ABSTRACT

Objective@#To investigate the diagnosis and management of laryngeal cleft.@*Method@#The clinical data of 13 cases of laryngeal cleft treated between 2007 and 2015 was analyzed retrospectively.@*Results@#The children with laryngeal cleft were classified according to the classification of Benjamin-Inglis, as type Ⅰ(11 cases), typeⅡ(1 case) and type Ⅲ(1 case). All patients were confirmed by microlaryngobronchoscopy under general anaesthetic. Eleven typeⅠ and 1 type Ⅱ clefts were managed conservatively, with which all type Ⅰ patients were successfully managed, while the type Ⅱ patient was resolved by surgical endoscopy. The type Ⅲ patient was treated by open repair but the results was poor.@*Conclusions@#Patients who suffered with choking on feeding or recurrent aspiration pneumonia, especially coexisted with other congenital malformation, needed detailed evaluation for laryngeal cleft, although which was a rare congenital abnormality. Electronic laryngoscope could be the first step to screen the cleft, while microlaryngobronchoscopy is the gold standard for diagnosis of laryngeal cleft. The majority of children with lower type clefts can be managed conservatively. Surgical endoscopy has high success rate when strictly following the indication. Type Ⅲ and Ⅳ clefts have high mortality for usually combining with severe complications and abnormalities.

ABSTRACT

OBJECTIVE To understand the relationship between the detection rate of Pseudomonas aeruginosa in Children′s Hospital wards from Jan 2008 to Sep 2008 and disinfection and isolation in the department and investigate the change in antimierobial resistance of P.aeruginosa to provide basis for reasonable use of antibiotics in clinical practice.METHODS The clinically isolated P.aeruginosa strains were collected,cultured and identified by paper diffusing method.The results were evaluated according to the relevant documents of NCCLS of USA.RESULTS The resistant rates of P.aeruginosa to Ampicillin,Ampicillin/Sulbactam,ceftriaxone,cefazolin and SMI were higher than 98%.Their resistant rate to Levofloxacin and IMP was the lowest(about 2% or so).CONCLUSIONS Effective disinfection and isolation of P.aeruginosa should be performed.Selection of antimicrobial drugs should be according to the results of drug susceptibility,reduce the rate of bacterial resistance.

ABSTRACT

OBJECTIVE To summarize the clinical experience of endoscopic thyroidectomy via suprasternal approach. METHODS Endoscopic thyroidectomy via suprasternal approach was performed in 35 patients with ultrasonic scalpel. RESULTS Operations were successfully performed in 35 patients. The mean operation times were 130 (105~190) minutes in 24 cases with subtotal lobectomy and 4 case with total lobectomy, 60 (50~70) minutes in 2 cases with isthmectomy, 228 (185~270) minutes in 2 case with bilateral subtotal lobectomy, 163 (140~215) minutes in 3 case with subtotal lobectomy and the contralateral ademona resection. The bleeding during operation was 5 to 40ml and the average hospital stay time was 4 (3~5) days. CONCLUSION Endoscopic thyroidectomy via suprasternal approach is a safe way with good cosmetic value.
