ABSTRACT

Objective:To investigate the clinical effect of hysteroscopic scar excision for cesarean scar pregnancy(CSP) and its effect on serum β-human chorionic gonadotropin(β-HCG).Methods:From January 2017 to January 2019, 83 patients with endogenous CSP admitted to the People's Hospital of Wenzhou were divided into observation group(43 cases) and control group(40 cases) according to different surgical methods.The observation group underwent hysteroscopic scar lesion resection, while the control group underwent uterine artery embolization.The success rate, intraoperative bleeding volume, vaginal bleeding time and hospitalization time, postoperative complications and the recovery time of blood β-HCG were compared between the two groups.Results:The success rate of operation in the observation group was 97.67%(42/43), which was higher than that in the control group[82.50%(33/40)](χ 2=3.875, P<0.05). The amount of bleeding in the observation group[(43.28±10.28)mL] was less than that in the control group[(74.32±15.48)mL], the time of vaginal bleeding in the observation group[(5.79±1.45)d] was shorter than that in the control group[(8.54±1.89)d], and the time of hospitalization in the observation group[(7.32±1.48)d]was shorter than that in the control group[(10.80±1.97)d], the differences were statistically significant( t=10.832, 7.468, 9.140, all P<0.05). There was no statistically significant difference in postoperative complications between the two groups( P>0.05). The recovery time of blood β-HCG in the observation group[(19.54±3.52)d]was shorter than that in the control group[(28.73±6.57)d]( t=8.020, P<0.05). The total satisfaction rate of the observation group was 97.67%(42/43), which was higher than 77.50%(31/40) of the control group(χ 2=6.170, P<0.05). Conclusion:Hysteroscopy-assisted scar excision is effective in the treatment of CSP, and can shorten the recovery time of blood β-HCG.

ABSTRACT

OBJECTIVE@#To investigate the reasons that hearing-impaired patients who owned the indication of hearing aid fitting but were not successfully fitted.@*METHOD@#Analyzing the records of hearing aid fitting for 2 635 hearing-impaired people from 2005 to 2010, especially for the people who were not successfully fitted.@*RESULT@#In general, there were 1 700 people having proper hearing aids. The success rate of hearing aid fitting is 64.52%, which increased year by year. According to the analysis of records about hearing-impaired people who were not successfully fitting, the primary factor was economic constraints (23.64%), the second was the patients could not accept them (18.61%), and the third was acceptation and satisfaction for the professional centers (10.27%). The another factor also included the dissatisfaction for the hearing aid (8.13%). Meanwhile, it showed the success rate in the auditory specialist clinic was significantly lower than that in the general otolaryngologist clinic (P < 0.05). In addition, the success rate (61.29%) in the group patients' ages were lower than 50 years old was significantly lower than that (66.33%) in the group patients' ages were more than 50 years old (P < 0.05).@*CONCLUSION@#The research shows that so many factors could lead to the success of hearing aid fitting. So its necessary for us to do some special things to overcome them, including disseminating the knowledge about hearing rehabilitation and improving our abilities. They are the only solutions to improve the patients confidence and make them get the better rehabilitation from the hearing aids.

ABSTRACT

Objective To explore the signal-to-noise ratio loss (SNR loss) of the people with normal pure-tone audiograms .Methods A group of 10 patients were recruited into the study ,who complained they could not hear speech clearly in the real world environment but had normal pure-tone audiograms ,immittance and word rec-ognition score (WRS) results .The Mandarin hearing in noise test (M-HINT ) was administered to them and the results were compared to the normal ranges published in the journal .Results All of them had SNR loss when noise (speech spectrum) came from front of them ,but 9 patients had SNR loss when noise came from 90 degree left or 90 degree right of them .Conclusion The current study showed the patients had speech hearing loss in the noisy envi-ronment ,regardless of whether they had normal audiograms ,immittance and WRS results .So we should pay atten-tion to such patients and adopt some special tests (eg M-HINT) .It is better for us to communicate and counsel with them .

ABSTRACT

Parathyroid hormone related protein (PTHrP) has important biological functions in calcium metabolism. The aim of this study was to silence the expression of PTHrP by RNA interference and recombinant adenovirus, and to provide a material to investigate the relative functions of PTHrP in goat mammary gland epithelial cell. The Block-iT shRNA interference system was used in this experiment. We designed and synthesized two pairs of complementary single-strand DNA oligonucleotides (shRNA-322/357) targeting two different sites of PTHrP mRNA. Then the oligonucleotides were inserted into shuttle vector pENTR/CMV-GFP/U6. After detection of the interference efficiency by Western blotting, we chose pENTR/CMV-GFP/U6-322 and adenovirus backbone vector pAD/PL-DEST to produce recombinant vector pAD/PL-DEST/CMV-GFP/U6-322. The first generation recombinant adenovirus particles (AD-PTHrP-322) were produced and further amplified by transfecting HEK-293 cells. The titer of the recombinant adenovirus reached 2.0 x 1(9) PFU/mL determined by TCID50 assays. The result of real-time quantitative PCR indicated that mRNA expression levels of gene were reduced 29.2%, 68.1% and 82.6% (P < 0.05), respectively, when goat mammary gland epithelial cells were infected with AD-PTHrP-322 after 24, 48 and 72 h, in which PTHrP. Western blotting also showed that the expression of PTHrP was reduced by infecting the cells with AD-PTHrP-322. AD-PTHrP-322 has been proved with significant interference effect on expression of PTHrP.

ABSTRACT

deltaFosB, a naturally occurring truncated isform of fosB gene, existed in many tissues stably and played an important role in formation and differentiation of adipocyte and osteoblast. deltaFosB may be related to the metabolism of calcium in bone and mammary gland and regulate the signal pathway of calcium transfer from bone to mammary gland. We first sub-cloned deltafosB gene of goat into the vector pET32a to construct prokaryotic expression vector pET32a-deltafosB. Then we induced for deltafosB gene expression efficiently by IPTG. Finally we immunized the adult rabbits with purified recombinant deltaFosB to prepare rabbit anti-goat deltaFosB polyclonal antibody. iELISA analysis showed the antibody with the titer of 1:51 200, and Western blotting result showed that the antibody could specifically detect the deltaFosB protein expressed in prokaryotic cell and HEK-293 cell, respectively. Further Western blotting assay showed that deltaFosB expressed in various tissues of goat in vivo.
