ABSTRACT

Objective:To analyze the global research hotspots in the field of pediatrics based on the Essential Science Indicators (ESI) database and explore the inspiration to domestic editors and pediatrics researchers. Methods:The journal distribution, country (region) distribution, cooperation, organization distribution, funding, publication language, hot topic words and other data of highly cited papers in the field of pediatrics in ESI database were collected and analyzed. Results:A total of 682 highly cited pediatrics papers were collected from 77 pediatrics journals included in Science Citation Index(SCI). Most of the highly cited pediatrics papers (182) were found to be published in Pediatrics. All 682 paper were published in English and frequently, characterized by multiple authors, institutions and fund support.Of 682 highly cited pediatrics papers, 435 papers were published in the United States(the first), 123 papers in England(the second) and 86 paper in Canada(the third). Novel coronavirus pneumonia, coronavirus, SARS coronavirus, autism and multiple system inflammatory syndrome are the main frontiers of global pediatric research at present.Specifically, focal pediatric system diseases mainly include respiratory system diseases, digestive system diseases, cardiovascular diseases, etc. Conclusions:ESI-based analysis of global research hotspots in the field of pediatrics provides reference materials for domestic and foreign pediatrics researchers to understand the global academic frontiers and development trends in the field of pediatrics and select topics for future scientific research.More importantly, this analysis can help domestic editors of pediatrics journals to plan topics and organize hot papers, so as to improve the academic quality and international influence of the journals.

ABSTRACT

Objective To evaluate the effectiveness of comprehensive lifestyle modification in the primary health care system on status of pre-chronic diseases in a conventional healthcare program. Methods Two hundred and twenty-nine adult volunteers in 5 conventional healthcare centers were divided into 4 groups:group of prehypertension(n=67),group of prediabetes(n=38),group of abnormity serum cholesterol(n=71)and group of osteopenia(n=53).A muhicomponent behavioral intervention including education, physical activity, dietary practice, cease smoke, calcium supplement and exposure to sunlight was conducted for 12 months. The main outcome of each group was blood pressure, blood glucose, serum cholesterol and bone mineral density. Results Over 12 months, compared with the status before intervention, participants in each group were statistically significantly improved, the mean net reduction in systolic BP/diastolic BP was 9.4 mm Hg/6.6 mm Hg(1 mm Hg=0.133 kPa,t=5.93/8.29.P<0.05)in group of prehypertension;the reductions of blood glucose, serum cholesterol were 0.9 mmol/L(t=3.69,P<0.05)and 0.5 mmol/L(t=4.09,P<0.05)in group of prediabetes and group of abnormity serum cholesterol, and the increase of bone mineral density on lumbar spine was 0.015 g/cm2(t=2.03,P<0.05)in group of osteopenia,Conclusions For chronic disease prevention, ater one year the comprehensive lifestyle intervention produced beneficial changes in diet, physical activity, and biochemical parameters. This type of intervention is a feasible option to prevent chronic disease and should be implemented in the primary health care system.

ABSTRACT

Objective:To compare efficacy and time course of adrenocortical function recovery of oral prednisone and intrathyroidal injection of dexamethasone in patients with subacute thyroiditis. Methods: Sixty patients with subacute thyroiditis were divided randomly into 2 groups. Patients in group Dex (30 cases) were treated with intrathyroidal injection of dexamethasone (5 mg,twice per week, and then once per week.). Patients in group Pre (30 cases) were treated with prednisone. Prednisone was orally given in decreasing doses: 30 mg for the first 2 weeks, 20 mg for 2 weeks, thereafter the dose was tapered by 5 mg per week. Clinical outcomes of 2 groups were compared after 2 weeks, 4 weeks,6 weeks and 8 weeks of treatment. Rapid corticotropin stimulation test was used to assess adrenocortical function. A positive response, indicative of normal adrenocortical function, has been defined as a 30 min stimulated cortisol level of more than 552 nmol/L.The initial stimulation test was performed as a baseline study immediately following the diagnosis of subacute thyroiditis, before the initiation of glucocorticoid therapies. The tests were repeated 24 h after completion of glucocorticoid course and 2 weeks and every 4 weeks thereafter until a stimulated cortisol level of more than 552 nmol/L was achieved. Results: Curative rate of group Dex and group Pre after 8 weeks were 100% and 90% respectively. Duration of therapy was (5.7?2.0) weeks in group Dex and (8.8?1.8) weeks in group Pre respectively ( P
