ABSTRACT

OBJECTIVE@#To observe the clinical effect of elastic intramedullary nail in minimally invasive treatment of floating knee injury in children.@*METHODS@#From January 2009 to September 2017, 11 children with floating knee injury were treated with one-off open reduction and elastic intramedullary nail or external fixator fixation, including 7 males and 4 females, aged 5.0 to 11.0 years, with an average age of 8.3 years. The treatment results were evaluated according to karlstrom's standard.@*RESULTS@#Eleven patients were followed up for 8 to 48 months, with an average of 28 months. All the fractures healed at one time, and there were no complications such as nonunion, malunion and serious dysfunction of knee joint. The length of the affected limb in 2 cases was 1.2 to 1.5 cm longer than that in the opposite side without shortening. According to Karlstrom scoring standard, 8 cases were excellent, 1 case was good and 2 cases were middle.@*CONCLUSION@#Elastic intramedullary nail minimally invasive treatment of floating knee injury in children is a safe and effective treatment, which can effectively reduce the fracture and promote bone healing, which is conducive to early functional recovery.

ABSTRACT

To explore therapeutic effect of acupuncture combined electrical stimulation on aged patients with post‐stroke depression .Methods : A total of 78 patients with post‐stroke depression were randomly and equally divided into routine treatment group (received health education based on routine treatment ) and combined treatment group (received acupuncture combined low‐frequency electrical stimulation based on routine treatment group ).After four‐week treatment , score of Hamilton rating scale for depression (HAMD) and therapeutic effect were compared between two groups .Results : Compared with before treatment , there was significant reduction in HAMD score in two groups after treatment ;compared with routine treatment group after treatment , there was significant reduction in HAMD score [(19.72 ± 2. 04) scores vs.(14. 94 ± 1. 86) scores] in combined treatment group , P=0. 001 all.To‐tal effective rate of combined treatment group was significantly higher than that of routine treatment group (87.18% vs.64.1%, P=0.018).Conclusion : Acupuncture combined low‐frequency electrical stimulation possesses significant therapeutic effect on post‐stroke depression , which is worthy of clinical application .

ABSTRACT

BACKGROUND:β-tricalcium phosphate (β-TCP) and monocalciumphosphate monohydrate (MCPM) are traditionally considered as reactants for dicalcium phosphate dehydrate (DCPD) bone cement,but little is reported on the hydroxyapatite (HA) as a reactant.OBJECTIVE:To verify whether HA and MCPM can be used to prepare DCPD bone cement and to explore the physicochemical properties.METHODS:The HA and β-TCP were prepared by wet chemical precipitation method,and mixed with appropriate proportion of MCPM.Then,the HA-DCPD and β-TCP-DCPD were obtained by adding a proper amount of curing water.The composition and structure of the two materials were analyzed by X-ray diffraction,the morphology was observed by scanning electron microscope,and the mechanical strength was tested by Instron5567 universal material test machine.These two kinds of materials were placed in simulated body fluid for detecting the weight loss ratio,soaked for 14 days and taken out for X-ray diffraction and scanning electron microscope detection.RESULTS AND CONCLUSION:X-ray diffraction findings indicated that these two kinds of materials both belonged to high-purity DCPD bone cement.Under the scanning electron microscope,β-TCP-DCPD bone cement had dense crystal structure,with less pore number;however,the HA-DCPD bone cement presented with finer grains,loose structure,and higher pore number.With the increase of curing time,the mechanical strength of two kinds of bone cements was correspondingly increased,but the compressive strength of β-TCP-DCPD bone cement was significantly higher than that of HA-DCPD bone cement (P < 0.05).In the simulated body fluid,the weight loss ratio of β-TCP-DCPD bone cement was significantly lower than that of HA-DCPD bone cement (P < 0.05).At 14 days after soaking in the simulated body fluid,a layer of spherical particles that was formed on the surface of both materials was identified as hydroxyapatite by scanning electron microscope observation and X-ray diffraction analysis.In summary,HA-DCPD bone cement has good biodegradability,excellent bioactivity and bone conductivity,but poor mechanical properties.

ABSTRACT

Objective To understand the pathogenic distribution and epidemiological trend of hand-foot-and-mouth disease (HFMD),and provide evidence for the prevention and control of HFMD.Methods Children who were diagnosed with HFMD in a hospital between January and December 2015 were investigated,real time fluorescence PCR was used to detect enterovirus universal type EV,enterovirus 71 (EV71),and Coxsackievirus A16 (CoxA16) in specimens from children with HFMD.Positive rates and distribution of various types of EV among children of different months,genders,age groups,and infection types were analyzed.Results A total of 837 throat swab specimens from HFMD children were collected in 2015,380 (45.40%) of which were EV positive specimens.Virus typing showed that 110 (28.95 %),7 (1.84 %),6(1.58 %),and 257(67.63 %) were positive specimens for EV71,CoxA16,EV71 + CoxA16,and other types of EV.HFMD had a high prevalence since April,reached a peak in May-June,and remained high incidence in July-December.Positive rates of EV in children of different months were statistically different (P<0.05).The age of onset was mainly in children under 3 years.Positive rates of EV and constitute ratios of different types of EV in children of different age groups were all statistically different (all P<0.05).The positive rate of EV in severe HFMD cases was higher than common cases (65.34% vs 27.06%,P<0.001).The proportion of severe cases in children with EV71 infection and other types of EV infection were 90.00% and 60.70% respectively;children with EV71 + CoxA16 double infection were all severe cases.Constitute of EV types in children with different infection types was statistically different(P<0.001).Conclusion In 2015,EV infection in hospitalized children with HFMD in this hospital was mainly caused by other types of EV (nonEV71 and non-CoxA16),the high prevalence season,high-risk population under 3 years of age,and severe cases should be paid high attention,prevention and treatment should be performed well.

ABSTRACT

We revisited the "Xiao Chaihu Decoction event (XCHDE)" occurred in late 1980s in Japan and the Bupleuri Radix related adverse drug reaction (ADR) reports in China After careful review, comparison, analysis and evaluation, we think the interstitial pneumonitis, drug induced Liver injury (DILI) and other severe adverse drug envents (ADEs) including death happened in Japan is probably results from multiple factors, including combinatory use of XCHDE with interferon, Kampo usage under modern medicine theory guidance, and use of XCHD on the basis of disease diagnosis instead of traditional Chinese syndrome complex differentiation. There are less ADE case reports related to XCHD preparation in China compared to Japan, mostly manifest with hypersensitivity responses of skin and perfuse perspiration. The symptoms of Radix Bupleuri injection related ADEs mainly manifest hypersensitivity-like response, 2 cases of intravenous infusion instead of intramuscular injection developed hypokalemia and renal failure. One case died from severe hypersensitivity shock. In Chinese literatures, there is no report of the interstitial pneumonitis and DILI associated with XCHDG in Japan. So far, there is no voluntary monitoring data and large sample clinical research data available. The author elaborated the classification of "reevaluation" and clarified "re-evaluation upon events" included the reaction to the suspected safety and efficacy events. Based on the current status of the clinical research on the Radix Bupleuri preparations, the author points out that post-marketing "re-evaluation upon suspected event" is not only a necessity of continuous evaluation of the safety, efficacy of drugs, it is also a necessity for providing objective clinical research data to share with the international and domestic drug administrations in the risk-benefit evaluation. It is also the unavoidable pathway to culture and push the excellent species and famous brands of TCM to the international market, in that it enhances the pharmaceutical manufactures the ability to control enterprise risk and eliminate the negative impact on traditional Chinese medicine. With the widely used and ADR report concentrated Radix Bupleuri injection as an example, the author provide a new approach to post-marketing clinical research.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effects of marine protein peptide (MPP) on immunomodulating in mice and its possible mechanism.</p><p><b>METHODS</b>Female ICR mice (6-8 weeks old) were administered the MPP for 4 weeks with the dose of 0.22 g/kgBW, 0.45 g/kgBW and 1.35 g/kgBW. Spleen and thymus were weighted and cell-mediated immune functions, humoral immune functions, phagocytic functions of mononuclear phagocyte, NK cell activity assays, the T cell subpopulation of the spleen tissue by the flow cytometer and the concentrations of cytokines in serum by cytometric bead array were examined.</p><p><b>RESULTS</b>The capacity of lymphocyte proliferation induced by Con A (0.33 +/- 0.21), DTH response (0.36 +/- 0.11) mm in MPP 0.22 g/kgBW group were significantly increased in comparison with these values in control group (0.15 +/- 0.10) and (0.21 +/- 0.10)mm, respectively, P < 0.05. IgM-PFC number of MPP 0.22 g/kgBW group (1.64 +/- 0.06), 0.45 g/kgBW group (1.59 +/- 0.05) and 1.35 g/kgBW group (1.56 +/- 0.10) were higher than those in control group (1.38 +/- 0.10), P < 0.01; and the level of serum HC50 of MPP 0.22 g/kgBW group (141.00 +/- 23.00) and 0.45 g/kgBW group (130.40 +/- 33.20) were greater than the control (100.30 +/- 19.40) , P < 0.01. The activity of NK cells in MPP 0.22 g/kgBW group (1.672 +/- 0.142) was significantly elevated in comparison with this value in control group (1.392 +/- 0.182), P < 0.05. The percentage of CD4 T helper (Th) cell in spleen of MPP 0.22 g/kgBW group (32.84 +/- 3.776)% and 0.45 g/kgBW group (32.42 +/- 3.507) % was higher than those in control group (25.06 +/- 0.354) %, P < 0.05. The concentrations of IL-2 in serum of MPP 0.22 g/kgBW group 181.06 pg/ml, 0.45 g/kgBW group 94.84 pg/ml and 1.35 g/kgBW group 102.61 pg/ml were higher than those in control group 0.50 pg/ml, P < 0.05; and the level of IL-5 of MPP 0.22 g/kgBW group (38.31) pg/ml was greater than the control 0.50 pg/ml, P < 0.05. Nevertheless, no obvious effects on weight increasing, the ratio of immune organ and body weight and phagocytosis capacity were observed in our study.</p><p><b>CONCLUSION</b>MPP could improve the immune functions in mice, and might be by the mechanism of enhancing the function of Th cells stimulating the secretion of Th1 and Th2 type cell cytokines.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To identify proteomic patterns in hepatic tissues for diagnosing early HBV related HCC.</p><p><b>METHODS</b>Proteomic spectra were generated by two-dimensional gel electrophoresis (2-DE), A preliminary "raining" set of spectra derived from analysis of 14 cancer tissues and 14 non-cancer tissues, a proteomic patterns that completely discriminated cancer from non-cancer was identified. The discovered pattern was then used to classify an independent set of 48 masked samples: 24 from cancer tissues, and 24 from non-cancer tissues.</p><p><b>RESULTS</b>The discriminatory pattern correctly identified all cancer tissues and non-cancer tissues in the masked set. This result yielded a sensitivity of 100%, specificity of 100%.</p><p><b>CONCLUSION</b>Further analysis on these proteins in the proteomic pattern will be helpful to screen tumor markers for HBV related HCC. These findings justify a prospective assessment of proteomic pattern technology as a screening tool for cancer in high-risk and general populations.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To understand iron nutritional status in school-age children, incidence of the sub-clinical iron deficiency (SID) and effect of iron supplementation on SID in the rural school-age children from mountainous areas of Beijing.</p><p><b>METHODS</b>The dietary survey and food frequency questionnaire survey were conducted in 1,012 school children aged 7 - 13 at rural mountainous areas of Fangshan District, Beijing, and their blood samples were collected for analyzing biochemical indicators for iron nutrition. Two hundred and sixty-seven children with IDs (iron deficiency store) and IDE (iron deficiency erythropoiesis), based on screening criteria for iron-deficiency anemia, received an iron supplementation (NaFeEDTA) capsule (containing 60 mg iron element) weekly, and those with IDA (iron deficiency anemia) received NaFeEDTA capsule thrice weekly for nine weeks. Blood biochemical indicators for iron nutrition were determined repeatedly and compared with those before and after the intervention.</p><p><b>RESULTS</b>The daily average intakes of energy, protein, iron and vitamin C in school children of all age groups reached the daily recommended criteria (> 85% of the RNIs), but the proportion of heme in dietary iron constitution was lower. The average blood biochemical indicators for iron nutrition were as follows: serum ferritin (SF) (50.83 +/- 33.09) micro g/L, free erythrocyte protoporphyrin (FEP) (489.44 +/- 219.61) micro g/L, hemoglobin (Hb) (130.57 +/- 10.82) g/L, and the ratio of FEP/Hb (3.83 +/- 1.96), respectively. Incidence of total iron deficiency in rural children was 26.5%, with proportions of iron deficiency (IDs), iron deficiency erythropoiesis (IDE), and iron deficiency anemia (IDA) of 15.5%, 7.1%, and 3.9% respectively. SID accounted for 85.4% of the total iron deficiency, which was 5.8 times as much as IDA. With iron supplementation for 9 weeks, the hematdogical index of iron increased significantly and returned to the normal level.</p><p><b>CONCLUSIONS</b>The incidence of subclinical iron deficiency in the rural school-age children was insidious and should be attached more importance, which was helpful to its early recognition and intervention. Iron supplementation is important for children with SID to prevent and decrease the occurrence of IDA.</p>
