ABSTRACT

@#【Objective】To explore the clinical manifestation of COVID- 19 severe cases.【Methods】Clinical data of one severe case with COVID-19 including the clinical characteristic ,laboratory testing results,radiography,treatment,complication and outcome of the patient were retrospectively collected and analyzed.【Results】 The patient with COVID-19 was a 61-year old male,He suffered with underlying disease. His symptoms included fever,cough,myalgia, fatigue,and dyspnea. Laboratory testing results included normal WBC count,decreased lymphocyte cells,elevated LDH and hypoxemia. Radiography findings showed bilateral lung infiltration. His condition deteriorated after intensive treatment for one week. He was intubated and treated with mechanical ventilation because of complicating with severe acute respiratory distress syndrome(ARDS).【Conclusion】COVID-19 is an emerging acute communicable disease,which lack specific and effective treatment. Most patients have a good prognosis but mortality in severe cases is high. More attention should be paid on the high risk of progression in COVID-19 cases.

ABSTRACT

@#【Objective】To investigate the differences of lung involvement between dengue and severe dengue.【Methods】227 dengue patients admitted in The Third Affiliated Hospital of Sun Yat-sen University from July 2014 to October 2018 were enrolled. The clinical characteristics,treatment and outcome of the patients were analyzed to explore the differences of lung involvement between dengue and severe dengue (SD). 【Results】 The rate of old age ,smoking ,hypertension,diabetes and cerebrovascular disease was higher in dengue with lung involvement group(DWLI)than dengue without lung involvement group(DWOLI)(χ2 were 25.146,3.847,10.326,7.177,and 5.355,P was 0.050 for smoking,the others were < 0.05). The rate of cough and breathlessness was higher in DWLI(χ2 were 11.465 and 6.068,P were 0.001 and 0.014),as well as in SD subgroup(χ2 were 4.585 and 6.717,P were 0.032 and 0.010). C-reactive protein and procalcitonin were increased in DWLI(Z were - 2.591 and - 3.033,P were 0.010 and 0.002). The rate of pleural effusion was higher in SD subgroup(χ2 = 4.987,P = 0.026). Bilateral lung infiltration was correlated with SD(χ2 = 5.910,P =0.015). The rate of acute liver injury,acute kidney injury and multi-organ dysfunction syndrome(MODS)was higher in DWLI(χ2 were 7.044,7.059,and 11.315,P were 0.008,0.008 and 0.001). The rate of anti-virus,anti-bacteria and combined therapy was higher in DWLI(χ2 were 13.156,32.845,and 12.684,P all were < 0.001).【Conclusion】Dengue patients who were with old age,smoking,or suffered from underlying disease of hypertension,diabetes and cerebrovascular disease were vulnerable to lung involvement. Cough,breathlessness,pleural effusion and bilateral lung infiltration were signs of severe dengue. Attention should be paid to dengue with lung involvement.

Objective@#To evaluate the efficiency and safety of transurethral holmium laser enucleation of the prostate (HoLEP) in the treatment of BPH in patients with a history of transrectal prostate biopsy (TRPB).@*METHODS@#We retrospectively analyzed the clinical data on 102 cases of BPH treated by HoLEP in our hospital between November 2015 and May 2017, of which 42 had received TRPB prior to HoLEP (the PB group) but not the other 60 (the non-TRPB [NPB] group). We compared the preoperative, perioperative and postoperative follow-up data between the two groups of patients.@*RESULTS@#There were no statistically significant differences in the mean age, prostate volume, and preoperative post-void residual urine volume (PVR), IPSS, quality of life (QOL) score and maximum urinary flow rate (Qmax) between the two groups of patients. The preoperative PSA level was significantly higher in the PB than in the NPB group ([10.30 ± 3.62] vs [2.62 ± 1.75] μg/L, P < 0.01), and the operation time markedly longer in the former than in the latter ([78.00 ± 18.25] vs [67.93 ± 15.89] min, P < 0.01), particularly in the patients with an interval of <2 weeks between HoLEP and TRPB than in those with an interval of ≥2 weeks ([91.17 ± 16.51] vs [68.13 ± 12.45] min, P < 0.01). Statistically significant differences were not found in the postoperative hemoglobin level, continuous bladder irrigation duration, catheter-indwelling time and hospital stay, nor in the incidence rate of transient urinary incontinence between the PB and NPB groups (47.62% vs 45%, P = 0.794). There were no transurethral resection syndrome, bladder or rectal injury, or blood transfusion in either group, nor statistically significant differences in PVR, Qmax, IPSS and QOL score between the two groups of patients at 3, 6 or 12 months after operation.@*CONCLUSIONS@#HoLEP is a safe and effective surgical treatment of BPH for patients with a history of TRPB, which can reduce the time and increase the safety of operation when performed at ≥2 weeks after TRPB.

ABSTRACT

OBJECTIVE: To analyze the polymorphs of carbamazepin tablets by laser confocal micro-Raman spectrometer and establish the identification test model. METHODS: Reference Raman spectra of the four crystal forms of carbamazepin were established; then the polymorphs of the carbamazepin tablets from different manufacturers were analyzed, and an identification test model was established to distinguish the unknown samples. RESULTS: According to the results of the cluster analysis, the four batches of carbamazepin tablets from domestic manufacturers belonged to crystal form III. A qualitative model was created by choosing the first derivative and vector normalization method to process the spectra, using the factorization method to calculate, and selecting the 200-500, 650-850, 1 000-1 100, 1 500-1 700 cm-1 regions to compare. This model was verified to have high sensitivity, specificity, precisicion, negative prediction ability, and detection efficiency. CONCLUSION: The laser confocal micro-Raman spectrometer canbe used to analyze the polymorphs of carbamazepin tablets and the qualitative identification is rapid and accurate.

ABSTRACT

<p><b>OBJECTIVE</b>To study the different proportions of intermediate epithelial cells in human prostate cancer tissue and their clinical significance.</p><p><b>METHODS</b>We performed immunohistochemical staining for Cytokeratin 5 (CK5) and Cytokeratin 8 (CK8) on 60 samples of human prostate cancer, determined the proportions of intermediate epithelial cells in the cancer tissue, and classified the samples into 2 types, one with a majority of intermediate epithelial cells (CaP-INT, n = 32), and the other composed mostly of luminal epithelial cells (CaP-LUM, n = 28). Then we compared the 2 types of prostate cancer in the expression of the androgen receptor (AR), age of the patient, serum t-PSA, prostate volume, Gleason score, clinical stage, androgen resistance, and incidence of distant metastasis.</p><p><b>RESULTS</b>CaP-INT showed a significantly lower expression of AR ([24.42 +/- 11.41] %) and a higher incidence of distant metastasis (n = 14) than CaP-LUM ([77.21 +/- 10.22] % and n = 4) (P < 0.05). In the CaP-INT group, 6 of the 26 endocrinologically treated cases developed into androgen-independent prostate cancer (AIPC), while in the CaP-LUM group, only 1 out of 23 (P < 0.05). The former also showed remarkably higher clinical stages than the latter (P < 0.05), but no significant differences were found in age, serum t-PSA, prostate volume and Gleason score between the two groups (P > 0.05).</p><p><b>CONCLUSION</b>A higher proportion of intermediate epithelial cells may lead to increased invasiveness and metastasis of human prostate cancer.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate targeted degradation of the androgen receptor (AR) by chimeric molecules (DHT-PROTAC) via the ubiquitin-proteasome pathway in androgen-independent prostate cancer CA-2B cells, and explore the proliferation, secretion and apoptosis of the treated cells.</p><p><b>METHODS</b>C4-2B cells were treated with DHT-PROTAC, and then the expressions of the AR protein and caspase3 in the C4-2B cells were detected by immunohistochemistry and Western blot. The concentration of PSA in the supernatant was examined by ELISA. The cells were counted and their proliferation analyzed by a growth curve. The inhibitory effect on the growth of C4-2B cells was evaluated by MIT assay.</p><p><b>RESULTS</b>Compared with the control group, the DHT-PROTAC-treated group showed an obviously decreased expression of AR proteins with a significant attenuation of the band signals (P < 0.05), a 40% reduction of the AR-positive cells and a 60% decrease of the PSA concentration in the supernatant (P < 0.05). DHT-PROTAC exhibited an inhibitory effect on the C4-2B cells in a time-dependant manner (P < 0.05).</p><p><b>CONCLUSION</b>The chimeric molecule (DHT-PROTAC) can target the degradation of androgen receptors, reduce the secretion of PSA and repress the in vitro growth of C4-2B cells.</p>
