ABSTRACT

Jianwei Xiaoshi tablets, as a common variety of Chinese patent medicine with "one product with many manufacturers", have many manufacturers and huge market sales. However, the phenomenon about uneven quality and discrepant price is prominent. Based on this, this study was carried out for the quality evaluation of Jianwei Xiaoshi tablets by applying the high-quality evaluation criteria with the quality as core for Chinese patent medicine, which was based on the full production cycle, from the multi-dimension including raw material selection, production process, quality control, post-marketing research and so on. The evaluation results showed that the quality evaluation scores of Jianwei Xiaoshi tablets from different manufacturers varied greatly (ranging from 35 to 66), indicating that the quality was significantly different. In the actual production, generally inadequate attention was paid to the quality of raw materials, and the quality of raw materials was insufficient with the score ratio of 43%, especially the poor consistency control of them. The role of good manufacturing practice was obvious, and the scores of production process were generally high with the average score ratio of 62%, and the maximum up to 80%. The technological advancement of the manufacturer was outstanding. The score ratio of quality control was only 31% that the internal quality standard of each manufacture almost stayed at the qualified line, which was equal to the national standard, and the consistency of products was insufficient. The post-marketing research was lacking with the score ratio of 37%. Manufacturers with high brand awareness and market share were upper scores, while the others lagged far behind. The results of this evaluation are in line with the overall prediction, which can provide a reference for the high-quality evaluation of Chinese patent medicine, and supply the scientific data for high-quality and high-price application.

ABSTRACT

Objective To compare the clinical effects and safety between laparoscope radical surgery after neoadjuvant chemoradiotherapy and traditional surgery alone for treatment of rectal cancer patients. Methods A total of 122 patients with rectal cancer from January 2013 to January 2015 in Qingdao Tumor Hospital were divided into the observation group (61 cases) and the control group (61 cases) according to the different therapies. The patients in the observation group received laparoscope radical surgery after neoadjuvant chemoradiotherapy, and the patients in the control group received laparoscope radical surgery of rectal carcinoma. The operative, pathological, immunological indicators and the complications were analyzed retrospectively. Results There were no statistical differences in conversion to open laparotomy rate [6.6 % (4/61) vs. 4.9 % (3/61)], anus preservation rate [80.3 % (49/61) vs. 67.2 % (41/61)], radical rate [77.0 % (47/61) vs. 85.2 % (52/61)] in the observation group and the control group (χ2: 0.152, 2.711, 1.339 respectively, all P > 0.05). The proportion of terminal ileum in the observation group was significantly higher than that in the control group [60.3 % (38/61) vs. 21.3 % (13/61), χ 2= 21.058, P < 0.05]; the number of cleaned lymph nodes in the observation group was significantly lower than that in the control group (8±5 vs. 15±7, t= 5.834, P < 0.05). There were no significant differences between before the treatment and 7 d after surgery of the two groups in the levels of T cells, CD4+, CD8+, CD4+/CD8+and natural kill cells (P > 0.05). There were no significant differences between the observation group(24.6 %)and the control group(27.9 %)in postoperative complication rates (P> 0.05). Conclusion Laparoscope radical surgery after neoadjuvant chemoradiotherapy can reduce the number of lymph nodes dissection with low incidence rate of postoperative complications and implicit impacts on immune system, which is safe and worthy of wide application.

ABSTRACT

This study was aimed to investigate the efficacy ofTong-Bu San-Sheng(TBSS) Decoction to reduce the toxicity and side effects of chemotherapy, as well as prolong progression-free survival (PFS) for advanced lung squamous carcinoma patients who received chemotherapy. A total of 83 lung squamous carcinoma cases were divided into two groups by patients’ wishes. The control group contained 41 cases were treated by the chemotherapy of gemcitabine plus cisplatin (GP). The trial group contained 42 cases were treated by chemotherapy plus Chinese herbal medicine TBSS decoction. The toxicity and side effects of chemotherapy, as well as short-term outcome were evaluated. PFS of patient was recorded. The results showed that there were no differences on granulocytopenia (P = 0.115) or short-term outcome (P = 0.081) for patients of both groups after chemotherapy. The percentages of nausea, vomiting and thrombocytopenia in the trial group were lower than that in the control group (P = 0.037,P = 0.040). The PFS of patients in the trail group were prolonged compared to patients in the control group (4.31 ± 0.24 VS 3.78 ± 0.16 month;P = 0.043). It was concluded that Chinese herbal medicine TBSS decoction cannot reduce granulocytopenia caused by chemotherapy, or improve the tumor response rate (RR) of short-term outcome. However, it can prolong PFS, relieve nausea, vomiting and thrombocytopenia during chemotherapy.

ABSTRACT

BACKGROUND: Vertigo caused by vertebral artery stenosis accounts for a corresponding large percent in vertigo patients. There is not a unified criterion in diagnostic method, which affects accuracy of diagnosis. Application of simply check such as neck rolling-flexion and head hypsokinesis test can give first diagnosis to patients with vertebral artery stenosis in clinic. Then examination of transcranial Doppler(TCD) and nuclear magnetic resonance angiography (MRA) can be given to increase accuracy of clinical diagnosis.OBJECTIVE: To appraise diagnostic value of vertebral artery stenosis by neck rolling-flexion test, TCD and MRA examination.DESIGN: Cases analysis based on patients.SETTING: Department of Neurology, Affiliated Hospital of Inner Mongolia Medical College.PARTICIPANTS: Totally 41 patients with diagnosis of vertigo were chosen in Clinic of Neurological Department in Affiliated Hospital of Inter Mongolia Medical College from February to April 2004. The cases with vertigo that were not caused by vertebral artery stenosis were excluded.METHODS: Totally 41 patients with vertigo whose symptoms were induced by neck rolling-flexion and head hypsokinesis test were given later examination of TCD and MRA to vertebral arteries and basilar artery.MAIN OUTCOME MEASURES: Three checking results of 41 patients.RESULTS: Totally 41 patients with vertigo whose symptoms were induced by neck rolling-flexion and head hypsokinesis test were chosen. Vertigo occurred after left-right neck rolling in 32 cases, which accounted for 78% in detectable rate, and vertigo occurred after neck flexion and head hypsokinesis in 25 cases, which accounted for 61% in detectable rate. Thirty cases were found dissymmetry of blood flow in vertebral arteries on two sides and diameter differences of vertebral arteries on two sides between 0.5-2.0 mm by examination of TCD, which accounted for 73% in detection rate. Thity-nine cases were found dissymmetry in inner diameter of vertebral arteries and the differences in diameter were between 0. 5-2.6 mm, which accounted for 95% in detection rate. The other two cases were detected vertebral artery stenosis.CONCLUSION: Neck rolling-flexion and head hypsokinesis test can induce vertigo symptom, which helps to give final diagnosis whether it is caused by vertebral artery stenosis. TCD examination has a corresponding higher accuracy in diagnosis of vertebral artery stenosis. MRA examination has the highest accuracy in diagnosis of vertebral artery stenosis.

ABSTRACT

Anopheles minimus was once a main malaria vector in Hainan Island and had been e-liminated basically through the campaign of indoor residual spraying launched in 1959. It again became an incriminated vector of some focal malaria outbreaks in recent years. The present study was conducted in a selected county-Danxian and a typical hilly area-Feibar in the west part of Danxian county in 1989-1990.An. minimus was found in 50% and 62. 5 % of the surveyed sites at mountainous and hilly area of Danxian county, but not found in coastal region. An. minimus was found in all 18 sites surveyed in Feibar district constituting 52% of anopheline composition. Man-biting rate made by human-baited collection was 3. 2 before midniaght and 38. 2 when collected through whole night in some sites. However, the behaviour characteristics of An. minimus has changed. It has become exophilic, exophagic, and has an equal preference for man and cattle. The vectorial capacity of An. minimus estimated by quantitative data was in accord with malaria infection rate in Feibar district ,and the malaria infection rate among the inhabitants in three types of residential quarter with different socioeconomic conditions. Malaria infection rates of residential quarter of land-reclamation outcomers, villagers and state farm residents were 10%,2. 9% and 0. 5% respectively during 40 days from July to August,1990.Owing to the fact that An. minimus has become a secondary vector only next to An. dirus, with a wide range of distribution and a considerable different characteristics in behaviour compared to that before spraying campaign , it is suggested that a malaria control programme must be seriously planned to adjust the new problem of malaria epidemiology in Hainan Province.
