ABSTRACT

To investigate the clinical characteristics of metastatic hormone sensitive prostate cancer and explore the strategy of combination of new endocrine drugs. In April 2019, an 69-year-old man was admitted to the First People’s hospital of Changzhou with "gross hematuria" . Physical examination showed prostatic hyperplasia with an unsmooth hard surface. CT showed a mass in bladder and possible metastasis in right lung. Diagnostic TUR-Bt pathology showed prostatic acinar adenocarcinoma, and PET-CT showed malignant lesion of prostate with bladder invasion, multiple pelvic lymph node metastasis and lung metastasis. The diagnosis of mHSPC with lymphatic and lung metastasis was considered. The patient was treated with bicalutamide and then switched to goserelin plus acetate abiraterone with prednisone. Total prostate specific antigen (tPSA) decreased to 0.705 ng/ml after 1 month of ADT+ AAP treatment, and decreased to 0.007 ng/ml after 4 months, and then maintained at 0.003 ng/ml until January 2021. Serum testosterone decreased to 0ng/dl and maintained the whole follow-up period. After 3 months of treatment, the pulmonary metastasis was not obvious. Till the last follow-up at January 2021, the patient reported good quality of life with no serious adverse events. The efficacy of ADT combined with acetate abiraterone in the treatment of mHSPC with lung cancer was significant.

ABSTRACT

Objective:To investigate the effects of artesunate (ART) on epithelial-mesenchymal transformation (EMT) of colorectal cancer HCT-8 cells, and explore the effects of ART on cell migration, invasion,EMT ability, and protein kinase B (Akt)/Snail signaling pathway of colorectal cancer. Method:3-(4-5-Dimethylthiazol-2-yl)2,5-diphenyltetrazolium bromide (MTT) assay was used to detect the effects of ART at different concentrations on the proliferation of HCT-8 cells. Wound healing assay and Transwell assay were used respectively to detect the effects of ART on migration and invasion of colorectal cancer cells. The effects of different concentrations of ART on the distribution of EMT-related proteins vimentin and E-cadherin in HCT-8 cells were detected by double-immunofluorescent staining. The effects of ART on protein expression levels of EMT markers E-cadherin,vimentin and N-cadherin in HCT-8 cells and the expression of Akt1, p-Akt1, and Snail1 in the Akt/Snail signaling pathway were determined by Western blot. Result:The dose-dependent inhibitory effects of ART on the proliferation of HCT-8 cells were determined and the inhibition rate was calculated. A dose-response curve was plotted accordingly. The half-maximal inhibitory concentration (IC50) of ART on HCT-8 cells was (16.67±1.95) μmol·L-1. The following four groups were set up: a control group (0 μmol·L-1),and low-, medium-, and high-dose ART groups(2, 10, 50 μmol·L-1). Compared with the results in the control group,ART inhibited the migration and invasion of HCT-8 cells(P<0.05). Specifically, the expression of E-cadherin in HCT-8 cells was significantly up-regulated,and that of vimentin and N-cadherin was significantly down-regulated (P<0.05). The expression levels of p-Akt1 and Snail1 were significantly decreased after ART treatment, thus inhibiting EMT(P<0.05). Conclusion:The findings of this study suggested that ART inhibited the EMT-triggered migration and invasion of HCT-8 cells presumedly by inhibiting the activation of the Akt/Snail pathway to reverse EMT.

ABSTRACT

Objective To investigate the causal association between hip circumference (HC) and type 2 diabetes mellitus (T2DM) based on Mendelian randomization. Methods The genetic variants data of the HC and T2DM from the Genetic Investigation of Anthropometric Traits (GIANT) and DIAbetes Genetics Replication And Meta-analysis (DIAGRAM) database were matched according to the single nucleotide polymorphism (SNP) rsID. Genetic loci strongly related to the HC were used as instrumental variables; and the inverse-variance weighting, MR-Egger regression model and weighting median method were carried out to analyze the causal effect of HC on T2DM. Results Fifty-two, nine and fifteen SNPs were matched in the total cohort, female cohort and male cohort, respectively. Heterogeneity test suggested the SNPs were homogeneous. We found HC to be positively associated with T2DM risk (OR=1.065, 95% CI: 1.030-1.100, OR=1.103, 95% CI: 1.057-1.150 and OR=1.583, 95% CI: 1.273-1.968, respectively) in above three cohorts, respectively. Sensitivity analysis showed the results were robust. Conclusions There is a relationship between HC and T2DM of people, and HC may be the risk factor of T2DM.

ABSTRACT

The aim of the present study was to establish a cell model of volume-regulated anion channel subunit LRRC8A and investigate the physiological characteristics of LRRC8A. The eukaryotic expression vectors of LRRC8A and YFP-H148Q/I152L were constructed and transfected into Fischer rat thyroid (FRT) cells by Lipofectamine 2000. The FRT cell lines co-expressing LRRC8A and YFP-H148Q/I152L were obtained by antibiotic screening. The expression of LRRC8A and YFP-H148Q/I152L in FRT cells was detected by the inverted fluorescence microscope. The fluorescence quenching kinetic experiment was done to verify the function and effectiveness of the cell model. Then the cell model was utilized to study the physiological characteristics of LRRC8A, such as the characteristics of anion transport, the opening of LRRC8A by osmotic pressure, the effect of anion transport velocity, and the effect of chloride channel inhibitors on LRRC8A anion channel. The results of the inverted fluorescence microscope showed that LRRC8A was expressed on the cell membrane and YFP-H148Q/I152L was expressed in the cytoplasm. The results of fluorescence quenching kinetic test showed that under the condition of low osmotic state, LRRC8A could transport some kinds of anions, such as iodine and chloride ions. Osmotic pressure played a key role in the regulation of LRRC8A volume-regulated anion channel opening. Chloride channel inhibitors inhibited ion transport of LRRC8A channel in a dose-dependent manner. It is suggested that LRRC8A has the characteristics of classic volume-regulated anion channels by using the cell model of FRT cells co-expressing LRRC8A and YFP-H148Q/I152L.

ABSTRACT

Life course epidemiology should practically illustrate how risk exposures and their dynamic changes influence the occurrence, development and prognosis of chronic diseases from early life to the elderly. This paper develops the lifespan risk exposure measurement instrument (LREMI) in the framework of retrospective study to collect lifestyle, diet, physical activity information across subjects'life courses from 18-years-old to current age. Through a pilot study, the result of the test-retest analysis demonstrated the reliability of LREMI. In Shandong Multicenter Cohort, the LREMI showed its feasibility, for it could measure the exposure spectrum on both individuals and population with different life experiences. Moreover, it had good differentiation ability for identifying cases versus controls in population-based case-control study. However, further studies should be conducted in an already available prospective cohort to ascertain that our results could match prospective data.

ABSTRACT

Blending process is one of the key links of the pharmaceutical process, since the uniformity of drug products not only affects the appearance but also influences homogeneity and stability of the intrinsic quality, and then affects the efficacy of the products. Therefore, to ensure the safety and effectiveness of drugs, it is necessary to analyze and control the uniformity of the drug powder in the process of blending. Compared with the traditional method for the end point determination of the blending process, online monitoring technology has obvious advantages and broad application prospects. This paper summarized the research progress of online monitoring technologies for blending process in the pharmaceutical industry. Firstly, four kinds of online monitoring technologies were introduced and compared, which were the near infrared spectroscopy, thermal sensing technology, Raman spectroscopy and chemical imaging technology. Then the methods and principles of on-line monitoring algorithms were summarized and expounded. After that, a brief introduction of the application of near infrared monitoring equipment for pharmaceutical blending process was given. And then the research progress of the online monitoring of the blending process in the domestic pharmaceutical industry was summarized. Finally, the development trend of online monitoring technology of blending process was looked into the future.

ABSTRACT

The study is aimed to research the relationship between the seedling grade of Codonopsis pilosula and yield and quality of medicinal materials, so as to provide basis for establishing seedling standard. Thirty seedlings of C. pilosula were collected from the main production areas in Gansu province, such as Weiyuan, Minxian, Zhangxian, Dangchang and Longxi, root length and diameter and weight of all the samples were measured. According to the clustering results, seedlings were divided into 3 levels, and field experiments were conducted with three levels seedling, yield and quality were tested in laboratory. Results have showed that emergence of grades 1 was faster than that of grades 2 and 3. Yield of grades 1 was significantly higher than that of grades 2 and 3 (P<0.05). Propargyl glycoside content of grades 1 was the highest, and significantly higher than that of grades 3. Polysaccharide content of grades 3 was the highest and significantly higher than that of grades 1 and 2 (P<0.05). So considering yield, quality and investment cost of C. pilosula, planting seedlings of C. pilosula should select that root length>15.6 cm, root diameter>2.7 mm, root weight>0.56 g.

ABSTRACT

To investigate the clinical significance of aberrant hypoxia inducible factor-la [HIF-la], matrix metalloproteinase-2 [MMP-2], and E-cadherin expression in rectal adenocarcinoma. Prospective study.Chongqing Medical University, Chinese Ministry of Education Key Laboratory of Child Development and Disorders, Chongqing Provincial Key Laboratory of Pediatrics, Chongqing, China A total of 88 rectal adenocarcinoma tissues and 12 normal mucosa specimens were included in the study. The expression of HIF-la, MMP-2 and E-cadherin protein were detected by using immunohistochemistry. Clinico-pathological factors associated with expression of HIF-la, MMP-2, and E-cadherin were analyzed using the x[2] test. Survival curves were plotted according to the Kaplan-Meier method. HIF-la, MMP-2, and E-cadherin protein expression Results: HIF-la, MMP-2, and E-cadherin proteins were expressed in 65.9%, 71.6%, and 28.4% of rectal cancer tissues, respectively. However, HIF-la and MMP-2 proteins were not expressed, but E-cadherin protein was strongly expressed in 12 normal tissue samples. Altered HIF-la and MMP-2 protein expression was associated with lymph node and distant metastases and Duke's classification, while loss of E-cadherin protein expression was associated with tumor de-differentiation and lymph node and distant metastases. Combining these three markers together, the association was even more significant. Kaplan-Meier curve analysis showed that HIF-la and MMP-2 protein expression but loss of E-cadherin protein expression significantly contributed to poor overall patient survival. Altered HIF-la, MMP-2, and E-cadherin protein expression significantly contributed to the progression of rectal carcinoma and poorer patient survival

ABSTRACT

<p><b>BACKGROUND</b>The endovascular strategy of the huge dissecting aneurysms involving the basilar artery (BA) is controversial and challenging. This study was to investigate the clinical and angiographic outcomes of the treatment of the huge dissecting aneurysms involving the BA by the internal trapping (IT) technique.</p><p><b>METHODS</b>We retrospectively studied 15 patients with the huge dissecting aneurysms involving the BA treated by the IT technique between September 2005 and September 2014 in Department of Interventional Neuroradiology of Beijing Tiantan Hospital. Clinical and angiographic data were reviewed and evaluated.</p><p><b>RESULTS</b>All patients were treated by the IT technique. That meant the dissecting artery and aneurysm segments were completed occlusion. After the procedure, the angiography demonstrated that all the dissecting artery and aneurysm segments were completed occlusion. Follow-up angiography was performed at 3-6 months or 12-18 months after the endovascular treatment (median 8 months), 14 patients had a good recovery. Re-canalization occurred in one patient whose aneurysm involved in bilateral vertebral arteries and the two third of the middle-lower BA. After the second treatment, the patient died by the ventricular tachycardia.</p><p><b>CONCLUSIONS</b>The IT technique is a technically feasible and safe alternative for the treatment of BA dissecting aneurysms, but it is not necessarily the safest or most definitive treatment modality. The ideal treatment of the huge dissecting aneurysms involving the BA remains debatable and must be investigated on a case-by-case basis.</p>

ABSTRACT

Objective To evaluate the efficacy of DSA-guided percutaneous transhepatic gallbladder catheter drainage (PTGCD) in treating aged patients with acute cholecystitis complicated by severe diseases. Methods The clinical data of 15 aged patients with acute cholecystitis or complicated by severe diseases, who were encountered at authors’ hospital in the past three years and were treated with PTGCD, were retrospectively analyzed. The clinical results were discussed. Results PTGCD was successfully accomplished with single procedure in all 15 patients. Abdominal pain was relieved within one to three days, and the abdominal symptoms and signs subsided or disappeared. Reexamination of routine blood test showed that the white blood cell count decreased to normal range in 1 - 2 weeks, and complete cure was achieved in some patients. Secondary surgery was carried out in some patients after the clinical condition was improved. During the follow-up period no complications occurred in all patients except one who developed biliary leakage after the catheter was retrieved two weeks after the treatment. Conclusion For the treatment of complicated acute cholecystitis in aged patients who are not suitable to receive surgery, DSA-guided percutaneous transhepatic gallbladder catheter drainage is an ideal therapeutic means as it can significantly relieve clinical symptoms.

ABSTRACT

Objective To investigate the spatial distribution and clustering areas of Budd-Chiari syndrome in Heze City,Shandong Province, and to provide epidemiological information for further exploring the etiology and related risk factors of the disease. Methods Detailed residential addresses of 342 cases of patients (residents of Heze City) with diaphragm type Budd-Chiari syndrome diagnosed between 1995 and 2004 in Heze Municipal Hospital,Heze Shan County Central Hospital,Affiliated Hospital of Xuzhou Medical College,Shandong Provincial Hospital and Beijing Xuanwu Hospital were collected.Geographic information system (GIS) was used as a platform for data management and display. The nearest neighbor index,Ripley's K(d) function,Ripley's L(d) function and the nearest neighbor clustering method were applied to detect the spatial characters of Budd-Chiari syndrome in Heze City,Shandong Province.Crimestat 3.0 was used for spatial analysis. Results The nearest neighbor distance analysis showed that the nearest neighbor index was 0.6767 (Z =-11.4387,P < 0.01).That was an aggregation at the first-order spatial scale.Within the study area, the first clustering radius of Budd-Chiari syndrome was 6.66 km,and the first clustering strength was 5.40; the average radius of the strongest clustering area was 126.61 km,and the clustering strength was 12.52,while the biggest clustering radius was larger than 222 km.After corrected by population, the gathering strength was slightly higher than that before the correction.Ten first-order hot spots were formed, and 95% confidence interval aggregation number was 7,which meant the results were statistically significant(P < 0.05),main clustering areas are in Mudan District,Shan County and Juancheng. One secondorder hot spot was gathered based on the first-order hot spot. Conclusions Spatial distribution of Budd-Chiari syndrome in Heze City,Shandong Province has showed spatial aggregation and heterogeneity. This study has a great epidemiological significance for further exploring the cause of Budd-Chiari syndrome.
