ABSTRACT

ObjectiveTo investigate the effects of Biling Qutong prescription (BLQT) on serum levels of NOD-like receptor thermal protein domain associated protein 3 (NLRP3), purinergic ligand-gated ion channel 7 receptor (P2X7R), fibronectin (FN), and hepatic steatosis in patients with type 2 diabetes mellitus (T2DM) complicated with gouty arthritis (GA). MethodSixty-four patients diagnosed with T2DM comorbid with GA and treated at the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2019 to December 2022 were enrolled and randomly divided into a BLQT group (Chinese medicine group, 32 cases) and the ibuprofen group (western medicine group, 32 cases). Thirty healthy individuals who underwent routine health examinations during the same period were assigned to the control group. The BLQT group and the western medicine group received basic treatment along with BLQT and ibuprofen, respectively. After 8 weeks of continuous treatment, the traditional Chinese medicine syndrome score (TCMSS) of the patients was evaluated before and after treatment. The differences in fasting plasma glucose (FPG), 2-hour postprandial plasma glucose (2 h PG), glycated hemoglobin (HbA1c), fasting insulin (FINS), homeostasis model assessment of insulin resistance (HOMA-IR), serum uric acid (SUA), serum creatinine (SCr), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), alanine aminotransferase (ALT), aspartate aminotransferase (AST), controlled attenuation parameter (CAP), liver stiffness measurement (LSM), NLRP3, P2X7R, and FN levels before and after treatment were compared. Adverse drug reactions that occurred during treatment were recorded. ResultThe TCMSS for joint redness, swelling, pain, joint burning, yellow urine, and red tongue with yellow and greasy coating, as well as total score were significantly reduced in both the BLQT group and the western medicine group as compared with those before treatment (P<0.05, P<0.01). The BLQT group also showed a significant reduction in symptom scores such as dry mouth, polyuria, polydipsia, and slippery and rapid pulse (P<0.01). Compared with the western medicine group after treatment, the BLQT group exhibited a more significant reduction in all symptom scores and total score (P<0.05, P<0.01). The BLQT group and the western medicine group showed a decrease in FPG, 2 h PG, HbA1c, SCr, SUA, TG, TC, and LDL-C levels (P<0.05, P<0.01) after treatment, and the BLQT group showed decreased HOMA-IR, ALT, AST, and HDL-C levels (P<0.05, P<0.01) compared with those before treatment. When compared with the western medicine group after treatment, the BLQT group showed a more significant reduction in all laboratory parameters except for HDL-C (P<0.05, P<0.01). Before treatment, NLRP3, P2X7R, and FN levels in both the BLQT group and the western medicine group were higher than those in the control group (P<0.01). After treatment, NLRP3 and P2X7R levels in both groups significantly decreased (P<0.01), and FN levels in the BLQT group also decreased significantly (P<0.01). When compared with the western medicine group after treatment, the BLQT group showed a more significant reduction in NLRP3, P2X7R, and FN levels (P<0.01). Before treatment, CAP and LSM levels in both the BLQT group and the western medicine group were higher than those in the control group (P<0.01). After treatment, CAP and LSM levels in both groups decreased (P<0.05, P<0.01). Compared with the western medicine group after treatment, the BLQT group showed a more significant reduction in CAP and LSM (P<0.01). The incidence of adverse reactions was 3.13% (1/32) in the BLQT group and 15.63% (5/32) in the western medicine group, with no significant difference. ConclusionBLQT has good efficacy in patients with T2DM complicated with GA, which can significantly alleviate joint redness, swelling, heat, pain, limited mobility, dry mouth, and polydipsia, reduce blood glucose, uric acid, and lipid levels, suppress the high expression of NLRP3, P2X7R, and FN, and improve hepatic steatosis.

ABSTRACT

Objective To investigate the application value of self-expanding metallic stent (SEMS) implantation under fluoroscopy combined with endoscopy for acute left-side malignant colorectal obstruction. Methods Clinical date of 49 patients with left-side malignant colorectal obstruction who underwent SEMS implantation under fluoroscopy combined with endoscopy were retrospectively analyzed. Results Stents were implanted successfully in 48 patients. The technical success rate was 97.96% (48/49). One patient was transferred to emergency surgery because the guidewire could not pass through the stenosis. After stent implantation, colorectal obstruction relieved in 47 patients, while one patient was transferred to emergency surgery for unimproved obstruction. Postoperative perforation occurred in one patient after stent implantation, while a small amount of blood in excrement were found in 5 patients and then relieved after symptomatic treatment. Stent migration occurred in 2 patients. Totally 21 patients received tumor resection procedure after the symptoms of intestinal obstruction remission, and the success rate of surgery was 100% (21/21). Conclusion SEMS implantation under fluoroscopy combined with endoscopy is safe and effective to alleviate acute left-side malignant colorectal obstruction.

ABSTRACT

This study chiefly aimed at investigating the effects of Dan Zhi Jiang Tang Capsule (DJC) on myocardial Toll-like receptor 4 (TLR4),tumor necrosis factor α (TNFα) and interleukin-8 (IL-8) and the cardiopathologic changes in diabetic rats.Fifty male rats, excluding 8 rats in the control group, had been administered with high fat forage for 4 weeks, which established the diabetic rat model combined with the intraperitoneal injection of streptozotocin (STZ) at 35 mg· kg-1.The established model rats were randomly divided into the model group, the DJC high-dose group, the DJC low-dose group and the pioglitazone group. The rats in the model group and the control group were intragastrically administered with DJC at 1.08 and 0.54 g· kg-1 each day, while those of the pioglitazone group were treated with pioglitazone at 10 mg· kg-1 each day,and those of the model group and control group with saline of the same volume. The administrations lasted 8 weeks with the continues high-fat diet. The levels of TLR4,TNFα and IL-8 protein in myocardium of the rats were determined, and so were the blood glucose,HbA1c and blood lipids.Compared with the control group, the expressions of myocardial TLR4,TNFα and IL-8 in the model group were significantly increased (P< 0.01).The expressions of TLR4,TNFα and IL-8 in the DJC high-dose and low-dose groups were significantly decreased (P<0.05 or P<0.01).The cardiopathological report showed severe myocardial lesion in the model group. However, myocardial lesion in the DJC high-dose group was significantly mitigated, which presented better therapeutic effects than the DJC low-dose group and the pioglitazone group. The levels of blood glucose,HbA1c,TG,TC,LDL-C in the model group were significantly higher than those in the control group (P<0.05 or P<0.01).The levels of blood glucose,HbA1c,TG,TC and LDL-C were significantly decreased in the DJC high-dose and low-dose groups in comparison with the model group (P< 0.05 or P< 0.01).In conclusion,DJC down-regulated the expressions of TLR4,TNFα and IL-8 and improved the degree of myocardial lesion in the diabetic rats, showing favorable hypoglycemic and lipid-lowering effects.

ABSTRACT

Transforming growth factor-β1(TGF-β1)has become an important biological marker and therapeutic target of clinical progression of chronic kidney diseases. Sma- and Mad-related protein (Smad)is a downstream signal transduction protein of the TGF-β family. TGF-β1 activates Smad2 and Smad3 before increasing the transcription of connective tissue growth factors in the nucleus. Smad3 promotes mesangial cell proliferation, extracellular matrix accumulation,epithelial-mesenchymal transition, leading to renal fibrosis. However,Smad2 and Smad7 play a negative regulatory role by inhibiting renal fibrosis. TGF-β1 specific inhibitor (SB431542,etc.) has antifibrosis effect, most of which is in the preclinical stage. The drugs on the market that are effective for DN,such as benzodiazepines, atorvastatin, losartan, and pirfenidone, can inhibit the expression of TGF-β1,while tripterygium wilfordii, cordyceps sinensis, and berberine can delay the process of diabetic nephropathy by reducing TGF-β1 levels.

ABSTRACT

Objective To investigate the effects of Danzhi Jiangtang Capsule on CXCL9 and IL-8 in type 2 diabetic model rats, and discuss the mechanism in TCM intervention in type 2 diabetes. Methods Fifty male SD rats were randomly divided into 5 groups:normal control group, high-dosage group of Danzhi Jiangtang Capsule, low-dosage group of Danzhi Jiangtang Capsule, pioglitazone group and model group. Normal control group was fed with basal diet, while the other groups were fed with high fat diet. A single intraperitoneal injection of STZ of 35 mg/kg was employed to establish the animal model of diabetes mellitus. When the diabetic rodent model was built, high-dosage group of Danzhi Jiangtang Capsule and low-dosage group of Danzhi Jiangtang Capsule were treated with 1.08 g/(kg?d) and 0.54 g/(kg?d) by gavage, pioglitazone group was treated with 10 mg/(kg?d) by gavage, the blank group and the model group were treated with physiological saline 5 mL/(kg?d) by gavage for 8 weeks. CXCL9, IL-8 and blood glucose, and lipid in serum of rats were determined with double antibody sandwich method. Results Expressions of CXCL9 and IL-8 in model group increased significantly, compared with the normal control group (P<0.05, P<0.01). Expressions of CXCL9 and IL-8 in high and low dosage Danzhi Jiangtang Capsule group significantly decreased, the blood glucose and lipid also decreased, compared with model group (P<0.01). Conclusion Danzhi Jiangtang Capsule has inhibitory effects on the high expression of inflammatory factors in diabetes, and can also lower bloodglucose, and regulate blood lipid.
