@#Abstract: Objective To analyze the epidemiological characteristics of pulmonary tuberculosis (PTB) in Ankang City from 2011 to 2021, so as to provide a scientific basis for the formulation of PTB prevention and control strategy. Methods Descriptive statistics were used to analyze the epidemiological characteristics of PTB in Ankang City from 2011 to 2021, and a time series model was established to quantitatively predict the incidence of pulmonary tuberculosis in 2023. Results The incidence rate in Ankang City showed a significant upward trend from 2011 to 2017, and a more obvious downward trend in 2017-2021 (P<0.05), and the decrease rate in 2021 was 40.36% compared with that in 2017. The proportion of etiological positivity increased from 12.5% in 2014 to over 50.00% after 2019. The incidence season was mainly concentrated in the first quarter, accounting for 28.39% of the annual incidence. High incidence areas were concentrated in the south of Ankang: Langao County, Ziyang County and Zhenping County, with 128.32/100 000, 117.07/100 000 and 110.44/100 000, respectively. Low incidence areas were located in the north of Ankang: Ningshan County, with 60.62/100 000. Farmers and students were the high incidence groups, accounting for 81.80% and 4.97% of the total cases respectively. The incidence of young children was relatively low, but cases were reported every year. The incidence rate of male was 2.39 times that of female. The age of onset increased significantly from 15 years old, and the peak incidence was in the age group of 60-<80 years old, followed by the age group of 45-<60 years old, the average annual incidence was 136.44/100 000 and 104.47/100 000, respectively. The model ARIMA(0,1,1)(0,1,1)12 predicted that the incidence of the disease generally increased from October 2022 to March 2023, then steadily decreased, and increased again in December. Conclusions The incidence of tuberculosis varies in different areas of Ankang City, and males, farmers, students and the elderly are all factors of high incidence of tuberculosis. Therefore, different prevention and control strategies should be adopted according to the characteristics of population in different areas. The number of cases in Ankang City in 2023 showed an overall downward trend, which can provide a reference for the prevention and control of PTB.

ABSTRACT

Technical Specifications for Revision of Safety Information in Marketed Chinese Patent Medicine Instructions,a series of group standards,were proposed by Professor ZHANG Bing from Research Center for Pharmacovigilance and Rational Use of Traditional Chinese Medicine,and underwent centralized management by Chinese Association of Chinese Medicine. They were officially released on July 23 and implemented on July 31,2021. The series of group standards consist of six sections,including general principles,adverse drug events,contraindications,precautions,application for special populations,and warnings. The section of general principles is comprised of holistic and programmatic expressions,which explain the general technical requirements for revising the marketed Chinese patent medicine instructions. The other five sections focus on information collection,screening,transformation,and illustration of specific items,forming a standardized revision technical process. This series of standards is the result of multiple rounds of research and the suggestions of more than 200 experts in different professional fields of " medicine-pharmacy-management-law-enterprise" have been gathered therein to reach a consensus. With the purposes of establishing standardized technical specifications for the revision of safety information in the marketed Chinese patent medicine instructions,guiding marketing authorization holders in revising the instructions,filling the gaps in the research of Chinese patent medicine instructions,promoting the deve-lopment of pharmaceutical care and academic research,and encouraging the rational and safe medication of Chinese patent medicine,the series of group standards is of great significance.

ABSTRACT

Drug instructions,the statutory and technical documents recording effectiveness and safety information,are an important basis for guiding doctors,pharmacists,and patients to use drugs rationally,and their scientificity,standardization,and accuracy directly affect the medication safety of the public. The sections of adverse drug events,contraindications,precautions,warnings,and application for specific populations in drug instructions directly express safety information and measures for rational use of drugs. In the drug life cycle,marketing authorization holders( MAHs) need to update safety information in the instructions promptly to ensure the safety and effectiveness of clinical drug medication. At present,revising instructions is an important measure to control drug risks. In the drug life cycle,in order to standardize the revision of safety information in the instructions by MAHs and eliminate inexact terms such as " unclear",the Technical Specifications for Revision of Safety Information in Marketed Chinese Patent Medicine Instructions,a series of group standards,have been established under the guidance of Standardization Department,China Association of Chinese Medicine. Therefore,on the basis of the existing rules and regulations,the standardized technical procedures for revising instructions came into being to help clinical safe and rational medication of drugs,and implement the strategy of " Healthy China".

ABSTRACT

OBJECTIVES: This study assessed the protective effect of calcium dobesilate against contrast-induced nephropathy (CIN) after coronary angiography (CAG) or percutaneous coronary intervention (PCI) in patients with diabetes and chronic kidney disease (CKD). METHODS: A total of 130 patients with diabetes and CKD estimated glomerular filtration rate: 30-90 mL/min/1.73m2 were enrolled and included in the analysis. They were divided into experimental (n=65) and control groups (n=65). Patients in the experimental group were administered oral calcium dobesilate (500 mg) three times daily for 2 days before and 3 days after the procedure. The serum creatinine (SCr), cystatin C (Cys C), and neutrophil gelatinase-associated lipocalin (NGAL) levels were measured before and after the procedure. RESULTS: The mean SCr level at 24h after the procedure was found to be significantly lower in the experimental group than in the control group (79.1±19.6 μmol/L vs. 87.0±19.3 μmol/L, p=0.023). However, the Cys C and NGAL levels were not significantly different between the two groups at all measurement time points (all p>0.05). The incidence of CIN defined by the SCr level was significantly lower in the experimental group than in the control group (3 [4.6%] vs. 13 [20.0%], p=0.017). However, the incidence of CIN defined by the Cys C level was not statistically different between the two groups (7 [10.8%] vs. 7 [10.8%], p=1.000). CONCLUSIONS: This study revealed that calcium dobesilate has no preventive effect against CIN in patients with diabetes and CKD.

ABSTRACT

To investigate the source of serum exosomal HOTAIR, to uncover the diagnostic and prognostic values of serum exosomalHOTAIR, and to discern the expression of serum exosomal HOTAIR between neoadjuvant chemotherapy and response totamoxifen therapy. Samples were collected from the Third Affiliated Hospital of Kunming Medical University, TumorHospital of Yunnan. Exosomes were isolated from serum, cell culture medium and tumor tissues. We used transmissionelectron microscopy and western immunoblotting assay to characterize exosomes, and real-time PCR (qPCR) to assessHOTAIR expression. Neoadjuvant chemotherapy and tamoxifen therapy were carried out according to establishedguidelines. Breast cancer patients expressed higher serum exosomal HOTAIR than did healthy individuals (P \ 0.001).Serum exosomal HOTAIR levels 3 months after surgery were markedly decreased compared with levels before surgery(P \0.001), and the expression level of exosomal HOTAIR in cell culture medium increased with time in both breastcancer cell lines (72 h [48 h[ 24 h, 48 h vs 24 h [P \ 0.05]; 72 h vs 24 h [P \ 0.01]). Expression of serum exosomalHOTAIR in nude mice was notably greater than in the mock control group (P \0.001). The results of the ROC analysisrevealed an AUC for serum exosomal HOTAIR of 0.9178 with a 95% CI of 0.8407–1.017 (P \0.01). The AUC for theCA15-3 cell line was 0.7378 (95% CI, 0.5585–0.9170; P = 0.03). High expression of exosomal HOTAIR led to a worsedisease-free survival (P = 0.0481) and overall survival (P = 0.0463). In the high-expression chemotherapy group, sixpatients achieved a partial response (PR) and eight demonstrated stable disease (SD) and nine patients achieved PR and twoSD in the low-expression group (P = 0.048). In the low-expression tamoxifen group, one patient had a recurrence of breastcancer and another 10 patients exhibited no recurrence, while six showed recurrence, and seven had none in the highexpression group (P = 0.035). We isolated exosomes successfully, and demonstrated that serum exosomal HOTAIRoriginated from primary breast cancer tissue. We conclude that serum exosomal HOTAIR exhibits the potential to be adiagnostic and prognostic biomarker. High expression of serum exosomal HOTAIR was also correlated with poorneoadjuvant chemotherapy and response to tamoxifen therapy.

Myeloproliferative neoplasms(MPN) are high risk factors of atherosclerosis and arterial thromboembolism. This article gives a review on the incidence, pathogenesis, clinical features and treatment of MPN with coronary artery disease, emphasizing the importance of knowing MPN as possible causes of acute cardiovascular events in patients, in order to help clinicians to learn about the disease and make a proper diagnosis and treatment.

ABSTRACT

<p><b>OBJECTIVE</b>Clinical transplantation evidence has been indicated that umbilical cord blood (UCB) can be useful in the hematopoietic reconstitution in the children, but can not be well in the adult hematopoietic transplantation because of the low count. This study was to evaluate a new method for collecting stem cells from human placenta and umbilical cord, and to comparatively analyze the similarity and difference of quality and quantity of the cells.</p><p><b>METHODS</b>The UCB was collected, in same time the placental tissue was sterily collected; the umbilical placenta was collected by perfusing the blood (UPB) cord arterial and venous vascules with 0.9% saline; the mesenchymal stem cells (MSC) from same source of umbilical cord and placental tissues were isolated and cultured. The cell colony assay and flow cytometry were performed to determine the proliferation capacities and cell markers of UMSC and PMSC.</p><p><b>RESULTS</b>The total nuclear cells (NC) and hematopoietic stem cells (CD34(+)) from UPB and UCB were (17.45 ± 16.86) × 10(8), (6.9 ± 4.61) × 10(8) and (2.97 ± 2.25) × 10(6), (1.91 ± 1.7) × 10(6), respectively. Furthermore, the UPB contained more early precursor of hematopoietic stem cells (CD33(+) CD34(-)) (6.2 ± 13.5) × 10(5), (0.2 ± 0.8) × 10(5) ; and high proportion of MSC to NC (25.21 ± 18.69, 0.05 ± 0.10)%, respectively in 62 samples. There were no difference of the MSC level in UPB and UCB, as well as in the morphology and cell markers.</p><p><b>CONCLUSION</b>UPB has rich hematopoietic stem cells and mesenchymal stem cells. Placenta may offer another source for hematopoietic stem cells in research of hematopoietic stem cells and regeneration medicine.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the synergistic anti-tumor effect of radiotherapy and horseradish peroxidase/prodrug indole-3-acetic acid (HRP/IAA) gene therapy system using chimeric hTERT promoter responsive to ionizing radiation.</p><p><b>METHODS</b>The synthetic hTERT promoters containing four tandem-repeat copies of radio-inducible CArG elements, and the chimeric promoter containing cytomegalovirus (CMV) early promoter were both constructed. The activities of the chimeric promoters in cancer cell lines (HeLa, A549, and MHCC97) and normal cell line (MRC-5) were detected using luciferase reporter gene expression analysis after a (60)Co γ-irradiation treatment at a series of doses(a single dose of 0 to 10 Gy). The anti-tumor effect of combining irradiation with HRP/IAA gene-directed enzyme prodrug therapy system controlled by the chimeric promoter was tested by colony formation assay, cell counting and apoptosis analysis.</p><p><b>RESULTS</b>The chimeric promoters were ineffective in normal human cells, even after irradiation, but the expression of luciferase gene in tumor cells was significantly higher. The activity of the chimeric promoter in MRC-5 cells was 22.3%, 12.9% and 13.6% of that in HeLa, A549 and MHCC97 cells, respectively. After irradiation, the ratios were 11.7%, 8.7% and 8.8%, respectively. Furthermore, the chimeric promoters could successfully induce the expression of luciferase gene following different doses of radiation, with maximal inducible activity seen after 6 Gy irradiation. The chimeric promoter containing four tandem-repeat copies of radio-inducible CArG elements and CMV early promoter showed the highest activity with 6 Gy irradiation. The relative luciferase activities in HeLa, A549 and MHCC97 cells were 1.7 ± 0.2, 2.3 ± 0.2 and 2.3 ± 0.1, respectively. The chimeric promoter mediated suicide gene therapy system could increase radio-sensitivity in different cancer cells. Compared with the control system, it plus irradiation showed stronger cell proliferation inhibition, 67.3% vs. 26.1% in HeLa, 69.0% vs. 28.3% in A549, 64.6% vs. 20.8% in MHCC97 cells, and also higher apoptosis-inducing effect, 39.6% vs. 14.2% in HeLa, 33.0% vs. 12.4% in A549, and 33.2% vs. 14.2% in MHCC97 cells.</p><p><b>CONCLUSIONS</b>Chimeric promoter containing hTERT promoter, CArG elements and CMV promoter preserve the tumor-specificity in telomerase-positive tumor cells, and irradiation-responsive to low dose of radiation. The suicide gene therapy using this promoter plus radiotherapy show a strong anti-tumor effect in vitro. It is expected to have a good potential for future application in gene radiotherapy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the therapeutic efficiency of human telomerase reverse transcriptase promoter (hTERTp) mediated horseradish peroxidase (HRP) catalyzed effects of indole-3-acetic (IAA) on laryngeal squamous cell carcinoma with different radiosensitivity in vivo.</p><p><b>METHODS</b>Human laryngeal squamous cell carcinoma Hep-2 and Hep-2R cells were transplanted into nude mice. After growing to about 30 approximately 50 mm3, the tumor-bearing mice were randomly divided into eight groups: Hep-2 line: combined group (A), gene group (B), radiation group (C) and blank group (D); Hep-2R line: combined group (AR), gene group (BR), radiation group (CR) and blank group (DR). The phTERTp-HRP was delivered by intratumoral injection and the IAA by intraperitoneal injection, combined with 2 Gy daily radiation to a total dose of 30 Gy. The tumor volume was recorded. The cell apoptosis was detected by terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL) assay. The expression of HRP protein was detected by AP immunohistochemisty.</p><p><b>RESULTS</b>The tumor growth of combined groups was attenuated significantly and the tumor volume of Hep-2R blank group was the largest. The inhibition rate of each group was: A: 54.8%, B: 10.0%, C: 31.9%; AR: 52.7%, BR: 24.8%, CR: 17.0%. In the combined groups, necrosis and apoptosis of tumor cells were observed under the light microscope and the apoptotic index [A (16.6 +/- 1.3)% vs. AR (17.6 +/- 1.3)%] of tumor cells was highest (P < 0.05). The HRP protein expression of BR (33.3 +/- 8.9)% was higher than that of B (21.9 +/- 5.7)%, which was directly up-regulated in the tumors (45.0% vs. 54.8%, P < 0.05) after radiation.</p><p><b>CONCLUSION</b>In the Hep-2- and Hep-2R-transplantation tumors in nude mice, hTERTp can be induced by radiation and enhance the expression of horseradish peroxidase (HRP) gene according to telomerase activity. hTERTp-HRP/IAA system, which has synergistic effects with radiation and inhibits the tumor growth by induction of apoptosis and necrosis, may be a new gene-radiation strategy for the treatment of laryngeal carcinoma.</p>

ABSTRACT

Objective To evaluate the effect of laparoscopy in treatment of renal cysts.Methods Twenty- eight and thirty-five patients with renal cysts were performed with transperituneal and retroperitonel laparoscopy,re- spectively.Results A total of 62 patients were treated successfully,the average operation time was 60 minutes,no complications presented,and there was no recurrence in follow-up.Conclusion Laparoseopy survey had the advan- tages of less trauma and fewer complications and rapid recovery in treatment of renal cysts,which was suggested for clinical applications.
