ABSTRACT

Objective To assess the impact of immunization with pneumococcal conjugate vaccines on all-cause pneumonia hospitalizations among children in Soweto, South Africa. Methods We used data collected at the Chris Hani Baragwanath Hospital in Soweto between 2006 and 2014 ­ i.e. before and after April 2009, when a pneumococcal conjugate vaccine was first included in South Africa's routine immunization programme. Using a Bayesian generalized seasonal autoregressive moving-average model and the data collected in 2006­2008, we estimated the numbers of children that would have been hospitalized for pneumonia between 2010 and 2014 if no pneumococcal conjugate vaccines had been used. These estimates were then compared with the corresponding numbers of hospitalizations observed. Findings Between 2006 and 2014, 26 778 children younger than five years ­ including 3388 known to be infected with human immunodeficiency virus (HIV) ­ were admitted to the study hospital for pneumonia. We estimated that, for the children known to be infected with HIV and for the other children, pneumococcal conjugate vaccines reduced the numbers of hospitalizations for pneumonia in 2014 by 33% (50% credible interval, CrI: 6 to 52) and 39% (50% CrI: 24 to 50), respectively. In the study hospital in 2012­2014, as a result of immunizations with these vaccines, there were an estimated 3100 fewer pneumonia hospitalizations of children younger than five years. Conclusion. In our study hospital, following the introduction of pneumococcal conjugate vaccines into the national immunization programme, there were significant reductions in pneumonia hospitalizations among children

ABSTRACT

We conducted a household survey among Sa Kaeo residents to characterize self-reported health-seeking behavior for pneumonia and the proportion of individuals who seek care at a hospital to determine the coverage of a surveillance system. A 2-stage cluster sample was used to select households. A case of pneumonia was defined as a self-reported history of cough and difficulty breathing for at least 2 days or being given a diagnosis of pneumonia by a healthcare provider in the 12-month period beginning February 1, 2002, and ending January 31, 2003. Interviewers administered a structured questionnaire that asked about clinical illness and utilization of healthcare services. Among 1,600 households, 5,658 persons were surveyed, of whom 62 persons met the case definition. Of the 59 persons with complete data, 53 (90%, 95% CI: 79-96) sought medical care and 47 (80%, 95% CI: 67-89) sought care at a hospital facility in the province. Neither distance nor cost was reported as a barrier to seeking care. Most individuals with self-reported pneumonia sought care at the hospital level. Population-based surveillance can provide reliable estimates of hospitalized, chest radiograph-confirmed pneumonia in Sa Kaeo if adjustments are made to account for the proportion of individuals who access a hospital where radiologic assessment is available.

ABSTRACT

Según lo establecido por la Organización Panamericana de la Salud (OPS), conseguir una alta cobertura de vacunación es una meta esencial para la Región de las Américas. Es indispensable lograr niveles de cobertura de 95 por ciento o mayores para poder alcanzar los objetivos de la OPS de eliminar el sarampión y la rubéola, controlar las enfermedades prevenibles mediante la vacunación, y hacer perdurar la eliminación de la poliomielitis en territorio americano. Para poder alcanzar esos niveles, es imprescindible que las estadísticas de vacunación sean fiables y que las autoridades sanita- rias midan y monitoreen los niveles de cobertura a lo largo del tiempo. Los métodos elegidos por los directores de los programas de vacunación para calcular la cobertura dependerán de la información que haga falta. En general, los directores del Programa Ampliado de Inmunización (PAI) necesitarán información acerca de la cobertura para poder: 1) determinar la verdadera cobertura en los niveles nacional y local, 2) determinar cuán adecuada es la cobertura en una zona determinada, 3) monitorear las tendencias a lo largo del tiempo, y 4) monitorear las actividades de vacunación mientras se están llevando a cabo. Para lograr lo primero -determinar cuáles son los niveles verdaderos de cobertura-, los administradores tienen dos opciones: a) valerse de los datos acerca de las dosis administradas (es decir, el número de dosis de la vacuna que se ha administrado, dividido por la población que debió recibir una dosis) o b) llevar a cabo una encuesta para determinar la cobertura. Para lograr lo segundo -saber si la cobertura en una zona determinada es adecuada (por ej., mayor de 90 por ciento)-, se puede realizar un muestreo por lotes para garantizar la calidad (MLGC). El MLGC es una metodología de encuesta basada en el uso de muestras pequeñas que permite determinar si la cobertura en una zona determinada es adecuada o no, pero no sirve para estimar el nivel de cobertura. Para el tercer propósito -monitorear las tendencias a lo largo del tiempo-, se pueden usar los datos correspondientes al número de dosis administradas. Para lograr el cuarto propósito -determinar si procede vacunar o llevar a cabo una campaña de vacunación u otra actividad afín-, la "herramienta de monitoreo rápido" creada por la OPS es una magnífica solución.
