ABSTRACT

In this study, our aim was to evaluate and classify the voluntary error reports in the hospitals of Mashhad University of Medical Sciences. Patients have the right to receive health care in accordance to the best standards. Health care carries a risk of harm for patient safety, and with respect to today's stressful systems with a large number of patients, it would be inevitable. The meaning of risk management is to predict adverse events and reduce their occurrence. A voluntary medical error reporting form was designed and approved by the clinical governance team of Mashhad Medical University. They were then distributed inside hospitals in the way in which everyone [health providers and patients] could access them easily. The forms were collected and classified monthly in all wards. Classification was performed on the base of type, outcome and reporter. Data gathering took place from spring to autumn 2012. The data was analyzed by the SPSS software. 2500 errors were extracted from 1000 voluntary error reporting forms of the 12 hospitals of Mashhad Medical University. The most frequent error type was treatment errors [36%] related to drug administration, standard procedures and surgical events. Error reporting as a basic activity has an important role in discovering pitfalls of the health care system. To promote the reporting culture, its non punitive base must become clear for all professors and staff members, because this kind of reporting could lead to fewer medical errors and higher staff awareness about probable errors.

ABSTRACT

Job satisfaction is a general attitude of a person toward his job. Higher level of satisfaction leads to better results and efficacy. Job satisfaction provides an opportunity for attaining significant occupational values compatible with satisfaction of an individual's basic needs and influential in their achievement. One of the personal characteristics affecting organizational behaviors is self-esteem. In fact, self-esteem determines our life satisfaction. This descriptive study was conducted to determine job satisfaction and self-esteem of nurses at emergency departments [EDs] of teaching hospitals in Mashhad city. Data collection tools included a job satisfaction questionnaire and self-esteem Ayznk test. Data were analyzed by t- and Mann-Whitney U tests as well as Pearson correlation. Finding showed that 69.6%, 16.4% and 14% of the subjects had moderate, low, and high levels of job satisfaction respectively. No subject had very low job satisfaction. 63.5%, 20.4% and 16.1% of the subjects had moderate, high and low level of self-esteem respectively. There was a direct correlation between self-esteem and job satisfaction. Findings showed that job satisfaction and self-esteem of ED nurses were, moderate. There was a positive relation between marital status and job safisfaction as well as self-esteem. It seems that single nurses have lower job satisfaction and self-esteem

ABSTRACT

Background: Controlling is a managerial function in which performance is measured and corrective action is taken to ensure the accomplishment of organizational goals. An Audit is a systematic and offical examination of a record, process, structure, enviroment or account to evaluate performance


Purpose: This descriptive study was conducted to audit nursing care provided to CABG patients at an open-heart ICU of a teaching hospital in Mashad in 2004


Method: 36 CABG patients were selected by convenient sampling. Data collection tools included a demographic questionnaire and a checklist of nursing care for CABG patients


Results: Findings showed that nursing care was mostly provided in cardiovascular and respiratory systems [50 - 75]. In addition, the quality of nursing care in urinary system was high [75 - 100] and in nervous system was low [less than 50]


Conclusion: According to the results, it can be concluded that education and training for nurses should be contiued with respect to diseases and care
