ABSTRACT

Objective:To investigate the clinical effect of facial depression correction by using free anterolateral thigh adipofascial flap and human acellular dermal matrix. Methods:Nineteen facial depression patients (3 males and 16 females), age ranges from 16 to 56 years (average age: 29.1±10.37 years), caused by different reasons were selected in this research from Jan. 2008 to Dec. 2020. In the stage I operation, anterolateral thigh adipofascial flap was designed according to facial depression area and harvested to fill in the depression with vascular anastomosis; In the stage II operation, the human acellular dermal matrix was used to fill in the remained depression edge according to facial subunit. Results:All free anterolateral thigh adipofascial flap survived well. All patients revealed fascial subunit tissue atrophy of different degree in infraorbital, nasolabial sulcus and temporal areas which were repaired by using cellular dermal matrix and obtained satisfying effect during 1 to 8 years of follow-up with no obvious rejection. 18 of 19 patients showed flap hypertrophy and corrected by liposuction. All patients recovered well after 1 to 8 years of follow-up.Conclusions:Humana cellular dermal matrix could make up the limitations of treating fascial depression deformity when using anterolateral thigh adipofascial flap. The combination method of above approaches could obtain satisfying clinical effect, which is worthy of clinical promotion.

ABSTRACT

Objective:To evaluate the clinical effect of autologous nanofat combined with pearl fat transplantation in comprehensive improvement of lacrimal groove depression.Methods:Seventy-eight patients (age ranges from 28 to 56 years, with average 38 years) who desired for lacrimal groove improvement were involved in this study from Jan. 2019 to Jun. 2020 in the Department of Plastic Surgery in Peking Union Medical College Hospital. Primary fat tissue was obtained and purified by liposuction. Nanofat and pearl fat were prepared and injected into lacrimal groove area in different layers and multiple points evenly to ameliorate depression. Visual analogue scoring (VAS) was used for evaluating injection pain. Dark eye circles, faint lines on lower eyelid, color spots and lacrimal groove depression between pre-operation pictures and post-operation pictures of 1 year follow-up were evaluated by patients' satisfaction scores. Results:All 78 patients revealed mild swelling and disappeared in 3 to 4 days. Injection areas were stable in 3 months. All patients appeared no complications such as infection, hematoma, fat liquefaction, local induration and so on. Among 78 patients, 2 patients showed ecchymosis after surgery which disappeared in 10 days, and 1 patient showed uneven appearance which disappeared after timely treatment. After 1 year of follow-up, the average satisfaction score of improvement was 8.9±0.5, which showed satisfied post-operative effect. Conclusions:Autologous nanofat combined with pearl fat transplantation has high feasibility, short operation time, which could achieve good effect of facial rejuvenation with high patients' satisfaction. In this case, this technique is worthy of clinical promotion.

ABSTRACT

Objective To explore the distribution and drug resistance of pathogens isolated from patients with cholesteatoma otitis media so as to provide guidance for clinical use of antibiotics. Methods This survey analyzed the spectrum of organisms causing cholesteatoma otitis media and their sensitivities to commonly antimicrobial agents from Hebei province eye hospital in 2014.Results There was 86 positive speciments were cultured from 89 samples, the positive rate was 96.6%.A total of 90 strains of pathogens have been isolated, including 52 strains of gram-positive coccus (57.8%),35 strains of gram-negative bacilli (38.9%),3 strains of gram-positive bacilli (3.3 %),and 0 strain of fungi.Staphylococcus epidermidis,Staphylococcus aureus and Staphylococcus chromogenes ranked the top three species of pathogens, accounting for 20.0%,16.7%,and 12.2%,respectively.The gram-positive cocci were susceptible to vancomycin, rifampicin and amikacin, and showed higher drug-resistancerate to penicillin, amoxicillin and azithromycin. When applied to gram-negative bacilli, the drugs with best resistance were penicillin and cefazolin, and the drugs with the highest sensitivity were levofloxacin and pipercillin/ sulbactam. Conclusion Staphylococcus is the predominant pathogens of cholesteatoma otitis media in hospital, and the bacteria have a high antibiotic resistance.Enhanced monitoring on pathogenic bacteria distribution and drug resistance analysis of cholesteatoma otitis media could benefitthe guide of clinical rational use of antimicrobial agents.

ABSTRACT

OBJECTIVE:To study the distribution law of Valsartan capsules in the ocular tissue of rabbit eyes after intragastri-cal administration. METHODS:3 rabbits(6 eyes)were intragastrically administrated Valsartan solution,7.5 mg every time, twice a day,for 6 d. Rabbits were executed after last administration, and the cornea, iris, aqueous humor, lens, vitreous and retina tis-sues were collected. HPLC was used to determine the content of valsartan in each tissue. RESULTS:Content of valsartan in retina, iris, cornea, aqueous humor, vitreous and lens were (4.274 ± 1.75),(2.233 ± 0.72),(0.871 ± 0.66),(0.713 ± 0.49),(0.177 ± 0.07),(0.083 ± 0.06)μg/g,respectively. CONCLUSIONS:After multiple administrations, valsartan distributes unevenly in each tissue of eyes, showing highest distribution content in retina, followed by iris, and lowest content in lens and vitreous.

ABSTRACT

Objective To study intraoperative neural electrophysiological monitoring applied in lumbosacral spinal cord tumor resection. Methods We retrospectively reviewed the clinical data of 212 patients undergoing lumbosacral spinal cord tumor resection with or without intraoperative neural electrophysiological monitoring in our hospital. The patients were divided into two groups:124 patients in the monitored group received intraoperative neural electrophysiological monitoring while 88 ones in the control group did not.The monitoring was performed by recording the cortical somatosensory evoked potential (CSEP),dermatomal somatosensory evoked potential (DSEP) and electromyography (EMG).The patients were followed up for 3-6 months and their postoperative outcome was analyzed. Results There were significant differences in the outcome (P <0.05),but no difference was found in the incidence of complications between the monitored group and the control group. The sensitivity of CSEP +DSEP+EMG was 100%,and the specificity was 55.9% in the former group. Conclusion Combined monitoring with CSEP,DSEP and EMG during lumbosacral spinal cord tumor resection is valuable in protecting the spinal nerve roots and ensuring better operation safety.

ABSTRACT

Objective To investigate the therapeautic effects of ulinastatin on ulcerative colitis(UC) and it's possible mechanism. Methods Forty adult Wistar rats were randomly divided into four groups;normal group(group A),UC group(group B),SASP(salicylazosulfapyridine) therapeautic therapeutic group(group C),ulinastatin(UTI) therapeautic group(group D). UC models were induced by trinitrophenylmethylnitramine(TNBS), Group A and Group B received 0 9% saline solution instead of ulinastatin and SASP as a control.Serum level of tumor necrotic factor-?(TNF-?),malondialdehyde(MDA),superoxide dismutase(SOD) and pathological change were measured respectively after drugs treatment for two weeks. Results Both UTI and SASP decreased the level of serum TNF-?,MDA ,increased the level of serum SOD ,and relieved mucosa lesion and pathological change in UC rats . UTI was better than SASP in the treatment of UC.Conclusions UTI has apparent therapeautic effects. The mechanism may be associated with inhibition of pro-inflammatory cytokines and clearance of free radicals.
