ABSTRACT

For patients with chronic hepatitis B, the indications of antiviral treatment are gradually expanding, and the pursuit of various degrees of cure (partial cure, functional cure and complete cure) is consistently improving, which enhances the urgent clinical need for improving the detection sensitivity of hepatitis B virus (HBV) surface antigen and DNA. Based on the availability of commercial highly sensitive hepatitis B surface antigen and HBV DNA detection reagents, we summarized their applications in the diagnosis of HBV infection, their role on guiding selection of antiviral treatment agents and treatment plans, their prediction efficacy and indication for drug withdrawal, their role on monitoring therapy efficacy and the prediction of disease outcomes. Taken together, highly sensitive hepatitis B virus surface antigen and DNA assays and related detection technology play an important role in the whole management process of chronic HBV infection.

ABSTRACT

Objective To systematically review the epidemiological studies on primary biliary cholangitis (PBC), and to investigate the prevalence rate of PBC in the Chinese general population and its influencing factors. Methods PubMed, Embase, The Cochrane Library, CNKI, and Wanfang Data were searched for articles on the epidemiology of PBC in China published up to 31th March 2022. Two researchers independently performed screening and data extraction, and then related analyses were performed. Results A total of 9 articles were included. The positive rate of AMA was 1 049.05/100 000 (ranging fr om 159.65/100 000 to 2287.40/100 000), and the prevalence rate of PBC was 123.68/100 000 (ranging from 42.70/100 000 to 276.59/100 000). The positive rate of AMA was 636.51/100 000 (ranging from 52.55/100 000 to 1 164.33/100 000) in men and 1 265.47/100 000 (ranging from 225.23/100 000 to 1 704.93/100 000) in women, with a male/female ratio of 1∶1.99 for the prevalence rate of AMA. The prevalence rate of PBC was 40.81/100 000 (ranging from 23.54/100 000 to 75.10/100 000) in men and 148.71/100 000 (ranging from 77.36/100 000 to 214.91/100 000) in women, with a male/female ratio of 1∶3.64 for the prevalence rate of PBC. Conclusion Different studies show great differences in the positive rate of AMA and the prevalence rate of PBC in the Chinese general population, which is mainly affected by sex, age, and region. The positive rate of AMA and the prevalence rate of PBC increase with age, and the patients aged ≥50 years have a significantly higher positive rate of AMA than those aged < 50 years. The positive rate of AMA is significantly higher than the prevalence rate of PBC. There are significantly more women than men in the AMA-positive population and the PBC patients, and the influence of sex on AMA is lower than that on PBC.

ABSTRACT

Objective:To systematically evaluate the correlation of high sensitivity C-reactive protein (hs-CRP) with contrast-induced nephropathy (CIN) in patients following coronary angiography (CAG) or percutaneous coronary intervention (PCI).Methods:PubMed, web of science, CBM, CNKI and Wanfang Data were searched for studies on hs-CRP levels in patients undergoing CAG or PCI patients from the incipience of the database to March 7, 2021. Meta-analysis was performed by RevMan 5.3 and Stata 12.0 software. Results:Fourteen related studies were included involving 11 885 patients undergoing CAG or PCI (1 034 cases with CIN and 10 851 cases without CIN). The results of meta-analysis showed that the level of hs-CRP in CIN group was significantly higher than that in non-CIN group (WMD=3.77,95 %CI:2.80—4.74, P<0.001, I2=93%), patients with higher levels of hs-CRP before CAG or PCI were more likely to develop CIN. Sensitivity analysis shows that the results of this study had good stability. The results of subgroup analysis show that the differences in sample size, study population, geographical location and the definition of CIN were statistically significant. Conclusion:Available evidence shows that high hs-CRP level is a risk factor for CIN in patients undergoing CAG or PCI, large sample trials are still needed to support this conclusion.

ABSTRACT

Objective:To explore the clinical value of continuous blood purification(CBP) in patients with severe heart failure combined with renal failure and its effect on serum p66Shc protein, soluble fms-like tyrosine kinase receptor 1 (sFlt-1), and tissue inhibitor of metalloproteinase-1 (TIMP-1).Methods:Ninety-seven patients with severe heart failure combined with renal failure admitted to the Chaoyang Central Hospital from March 2017 to October 2019 were enrolled and they were divided into the control group (48 cases) and the observation group (49 cases) according to the random number table method. The control group was treated with intermittent hemodialysis (IHD), while the observation group was treated with CBP. Changes of the efficacy, the renal function indexes, cardiac function indexes, p66Shc protein, sFlt-1, TIMP-1 before and after treatment were compared between the two groups. The occurrence of adverse reactions were recorded. Results:The total effective rate in the observation group was better than thatin the control group: 79.59% (39/49) vs. 60.42% (29/48), χ 2 = 4.25, P<0.05. After treated for 1 week, the levels of blood urea nitrogen, serum creatinine, serum phosphorus, blood uric acid and β2 microglobulinin the observation group were lower than those in the control group: (12.63 ± 3.14) mmol/L vs. (16.23 ± 4.74) mmol/L, (175.52 ± 39.57) μmol/L vs. (240.15 ± 50.18) μmol/L, (1.20 ± 0.23) mmol/L vs. (1.37 ± 0.31) mmol/L, (265.15 ± 34.79) μmol/L vs.(297.52 ± 50.07) μmol/L, (28.75 ± 5.14) mg/L vs. (33.52 ± 7.39) mg/L, the differences were statistically significant ( P<0.05). The levels of left ventricular ejection fraction, cardiac output and stroke volume in the observation group were higher than those in the control group: (53.63 ± 7.96)% vs. (49.52 ± 5.14)%, (58.45 ± 15.23) ml vs. (49.58 ± 9.52) ml, (4.59 ± 0.52) L/min vs. (4.01 ± 0.23) L/min, the differences were statistically significant ( P<0.05). The levels of p66Shc, sFlt-1, TIMP-1 in the observation group were lower than thosein the control group: 1.11 ± 0.36 vs. 1.45 ± 0.42, (15.76 ± 4.34) μg/L vs. (19.87 ± 5.66) μg/L, (59.14 ± 10.57) μg/L vs. (65.39 ± 9.45) μg/L, the differences were statistically significant ( P<0.05). The total adverse reaction rate in the observation group was lower than that in the observation group: 14.29% (7/49) vs. 31.25% (15/48), χ2 = 3.98, P<0.05. Conclusions:CBP therapy for patients with severe heart failure combined with renal failure has better efficacy than IHD, and can improve the patient′s cardiac and kidney function, reduce the levels of p66Shc protein, sFlt-1 and TIMP-1, reduce adverse reactions. It is safe and feasible.

ABSTRACT

Objective:To evaluate the diagnostic value of serum prostate-specific antigen (PSA) levels and multi-parameter magnetic resonance imaging (mpMRI) in patients with granulomatous prostatitis after intravesical Bacillus Calmette-Guérin (BCG) therapy. Methods:The medical records of eight patients with pathologically proven granulomatous prostatitis in Shandong Provincial Hospital Affiliated to Shandong University from January, 2015 to June, 2020, were enrolled and analyzed in this retrospective study. All 8 patients (ages 47-76, mean 63.6) underwent pelvic mpMRI and serum tPSA levels before TURBT, which showed the results of tPSA, f/t and mpMRI were normal before TURBT (0.45-3.62 ng/ml, 0.20-0.51 and normal signal intensities on T1WI and T2WI, respectively). All patients underwent intravesical BCG therapy after post-TURBT 4-6-weeks’ intravesical gemcitabine therapy as a result of pathologically proven middle and high risk NMIBC via cystoscopy. Results:The results of tPSA levels in all 8 patients were elevated after intravesical BCG therapy after 9-15 months (mean 10.5 months), with 4 patients above 4 (6.77-12.89)ng/ml and 4 patients within the normal ranges(2.02-2.68)ng/ml, and f/t levels decreased to lower than 0.16 (0.09-0.15)in all patients. The mpMRI abnormal signals in all patients were all located in the peripheral zone of prostate. All nodular lesions of prostate mpMRI showed lower signal intensity (SI) on T2WI, higher SI on DWI and lower SI on ADC after BCG therapy. All patients underwent prostate biopsy for abnormal signal on prostate mpMRI. The biopsy pathologic results of all patients were granulomatous prostatitis. Conclusions:When elevated PSA and abnormal signals on prostate mpMRI after intravesical BCG therapy occurred, prostate biopsy may not be required for secondary granulomatous prostatitis patients with non-muscle invasive bladder cancer in combination of clinical history.

ABSTRACT

Programmed cell death protein 1 (PD-1) is an immune checkpoint modulator and a major target of immunotherapy as anti-PD-1 monoclonal antibodies have demonstrated remarkable efficacy in cancer treatment. Accumulating evidence suggests an important role of PD-1 in the central nervous system (CNS). PD-1 has been implicated in CNS disorders such as brain tumors, Alzheimer’s disease, ischemic stroke, spinal cord injury, multiple sclerosis, cognitive function, and pain. PD-1 signaling suppresses the CNS immune response via resident microglia and infiltrating peripheral immune cells. Notably, PD-1 is also widely expressed in neurons and suppresses neuronal activity via downstream Src homology 2 domain-containing protein tyrosine phosphatase 1 and modulation of ion channel function. An improved understanding of PD-1 signaling in the cross-talk between glial cells, neurons, and peripheral immune cells in the CNS will shed light on immunomodulation, neuromodulation, and novel strategies for treating brain diseases.

ABSTRACT

Objective To determine the nucleotide sequence of the LACK (Leishmania homologue of receptors for activated protein kinase C) gene of Leishmania donovani isolates from plain foci (L.d SD1) and desert foci (L.d XJ771) of China, and to find out the difference of the sequence of LACK gene with other Leishimania spp. and also the isolate from hill foci of China.. Methods. The LACK genes of L.d SD1 and L.d XJ771 were amplified by RT-PCR and cloned into pUC18 vector respectively, sequenced by the dideoxy chain termination method, then analyzed and compared with that of other isolates.. Results . The LACK genes of the two isolates were successfully cloned. Both of the 2 fragments were 942 bp in length. Comparison of the two nucleotide sequences with that of other Leishmania spp. in GenBank showed that the identities were more than 97%, and the identities of the nucleotide sequences of LACK genes of the three L.d isolates from plain, desert and hill foci of China were more than 95%.. Conclusion . High identities exist among the nucleotide sequences of LACK genes of the three L.d isolates from the three foci of China.
