ABSTRACT

Objective:To investigate the clinical efficacy and adverse reactions of linezolid in the treatment of gram-positive coccal infections after chemotherapy in older adult patients with leukemia.Methods:Ninety-two older adult patients with leukemia complicated by gram-positive coccal infections, who received treatment in Yiwu Central Hospital from January 2017 to December 2019, were included in this study. They were randomly assigned to receive routine anti-infection treatment (control group, n = 46) or linezolid treatment (observation group, n = 46). Clinical efficacy, the time required for body temperature restoring to normal, and medication time were compared between the two groups. Results:Total response rate was significantly higher in the observation group than in the control group [95.65% (44 /46) vs. 78.26% (36/46), χ2 = 6.13, P = 0.013]. The time required for body temperature restoring to normal and medication time in the observation group were (7.98 ± 1.04) days and (8.58 ± 1.31) days, respectively, which were significantly shorter than those in the control group [(8.85 ± 1.47) days, (9.46 ± 2.52) days, t = 3.27, 2.10, P = 0.001, 0.019). The incidence of adverse reactions was significantly lower in the observation group than in the control group [4.35% (2/46) vs. 19.57% (9/46), χ2 = 5.05, P < 0.05]. Conclusion:Linezolid is highly effective on gram-positive coccal infections after chemotherapy in older adult patients with leukemia. Linezolid treatment requires comparatively shorter time required for body temperature restoring to normal and shorter medication time and is safer than routine anti-infection treatment.

ABSTRACT

Objective:To summarise the clinical experience in replantation of the severed auricle distal to helix with microsurgical technique.Methods:From December 2018 to October 2021, a total of 5 patients with severed auricle injury were treated in the Department of Hand and Foot Surgery of The Affiliated Hospital of Qingdao University. They were 4 males and 1 female with 23-62 years old. After complete debridement of the auricular pinna, the retrograde replantation method was used. For arteries: a dorsal vein of the foot was used to bridge the posterior auricular artery. For veins: 2 patients had the veins directly anastomosed, 2 had the arteriovenous bridging to the posterior ear vein with dorsal foot veins, and the veins in 1 case were not anastomosed. Among the patients, 2 developed venous occlusions after severed auricle, and were treated with bloodletting through small incision at the skin margin. Two patients who received the arteriovenous of the severed auriclse achieved good blood supply. All the patients underwent treatments including anti-freezing, anti-spasm and anti-infection after the emergency surgery. The follow-ups were conducted regularly by telephone and by display photos via WeChat after surgery.Results:All 5 severed auricles were successfully replanted and survived. Postoperative follow-up ranged from 3 months to 2 years, with an average of 10 months. In the 2 cases with venous crisis, the auricles had mild atrophy. All auricles had no obvious pigmentation, and had the sensation recovery back to normal in 1 year after surgery.Conclusion:The pre-judgment of blood vessel quality and high-quality microsurgery skills are the necessary pre-conditions for auricle replantation. For replantation of severed auricle, it is the key to prevent vascular crisis by having the injured blood vessels thoroughly removed.

ABSTRACT

Objective:To investigate the clinical effect of free posterior tibial artery perforator flap in repair of forefoot soft tissue defect.Methods:From January 2017 to January 2021, a retrospective study was conducted on 13 patients with forefoot soft tissue defect, metatarsal head exposed, and forefoot transverse arch integrity, including 9 males and 4 females. The age was (40.0±13.0) years old. Cause of injury: 8 cases of traffic accident injury, 5 cases of heavy object smashing injury. Seven cases had forefoot skin defect and toe damage, and 6 cases had forefoot skin avulsion injury, open toe fracture with tendon, blood vessel and nerve injury. The wound area was 4.5 cm×3.0 cm-8.0 cm×6.0 cm. VSD treatment was performed in the first stage, and free posterior tibial artery perforator flap was used for the second stage. The flap area was 5.5 cm×4.0 cm-9.0 cm×7.0 cm. Outpatient reviews scheduled at 1, 2, 3, and 6 months after surgery, through outpatient clinic, telephone or WeChat. The flaps were evaluated according to appearance, texture, sensory recovery, and the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hind foot function scoring system.Results:All 13 flaps survived. The follow-up lasted for 6 to 24 months. The feet were in good shape, walking with weight beries, and the flaps had satisfactory appearance without wear and tear. Five cases were S 3, 6 were S 2, and 2 were S 1. According to AOFAS ankle-hindfoot function score, 4 had excellent scores, 7 were in good, and 2 in fair. Conclusion:The free posterior tibial artery perforator flap has relatively constant perforators, and the pedicle of the middle and upper perforators is longer, and the flap can build part of the sensation. Posterior artery perforator flap is a good flat for repairing the soft tissue defects of the metatarsal head of the forefoot.

ABSTRACT

To report a case of completely amputated ear beyond the anthelix with a severe cross section contusion. After the inspection and exclusion of coronavirus disease (COVID-19) as per prevention and control procedure, an ear replantation surgery was performed under strict protective measures. At 2 week of followed-up, the replanted auricle survived well and the left ear showed a good appearance.

ABSTRACT

Objective To evaluate the use of endoscopic papillary balloon dilation in treatment of choledocholithiasis associated with peripapillary duodenal diverticulum.Methods From January 2017 to July 2018,a retrospective study was conducted on 124 patients with choledocholithiasis associated with peripapillary duodenal diverticula at the Department of Hepatobiliary Surgery,Second Hospital of Hebei Medical University.These patients were divided into the small endoscopic sphincterotomy combined with balloon dilation group (sEST+EPBD,n =60) and the simple papillary balloon dilation group (EPBD n =64).The operation time,one-time success rate of stone removal,complication and hospitalization stay were compared between the two groups of patients.Results The hospitalization expenses of the EPBD group was significantly less than the sEST+EPBD group (P<0.05).The operation time of the EPBD group was significantly shorten than the sEST+EPBD group (P<0.05).There were no significant differences in the one-time success rate of stone removal,complication rates and hospitalization stay between the two groups (P>0.05).Conclusions Compared with sEST+EPBD,treatment of choledocholithiasis in patients with peripapillary duodenal diverticula using simple balloon dilation shortened the operation time,did not increase the complication rates and hospitalization stay.The procedure was safe and effective,and resulted in almost the same one-time success rate of stone removal.

ABSTRACT

Objective:To observe the effect of distraction before discectomy (DBD) in the distraction of the height of anterior cervical space,and to explore its feasibility in restricting the over distraction.Methods:A total of 31 patients with cervical spondylotic myelopathy were treated with anterior cervical discectomy and fusion (ACDF).During surgery,the intervertebral space was distracted before discectomy,the procedure was defined as DBD technique.Before surgery,the distance from the arch top of inferior endplate of upper vertebrae of the index level to the midpoint of superior endplate of lower vertebrae was measured (H0).The same method was used to measure the adjacent proximal and distal intervertebral space heights (Hp and Hd),and the mean value of Hp and Hd,H,was regarded as a referential height that the index intervertebral space should be restored.During operation,the intervertebral space heights before (H1) and after (H2) discectomy with application of DBD were measured withthe aforementioned method.The pre-and post-operation index segment heights AB and A'B'were measured respectively.The change of index segment height was defined as △H (A'B'-AB),the index intervertebral space height after operation was defined as Hs (H0 +△H).The patients were divided into neck pain group and non-neck pain group according to their post-operative neck pain VAS scores.The radiographic data of the patients in two groups was analyzed and compared with statistical methods.Results:The post-operative neck pain incidence rate was 19.35%.During operation,after DBD,the intervertebral space height change △H1 was (1.19±0.51) mm.In non-neck pain group,the difference between H1 (6.95 mm±0.84 mm) and H (6.98 mm±0.70 mm) was not significant (P=0.80),and the difference between H2 (7.31 mm±0.90 mm) and H1 (6.95 mm± 0.84 mm) was significant (P<0.01).In neck pain group,the difference between H2 (8.33 mm± 1.39 mm) and H1 (7.87 mm±1.35 mm) was significant (P<0.01).In neck pain group,the △H was (3.04±0.42) mm;in non-neck pain group,the △H was (1.70±0.51) mm;the difference between two groups was significant (P<0.01).In nonneck pain group,H1,H2 and H3 had good consistency with H.Conclusion:DBD can effectively control the distraction height of the index intervertebral space within 2 mm,also convenient to let the index intervertebral space be similar with the adjacent segment and easy to follow.

ABSTRACT

Objective To explore the mechanism of tripterygium wilfordii polyglycoside tablets for elderly patients with relapsing refractory immune thrombocytopenic purpura (ITP),and to seek the theoretical basis for Chinese medicine treatment of this disease.Methods The clinical data of 79 patients with relapsing refractory ITP were retrospectively analyzed.According to whether the combined use of tripterygium wilfordii polyglycoside,they were divided into the control group (35 cases) and the observation group (44 cases).The control group was treated with platelet and tranexamic acid,sulfasalazine,sulforaphane sodium,hemagglutinin and other symptomatic hemostasis treatment.The observation group in symptomatic hemostasis support on the basis of treatment with tripterygium wilfordii polyglycoside tablets.The CD4+/CD8+ ratio and CD4+ CD25+ Treg expression were compared between the two groups.Results The CD4+/CD8+ ratio,CD4+ CD25+ Treg and platelet count in the control group before treatmentwere (0.96 ± 0.36),(1.21 ± 0.67) %,(13.14 ± 6.92) × 109/L,respectively,which of the observation group were (0.92 ± 0.37),(1.19 ± 0.59) %,(11.51 ± 6.21) × 109/L,respectively,there were no statistically significant differences between the two groups (all P > 0.05).The CD4+/CD8+ ratios in peripheral blood of the observation group at 2 weeks,3 weeks and 4 weeks after treatmentwere (1.04 ±0.56),(1.55 ±0.34),(1.59 ±0.41),respectively,there were statistically significant differences between the two groups (t =9.994,9.797,all P < 0.05).The CD4+ CD25+ Treg proportions in the observation group at 2 weeks,3 weeks and 4 weeks after treatmentwere (1.01 ± 0.61) %,(1.06:±:0.57) %,(5.92 ± 0.65) %,respectively,there was statistically significant difference between the 4 weeks after treatment and before treatment(t =5.378,P < 0.05).The CD4+/CD8+ ratios in the peripheral blood of the control group were (1.01 ±0.60),(0.89 ±0.50) and (0.96 ±0.51),respectively,and the CD4+ CD25+ Treg in control group at 2 weeks,3 weeks and 4 weeks after treatment proportions were (0.99 ±0.72)%,(1.15 ±0.66)%,(1.22 ±0.56)%,respectively,there were no statistically significant differences between before and after treatment (all P >0.05).There were statistically significant differences in the CD4+/CD8+ ratio and CD4+ CD25+ Treg expression between the observation group and control group at 4 weeks after treatment (t =8.589,P < 0.01;t =2.369,P < 0.05).There was no statistically significant difference in the platelet count between the two groups(P > 0.05),but the symptoms of bleeding of the observation group was lighter at 3 weeks after treatment.Conclusion Tripterygium wilfordii polyglycoside improves the expression of CD4+/CD8+ and CD4+ CD25+ Treg in peripheral blood of elderly patients with relapsed or refractory ITP.It is an ideal drug for the treatment of relapsed and refractory ITP in the elderly,it is worth further study.

ABSTRACT

Objective:To explore the effect and molecular mechanism ofSPHK1 in the invasion and metastasis process of non-small-cell lung cancer cells(A549).Methods:Recombinant retrovirus was used to mediate the production ofA549/vector,A549/SPHK1,A549/scramble, andA549/SPHK1/RNAi that stably expressed or silencedSPHK1.The invasion and migration capacities of A549 cells overexpressing or silencingSPHK1 were determined usingTranswell invasion assay and scratch wound repair experiment.The protein and mRNA expression levels ofE-cadherin, fibronectin, vimentin inA549/vector,A549/SPHK1,A549/scramble,A549/SPHK1/RNAi were detected withWestern blot(WB) and quantitativePCR(QPCR) methods, respectively.Results:Transwell invasion assay and scratch wound repair experiments showed that over-expression of SPHK1 obviously enhanced the invasion and migration capacities ofA549 cells.WB andQPCR detection results showed that, the expression ofE-cadherin(a molecular marker of epithelial cells) and fibronectin, vimentin(molecular markers of mesenchymal cells) inA549 cells was upregulated after overexpression ofSPHK1; whileSPHK1 silencing significantly reduced the invasion and metastasis capacities ofA549cells, upregulated the expression of molecular marker of epithelial cells, and downregulated the expression of molecular marker of mesenchymal cells. Conclusions:SPHK1 promotes epithelial mesenchymal transition of non-small-cell lung cancer cells and affects the invasion and metastasis capacities of these cells.

ABSTRACT

Objective To observe the clinical curative effect of ankle chute tibia bone graft fusion treatment of late kaschin-beck disease.Methods Thirty-three patients with late kaschin-beek disease performed ankle arthrodesis,the ankle surgery in 5 cases,simple right ankle surgery in 13 eases and 15 cases of left ankle surgery.Intraoperative used tibial chute hollow screw internal fixation and bone graft plaster external fixation postoperatively.Followed up 0.5-6.0 years,compared the American orthopaedic food and ankle society (AOFAS) score before surgery to determine the efficacy of last follow-up,X ray film fusion situation assessment in the postoperative period.Results Thirty-three cases did not occur postoperative incision infection,vascular or nerve complications.Last follow-up,30 cases were no obvious discomfort,3 cases of mild ankle pain,accompanied by walking or standing for a long time after the foot heel had mild acid bilges feeling,did not affect the daily life.The AOFAS score before surgery was significantly lower than that last follow-up [(25.84 ± 16.81) scores vs.(87.32 ± 4.68) scores] (P < 0.01).Clinical efficacy excellent in 25 cases,good in 5 cases,general in 2 cases,poor in 1 case.Follow-up X line piece was showed that in all cases to achieve bony fusion.Conclusion Using sliding kashin-beck terminal patients treated ankle bone graft fusion,simple operation,high bone graft fusion rate,less complications,curative effect is positive.

ABSTRACT

Objective To compare the clinical outcomes of using the Hukou artery and ulnar proper digital artery of index finger translocation to reconstruct artery supply in replanting of rotated and avulsed thumb.Methods From January 2011 to May 2012,replantation surgery were carried out in 16 patients with rotated and avulsed injury of thumb.In group A,all cases were replanted by Hukou artery translocation,in which 6 males and 2 females and (27.0 ±9.5) years old in average.The causes of injury were machine strangulation (7 patients) and traffic accident (1 patient).The average time from injury to operation were(6.1 ± 3.1) hours.The amputed level were at the base of proximal phalanx (5 patients) and metacarpophalangeal joint (3 patients).In group B,all patients were replanted using ulnar proper digital artery of index finger translocation,in which 5 males and 3females and (29.7 ± 13.7) years old in average.The causes of injury were machine strangulation (6 patients) and cord wring injury (2 patient).The average time from injury to operation were(7.1 ±2.2) hours.The amputed level were at the base of proximal phalanx (4 patients) and metacarpophalangeal joint (4 patients).Results The operation time were (3.2 ± 0.8) minutes in group A and (5.3 ± 0.6) minutes in group B.All fingers were survived in both groups.All patients were followed up 3-10 months (6.9 months on average).Excellent rate reached 75.0% in group A and 75.0% in group B according to Criterion on Functional evaluation on Finger Reconstruction issued by Chinese society of hand surgery.There were significant differences between the two groups in operation time (P < 0.05),and without differences in excellent rate (P > 0.05).Conclusion There are same outcome by using the Hukou artery and ulnar proper digital artery of index finger to reconstruct artery supply in replantation of rotated and avulsed thumb.However,using the Hukou artery translocation were more simple and minimally invasive.

ABSTRACT

Objective To explore the inflnence of autologous peripheral blood hematopoietic stem cell transplantation (auto-HSCT) for lymphoma process on the quality of patients blood coagulation.Methods Plasma samples were collected before the conditioning and on day 0,7,14,28,35 follow auto-HSCT from 20 patients.The following parameters were measured:prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen (FIB) and D-dimer(D-Di).Results Compared with the values before conditioning,a significant rise in the FIB values was detected on day 7 and day 14 after auto-HSCT,no essentially change in the FIB values was detected on day 21 and day 28 and day 35 after auto-HSCT.Other coagulation parameters investigated(PT.APTT.D-Di) remained essentially unchanged.All of the patients not concurrent thrombotic disease.Conclusion Lymphoma patients with autologous peripheral blood hematopoietic stem cell transplantation appear abnormality in coagulant function.Clinical doctors should concern.

ABSTRACT

ObjectiveTo investigate the technique and clinical results of curved osteotomy under the metatarsal head's cartilage for improving reconstructed metacarpophalangeal joint function of fingers with second toe transplantation. MethodsThere were total 21 cases with 21 digits. During second toe transplantation with the metatarsophalangeal joint, the bottom of the second metatarsal head was incised. Then a curved osteotomy were carried out on about 5.0 mm under the metatarsal head's cartilage until the passive range of motion could be 90°.Longitudinal or cross-Kirschner wires were used to fix the joint.Finally,the conventional methods were used to reconstruct the blood supply, movement and nerves. ResultsAll 21 digits of the 21 cases survived uneventfully.The follow-up time was ranged from 6 to 24 months.Their average passive range of motion of the reconstructed metacarpophalangeal joint was 75°,ranging from 65° to 85°.The average active range of motion was 65°, ranging from 45° to 80°. Postoperative X-ray revealed fracture healed well without joint degeneration. ConclusionCurved osteotomy under the metatarsal head's cartilage is an effective way to improve active and passive activities function of the reconstructed metacarpophalangeal joint with second toe transplantation.

ABSTRACT

Objective To investigate the replantation methods and outcomes of mini tissue mass of amputated finger. Methods Twenty-six fingers of 20 patients were replanted with multiple vessel anastomosis methods to restore blood supply.The methods include vascular anastomosis,vascular bridge,arteriovenolization,veno-arteriolization,et al. Results Twenty-five replanted tissues were survived completely.Partly necrosis occur in 1 case,and rehabilitation by change dressings.The patients were follow up from 6 to 12 months.The contour and function of the replantation fingers recovered satisfactory. Conclusion By using the rational anastomosis according to the traumatic condition,it could be obtain good outcome of mini tissue mass replantation

ABSTRACT

Objective To summarize and investigate the therapeutic effects of reconstruction of contracture of the first web space with snuff-box flap. Methods Eighteen patients with contracture of the first web space were treated by snuff-box flap. The width and the angle of the first web space was 19 mm and 20°on average. According to the first web space skin defects, the flap designed to nasopharyngeal fossa centers as rotation point, the radial line when the forearm in the neutral position as the axis. Results The followedup for 5-26 months revealed that 17 flaps had a success, but 1 case was partly necrosis. The width of the first web space was augmented by an aveage of 45 mm, the angle of the first web space was augmented by an average of 50°. Conclusion It is ralatively simple and reliable to repair the contracture of the first web space using the snuff-box flap.

ABSTRACT

Objective To introduce the improvement of the flap from the dorsal site of the index in the application of treatment in the thumb web contracture. Methods Apply improved first dorsal metacarpal flap for 111 cases of patients of the thumb web contracture releasing. The original island or tongue-shaped flaps was modified to be the local flap transfer to the thumb web. Surgical method simplified. Before thumb web contracture was released, the flap was taken from ulnar aspect to radial aspect on the dorsum of the index. The flap was transferred to cover the soft tissue defect on the first web space following the contracture releasing. Flap donor area was closed by split-thickness skin graft. Results One hundred and eleven cases of flaps survived. After the 3-18 months (mean follow-up of 8.5 months), the local appearance, thumb function recovered well for 94.6 percent. Conclusion Improved first dorsal metacarpal flap is effective methods in thumb web contracture releasing.

ABSTRACT

Objective To evaluate the safety and efficacy of the human acellular nerve allograft (hANG)for nerve repair in the clinical setting,and report the early outcomes of bridging digital nerve defect with the hANG. Methods Four patients with 5 digital nerve injuries were included in this pilot study.The nerves defect ranged from 10-20 mm and were bridged with the hANG(manufactured by Zhongda Medical Equipment Co.,Ltd,Guangzhou,China).Four digital nerve acute injuries in 3 patients were repaired with hANG primarily,while the nerve in another patient was reconstructed secondarily.The procedure was performed under a 10-manifying operating microscope.The nerve stumps were debrided until the normal fascicles could be seen.hANG was inserted between the proximal and distal stumps and end-to-end neurorrhaphy was performed with 9-0 sutures.Postoperative cares included dressing change and administration of antibiotics.No immunosuppressants had been used.The follow-up time ranged from 1 to 3 months.The wound and blood sample were examined for the safety of hANG.The nerve function Wag evaluated according to the scoring system proposed by the Nerve Injuries Committee of the British Medical Research Council. Results All wounds healed primarily.The adverse effects,such as rejection,allergy,infection,and toxicity to the liver and kidney were absent.The results of blood biochemistry test were within the normal range.The injured nerve achieved good functional recovery.In 2 cages,the 2 point discrimination(2PD)was 8mm(S3~+,excellent). Conclusion Based on the short term follow-up,using hANG to repair digital nerve defect as long as 20mm was safe,and the nerve functional recovery is pretty good.

@#Objective To explore the concept,denomination and technique of miniature tissue transplantation in hand clinic. Methods In 537 cases,local skin or composite tissue defect in 756 thumbs or fingers were repaired with 4 kinds of miniature island flap and 10 kinds of miniature vascularized composite tissue.Some of them were created in clinic practice.The precess,main points and indication of the operation were expounded and compared. Results In 639 fingers covered with miniature island flap,the transferred flaps were surviverd completely in 621 fingers(97.2%),survivered partly in 11 fingers(1.7%)and necrosised in 7 fingers(1%).In 117 fingers repaired with miniature vascularized composite tissue,all the transplanted tissues were survivered.In all the injuried fingers,beautiful outlook were recovered and no fat and clumsy were found,two-point difference was 6-8 mm,effctive sensibility and good function were restored,and all the patients were satisfied. Conclusion Miniature tissue transplantation were good methods to cover the local defect in fingers or thumbs,worth of generalization and aplication in hand clinic,and have an important significance for improve the trauma repair quality and treatment level in hand surgery.

ABSTRACT

Objective To explore small size toe tissue flap for aesthetic reconstruction of the thumb and / or finger. Methods Six kinds of small size toe tissue transplants had been applied in repairing skin-bone-joint composite tissue defects of the thumb or finger in 74 cases. Results Among 83reconstructed flaps of the 74 patients, 81 flaps survived completely. Follow-up examination made three to forty-eight months postoperatively showed that the outward appearance were excellent in most cases. The function of the thumbs or fingers were good. The donor feet can walk normally with no pain. Conclusion A variable combinations of toe tissues including skin, soft tissue, bone and joint can be harvested to form a lot of small size transplants for refined aesthetic reconstruction of thumb and finger. The functional and aesthetic results are good and the treatment course is shortened.

ABSTRACT

@#Objective To introduce the concept of long length finger reconstruction and our corresponding three operative methods. Methods In a series of 10 finger defect cases with one of their long finger amputated at or proximal to proximal phalanx, long finger reconstruction were accomplished with one of the three methods. First method: For emergency patients whose proximal finger segment were demolished, the donor second toe was transplanted intercalatedly with microsurgical technique between the original proximal finger stump and the saved distal finger segment. Second method: Bilateral second toes were harvested and connected together to form a long transplant in order to reconstruct a normal length finger. Third method: From one foot, the donor second toe is harvested with its dorsal and plantar skin flap. From the other foot, the second toe is harvested with its metatarsophalangeal joint and skin flaps from neighbouring sides of great and third toes. The skin covering will be perfect. During transplantation of the proximal transplant, the MPJ should be fixed at 90°plantar rotation position for better flexion. Results Uneventful survival of reconstructed fingers were obtained in all ten cases. Postoperative functional evaluation of the patients with standard set by Chinese Society of Hand Surgery showed to be excellent in 1 case, good in 5 cases and fair in 4 cases. The overall excellent/good rate was 60%. Conclusion By application of these three reconstruction methods, the challenging problem of long length finger can be solved to reasonable extent.

ABSTRACT

Objective To expore the method of clinical application and the final result of pedicled flap supplied by radial osteo cutaneous branch of anterior interosseous artery Methods Basing on the anatomic basis that the osteo cutaneous and myocutaneous branches of the anterior interosseous artery have constant collateral anastomoses which can give forth a large size dorso radial flap on dorsum of the forearm Four cases of skin defect of first web had been reconstructed by this flap Results All 4 cases were successful with good functional and cosmetic results Conclusion Reconstruction with dorso radial flap of forearm is simple,reliable procedure which bring minimum trauma to the patient and is an ideal alternative operation
