ABSTRACT

ObjectiveTo investigate the role and mechanism of hyodeoxycholic acid (HDCA) in the progression of metabolic associated fatty liver disease (MAFLD), and to provide a new theoretical basis for further clarifying the pathogenesis of MAFLD. MethodsL02 hepatocytes were used as experimental cells, and palmitic acid was used to induce steatosis in L02 cells. The farnesoid X receptor (FXR) siRNA interference chain technique was used to construct a hepatocyte cell line with low FXR expression. CCK8 assay was used to observe the effect of HDCA on L02 steatosis hepatocytes at different concentrations (0, 100, 200, 300, and 400 μmol/L) and time points (12, 24, 36, and 48 hours). The method of qRT-PCR was used to measure the mRNA expression levels of FXR, proliferating cell nuclear antigen (PCNA), Cyclin D1, phosphatidylinositol 3-kinase (PI3K), and protein kinase-B (AKT), and Western blot was used to measure the protein expression levels of FXR, Cyclin D1, PCNA, PI3K, phosphorylated PI3K (p-PI3K), AKT, and phosphorylated (p-AKT). A one-way analysis of variance was used for comparison of normally distributed continuous data with homogeneity of variance between multiple groups, and the Tukey HSD test was used for further comparison between two groups; the Welch analysis of variance was used for comparison of normally distributed continuous data with heterogeneity of variance between multiple groups, and the Games-Howell test was used for further comparison between two groups. The independent-samples t test was used for comparison between two groups. ResultsCCK8 assay showed a significant reduction in the viability of L02 cells and steatosis hepatocytes treated by 300 μmol/L HDCA (P<0.05), and qRT-PCR showed a significant increase in the mRNA expression level of FXR and significant reductions in the mRNA expression levels of PCNA, Cyclin D1, PI3K, and AKT (all P<0.05). Western blot showed a significant increase in the protein expression level of FRX (P<0.05), and after interference of FXR expression in L02 cells, there were significant increases in the protein expression levels of PCNA, PI3K, p-PI3K, AKT, and p-AKT (all P<0.05). ConclusionHDCA inhibits the PI3K/AKT signaling pathway by upregulating FXR expression, thereby inducing a reduction in the viability of steatosis hepatocytes.

ABSTRACT

Objective:To explore the influence of peer support education on psychosocial adaptation and psychological distress of breast cancer patients.Methods:Using non-simultaneous experimental research methods, a total of 51 breast cancer patients admitted to the department of breast surgery, the First Affiliated Hospital of Nanchang University from June to August 2019 were selected as the control group, and 53 breast cancer patients admitted from September to December 2019 were selected as the intervention group. The control group received routine education, while the intervention group received peer education on the basis of routine education. After 4 months of intervention, the differences of psychosocial adaptation and psychological pain were compared between the two groups.Results:After 4 months of intervention, 49 cases in the control group and 51 cases in the intervention group completed the study. In the psychosocial adaptation scale, the scores of anxiety/depression, self-esteem, self-acceptance, self-control and self-efficacy belonging in the intervention group were (29.43±2.83), (32.08±3.07), (28.10±2.62), (28.08±3.27), (31.53±3.73) points, which were higher than (27.67±3.96), (30.14±2.54), (25.94±2.91), (25.16±2.95), (30.02±2.70) points in the control group after the intervention, and difference statistically significant ( t values were -4.680 to -2.312, P<0.05). The score of psychological pain in the intervention group (2.37±1.15) points was significantly lower than that in the control group (3.57±0.98) points, and difference was statistically significant ( t value was 4.867, P<0.05). The number of patients with mild and moderate psychological pain in the intervention group was 51 cases accounting for 100% (51/51), which was higher than 48 cases accounting for 98% (48/49) in the control group, and difference was statistically significant ( χ2 value was 12.272, P<0.05). Conclusions:Peer education can effectively improve the psychosocial adaptability of breast cancer patients and reduce their pain level.

ABSTRACT

Purpose To study the clinicopathological features of extraventricular neurocytoma. Methods 12 cases of extraventricular neurocytoma were retrospectively analyzed for their clinical information and histopathological features. Results The distribution of ex-traventricular neurocytoma was as follows: cerebral hemisphere (5 cases), right thalamus (2 cases), cerebellar vermis (2 cases), saddle area (2 cases), and spinal cord (1 case). The histological features were round uniform cells, round or oval nuclei, with peri-nuclear clearing commonly seen. Neuropil-like structure, ganglion cells or ganglion-like cells, perivascular rosettes, calcification, cap-illary wall thickening and hyaline degeneration and other pathological changes were also observed. All cases showed strong immunoreac-tivity for Syn. 7 cases exhibited Ki-67 labeling index of≤2%, and other 5 cases showed 3% ~8%. 3 cases were performed with fluo-rescence in situ hybridization ( FISH) to detect loss of chromosome 1p/19q, but no loss was detected. Postoperative follow-up for 4 months to 4 years showed one case had tumor recurrence and intracranial spread in postoperative 2 years. Conclusions Extraventricu-lar neurocytoma shares similar histological features with central neurocytoma, but often more complex. Ganglion cells or ganglion-like cells, calcification, capillary wall thickening and hyaline degeneration are more frequently seen, but neuropil-like structure is less seen. Extraventricular neurocytoma needs differential diagnoses from oligodendroglioma, ependymoma, dysembryoplastic neuroepitheli-al tumor and others. Immunohistochemistry and chromosome 1p/19q loss can help in the differential diagnosis. The treatment for ex-traventricular neurocytoma is grossly total resection, with post-operative radiotherapy for subtotal resection, atypical or recurrent cases.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinicopathologic features and 19q13.42 gene changes in embryonal tumors with multilayered rosettes (ETMR).</p><p><b>METHODS</b>Immunohistochemistry and fluorescence in situ hybridization (FISH) were performed in three ETMRs.</p><p><b>RESULTS</b>The average age of the patients were 34 months. Imaging revealed huge masses with inhomogeneous enhancement and two cases showed cystic lesions. Follow-up data showed 14 and 38 months survival in two children, the third had a recurrence 4 months after operation. Morphologically, the tumor was mainly composed of dense small primitive neuroepithelial cells in patchy or multilayer rosettes within a background of advanced neuronal differentiation, containing neurocytes, ganglion cells, and neuropil-like background. Immunohistochemical staining showed the neuronal marker, synaptophysin, was positive in differentiated areas. Nestin as a neural stem cell marker was immunoreactive in the primitive neuroepithelial cells including multilayered rosettes. Neurons with positive expression of NeuN were observed occasionally. Ki-67 index was up to 40%-80% in the undifferentiated cells and rosettes, but was only 1%-3% in the differentiated areas. CD99 was positive in perivascular papillary pattern areas in one case. 19q13.42 amplification was detected in more than 30% of tumor cells in all cases.</p><p><b>CONCLUSIONS</b>ETMR is a unique entity with distinctive clinical and pathological features. Chromosome 19q13.42 abnormality is valuable for confirming the diagnosis and for further treatment research.</p>

ABSTRACT

Objective To investigate the clinic-pathological significance of von Willebrand factor (vWF) and P-selectin expression in patients with lupus nephritis (LN).Methods Biopsy specimens from 71patients with LN whose disease course ranged from 2 months to 10 years were examined and scored according to a standardized method.SLE activity was evaluated by the SLE disease activity index (SLEDAI) and the British Isles Lupus Assessment Group index.Immunohistochemistry was used to detect the expressions of vWF and P-selectin in the renal tissue of LN patients and 10 normal controls.Of all the patients,7 had World Health Organization type Ⅱ,10 had type Ⅲ and 36 had WHO type Ⅳ,18 had type Ⅴ lupus nephritis.At the same time,the 24-h urine protein (UP) excretion level,the serum creatinine,the serum C3,C4,album level,antinuclear antibody (ANA),anti-double stranded DNA and disease activity scores of all patients were detected.The relationship of their expressions with the clinicopathological parameters of lupus nephritis was analyzed.ANOVA,Nemenyi test and Pearson's analysis were used for statistical analysis.Results Compared with the control group,the in situ expressions of vWF and P-selectin in the lupus nephritis patients were significantly higher.Among these patients,renal expressions of vWF were higher in type Ⅳ (0.30±0.10) than those with type Ⅱ and Ⅲ (0.12±0.05,0.22±0.14)(x2=9.273,x2=8.712,P<0.05),while the expressions of P-selectin were higher in type Ⅳ and Ⅴ (0.29±0.05,0.36.±0.10)than those in type Ⅱ and Ⅲ (0.27±0.09,0.29±0.05 )(P<0.05).Correlation analysis indicated that vWF was correlated significantly with AI and 24-h UP (r=0.403,0.332,P<0.05 ),while P-selectin was correlated significantly with the scores of activity index (AI),24-h UP excretion rate and SLEDAI (r=0.283,0.453,0.297,P<0.05 ).The expression of vWF was also closely related to P-selectin expressions (r=0.371,P=0.001).Conclusion These results indicate that vWF and P-selectin play a critical role in the pathogenesis of LN.The detection of these two factors will provide a valuable clinical reference for renal damage.These information provide valuable clues for making better treatment regimen plan and achieving more favorable prognosis.

ABSTRACT

Objective To discuss the expression and mutation of β-catenin gene in breast carcinoma.Methods 119 breast carcinomas, 72 adenosis of breast tissues and 40 normal breast tissues were studied.Using immunohistochemistry studies to detect the expressions of β-catenin, then analyzing their relationship with the clinicopathological features of breast carcinomas. Gene mutation of β-catenin (exon 3) in 44 cases of breast carcinoma were analyzed by polymerase chain reaction (PCR) and then by direct sequencing.ResultsIn 40 cases of normal breast tissues, epithelial cells showed equally strong membranous of β-catenin protein at the cell-cell boundaries. The abnormal rate of β-catenin in breast carcinomas (78.15 %) was much higher than that of adenosis of breast tissues (44.44 %), the significant correlation was found (P<0.01). The abnormal expression of β-catenin was associated with lymph node metastasis (P=0.031). Mutation of β-catenin gene was not detected in 44 cases of breast carcinoma, while the abnormal rate of β-catenin in 44 cases of breast carcinoma was 77.30 %. Conclusionβ-catenin abnormal expression could be considered as an useful marker for determining metastasis and prognosis of human breast carcinoma and for guiding treatment. β-catenin abnormal expression is not caused by β-catenin gene mutation, maybe by other mechanisms. It needs further study.

ABSTRACT

Objective To investigate the feasibility, safety and short-term efficacy of endoscopic implantation of 5-FU Slow-release Particles for advanced gastroenteric tumor. Methods During the endoscopy procedure,slow-releasing 5-FU agents were implanted densely into the tumors and infiltrated area. Forty-five to sixty pieces of agents ( each piece equivalent to 1.67 mg 5-FU)were injected, which containing an average dose of 100 mg 5-FU. Results A total of 13 advanced gastric cancer patients were enrolled into this study. Significant effects were observed in 3 patients and good effects in 8 patients, but 2 cases with no effects. The total effective rate was 84.62%. The endoscopy examination showed that the size of tumors reduced in various degree at 1 - 2months after the implantation. No hemorrhage or perforation was observed. Parameters of hepatorenal function and routine blood test were stable after implantation. Conclusion Endoscopic implantation of 5-FU Slow-release Particles can relieve the symptoms of patients and limit tumor growth in advanced gastroenteric tumor with no marrow and hepatorenal functional impair.

ABSTRACT

Objective To investigate the effects of different cardiopulmonary resuscitation (CPR) qualifies under Q-CPR control on hemodynamics and aerobic metabolism in pigs with cardiac arrest. Method After eighteen pigs (mass [ (30+ 1) kg] were anesthetized,a Swan-Ganz catheter was positioned through the right femoral vein and linked with a continuous cardiac output monitor. An electrode cable was positioned into right ventricle through left internal jugular vein. Then different catheters were positioned in the arteriae aorta, carotid artery and fight atrium successively. Ventricular fibrillation (VF) was induced by a programmed electrical stimulation equipment. All animals underwent 4 minutes of untreated VF and then were randomly assigned to either standard CPR or nonstandard CPR. A Philips HeartStart MRx Monitor/Defibrillator was used to ensure qualifies of chest compression (e.g., depth, rate and chest recovery) by Q-CPR technology. Pigs in standard CPR group were treated with standard compression with a compression rate of 100 beat/min, a compression-ventilation ratio of 30:2, and a depth of with 38~51 mm with 100% decompression. Pigs in nonstmdard CPR group were treated with nonstandard compression with a depth of 60%~70% of standard depth with unsatisfactory decompression. Defibrillations were administered after 9 minutes of compressions and vetilations. Cardiac output (CO), mean aortic pressure (MAP) and coronary perfusion pressure (CPP) were recorded at different periods. Oxygen delivery (DO2) and oxygen consumption (VO2) were calculated according to blood gas analysis. At last,number of the survivals was recorded.Data were calculated by SPSS 11.5 statistics software. Statistical analysis was performed using Chi-Square Test and two independent samples t testing. Results The return of spontaneous circulation (ROSC) achievement ratio with nonstandard CPR was 28.6% and with standard CPR was significantly greater (90.9%, P=0.013).Hemodynamics (e. g., CPP, CO and MAP) and systemic oxygenation with standard CPR were much better. Conclusions In this animal model of cardiac arresr in pigs, standard CPR could improve hemodynarmics by increasing CPP and CO. And standard CPR could beneficial aerobic metabolism by increasing DO2 and VO2. As a result, ROSC achievement ratio of pigs with standard CPR was much greater.
