ABSTRACT

Objective:To observe the clinical efficacy of addition and subtraction therapy of Jinkui Shenqiwan combined with Buzhong Yiqitang to postmenopausal osteoporosis (PMO) with deficiency of spleen and kidney, and to investigate its regulation effect on immune inflammatory factors. Method:One hundred and sixty patients were randomly divided into observation group and control group, with 80 cases in each group. Both groups got comprehensive western medicine treatment measures. Patients in control group additionally got Zhuanggu Zhitong capsule, 4 capsules/time, 3 times/day. Patients in observation group additionally got addition and subtraction therapy of Jinkui Shenqiwan combined with Buzhong Yiqitang, 1 dose/day. The treatment was continued for 24 weeks. Before and after treatment, lumbar L2-4 bone mineral density (BMD) was detected by Dual energy X-ray absorptiometry (DXA) and lumbar BMD was detected by quantitative CT (QCT). Scores of traditional Chinese medicine(TCM) syndromes and Chinese osteoporosis-targeted quality of life questionnaire (COQOL) were graded. Levels of Estradiol (E<sub>2</sub>), type Ⅰ procollagen amino terminal pro peptide (PINP), serum osteocalcin (OC), osteoprotegerin (OPG), type Ⅰ collagen cross-linked C-terminal peptide (S-CTX), tartrate resistant acid phosphatase (TRACP) and urinary pyridinoline (PYD) were detected. Levels of CD4<sup>+</sup> T cells, CD8<sup>+</sup> T cells, interleukin-17 (IL-17), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), <italic>γ-</italic>interferon(IFN-<italic>γ</italic>) and interleukin-4 (IL-4) were calculated. The proportion of T helper cell (Th)17 and regulatory T cell (Treg) in CD4<sup>+</sup> T cells was calculated. Besides, the safety was evaluated. Result:Bone density was detected by DXA in observation group, and its T-value and bone density detected by QCT were all higher than those in control group (<italic>P</italic><0.01). After treatment, scores of TCM syndrome and COQOL were lower than those in control group (<italic>P</italic><0.01). Levels of PINP, OC, S-CTX, TRACP and PYD/Cr were all lower than those in control group (<italic>P</italic><0.01). Levels of OPG, CD8<sup>+</sup> and Treg were higher than those in control group (<italic>P</italic><0.05), levels of Th17, Th17/Treg, CD4<sup>+</sup>/CD8<sup>+</sup>, IL-17, TNF-<italic>α</italic> and IFN-<italic>γ </italic>were lower (<italic>P</italic><0.01), and levels of IL-4 and E<sub>2</sub> were higher than those in control group (<italic>P</italic><0.01). The clinical efficacy in observation group was better than that in control group (<italic>Z</italic>=2.103, <italic>P</italic><0.05). Conclusion:On the basis of calcium and vitamin D supplementation, Jinkui Shenqiwan combined with Buzhong Yiqitang can improve levels of E<sub>2</sub> and bone density, reduce clinical symptoms, improve quality of life, regulate bone metabolism index and immune inflammation reaction, with better clinical efficacy and safety.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the adrenocortical function in children with severe and critical enterovirus 71 infection by using a high-dose (250 µg) adrenocorticotropic hormone (ACTH) stimulation test. And to at provide experimental basis for glucocorticoid in the treatment of hand-foot-and-mouth disease (HFMD).</p><p><b>METHOD</b>This was a prospective multi-center study which was carried out in PICUs of Beijing Children's Hospital, Zhengzhou Children's Hospital, Kaifeng Children's Hospital and Linyi People's Hospital in Shandong province. Children with severe and critical hand-foot-mouth disease admitted to PICUs of the four hospitals from June 2009 to April 2010 were enrolled in this study, and EV71 virus nucleic acid test and high-dose (250 µg) ACTH stimulation started at the same time. EV71 virus nucleic acid positive 51 cases were eventually enrolled in the study. Cortisol test was performed at baseline (T0) and after high-dose (250 µg) ACTH stimulation at 30 minutes (T30), 60 minutes (T60) in the first 6 hours after admission, but before glucocorticoid was given. The adrenocortical function was evaluated according to ΔTmax [ΔTmax=(T30, T60 maximum)-T0]. Diagnostic criteria of adrenal insufficiency (AI) is increment (ΔTmax)≤9 µg/dl.</p><p><b>RESULT</b>The incidence of AI in 51 cases was 52.94% (27/51). The incidence of AI in severe group was 44.74% (17/38), which was significantly higher in critical group 76.92% (10/13), P<0.05. Of the cases with a pediatric critical illness score (PCIS)≤70, 81.82% (9/11) had adrenal insufficiency, and it was 28.57% (4/14) when PCIS≥90. The incidence of AI was 75% (6/8) and 48.84% (21/43) in death and survivor group respectively, but there were no significant difference between the two groups (P>0.05). Baseline (T0) cortisol in death group was higher than survivor group (P<0.05).</p><p><b>CONCLUSION</b>AI may occur in children with enterovirus 71 infection. The critical enterovirus 71 infection had a high incidence of AI. AI may affect the prognosis of patients with severe and critical enterovirus 71 infection. Exogenous glucocorticoids administration may be considered when AI is identified or highly suspected. The timing, dosage and regimen of glucocorticoid are still unclear. Further animal experiments and clinical trials are needed.</p>

ABSTRACT

Objective To evaluate the effects of regular follow-up and systematic management in premature infants on early discovering of cerebral palsy and intervention efficacy in early phase. Methods Fifty premature infants with cerebral palsy recruited from early children development outpatient service, were classified as treatment group; 40 premature infants with cerebral palsy recruited from general outpatient service, were classified as control group. The infants of treatment group received systematic follow-up and intervention. while the infants of control group received no systematic management. Age and therapeutic efficacy were compared between the two groups. ResultsThe average age in the treatment group ( [4. 0 ± 1.2] months) was significantly younger than that of the control group ( [7.0 ± 1.4] months) ( P < 0. 05 ). The mental development index ( MDI )and physical development index (PDI) in the treatment group was 91.8 ± 10. 2 and 90. 2 ± 11.2,respectively,which were significantly higher than that of the control group(80. 2 ± 11.7 for MDI and 79. 3 ± 10. 2 for PDI)( P < 0. 05 ) . Conclusion Systematic management could help discover cerebral palsy in premature infants in time and could improve physical and mental development of these infants.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the migration and differentiation of the neural precursor cells (NPCs) that derived from murine embryonic stem cells (ESCs) when they were transplanted into amyloid beta (A beta)-treated rat hippocampus.</p><p><b>METHODS</b>MESPU35, a murine ESC cell line that express the enhanced green fluorescent protein (EGFP), was induced differentiation into nestin-positive NPCs by modified serum-free methods. The A beta plaques and the differentiation of the grafted cells were observed by immunofluorescent staining.</p><p><b>RESULTS</b>Comparing 16 weeks with 4 weeks post-transplantation, the migration distance increased about 5 times; the rate of migratory NPCs differentiating into glial fibrillary acidic protein (GFAP)-positive cells kept rising from (30.41+/-1.45) % to (49.25+/-1.23) %, and the rate of NPCs differentiating into neurofilament 200 (NF200) positive cells increased from (16.68+/-0.95) % to (27.94+/-1.21) %. Meanwhile, the GFAP-positive cells targeting to the ipsilateral side of A beta plaques increased from 60.2% to 81.3%, while the NF200-positive cells increased from 61.3% to 84.1%. The migration distance had significant positive linear correlations to the neuronal differentiation rate (r = 0.991) and to the astrocytic differentiation rate (r = 0.953).</p><p><b>CONCLUSION</b>Engrafted NPCs migrate targetedly to the A beta injection site and differentiate into neurons and astrocytes.</p>
