 358 new residents in 36 survey sites in 7 districts of Shanghai from July to September 2020. The content includes fertility history, induced abortion history, demand for reproductive health-related services, awareness rate of the Shanghai Family Planning Association and service access rate, etc. Results Among the new residents themselves and their spouses/sexual partners, 31.3% (374/1 194) had been pregnant once and 33.6% (401/1 194) had been pregnant twice; 46.3% (533/1 194)had one child and 29.7% (355/1 194)had two children. The difference of number of births among new residents with different residence time, those who did or did not possess permanent residency or residence permits, and those from different sources (urban or rural) was statistically significant ( χ 2=158.664, 50.263, 16.011, 114.419, all P <0.001). Among the new residents themselves and their spouses / sexual partners, the proportion of induced abortion of more than once was 36.1%. The difference of the number of abortions of new residents with or without permanent residency was statistically significant ( χ 2=19.389, P <0.001). The awareness rate of new residents of the harm of induced abortion to health was 92.1% (1 100/1 194); There were significant differences in the scientific knowledge of harm of induced abortion to health among new residents with different local residence time and those with or without a residence permit ( χ 2=36.590, 20.926, both P <0.001). The awareness rate of the Family Planning Association was 82.6% (986/1 194), and the service access rate was 51.3% (613/1 194). Permanent residency and residence permits are the main factors that affect the service accessibility of the Shanghai Family Planning Association. 44.8% (535/1 194) of new residents hope to receive reproductive health services in their place of residence, and they are most concerned about knowledge on good prenatal and postnatal care. Conclusion We should further publicize the "three-child" fertility policy, advocate a friendly fertility culture, and provide new residents with people-centered and accurate reproductive health services adapted to their needs through multi-sectoral cooperation, so as to improve their reproductive health level.

ABSTRACT

Objective:To observe the efficacy and safety of BD (bortezomib, dexamethasone) regimen and PCD (bortezomib, cyclophosphamide, dexamethasone) regimen in treatment of multiple myeloma (MM).Methods:The clinical data of 31 patients with MM treated in Huangshan People's Hospital of Anhui Province from January 2015 to October 2019 were retrospectively analyzed. The patients were divided into BD group (11 cases) and PCD group (20 cases) according to different treatment schemes. The treatment efficacy and adverse reactions of the two groups were compared.Results:MM patients were treated with BD and PCD regimens for 2-6 courses, with a total response rate of 83.9% (26/31) and a median onset time of 2.5 months (1.3-6.2 months). Among them, there were 2 cases of complete remission (CR), 4 cases of very good partial remission (VGPR), 2 cases of partial remission (PR), 2 cases of stable disease (SD), and 1 case of progression of disease (PD) in the BD group; in the PCD group, there were 6 cases of CR, 8 cases of VGPR, 4 cases of PR, 1 case of SD, and 1 case of PD; there was no statistically significant difference in clinical efficacy between the two groups ( χ2 = 0.186, P = 0.758). There were 1 case of leukopenia, 1 case of anemia, 1 case of thrombocytopenia, 1 case of liver damage, 3 cases of neurological adverse reactions, 2 cases of pulmonary infection, 2 cases of diarrhea in the BD group; there were 3 cases of leukopenia, 1 case of anemia, 3 cases of thrombocytopenia, 1 case of liver damage, 5 cases of neurological adverse reactions, 4 cases of pulmonary infection and 3 cases of diarrhea in the PCD group; there was no significant difference in adverse reactions between the two groups ( P > 0.05). Conclusions:The bortezomib-based combined chemotherapy regimens can achieve CR and VGPR for MM patients in a short time, and effectively delay the disease progression. The efficacy of the three-drug combination regimen may be better than the two-drug combination regimen.

ABSTRACT

@#Objective To explore the effect of surface electromyography biofeedback combined with the routine comprehensive rehabilitation treatment on walking ability of stroke patients with hemiplegia. Methods 80 stroke patients with hemiplegia were randomly divided into control group (n=40) and treatment group (n=40). All the patients received routine comprehensive rehabilitation treatment. The treatment group received electromyography biofeedback electrostimulation in addition. Fugl-Meyer Assessment was used to assess motor and balance function; active range of movement of ankle joint, the integrated electromyogram (iEMG) of isometric contraction of anterior tibialis muscle under maximum ankle dorsiflexion and Holden walking function were determined before and 8 weeks after treatment. Results 8 weeks after treatment, the scores of motor and balance function, the active range of movement of ankle, the iEMG, and Holden walking function significantly improved in both groups (P<0.001), and improved more in the treatment group than in the control group (P<0.05).Conclusion The electromyography biofeedback combined with routine comprehensive rehabilitation treatment can improve ankle motion control ability and walking ability of stroke patients with hemiplegia.

ABSTRACT

Objective To investigate the diagnostic value of intraductal ultrasonography for the quality of biliary stricture.Methods Data of the patients who had received operation because of biliary stricture after IDUS examination from 2006 to 2010 were collected.IDUS results were compared with those of operation.Results There were 43 cases of malignant strictures and 6 benign strictures in total.The sensitivity,specificity,positive predictive value,negative predictive value and diagnostic accuracy of intraductal uhrasonography for the quality of biliary stricture were 97.7% ( 42/43 ),83.3% ( 5/6 ),97.7% ( 42/43 ),83.3% (5/6) and 95.9% (47/49),respectively,which were significantly higher than conventional imaging like ultrasound B,CT and MRCP.Twenty one cases in 32 were diagnosed as malignant biliary stricture with cytological brushing,with the diagnostic accuracy of 65.6%.All cases had been diagnosed by IDUS.Conclusion Intraductal ultrasonography is of high diagnostic value for biliary stricture.However,cytological brushing based on IDUS is of limited diagnostic value for malignant biliary stricture.

ABSTRACT

@#Objective To investigate the effect of neuromuscular activation on patients with low back pain after lumbar vertebra fracture operation. Methods 60 low back pain patients who received lumbar vertebra fracture operation were divided into treatment group (n=30)and control group (n=30). Both groups received 20 times of frequency treatments for 4 weeks. While the treatment group received the lumbar stability treatment with neuromuscular activation, and the control group received back muscle strengthen treatment. They were assessed with visual analogue scale (VAS) for pain and Short Form of Health Survey (SF-36) before, 4 weeks and 6 months after treatment. Results The scores of VAS and SF-36 was not significantly different between 2 groups (P>0.05) before treatment, and was significantly different (P<0.05) 4 weeks after treatment and later. Both the scores of VAS and SF-36 improved in both groups after treatment (P<0.05). Conclusion Neuromuscular activation can relieve low back pain after lumbar vertebra fracture operation and improve the quality of life.

ABSTRACT

Objective To study the effect of surface electromyogram-triggered electrical stimulation on lower limb function in hemiplegic stroke patients. Methods Thirty hemiplegic stroke patients were divided into a treatment group ( 15 cases) and a control group ( 15 cases). Both groups were given conventional rehabilitation training.Additionally, the treatment group was given surface electromyogram-triggered electrical stimulation training, while the control group was given common low frequency electrical stimulation. Brunnstrom's recovery stages, the Fugl-Meyer assessment (FMA) and electromyographic parameters were assessed before and after 3 courses of treatment. Results After treatment both groups had significantly higher Brunnstrom and FMA scores and better integrated electromyograms(iEMG), but the effects in the treatment group were significantly better than in the control group. Conclusions Surface electromyogram-triggered electrical stimulation training can provide satisfactory rehabilitation of lower limb function in hemiplegic stroke patients.

ABSTRACT

Objective To evaluate the safely and efficacy of therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and its related procedures in patients older than 85 years.Methods Data of patients older than 85 who underwent ERCP between 2004 and 2009 (group A, n =52) were reviewed and compared with those of patients aging 70-84 years (group B, n = 329).Results The rate of patients with underlying disorders of other systems in group A was significantly greater than that of group B (100.0% vs.77.5% , P < 0.05) , as well as the rate of multiple procedures to achieve stone clearance (42.3% vs.28.8% , P <0.05).There were no significant differences between 2 groups regarding the rates of complication (7.7% vs.4.9% ) and post-procedure mortality (1.9% vs.0.6% ).Conclusion Therapeutic ERCP and related procedures are safe and effective in patients aging over 85, and advanced age should not be regarded as the contraindication of the procedure.

ABSTRACT

BACKGROUND: Previous results of our study show that oyster extract has some protective effects on apoptosis of the neuroepithelium in neural tube defects induced by hyperthermia in vivo.It remains unclear whether the extraet also protects in vitro cultured neural stem cells.OBJECTIVE: To investigate the protective effect of oyster extract on apoptosis of cerebral neural stem cells induced by hyperthermia.METHODS: The cerebral neural stem cells of embryonic mice of 13 days were cultured in vitro.Nestin expression was detected by immunofluorescence method to identify neural stem cells.The neural stem cells of passage 3 were divided randomly into 4groups: hyperthermia control group and oyster treated Ⅰ,Ⅱ,Ⅲ groups(mass concentration 2.5,5,10 g/L oyster extract solution).In addition,culture solution control group(no cells),and culture solution+oyster extract control group(no cells)were designed.All oyster extract groups and control groups were treated by hyperthermia over 39 ℃.The survival rate and the vitality of neural stem cells were detected by trypan blue staining and MTT assay.Western-blotting was employed to explore the expression of p53 in cerebral neural stem cells of each group.RESULTS AND CONCLUSION: The survival rate and the value of MTT assay in oyster treated groups Ⅱ and Ⅲ were significantly greater than hyperthermia control group(P < 0.05),but the expression of p53 in oyster treated groups Ⅱ and Ⅲ were weaker than hyperthermia control group.Oyster extract plays an important protective role in the apoptosis of neural stem cells induced by hyperthermia.

ABSTRACT

Objective To discuss the influence of individualized health education based on combination of TCM and western medicine on the life quality of patients with diabetes mellitus in order to supply novel idea and method for carried out education about diabetes mellitus. Methods Hospitalized patients (140 cases) were randomized into the control group and the test group with 70 cases in each group. Both groups received routine diabetes drugs treatment. The patients and their family members received individualized health education based on combination of TCM and western medicine during their hospitalization days and 6 months after discharge. SF-36 inventory and diabetes cognitive questionnaire was adopted to investigate the two groups. Results The diabetes cognitive degree of the test group was higher than that of the control group (P<0.01). Six items such as social function, systemic health, body pain, psychological health, vigor and emotional role function of the test group was different from those of the control group (P<0.05). Conclusion Individualized health education based on combination of TCM and western medicine contributed to the improvement of patients' life quality.

@#Objective To investigate the effect of bed rest on acute nonspecific low back pain(ANLBP).Methods87 cases were randomly divided into bed rest group (42 cases) and free activity group (45 cases).The bed rest group would be proper activity indoor and step up activity outdoor after absolute in bed for a week.The free activity group would be prescribed free activity.At the same time,all of them received physiotherapy and non-steroidal anti-inflammatory drugs (NSAIDs).The visual analogous scale (VAS) were measured before and 2 weeks after treatment.The Roland-Morris disability questionnaire (RMDQ) were measured before and 2,6 weeks after treatment.ResultsThe difference of VAS and RMDQ scores between these 2 groups were not significant before treatment (P>0.05),but were significant 2 weeks after treatment(P<0.05).Further more,the difference of RMDQ scores between these 2 groups were not significant 6 weeks after treatment (P>0.05).ConclusionThe bed rest about patients could not promote,but delay the improvement of the pain and disability in the early period or later period recovery of ANLBP.

ABSTRACT

Epidemiologic studies suggest that intake of excess all-trans-retinoic acid (RA) during embryogenesis induces various developmental defects and the central nervous system (CNS) represents a major site of the teratogenic action of RA. It is therefore important to understand which parameters are affected early by excessive RA in order to devise and improve protective nutritional strategies. The modulations of beta-catenin and caspase-3 levels were investigated in the KM mouse embryo following maternal treatment with a single oral dose of 30mg/kg body weight of RA during the neurula period. In addition, retinoic receptors (RARs) are key transcription factors regulating gene expression in response to RA-activated signals. So the experiment was designed to evaluate whether the alterations in protein expression of RAR alpha and beta during the time of neural tube closure were induced by excessive RA. Maternal intake of excess RA induced early downregulation of RAR alpha and beta, beta-catenin and caspase-3 expression, which was followed by an increase in their expressive levels in the neural tube tissue of mouse embryos. This finding suggests that the alterations in the expression profile of RAR alpha and beta, beta-catenin and caspase-3 may be implicated in the teratogenesis induced by excess RA in KM mouse embryo.
