ABSTRACT

Objective: To explore the value of tissue Doppler imaging (TDI) combined with two-dimensional speckle tracking imaging (2D-STI) at rest on evaluating microcirculation dysfunction and left ventricular dysfunction in patients with angina and no obstructive coronary artery disease(ANOCA). Methods: This retrospective study recruited 78 ANOCA patients, who hospitalized in the People's Hospital of Liaoning Province from August 2019 to July 2021. These patients underwent conventional echocardiography examination, including TDI and 2D-STI, to evaluate the left ventricular dysfunction, and adenosine stress echocardiography (SE) to evaluate the coronary flow velocity reserve (CFVR). ANOCA patients were divided into coronary microcirculation dysfunction CMD group (CFVR<2) and control group (CFVR≥2) according to CFVR. Clinical data, routine echocardiographic parameters, TDI parameters including isovolumic contraction time (IVCT), isovolumic relaxation time (IVRT), ejection time (ET), and STI parameters including global longitudinal peak strain (GLS), time to peak (TTP); peak strain dispersion (PSD) were compared between the two groups. Binary logistic regression was used to analyze the risk factors of CMD and the predictive value of each parameter to construct a joint prediction model for the diagnosis of CMD in this patient cohort. Results: The mean age was (55.5±11.2) years, 43 (55%) patients were females in this patient cohort, 38 (49%) patienst were didvided into the CMD group and 40 (51%) into the control group. Age, prevalence of hypertension, diabetes, dyslipidemia, and smokers were significantly higher in the CMD group than in the control group (all P<0.05). Tei index was higher, IVCT and TTP were longer, PSD was higher, ET was shorter, and absolute GLS was lower in the CMD group than in the control group (all P<0.05). The results of logistic regression analysis showed that longer IVCT, higher Tei index, higher time to PSD and lower absolute GLS were the independent risk factors of CMD. The ROC curve revealed that the predicting efficacy on CMD was satisfactiory with the combined predictors: AUC=0.884, sensitivity of 82% and specificity of 80%. Conclusions: TDI combined with 2D-STI is associated with a good diagnostic value on the diagnosis of CMD and left ventricular dysfunction in patients with ANOCA, which provides a feasible non-invasive tool for the diagnosis of CMD and risk stratification of patients with ANOCA.

ABSTRACT

Objective To know about knowledge about prevention and treatment of venous thromboembolism (VTE) in nurses from tertiary hospitals in China, and to analyze influencing factors to provide guidance for nursing of VTE. Methods A questionnaire survey was conducted among 5279 nurses from 80 tertiary hospitals in 29 provinces (municipalities, and autonomous regions). Results The average score of knowledge about VTE prevention and treatment in nurses was 61.26%. The knowledge was obviously unbalanced. Nurses had poor knowledge of elastic stockings and injection of prefilling anticoagulant drugs. The scores were high for nurses who were over 30 years old, had worked for more than 10 years, and had higher titles. Conclusion Currently knowledge about pre-vention and treatment of VTE in nurses in our country is not ideal, and there is imbalance in knowledge. Training is still an effective approach. There should be standardized training according to level of knowledge of nurses.

ABSTRACT

Objective To establish evidence-based knowledge translation professional group in general hospital, and to explore its effects on clinical practice. Methods Double goals including evidence-based nursing and knowl-edge translation were built, inner organization structure of "Nursing Department-Core Group-Research Nurse-Practice Nurse" was set up,outer platform of "Evidence-Based Nursing and Knowledge Translation Center" was established,"double cycle" operation mode and "double track" management mode were developed. Results After establishment of evidence-based knowledge translation professional group,24 nurses received systematic training in evidence-based nursing knowledge translation. We successfully joined Best Practice Spotlight Organization of Canadian. Conclusion Evidence-based knowledge translation professional group in general hospital can promote the development of evi-dence-based nursing knowledge translation.

ABSTRACT

Objective To explore the role of infection control nurses in healthcare-associated infection(HAI)man-agement,and provide basis for HAI management. Methods Through setting up infection control nurses in clinical departments of the whole hospital, clarifying their responsibilities and duties, training, and supervising them, the effect of infection control nurses on HAI management was observed. Results A total of 67 infection control nurses were set up in the clinical departments of the whole hospital,HAI management knowledge among health care work-ers (HCWs)in 26 departments improved significantly, scores of HAI management knowledge among HCWs in April and December was compared, difference was statistically significant (Z = - 2.193,unilateral P = 0.014). Hand hygiene compliance rate of HCWs improved from 83.35%(1817/2180)in April to 89.53% (2002/2236)in December, difference was statistically significant(χ2 =36.13,P <0.01).A total of 56670 hospitalized patients were monitored from April to December 2015,the total length of hospital stay was 411164 days, utilization rate of three catheters was 27.18%,three catheter-related infection rate was 0.74‰.The median scores of supervision on HAI management in clinical departments improved from 95.30 in May to 97.70 in September(P <0.05).Conclusion Setting up infection control nurses is of great significance to strengthen the HAI management organization and pro-mote the quality of HAI management.
