ABSTRACT

bjective To observe the changes of upper extremity sensory function in patients with radicular cervical spondylosis and evaluate the treatment effect, by using the method of suspension exercise therapy (SET) and soft tissue manipulation. Methods A total of 72 patients with cervical spondylosis were divided into observation group (treated by SET combined with manipulation)and control group (treated by purely cervical traction) by simple random method. Both groups received 4-week continuous treatment for 6 times per week. The somatosensory evoked potential(SEP), current perception threshold (CPT) before and after the treatment in two groups were separatedly tested, and changes in visual analogue scale (VAS) pain scores of the affected limbs were examined, so as to determine the effective rate of treatment. Results After treatment, the latency of SEP was shortened mainly in brachial plexus potential N9 and cervical spinal potential N13 in two groups (P<0.05). The CPT levels and the VAS pain scores of the affected limbs were reduced in two groups (P<0.01), and the treatment effect in observation group was better that in control group (P<0.01).Conclusion SET combined with soft tissue manipulation can effectively promote the repair of nerve sense function of cervical spondylotic radiculopathy.

ABSTRACT

Objective:To investigate the clinical outcomes of anterolateral thigh flap combined with great saphenous vein transplantations for the treatment of extensive elbow soft tissue defects accompanied by brachial artery embolism. Methods:A retrospective case series study was carried out to investigate the clinical data of 19 patients with extensive elbow soft tissue defects accompanied by brachial artery embolism admitted to Fuyang People's Hospital of Anhui Medical University from March 2017 to March 2020. There were 15 males and 4 females, aged 14-59 years[(37.6±14.1)years]. The areas of elbow soft tissue defects ranged from 7 cm×3 cm to 12 cm×5 cm. Before operation, angiography test was performed to confirm brachial artery embolism of the affected upper limb. The length of embolization ranged from 5 to 12 cm[(7.3±1.6)cm]. All patients were treated using the anterolateral thigh flap combined with great saphenous vein transplantation. The operation time and intraoperative blood loss were recorded. Angiography test was re-examined one week after operation. The degree of flap swelling, elbow Broberg-Morrey functional score and scar contracture were evaluated at postoperative 3,6,and 12 months. Healing of donor site and postoperative complications were observed. Results:All patients were followed up for 12-24 months[(18.1±3.0)months]. The operation duration was 3.5-6.4 hours[(4.9±0.8)hours],with intraoperative blood loss of 200-600 ml[(338.7±101.6)ml]. The blood flow of repaired brachial artery was unobstructed in all patients one week after operation, with all flaps survived. The degree of flap swelling was(0.9±0.3)cm at postoperative 12 months, significantly reduced from that at postoperative 3 and 6 months[(1.2±0.3)cm,(1.1±0.3)cm]( P<0.05). The elbow Broberg-Morrey functional score was(87.8±4.8)points,significantly higher than those at postoperative 3 and 6 months[(71.4±7.0)points,(80.2±4.8)points]( P<0.05). The rating of elbow function score was excellent in 9 patients, good in 8 and fair in 2 at postoperative 12 months, with the excellent and good rate of 90%. The rate of scar contracture was 63%(12/19)at postoperative 12 months, significantly higher than that at postoperative 3[11%(2/19)]( P<0.05),while not different from that at postoperative 6 months[42%(8/19)]( P>0.05). The donor site was healed in all patients, without complications related to blood vessels or flaps. Conclusions:For extensive elbow soft tissue defect accompanied by brachial artery embolism, the anterolateral thigh flap combined with great saphenous vein transplantations can successfully restore the blood supply of the distal site of the affected limb with a high survival rate of the flap and well recovered limb function, and hence is a feasible method for severe elbow trauma. However, due to various factors such as elbow joint immobilization during perioperative period, scar contracture is prone to occur in the area of the flap.

ABSTRACT

ObjectiveTo investigate the efficacy and safety of CalliSpheres microsphere-transcatheter arterial chemoembolization (CSM-TACE) versus conventional transcatheter arterial chemoembolization (cTACE) in the treatment of hepatocellular carcinoma (HCC) through a meta-analysis. MethodsPubMed, Web of Science, Cochrane Library, CNKI, Wanfang Data, and VIP were searched for all Chinese and English articles on the application of CSM-TACE and cTACE in HCC published up to the end of October, 2020. After quality assessment was performed for the articles included, RevMan 5.3 software provided by Cochrane Library was used for analysis. ResultsA total of 15 studies were included, with 1535 patients in total. This meta-analysis showed that compared with the patients receiving cTACE, the patients receiving CSM-TACE had significantly higher 1-year overall survival rate (odds ratio [OR]=2.26, 95% confidence interval [CI]: 1.63-3.13, P<0.000 01), 2-year overall survival rate (OR=1.73, 95%CI: 1.20-2.50, P=0.003), and 2-year progression-free survival rate (OR=1.60, 95%CI: 1.05-2.43, P=0.03). In terms of safety, compared with the patients receiving cTACE, the patients receiving CSM-TACE had significantly lower incidence rates of postoperative vomiting (OR=0.65, 95%CI: 0.46-0.92, P=0.01), bone marrow suppression (OR=0.17, 95%CI: 0.05-0.54, P=0.003), and neutropenia (OR=0.18, 95%CI: 0.07-045, P=0.000 3), while there were no significant differences between the two groups of patients in postoperative pyrexia, abdominal pain, and ascites (all P>0.05). ConclusionCSM-TACE has significant advantages in improving 1- and 2-year overall survival rates and 2-year progression-free survival rates and can significantly reduce the incidence rates of postoperative vomiting, bone marrow suppression, and neutropenia. Therefore, CSM-TACE is a safe and effective treatment method.

ABSTRACT

OBJECTIVE@#To analyze gene expression profile of T cell lymphoma Jurkat cell line treated with paclitaxel by computational biology based on next generation sequencing and to explore the possible molecular mechanism of paclitaxel resistance to T cell lymphoma at gene level.@*METHODS@#IC50 of paclitaxel on Jurkat cell line was determined by CCK-8 assay. Gene expression profile of Jurkat cells treated with paclitaxel was acquired by next generation sequencing technology. Gene microarray data related to human T cell lymphoma were screened from Gene Expression Omnibus (GEO) database (including 720 cases of T cell lymphoma and 153 cases of normal tissues). Combined with the sequencing data, differential expression genes (DEGs) were intersected and screened. DAVID database was used for enrichment analysis of GO function and KEGG pathway to determine and visualize functional entries of DEGs, and protein-protein interactions network of DEGs was drawn. The levels of gene expression were detected and verified by RT-qPCR.@*RESULTS@#CCK-8 results showed that the proliferation of Jurkat cells was inhibited by paclitaxel depended on the concentration apparently. Treated by paclitaxel for 48 h, P<0.05 and |log2(FC)|≥1 were used as filter criteria on the results of RNA Sequencing (RNA-Seq) and GeoChip, 351 DEGs were found from Jurkat cells, including 323 up-regulated genes and 28 down-regulated genes. The GO functional annotation and KEGG pathway enrichment analysis showed that the role of paclitaxel was mainly concentrated in protein heterodimerization activity, nucleosome assembly and transcriptional dysregulation in cancer, etc. The results of RT-qPCR were consistent with those of the sequencing analysis, which verified the reliability of this sequencing.@*CONCLUSION@#Paclitaxel can affect the proliferation and apoptosis of T-cell lymphoma by up-regulating JUN gene, orphan nuclear receptor NR4A family genes and histone family genes.

ABSTRACT

Objective:To explore the relationship between their proficiency, needle penetration angle and the success rate of puncture by using a self-developed venipuncture angle monitor based on remote sensing technology, and measured the needle penetration angle of people with different venipuncture proficiency levels. Methods:A total of 30 sophomore medical school undergraduates without venipuncture experience and 30 nurses with more than 3 years of clinical experience each were recruited. Volunteers performed punctures on a rubber puncture model of the arm. The result of each puncture, the volunteer′s estimated insertion angle, and the actual insertion angle measured by the monitor were recorded and analyzed. Results:The puncture success rate of the nurse group was 96.7% (58/60), and the puncture success rate of the student group was 51.7% (31/60). The puncture success rate of the nurse group was higher than that of the student group( χ2 value was 29.4, P<0.05); The estimated difference of the average angle of the nurse group was (5.2 ± 1.9) °, and the estimated difference of the average angle of the student group was (8.9 ± 2.6) °. The estimated difference of the nurse group was less than the student group ( Z value was -7.10, P<0.05); the most suitable venipuncture angle range for beginners was (31.6 ± 6.4)°. Conclusions:There is a correlation between the success rate of venipuncture and the puncture angle, and optimum puncture angle for beginners in the controlled model was (31.6±6.4)°. At both skill levels, operator estimates of the puncture angle were commonly larger than the actual angle. The angle monitor based on remote sensing technology makes the precise study of venipuncture possible.

ABSTRACT

Objective:To explore the therapeutic effect and mechanism of Chaihu Yueju decoction on model rats induced by chronic unpredictable mild stress (CUMS). Method:The 60 SD rats were randomly divided into normal group, model group, Chaihu Yueju decoction low, medium and high-dose groups(0.3, 0.6, 1.2 g·kg-1) and fluoxetine hydrochloride group(0.2 mg·kg-1), 10 for each group. The model rats was established by chronic unpredictable mild stress for 5 weeks, and then Chaihu Yueju decoction and fluoxetine hydrochloride were given to the corresponding treatment group by gavage for 3 weeks. In the last week of gavage, Morris water maze training and testing were conducted. After the last day of gavage, sugar water preference and other behavioral experiment were tested. The sugar water preference test was used to detect the degree of pleasure deficiency in rats before and after treatment, the open field test was used to detect the depression of rats before and after treatment, the spatial memory ability was tested by Morris water maze. Western blot was used to detect the levels of tumor necrosis factor-α(TNF-α), 5-hydroxytryptamine receptor 1A (5-HT1A), extracellular signal-regulated kinase (ERK) and synapse associated protein in hippocampus of each group, Gloji and Nissl staining was used to observe the changes of dendritic spines and Nissl bodies in the hippocampus CA3. Result:Compared with normal group, the weight, sugar water preference rate, the scores of horizontal and vertical movement were significantly reduced (P<0.05), and the immobility time was significantly prolonged in model group (P<0.05). Meanwhile, the ability of learning and memory in model group decreased (P<0.05). Western blot results showed that the levels of TNF-α was significantly increased (P<0.05), and the levels of 5-HT1A, p-ERK, cyclic adenosine phosphate reactive element binding protein (CREB), p-CREB, Synapsin-1,Synaptophysin, glutamate receptor subtype-1(GluR-1)and postsynaptic membrane protein-95(PSD-95) in model group were significantly decreased (P<0.05). Gloji and Nissl staining results showed that the density of dendritic spines and the number of Nissl body in the hippocampal CA3 of the model group was obviously attenuate (P<0.05). The results show that the neurons were evidently damaged. Compared with model group, the weight, sugar water preference rate, the scores of horizontal and vertical movement were clearly increased (P<0.05) and the immobility time was significantly shortened in fluoxetine hydrochloride group, middle dose and high dose Chaihu Yueju decoction group (P<0.05). Western blot results showed that the levels of synaptophysin, GluR-1 and PSD-95 were significantly increased in fluoxetine hydrochloride group, middle dose and high dose Chaihu Yueju decoction group. The level of TNF-α was significantly decreased (P<0.05), the levels of 5-HT1A, p-ERK, CREB, p-CREB and Synapsin-1 were remarkably increased (P<0.05), especially the high dose group of Chaihu Yueju decoction. Gloji and Nissl staining results showed that the density of dendritic spines and the number of Nissl body in the hippocampal CA3 of high dose group were similarly increased in a dose-dependent manner. Conclusion:Chaihu Yueju decoction could improves the weight, the depressive despair, autonomous activity ability and learning and memory ability of DP model rats. Its mechanism is closely related to attenuating the inflammatory reaction and enhancing the levels of 5-HT1A receptor protein, ERK and synapse related protein, then activating 5-HT/CREB and ERK/CREB signaling pathways, increasing the number and distribution of dendritic spines and repairing damaged neurons in the DP model rat's hippocampus.

ABSTRACT

Objective:To investigate the effect of actin related protein 2/3 complex subunit 2 (ARPC2) gene silencing on the biological characteristics of papillary thyroid carcinoma (PTC) TPC-1 cells through lentivirus-mediated RNA interference.Methods:TPC-1 cells infected with nonsense short hairpin RNA (shRNA) sequence lentivirus (shCtrl) was used as the control group. TPC-1 cells infected with ARPC2 shRNA interference sequence lentivirus (shARPC2) was used as the experimental group, in which the expression of ARPC2 gene was specifically interfered. The effects of silencing the expression of ARPC2 gene on the proliferation of TPC-1 cells were detected by using methyl thiazolyl tetrazolium (MTT) assay, flow cytometry, Western blot and colony formation test. Flow cytometry and Western blot were conducted to detect the effect of silencing ARPC2 gene on TPC-1 cells apoptosis and related proteins.Results:shARPC2 could efficiently infect TPC-1 cells, and the expression efficiency of green fluorescent protein was over 85%. Compared with the control group, TPC-1 proliferation was inhibited in the experimental group. The ratio of S-phase cells in the experimental group was reduced compared with that in the control group [(14.79±0.21)% vs. (21.13±0.33)%, t = 27.77, P < 0.05]. The ratio of G 1 and G 2/M-phase cells in the experimental group was increased compared with that in the control group [G 1 phase: (67.57±0.08)% vs. (62.06±0.36)%, t=25.56, P < 0.05; G 2/M phase: (17.64±0.12)% vs. (16.91±0.17)%, t=6.154, P < 0.05]. Meanwhile, the expressions of cell cycle-related proteins CDK2, CyclinE and CyclinD were reduced in the experimental group. The number of clone formation in the experimental group was less than that in the control group, the difference was statistically significant [(10±2) vs. (161±6), t=9.011, P < 0.05]. In addition, the apoptotic ratio of cells in the experimental group was higher than that in the control group [(8.60±0.77)% vs. (4.08±0.40)%, t=9.011, P < 0.05]. Western blot showed that the expressions of anti-apoptotic factors p21 and bcl-2 were reduced in the experimental group, while the expression of pro-apoptotic factor bax was increased. Conclusion:The interference with the expression of ARPC2 regulated by shRNA can inhibit the proliferation, and promote the apoptosis of PTC TPC-1 cells, indicating that ARPC2 may be a possible biological new target for the treatment of PTC.

ABSTRACT

Objective:To explore the relationship between death indicators and unplanned return indicators on healthcare quality evaluation. Methods:A total of 836 976 medical record data were collected from 31 tertiary public general hospitals in a diagnosis-related groups(DRG) data platform in 2017. Multiple death indices(low and low-risk risk group mortality, high-risk group mortality, crude mortality, and risk adjusted mortality) and unplanned return indices(31-day unplanned readmission rate and 31-day unplanned return to surgery rate) were calculated. Pearson′s correlation coefficient was used to examine the relationships among those indices. Results:Death indicators were correlated with each other, but the unplanned readmission rate was not correlated with the unplanned reoperation rate( r=0.305). There was no correlation between unplanned re-entry rate and death rate. The correlation coefficients were as follows: unplanned readmission rate versus low and low-risk group mortality( r=-0.227), versus high-risk group mortality( r=-0.098), versus actual mortality( r=-0.130), versus risk adjusted mortality( r=0.010); unplanned reoperation rate versus low and low-risk group mortality( r=0.105), versus high-risk group mortality( r=0.030), versus actual mortality( r=-0.004), versus risk adjusted mortality( r=-0.141). Conclusions:The indicators of death and the indicators of unplanned return are not the same in terms of actual management technology and evaluation effect. They are complementary to each other and can form an ideal combination of quality evaluation indicators.

ABSTRACT

Our recent studies demonstrated that the natural product nobiletin (NOB) served as a promising multidrug resistance (MDR) reversal agent and improved the effectiveness of cancer chemotherapy . However, low aqueous solubility and difficulty in total synthesis limited its application as a therapeutic agent. To tackle these challenges, NOB was synthesized in a high yield by a concise route of six steps and fourteen derivatives were synthesized with remarkable solubility and efficacy. All the compounds showed improved sensitivity to paclitaxel (PTX) in P-glycoprotein (P-gp) overexpressing MDR cancer cells. Among them, compound exhibited water solubility 280-fold higher than NOB. A drug-resistance A549/T xenograft model showed that at a dose of 50 mg/kg co-administered with PTX (15 mg/kg), inhibited tumor growth more effective than NOB and remarkably increased PTX concentration in the tumors P-gp inhibition. Moreover, Western blot experiments revealed that inhibited expression of NRF2, phosphorylated ERK and AKT in MDR cancer cells, thus implying of multiple mechanisms to reverse MDR in lung cancer.

ABSTRACT

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can cause acute respiratory distress syndrome, hypercoagulability, hypertension, and multiorgan dysfunction. Effective antivirals with safe clinical profile are urgently needed to improve the overall prognosis. In an analysis of a randomly collected cohort of 124 patients with COVID-19, we found that hypercoagulability as indicated by elevated concentrations of D-dimers was associated with disease severity. By virtual screening of a U.S. FDA approved drug library, we identified an anticoagulation agent dipyridamole (DIP) , which suppressed SARS-CoV-2 replication . In a proof-of-concept trial involving 31 patients with COVID-19, DIP supplementation was associated with significantly decreased concentrations of D-dimers ( < 0.05), increased lymphocyte and platelet recovery in the circulation, and markedly improved clinical outcomes in comparison to the control patients. In particular, all 8 of the DIP-treated severely ill patients showed remarkable improvement: 7 patients (87.5%) achieved clinical cure and were discharged from the hospitals while the remaining 1 patient (12.5%) was in clinical remission.

ABSTRACT

Objective To understand the potential risk of schistosomiasis transmission in Xiuzhou District of Jiaxing City, so as to provide the scientific evidence for consolidating schistosomiasis control achievements. Methods Fixed and mobile surveillance sites were set up in Xiuzhou District of Jiaxing City from 2013 to 2015. Oncomelania hupensis snails was surveyed historical snail habitats, current snail habitats, and suspected snail habitats. The schistosome infections were identified using serological and parasitological testing among local residents and mobile populations. In addition, the survival and reproduction of snails imported into Xiuzhou District was observed, and the schistosome infection in wild reservoir hosts was detected. Results A total of 540.14 hm2 of settings were surveyed in Xiuzhou District, Jiaxing City from 2013 to 2015, and 1.65 hm2 of snail habitats were identified. The snail habitats were mainly located in dry lands, and no infected snails or importation of snails were found. During the period from 2013 to 2015, a total of 7 668 local residents and mobile populations were examined in Xiuzhou District, and no new local infections were detected; however, three imported schistosomiasis cases were identified. Field simulation experiment showed that the imported snails laid eggs and reproduced in Xiuzhou District, and no schistosome infections were found in wild animals. Conclusion There are still residual Oncomelania snails and imported schistosomiasis patients in Xiuzhou District of Jiaxing City; therefore, the surveillance and management of local Oncomelania snails and imported schistosomiasis should be intensified to reduce the risk of schistosomiasis transmission.

ABSTRACT

Objective To evaluate the surgical treatment of complex acetabular fractures through the lateral-rectus approach with pelvic reconstructive plate and antegrade posterior-column lag screws. Methods Between January 2014 and April 2018,29 patients were surgically treated for complex acetabular fractures at Department of Orthopaedic and Trauma,Taihe Hospital. They were 22 males and 7 females, aged from 19 to 72 years(mean,41.7 years).According to the Letournel-Judet classification, there were 4 transverse fractures,7 T-shape fractures,15 both column fractures and 3 anterior plus posterior hemitransverse fractures. In all the patients, the lateral-rectus approach was adopted;their anterior column fractures were fixated with pelvic reconstructive plate and screws and their posterior column fractures with antegrade lag screws. The operation time, intraoperative bleeding, fracture reduction, fracture union time, function of the affected hip and complications were recorded. Results The operation time ranged from 50 to 140 min, averaging 85 min;the volume of intraoperative bleeding ranged from 150 to 1,100 mL,averaging 315 mL.By the Matta criteria, the reduction was rated as excellent in 21 cases,as good in 7 and as poor in one, giving an excellent to good rate of 96.6%.The average follow-up was 16.2 months (from 6 to 30 months) for the 29 patients.Their fractures got united after an average of 11 weeks (from 7 to 13 weeks).According to the Merle d'Aubigné & Postel scoring system at the last follow-up,the function of the affected hip was excellent in 20 cases, good in 7 and fair in 2,yielding an excellent to good rate of 93.1%.There was one case of vasospasm of external iliac artery. No such complications were observed by follow-up as infection, deep venous thrombosis, heterotopic ossification, osteoarthritis or avascular necrosis of the femoral head. Conclusions The lateral-rectus approach with pelvic reconstructive plate and antegrade posterior-column lag screws is a suitable surgical treatment for complex acetabular fractures, leading to satisfactory clinical results. The lateral-rectus approach provides adequate exposure of the quadrilateral surface and facilitates insertion of the posterior column antegrade lag screws.

ABSTRACT

Immunosenescence is the decline in the structure and function of the body's immune system that involves organs, cells, im-mune factors, and regulatory networks. Decreased immunity and immune clearance, caused by immunosenescence, are key contribu-tors to tumorigenesis. This article begins by introducing the concept of immunosenescence and provides a comprehensive description of the relationship between immunosenescence and tumorigenesis. We also discuss the influence of the various components of the immune system on tumor incidence. Finally, the article provides ideas for immunosenescence intervention aimed at the prevention and treatment of cancer.
