ABSTRACT

Abstract Smog has become the fifth season of Pakistan especially in Lahore city. Increased level of air pollutants (primary and secondary) are thought to be responsible for the formation of smog in Lahore. Therefore, the current study was carried out for the evaluation of air pollutants (primary and secondary) of smog in Wagah border particularly and other sites (Jail road, Gulburg) Lahore. For this purpose, baseline data on winter smog from March to December on primary and secondary air pollutants and meteorological parameters was collected from Environmental Protection Department and Pakistan Meteorological Department respectively. Devices being used in both departments for analysis of parameters were also studied. Collected data was further statistically analyzed to determine the correlation of parameters with meteorological conditions and was subjected to air quality index. According to results, PM 10 and PM 2.5 were found very high above the NEQS. NOx concentrations were also high above the permissible limits whereas SO2 and O3 were found below the NEQS thus have no roles in smog formation. Air Quality Index (AQI) of pollutants was PM 2.5(86-227), PM 10 (46-332), NOx (26-110), O3 (19-84) and SO2 (10-95). AQI of PM 2.5 remained between moderate to very unhealthy levels. AQI of PM 10 remained between good to hazardous levels. AQI of NOx remained between good to unhealthy for sensitive groups levels. AQI of O3 and SO2 remained between good to moderate levels. Pearson correlation showed that every pollutant has a different relation with different or same parameters in different areas. It is concluded from the present study that particulate matter was much more responsible for smog formation. Although NOx also played role in smog formation. So there is need to reduce sources of particulate matter and NOx specifically in order to reduce smog formation in Lahore.


Resumo Smog tornou-se a quinta estação do Paquistão, especialmente na cidade de Lahore. Acredita-se que o aumento do nível de poluentes atmosféricos (primários e secundários) seja responsável pela formação de poluição atmosférica em Lahore. Portanto, o presente estudo foi realizado para a avaliação dos poluentes atmosféricos (primários e secundários) do smog na fronteira de Wagah em particular e em outros locais (Jail road, Gulburg) Lahore. Para este propósito, os dados de referência sobre a poluição atmosférica de inverno de março a dezembro sobre poluentes atmosféricos primários e secundários e parâmetros meteorológicos foram coletados do Departamento de Proteção Ambiental e do Departamento Meteorológico do Paquistão, respectivamente. Dispositivos sendo usados em ambos os departamentos para análise de parâmetros também foram estudados. Os dados coletados foram posteriormente analisados estatisticamente para determinar a correlação dos parâmetros com as condições meteorológicas e foram submetidos ao índice de qualidade do ar. De acordo com os resultados, PM 10 e PM 2,5 foram encontrados muito acima do NEQS. As concentrações de NOx também estavam muito acima dos limites permitidos, enquanto SO2 e O3 foram encontrados abaixo do NEQS, portanto, não têm papéis na formação de smog. O índice de qualidade do ar (AQI) de poluentes foi PM 2,5 (86-227), PM 10 (46-332), NOx (26-110), O3 (19-84) e SO2 (10-95). Conclui-se do presente estudo que o material particulado foi muito mais responsável pela formação de smog. Embora o NOx também tenha desempenhado um papel na formação do smog. Portanto, é necessário reduzir as fontes de partículas e NOx, especificamente para reduzir a formação de smog em Lahore.

ABSTRACT

Background: Pakistan is facing a great challenge in planning, implementing, and sustaining family planning and birth control policies. Considering this, one of the community health programs in the outskirts of a community health worked led family planning counselling. The purpose of this research study is to highlight perceptions of the beneficiaries regarding the benefits of the same. Methods: A qualitative descriptive study was undertaken for this investigation using a non-random sampling technique. The sample was chosen from the community population who were part of this intervention, with an inclusion criterion. The data was collected using in-depth interviews from 13 participants in total. Results: The key themes emerged across the interviews: Unfolding myths: Participants’ recognized that using family planning mediums does not mean that there is a medical deficiency outcome to it but a way to prevent large families, societal pressure: Participants shared the issues and cultural taboos attached to the family planning and how it is conceived in their societies, program support: participants shared that having a female lady health worker coming to the home brings in more access to the family planning and benefits: they found that effective and structured family planning is contributing to their enhanced personal life relations, economic gains and social comfort. Conclusions: A community health program model can be one of the outlets the government can collaborate to offer door to door provision, service quality and access to family planning to such communities in need.

ABSTRACT

Background: In the face of the COVID-19 pandemic, surgical personnel and patients experience many diagnosis and treatment-related challenges. COVID-19 is highly virulent, and thus, immediate, safe, and effective solutions to the patients’ management are required. Research collaboration and knowledge dissemination are necessary to optimize medical and surgical approaches to the individual patients’ needs. Gastric manifestations of SARS-CoV-2 infection are not uncommon and may even precede respiratory symptoms. Common gastrointestinal symptoms include loss of appetite, diarrhea, nausea, vomiting and abdominal pain. Case Presentation: A 50-year-old male patient complained of "blood in the stool and tenesmus for half a year" and was admitted to the hospital on March 2, 2023. Half a year ago, the patient had blood in the stool without obvious incentives, the bleeding was bright red, the habit of defecation changed, the stool became thinner, and the frequency of defecation increased, 3-5 times a day, and the amount was not much. At that time, he went to a regular medical institution for any treatment measures.

ABSTRACT

Background: Treatment given as a first step to shrink a tumor before the main treatment, which is usually surgery, is given. Examples of neoadjuvant therapy include chemotherapy, radiation therapy, and hormone therapy. It is a type of induction therapy. The most common pattern of esophageal cancer metastases (ECM) is to the lymph nodes, lung, liver, bones, adrenal glands, and brain. Case Presentation: A male patient, 47 years old, was admitted to the hospital on July 31, 2020, mainly due to "progressive dysphagia. Two months ago, the patient had difficulty eating without obvious incentives, especially when eating hard food such as rice. The liquid diet was acceptable, accompanied by retrosternal pain and discomfort, heartburn, and obvious weight loss. The symptoms gradually aggravated without obvious relief. Conclusion: In summary, there are few international investigations and an absence of relevant information and literature on the environment in which tumor cells survive after radiotherapy. Clinicians can't ignore the issue of whether radiotherapy will alter the tumor cells' natural environment, leading to tumor cell escape and an increase in brain metastases.

ABSTRACT

Objective: Tumor surgeons now pay special attention to the alteration in breast form following breast-conserving surgery. Safety of breast-conserving margins and postoperative breast appearance are typically challenging in conventional breast-conserving surgery due to the effect of variables such as big breast tumors or unusual placements. It's important to remember that postoperative breast deformity is a real possibility. Breast-conserving tumor plastic surgery combines tumor excision with plastic surgery, allowing for a more thorough removal of the tumor. It provides a more humane treatment option for the majority of patients by ensuring the safety of surgical margins, considering the aesthetic effects, increasing the breast-conserving opportunities for patients with tumors in special parts, and expanding the indications for breast-conserving surgery. It also greatly improves the postoperative quality of life and mental state. In this article, we deeply examine the current state of knowledge concerning the effectiveness, safety, and appropriate patient population for breast-conserving surgery in the context of tumor plastic surgery.

ABSTRACT

Abstract Dengue fever vectored by the mosquito Aedes aegypti is one of the most rapidly spreading insect-borne diseases. Current reliance of dengue vector control is mostly on chemical insecticides. Growing insecticide resistance in the primary mosquito vector, Aedes aegypti, limits the effectiveness of vector control through chemical insecticides. These chemical insecticides also have negative environmental impacts on animals, plants and human health. Myco-biocontrol agents are naturally occurring organisms and are found to be less damaging to the environment as compared to chemical insecticides. In the present study, entomopathogenic potential of local strains of fungi isolated from soil was assessed for the control of dengue vector. Local fungal isolates presents better alternative to introducing a foreign biocontrol strain, as they may be better adapted to environmental conditions of the area to survive and may have more entomopathogenic efficacy against target organism. Larvicidal efficacy of Fusarium equiseti and Fusarium proliferatum was evaluated against Aedes aegypti. Local strains of F. equiseti (MK371718) and F. proliferatum (MK371715) were isolated from the soil of Changa Manga Forest, Pakistan by using insect bait method. Larvicidal activity of two Fusarium spp. was tested against forth instar larvae of A. aegypti in the laboratory, using concentrations 105, 106, 107 and 108 conidia /ml. LC50 values for F. equiseti after 24h, 48h, 72h and 96h of exposure were recorded as 3.8x 108, 2.9x107, 2.0x107, and 7.1x106 conidia /ml respectively while LC50 values for F. proliferatum were recorded as 1.21x108, 9.6x107, 4.2x107, 2.6x107 conidia /ml respectively after 24h, 48h, 72h and 96h of exposure. The results indicate that among two fungal strains F. equiseti was found to be more effective in terms of its larvicidal activity than F. proliferatum against larvae of A. aegypti.


Resumo A dengue transmitida pelo mosquito Aedes aegypti é uma das doenças transmitidas por insetos de propagação mais rápida. A dependência atual do controle do vetor da dengue é principalmente de inseticidas químicos. O aumento da resistência a inseticidas no principal vetor do mosquito, Aedes aegypti, limita a eficácia do controle do vetor por meio de inseticidas químicos. Esses inseticidas químicos também têm impactos ambientais negativos sobre os animais, plantas e saúde humana. Os agentes de micobiocontrole são organismos que ocorrem naturalmente e são menos prejudiciais ao meio ambiente em comparação com os inseticidas químicos. No presente estudo, avaliou-se o potencial entomopatogênico de cepas locais de fungos isolados do solo para o controle do vetor da dengue. Isolados de fungos locais apresentam melhor alternativa para a introdução de uma cepa de biocontrole estrangeira, pois podem ser mais bem adaptados às condições ambientais da área para sobreviver e podem ter maior eficácia entomopatogênica contra o organismo-alvo. ABSTRACT

By applying the in-silico method, resveratrol was docked on those proteins which are responsible for bone loss. The Molecular docking data between the resveratrol and Receptor activator of nuclear factor-kappa-Β ligand [RANKL] receptors proved that resveratrol binds tightly to the receptors, showed the highest binding affinities of −6.9, −7.6, −7.1, −6.9, −6.7, and −7.1 kcal/mol. According to in-vitro data, Resveratrol reduced the osteoclasts after treating Marrow-Derived Macrophages [BMM] with Macrophage colony-stimulating factor [MCSF] 20ng / ml and RANKL 50ng / ml, with different concentrations of resveratrol (2.5, 10 μg / ml) For 7 days, the cells were treated with MCSF (20 ng / ml) and RANKL (40 ng / ml) together with concentrated trimethyl ether and resveratrol (2.5, 10 μg / ml) within 12 hours. Which, not affect cell survival. After fixing osteoclast cells with formaldehyde fixative on glass coverslip followed by incubation with 0.1% Triton X-100 in PBS for 5 min and after that stain with rhodamine phalloidin staining for actin and Hoechst for nuclei. Fluorescence microscopy was performed to see the distribution of filaments actin [F.actin]. Finally, resveratrol reduced the actin ring formation. Resveratrol is the best bioactive compound for drug preparation against bone loss.


Com a aplicação do método in-silico, o resveratrol foi ancorado nas proteínas responsáveis pela perda óssea. Os dados de docking molecular entre o resveratrol e o ligante do receptor ativador do fator nuclear kappa-Β [Receptor Activator of Nuclear Factor kappa-B Ligant (RANKL)] provaram que o resveratrol se liga fortemente aos receptores, mostraram as afinidades de ligação mais altas de −6,9, −7,6, −7,1, −6,9, - 6,7 e -7,1 kcal / mol. De acordo com dados in-vitro, o resveratrol reduziu os osteoclastos após o tratamento de macrófagos derivados da medula óssea [Bone Marrow derived Macrophage (BMM)] com fator estimulador de colônias de macrófagos [Macrophage Colony-Stimulating Factor (MCSF)] 20ng / ml e RANKL 50ng / ml, com diferentes concentrações de resveratrol (2,5, 10 μg / ml). Durante sete dias, as células foram tratadas com MCSF (20 ng / ml) e RANKL (40 ng / ml) juntamente com éter trimetílico concentrado e resveratrol (2,5, 10 μg / ml) em 12 horas, processo que não afeta a sobrevivência celular. Após a fixação de células de osteoclastos com fixador de formaldeído em lamela de vidro seguido de incubação com 0,1% Triton X-100 em PBS por 5 min, foi realizado posteriormente o procedimento para corar com rodamina faloidina a actina e Hoechst os núcleos. A microscopia de fluorescência foi realizada para ver a distribuição dos filamentos de actina [F.actina]. Finalmente, o resveratrol reduziu a formação do anel de actina. O resveratrol é o melhor composto bioativo para o preparo de medicamentos contra a perda óssea.

LC50 values for F. equiseti after 24h, 48h, 72h and 96h of exposure were recorded as 3.8x 108, 2.9x107, 2.0x107, and 7.1x106 conidia /ml respectively while LC50 values for F. proliferatum were recorded as 1.21x108, 9.6x107, 4.2x107, 2.6x107 conidia /ml respectively after 24h, 48h, 72h and 96h of exposure. The results indicate that among two fungal strains F. equiseti was found to be more effective in terms of its larvicidal activity than F. proliferatum against larvae of A. aegypti.


Resumo A dengue transmitida pelo mosquito Aedes aegypti é uma das doenças transmitidas por insetos de propagação mais rápida. A dependência atual do controle do vetor da dengue é principalmente de inseticidas químicos. O aumento da resistência a inseticidas no principal vetor do mosquito, Aedes aegypti, limita a eficácia do controle do vetor por meio de inseticidas químicos. Esses inseticidas químicos também têm impactos ambientais negativos sobre os animais, plantas e saúde humana. Os agentes de micobiocontrole são organismos que ocorrem naturalmente e são menos prejudiciais ao meio ambiente em comparação com os inseticidas químicos. No presente estudo, avaliou-se o potencial entomopatogênico de cepas locais de fungos isolados do solo para o controle do vetor da dengue. Isolados de fungos locais apresentam melhor alternativa para a introdução de uma cepa de biocontrole estrangeira, pois podem ser mais bem adaptados às condições ambientais da área para sobreviver e podem ter maior eficácia entomopatogênica contra o organismo-alvo. A eficácia larvicida de Fusarium equiseti e Fusarium proliferatum foi avaliada contra Aedes aegypti. Cepas locais de F. equiseti (MK371718) e F. proliferatum (MK371715) foram isoladas do solo de Changa Manga Forest, Paquistão, usando o método de isca para insetos. Atividade larvicida de dois Fusarium spp. foi testado contra larvas de quarto ínstar de A. aegypti em laboratório, nas concentrações 105, 106, 107 e 108 conídios / ml. Os valores de LC50 para F. equiseti após 24 h, 48 h, 72 h e 96 h de exposição foram registrados como 3,8x 108, 2,9x107, 2,0x107 e 7,1x106 conídios / ml, respectivamente, enquanto os valores de LC50 para F. proliferatum foram registrados como 1,21x108, 9,6 x107, 4,2x107, 2,6x107 conídios / ml, respectivamente, após 24 h, 48 h, 72 h e 96 h de exposição. According to in-vitro data, Resveratrol reduced the osteoclasts after treating Marrow-Derived Macrophages [BMM] with Macrophage colony-stimulating factor [MCSF] 20ng / ml and RANKL 50ng / ml, with different concentrations of resveratrol (2.5, 10 g / ml) For 7 days, the cells were treated with MCSF (20 ng / ml) and RANKL (40 ng / ml) together with concentrated trimethyl ether and resveratrol (2.5, 10 g / ml) within 12 hours. Which, not affect cell survival. After fixing osteoclast cells with formaldehyde fixative on glass coverslip followed by incubation with 0.1% Triton X-100 in PBS for 5 min and after that stain with rhodamine phalloidin staining for actin and Hoechst for nuclei. Fluorescence microscopy was performed to see the distribution of filaments actin [F.actin]. Finally, resveratrol reduced the actin ring formation. Resveratrol is the best bioactive compound for drug preparation against bone loss.

The emergence of multi-drug resistant (MDR) bacterial strains, which are posing a global health threat has developed the interest of scientists to use bacteriophages instead of conventional antibiotics therapy. In light of an increased interest in the use of phage as a bacterial control agent, the study aimed to isolate and characterize lytic phages from sewage effluent. During the current study, bacteriophage AS1 was isolated from sewage effluent against E.coli S2. The lytic activity of phageAS1 was limited to E.coli S2 strain showing monovalent behavior. The calculated phage titer was 3.5×109 pfu/ml. PhageAS1 was stable at a wide range of pH and temperature. The maximum stability was recorded at 37ºC and pH 7.0, while showing its normal lytic activity at temperature 60ºC and from pH 5.0 to11.0 respectively. At temperature 70ºC, phage activity was somewhat reduced whereas, further increase in temperature and decrease or increase in pH completely inactivated the phage. From the current study, it was concluded that waste water is a best source for finding bacteriophages against multi-drug resistant bacterial strains and can be used as bacterial control agent.


O surgimento de cepas bacterianas multirresistentes (MDR), que representam uma ameaça global à saúde, desenvolveu o interesse dos cientistas em usar bacteriófagos em vez da terapia convencional com antibióticos. Diante do crescente interesse no uso de fago como agente de controle bacteriano, o estudo visou isolar e caracterizar fagos líticos de efluente de esgoto. Durante o estudo atual, o bacteriófago AS1 foi isolado de efluente de esgoto contra E. coli S2. A atividade lítica de phageAS1 foi limitada à cepa E. coli S2, apresentando comportamento monovalente. O título de fago calculado foi de 3,5 x 109 ufp/ml. PhageAS1 foi estável em uma ampla faixa de pH e temperatura. ABSTRACT

India is trying to maintain stability in the current circumstances; another immediate threat has developed in the shape of coronavirus disease- associated mucormycosis. Mucormycosis is uncommon but lethal disease, caused by a group of molds known as mucormycetes. If not treated properly, an infection might be fatal. The most ordinary risk factor is diabetes mellitus, followed by hematological malignancies. Patients with postpulmonary tuberculosis and persistent renal diseases are further probable to extend mucormycosis in India. Trauma increases the risk of cutaneous mucormycosis. Despite a rise in illness awareness among treating physicians, disease-related morbidity and mortality remain high, as patients seek medical attention late in the disease process and medication is expensive. Despite the fact that it has been present since the beginning of the pandemic, the cause of this fungal infection remains unknown. In this review we aimed to summarize about mucormycosis and its epidemiology, pathogen, and treatment options in context to COVID-19.

ABSTRACT

Thyroid cancer is the prevalent endocrine cancers, and its incidence is growing all over the world, according to the World Health Organization. About 5–10 per cent of individuals with differentiated thyroid carcinoma may experience destructive behavior and metastasis, and their disease will be refractory to therapeutic techniques such as radiation therapy for an unknown cause, despite the favorable prognosis. Most aggressive, deadly, and unresponsive type of the cancer is thyroid carcinoma. Regrettably, existing treatments are not specific and are thus considered poor in treating thyroid malignancies. Consequently, mortality in this malignancy despite progress in diagnosis and treatment is a prominent issue in medicine. evidence linking cellular, molecular, and genetic to a diagnostic and therapeutic simplification. With the new idea of personalized therapy for thyroid cancer diagnosis, arranging the treatment, discovering the success of the treatment and assessing the visualization has improved in the last ten years. Personalized medicine treatment for thyroid cancer is supported by these studies. According to the findings of this review, cellular and molecular processes of cancer will lay concrete on the way for the development of narrative biomarkers for personalized medicine that take individual variations into account

BACKGROUND: Management of malignant bone and soft tissue tumors remains an overwhelming confront to orthopedic surgeons. The challenge is discriminating in developing countries due to inadequate diagnostic and therapeutic amenities and unawareness. A lot has been discussed about the neglected orthopedic trauma, but the published literature on the causes and management of neglected bone and soft tissue tumors is sparse. Hence, current study was undertaken to highlight the causes of neglect and therapeutic challenges for managing these neglected tumors in developing countries. AIMS AND OBJECTIVES: To determine the causes of neglect of malignant bone and soft tissue tumors, their epidemiology (including their relative frequencies, age, gender discrimination, anatomical sites of occurrence and histological characteristics) and difficult aspect of management due to neglect or delayed presentation. MATERIALS AND METHODS: This was an appraisal of the neglected malignant bone and soft tissue tumors presented to J. N. Medical College and Hospital from June 2008 to May 2013. Criteria for labeling the tumor as neglected malignant bone and soft tissue tumor was delayed presentation (>3 months), locally advanced disease, ulceration, sepsis, fungating mass or metastasis at the time of presentation. All the cases were reviewed and analyzed for age, gender, histological types, educational status and socioeconomic status of the family, any prior treatment by traditional bone setters or registered medical practitioner, cause of delay for seeking medical advice. We have also analyzed the treatment given at our institute and the outcome of the tumor. OBSERVATIONS AND RESULTS: Eighteen patients fulfilled the criteria for neglected malignant bone and soft tissue tumors, hence were included in study. Eight cases were of osteosarcoma, five cases were of Ewing’s sarcoma, three cases were of chondrosarcoma and 1 case each was of pleomorphic liposarcoma and primary lymphoma of bone. According to Enneking staging system 11 cases were of stage III (distant metastasis) and 7 were stage II‑B. Seven were females, and 11 were males. Age range was 5–68 years. 15 patients (83.3%) belonged to low socioeconomic status with 17 patients (94.4%) belonged to uneducated background. Cause of delay in seeking medical advice was neglect by the patient and family due to financial constraints, cultural and religious believes, lack of access to health care facilities, consultation with traditional bone setters and even misdiagnosis by qualified orthopedic surgeons. The tumors included were all unresectable and of huge sizes, hence were managed with amputation/dis‑articulation, chemotherapy or radiation. CONCLUSION: The current study tries to highlight the causes and quantity of neglect of malignant bone and soft tissue tumors prevalent in our country, which poses a therapeutic challenge for management and consequent mutilating surgeries with poor outcome resulting in loss of extremity and existence.

Past studies have shown that in attempts to prevent HIV; health education yields little change in condom use. The reason may be that education fails to target barriers for changing behaviour. The present controlled pilot study tested whether psychological inoculation (PI) reduces such barriers for using male condoms. Twenty-two Nigerian women with HIV were randomly assigned to receive PI or health education (control). In the PI condition; women learned to refute sentences reflecting barriers against condom use; while controls learned how to use condoms and the consequences of their non-use. Barriers for condom use; self-efficacy to negotiate condom use with partners and actual condom use were self-reported before and one week after interventions. Results revealed that only in the PI group were there statistically significant increases in condom use negotiating self-efficacy and reductions in barriers concerning motivation; sexual satisfaction and partners. Controls reported no statistically significant changes. However; actual reported condom use was unchanged in both groups. Thus; it is feasible to conduct PI interventions in an African sample of HIV patients. Furthermore; PI can reduce cognitive barriers for condom use; while health education yields little changes in such outcomes over time. If replicated in larger samples with longer follow-ups; these findings could eventually have implications for HIV prevention in several world regions

ABSTRACT

Thirty-six patients of type C intercondylar fractures of lower end of humerus who visited JN Medical College, Aligarh between January, 2001 and January, 2003 were included in the study. All patients were treated surgically by open reduction and internal fixation with 4mm cancellous screws, reconstruction plates, one-third tubular plates. Early physiotherapy was started and the results graded using Krishnamoorthy criteria.
