ABSTRACT

Background: it is reasonable to think that some biochemical characteristics of follicular fluid [FF] surrounding the oocyte may play a critical role in determining the quality of oocyte and the subsequent potential needed to achieve fertilization and embryo development


Objective: this study was carried out to evaluate the levels of FF homocysteine [Hcy] in IVF candidate polycystic ovary syndrome [PCOS] women and any relationships with FF glucose and estradiol [E[2]] levels


Materials and Methods: in this case control study which was performed in Dr. Tizro Day Care and IVF Center 70 infertile patients were enrolled in two groups: comprising 35 PCOS and 35 non PCOS women. Long protocol was performed for all patients. FF Hcy, glucose and E[2] levels were analyzed at the time of oocyte retrieval


Results: it was observed that FF Hcy level was significantly higher in PCOS patients compared with non PCOSs [p<0.01]. Observations demonstrated that in PCOS group, the Hcy level increased independent to E[2], glucose levels, BMI and age, while the PCOS group showed significantly higher BMI compared with non-PCOS group [p=0.03]. However, no significant differences were revealed between groups for FF glucose and E[2] levels


Conclusion: present data showed that although FF glucose and E[2] levels were constant in PCOS and non PCOS patients, but the FF Hcy levels in PCOS were significantly increased [p=0.01]

ABSTRACT

The repetitive micro traumatic stresses placed on the athletes shoulder joint complex during the throwing motion challenge the surrounding tissues. The purpose of this study was to compare shoulder rotational strength, range of motion and proprioception between the throwing athletes and non-athletic persons. Fifteen throwing athletes and 15 non-athletes participated in a nonrandom case - control study. Strength of shoulder rotational movements was tested with a hand held dynamometer. The ranges of internal and external rotation of shoulder were measured by a standard goniometer. The ability of subjects to replicate the target position and kinesthetic sense was examined on the subjects' right shoulder by using a continuous passive motion device. Independent and paired t tests were used to statistically analyze between and within group differences. No significant difference was detected on the range of internal rotation between throwing athletes and non-athletic candidates [P=0.3]. The range of external rotation was significantly more in athletic subjects [P=0.03]. The results also showed that throwing athletes demonstrated a significantly higher isometric strength of shoulder external and internal rotation than the nonathletic group [P<0.05]. However, the comparison of the internal and external rotation strength of dominant side in each group showed that throwing athletes showed a significant lower isometric strength of shoulder external rotation than internal rotation [P<0.001]. It was also demonstrated higher joint position acuity in the throwing athletes than non athlete subjects [P=0.01]. The repetitive nature of overhead throwing and the high forces that it causes result in adaptive changes of the dominant extremity. Throwing can lead to mobility, strength and neural adaptation

ABSTRACT

Anti-inflammatory drugs are used for reducing the pain after surgery especially in minor surgery and local anesthesia drugs, especially Markain, is also employed. Regarding performing little studies about the comparison of these two kinds of drugs, this study was done for comparison two methods. This double-blind randomized clinical trial study was done on 198 patients with inguinal hernia. Patients were divided into case group [intra wound Markain] and control group [the suppository of naproxen]. Surgery was done with standard method [Liechtenstein] and the severity of pain was assessed by the visual analogue scale [VAS] in one, six and 24 hours after surgery. Data were analyzed by SPSS- ver16 software. Mean age in case group were 52.6 +/- 5.7 years and in control group were 53.7 +/- 4.9 years. Mean pain intensity in the first and 6[th] hours in case group were 6.3 and 4.6 and in control group were 3.9 and 2.9, respectively [P<0.001]. The pain immediately and 24 hours after surgery were similar in both groups and there was no significant difference between the two groups. Need to opioid analgesics was further reduced in the naproxen group one and six hours after surgery. Although both methods were effective in reducing postoperative pain and less expensive, but naproxen suppositories was more effective pain reliever for optimal pain control, especially in the first and six hours after surgery and is more effective in reducing pain intensity than Markain. Naproxen also reduces the need to deal with housing has been opioid analgesics

ABSTRACT

Central nervous system malformations are the second most common congenital malformations after congenital heart diseases. These malformations are associated with many instances of morbidity and mortality which underline the importance of prevention and their early diagnosis. The objective of this study is the diagnosis of neural tube defect [NTDs] in the first trimester and its comparison to second trimester diagnoses in order to reduce the complications associated with late pregnancy terminations and its costs. This study was a trans-sectional study. A total number of 1074 patients who referred to Imam Khomeini Hospital were enrolled in this study. During the 11th-13th [+6 days] gestational week the patients were screened sonographically; subsequently they were re-scanned for fetal anomalies during 18[th]-20[th] gestational week, and we followed the babies after birth. Considering that Intracranial Translucency [IT] was introduced in the newer researches, it was, too, measured in 125 of the cases. In a total number of 1074 patients we had one patient with an anencephalous fetus whom was diagnosed in the first trimester of pregnancy. In the second trimester, we detected one case of myelomenigocele; when we referred to this patient's first trimester sonography, there was no visible IT. In the 125 cases in whom the IT length was measured, it was normal; the 2nd trimester sonographies in these patients were also normal. It must be noted that the diagnosis of NTD is more accurate in the second trimester of pregnancy. Consequently it is recommended that in high risk patients, the second trimester sonography be performed transvaginally, and in an earlier gestational age [14[th]-16[th] gestational weeks]

ABSTRACT

Brucellosis is one of the most common joint diseases of humans and livestock, which has a high prevalence levels. Diversity of clinical symptoms and sometimes unusual symptoms of the disease confuses physicians regarding the diagnosis and treatment. This increases duration of disease, treatment costs and also mortality and morbidity. A47-year-old man with rural agricultural job admitted to emergency room of Imam Hossein hospital of Shahroud, complaining prolonged fever, weakness, paralysis, nausea, anorexia and abdominal pain, and jaundice. Initial laboratory exams, including Wright, coomb's Wright and 2MB, were normal except increased value of BUN and creatinine. Ultrasound and CT scan of abdomen were normal. After 3 days, repetition of tests showed positive Wright and 2MB. Standard treatment with anti- brucellosis drugs was prescribed. After ten days, he discharged from the hospital and treatment continued at home. Awareness of different manifestations of brucellosis leads to early diagnosis and treatment and also prevention of complications
